L’intervention de Latarjet donne des résultats fonctionnels satisfaisants et un faible taux de récidive. Certains auteurs lui attribuent un caractère arthrogène lié au positionnement de la butée et au matériel. L’objectif principal était d’évaluer la prévalence de l’omarthrose à long terme, de déterminer les facteurs de risque d’omarthrose et le retentissement clinique de celle-ci. Notre hypothèse était que le risque d’arthrose pouvait être limité par le bon positionnement de la butée. L’objectif secondaire était d’évaluer le taux de récidive d’instabilité à long terme. Sur une cohorte de 102 patients (106 épaules), opérés entre 1984 et 1998, 77 avaient été revus avec un dossier radiologique complet en 2003 pour une précédente étude. Quarante-quatre de ces patients ont à nouveau été revus cliniquement et radiologiquement au recul moyen de 22 ans. L’évolutivité clinique et radiologique était comparée aux résultats observés au recul de 8 ans. Les patients étaient évalués cliniquement en relevant les mobilités, les scores de Constant et Walch-Duplay. Des radiographies étaient systématiquement réalisées pour déterminer le stade d’arthrose selon Samilson et un scanner pour 31 patients. Sur les 44 patients revus, le taux d’arthrose était de 34,1 %. Les patients étaient satisfaits dans 90,9 % des cas. Le score de Constant moyen était de 83,5 (±14) et le score de Duplay moyen de 71,6 (±23,4) points. L’âge élevé lors de l’intervention et à la révision, l’importance du recul et un âge supérieur à 30 ans lors du premier épisode d’instabilité étaient les principaux facteurs liés à la survenue d’une omarthrose et indépendants du geste chirurgical. Le débord latéral de la butée était lié à l’apparition d’une omarthrose (71,4 % vs 16,7 % en cas de butée médiale ou affleurante, p = 0,0004). La présence d’une encoche humérale ou d’une perte de substance glénoïdale et le nombre d’épisodes d’instabilité préopératoires étaient sans influence sur l’évolution arthrosique. Le débord latéral de la butée était le seul facteur de risque lié à la technique chirurgicale. Le retentissement clinique de cette omarthrose n’était significatif que pour les stades avancés (Samilson 3 et 4). Le débord latéral de la butée semblant être le seul paramètre lié à la technique chirurgicale pourvoyeur d’omarthrose à long terme, ce travail plaide pour un strict respect du positionnement coracoïdien auquel pourraient participer les ancillaires spécifiques ou les techniques arthroscopiques. Le traitement des fractures du corps du scaphoïde est généralement orthopédique chez l’enfant et chirurgical chez l’adulte, compte tenu du risque relativement élevé de pseudarthrose. La stratégie thérapeutique est moins codifiée chez les adolescents. L’objectif de cette étude était de comparer les paramètres radiographiques, cliniques et le taux de complications entre traitement orthopédique (TO) et chirurgical (TC) par vissage percutané de ces fractures chez les adolescents en fin de croissance. Le TO des fractures non déplacée du corps du scaphoïde chez l’adolescent permet une consolidation radiographique, un résultat fonctionnel et un taux de complication comparables à celui du TC. Une étude rétrospective monocentrique a été menée, incluant les patients ayant présenté une fracture non déplacée du corps du scaphoïde, avec un âge civil (AC) et un âge osseux (AO) entre 14 et 18 ans. Les paramètres cliniques, radiographiques et les complications ont été analysés lors du traumatisme et à un an, incluant des scores fonctionnels, entre deux groupes de patients, TO et TC. Trente-sept patients ont eu un TO (63,8 %) et 21 un TC (36,2 %). L’AC médian était de 16 ans [14,25–16]. L’AO médian était de 16 ans [15–17] selon Greulich et Pyle et correspondait à R9 [R7–R10] et U7 [U7–U8] selon le système radio-ulnaire-distal. Toutes les pseudarthroses ont été retrouvées dans le groupe TO (23,4 % vs 0 %, p = 0,019). La durée d’immobilisation (8 semaines) et le nombre de consultations étaient plus élevés après TO que TC. Les scores fonctionnels étaient inférieurs chez les patients ayant présenté une pseudarthrose après TO (p ≤ 0,002). Le TO des fractures du corps du scaphoïde chez l’adolescent entraîne un taux de pseudarthrose plus important que le TC, similaire au taux retrouvé chez les adultes. Une approche chirurgicale par vissage percutané est suggérée par ce travail. III ; étude rétrospective comparative.
Background: Treatment of scaphoid waist fractures is generally conservative in children but surgical in adults, given the relatively high risk of nonunion in adults. In adolescents, the required therapeutic strategy is less well defined. The objective of this study was to compare the radiographic and clinical parameters, and the rate of complications, between non-surgical orthopedic treatment (OT) and surgical treatment (ST) by percutaneous screw fixation of these fractures in adolescents approaching skeletal maturity. Hypothesis: ST of non-displaced scaphoid waist fractures in adolescents allows radiographic union, a functional result and a complication rate comparable to that of ST. Methods: This single-center retrospective study included patients who presented with a non-displaced scaphoid waist fracture, with a chronological age (CA) and a bone age (BA) between 14 and 18 years. Clinical and radiographic parameters and complications were analyzed during the trauma and at one year, including functional scores, between two groups of patients; OT and ST. Results: Thirty-seven patients had OT (63.8%) and 21 had ST (36.2%). The median CA was 16 years [14.25-16]. The median BA was 16 years [15;17] according to the Greulich and Pyle method and corresponded to R9 [R7-R10] and U7 [U7;U8] according to the Distal Radius and Ulnar (DRU) classification system. All nonunions were found in the OT group (23.4% vs 0%, p=0.019). The duration of immobilization (8 weeks) and the number of consultations were higher after OT than ST. Functional scores were lower in patients with nonunion after OT (p≤0.002) Conclusion: OT of scaphoid waist fractures in adolescents results in a higher rate of nonunion than ST, similar to the rate found in adults. Findings from this study recommend a surgical approach by percutaneous screw fixation. Level of evidence: III; comparative retrospective study.
INTRODUCTION:The Latarjet procedure provides satisfactory functional results, with low rates of recurrence. It is, however, sometimes claimed to induce osteoarthritis due to the positioning of the bone-block and hardware. The aims of the present study were to assess the long-term prevalence of osteoarthritis following the procedure, to determine risk factors and assess clinical impact. The study hypothesis was that osteoarthritis risk can be reduced by good bone-block positioning. The secondary objective was to assess long-term recurrence of instability. MATERIALS AND METHODS:Out of a cohort of 102 patients (106 shoulders) operated on between 1984 and 1998, 77 had complete radiology files for analysis in a previous study published in 2003. Forty-four of these patients were seen again for clinical and radiological assessment at a mean 22 years' follow-up, to be compared to results previously reported at 8 years. Clinical assessment comprised range of motion and Constant and Walch-Duplay scores. X-rays were taken systematically to determine Samilson osteoarthritis stage; 31 patients also underwent CT. RESULTS:In the 44 patients, the rate of osteoarthritis was 34.1%. 90.9% of patients were satisfied. Mean Constant score was 83.5±14 and mean Walch-Duplay score was 71.6±23.4. Advanced age at surgery and at assessment, length of follow-up and age >30 years at first episode of instability were the main factors for onset of osteoarthritis, independently of surgery. Lateral bone-block overhang correlated with onset of osteoarthritis: 71.4%, versus 16.7% in medial or flush bone-block (p=0.0004). Humeral notch, glenoid bone defect and number of preoperative episodes of instability were unrelated to onset of osteoarthritis. Lateral bone-block overhang was the only risk factor related to surgery. The clinical impact of the osteoarthritis was significant only in advanced stages (Samilson 3 and 4). CONCLUSION:Lateral bone-block overhang was the only surgical risk factor for osteoarthritis in the long-term. Strict coracoid positioning is therefore essential, and could be facilitated by specific instrumentation and by arthroscopy. LEVEL OF EVIDENCE:IV; retrospective study.
Background: The risk of recurrence after the first episode of anterior shoulder dislocation is high with nonoperative treatment in younger patients. Purpose/Hypothesis: The aim of this study was to compare the results of arthroscopic Bankart repair and nonoperative treatment for shoulder dislocation in patients younger than 25 years, with a minimum of 2 years of follow-up. The hypothesis was that surgery would decrease the risk of recurrence. Study design: Randomized controlled trial; Level of evidence, 1. Methods: We included patients aged between 18 and 25 years after a first episode of anterior shoulder dislocation and divided them into 2 groups. The first group was treated surgically with an arthroscopic Bankart repair within 2 weeks after the dislocation; the second group was treated nonoperatively. Both groups were immobilized for 3 weeks in internal rotation and followed the same physical therapy protocol. Standard radiography and computed tomography were performed immediately after reduction of the dislocation, and follow-up was performed at 3, 6, 12, and 24 months. The primary outcome measure was instability recurrence, defined as another anterior shoulder dislocation requiring closed reduction by another person (the patient was unable to reduce the dislocated joint themselves), a subluxation, or a positive apprehension test. Secondary outcome measures included range of motion, return to sport, and functional scores such as the short version of the Disabilities of the Arm, Shoulder and Hand score the Walch-Duplay score, and the Western Ontario Shoulder Instability Index (WOSI). Results: A total of 20 patients were included in each group. The mean ± SD age was 21 ± 1.8 years, and there were 33 men (82.5%) and 7 women (17.5%) in the total sample. Recurrence of instability was significantly decreased in the surgical treatment group compared with the nonoperative group (2 [10%] vs 14 [70%], respectively; P = .0001). Fewer patients in the surgical treatment group versus the nonoperative group had another episode of dislocation (0 vs 6 [30%], respectively), subluxation (2 [10%] vs 13 [65%], respectively; P = .003), or a positive apprehension test (1 [5%] vs 11 [58%], respectively; P = .0005). The Walch-Duplay score (88.4 vs 70.3 points; P = .046) and WOSI (11.5 vs 17.7 points; P = .035) were significantly better in the surgical group versus the nonoperative group after a 2-year follow-up. Level of sport was the same or better in 89% of the surgical treatment group vs 53% of the nonoperative treatment group (P = .012). No surgical complication was recorded. We did not find any significant difference in range of motion. Conclusion: In patients with first-time shoulder dislocations, arthroscopic labral repair (Bankart procedure) reduced the risk of secondary shoulder dislocation and improved functional outcome versus nonoperative treatment after a 2-year follow-up. Surgical treatment after a first episode of shoulder dislocation could be offered as a primary treatment option in a younger population if these results are confirmed by larger studies with a longer follow-up. Background: Conservative treatment after primary anterior shoulder dislocation (ASD) is associated with a high recurrence rate in young patients. The main objective was to compare primary arthroscopic Bankart repair and conservative treatment 2 years after primary ASD in patients less than 25 years old. Methods: A prospective randomized study was conducted in patients between 18 to 25 years old after primary ASD. The first group had arthroscopic Bankart repair within 15 days and the second group was treated conservatively. Patients in both groups were immobilized in internal rotation for 3 weeks and followed the same rehabilitation protocol. Patients were clinically monitored in 3, 6, 12 and 24 months. The main outcome criterion was recurrent dislocation, subluxation or apprehension when the arm was placed in 90° abduction and external rotation. Secondary outcome criteria were functional scores (Quick-DASH, Walch-Duplay, WOSI), joint mobility and resumption of sports. Results: 40 patients were included (20 in the surgical group and 20 conservative group). The average age was 21 +/- 1.8 years with 33 men (82.5%) and 7 women (17.5%). Patients in the surgical group underwent surgery within an average of 4.3 +/- 2.4 days. In the surgical group, instability at 2 years was significantly less than in the conservative group (2 (10%) compared to 14 (70%); p=0.0001). Fewer patients had a recurrence (0 versus 6 (30%)), subluxation (2 (10%) versus 13 (65%); p=0.003), and apprehension (1 (5%) versus 11 (58%); p=0.0005). The Quick-DASH (6.5 versus 11.2 points), Walch-Duplay (88.4 versus 70.3 points; p=0.007) and WOSI (11.5 versus 17.7 points; p=0.035) functional scores were better in the surgical group at 2 years. 95% had resumed sport in the surgical group compared to 68% in conservative group. No per- or postoperative complications were reported. There was no significant difference in joint mobility. Conclusion: After primary ASD in young patients, arthroscopic Bankart repair decreased recurrent instability at 2 years and provided better functional recovery than conservative treatment.
Background: Thoracic outlet syndrome (TOS) is challenging to diagnose, as the physical findings and investigations lack sensitivity and/or specificity. Magnetic resonance imaging (MRI) with dynamic manoeuvres can rule out a tumour and detect anatomical abnormalities potentially responsible for compression. The objective of this study was to assess the sensitivity and specificity of MRI for identifying anatomical structures responsible for compression in TOS, using intra-operative findings as the diagnostic reference standard. Hypothesis: MRI is effective in diagnosing the source of compression in TOS, notably within the scalene triangle and at the pleural apex. Methods: We retrospectively included 48 patients who underwent surgery for TOS after a work-up that included MRI (1.5-T, n=29 and 3-T, n=19). The MRI scans were reviewed for the study by a specialised radiologist who was unaware of the intra-operative findings. The sensitivity and specificity of MRI for diagnosing TOS were estimated using the intra-operative findings as the reference standard. Results: MRI identified a structure potentially responsible for TOS in 34 (71%) patients; thus, the false-negative rate was 14/48 (29%). The sensitivity of MRI was 28% for compression at the suspensory ligament of the pleural dome, 81% for hypertrophy of the anterior scalene muscle, and 50% for an accessory scalene muscle. For diagnosing a cervical rib, MRI had 100% sensitivity and 100% specificity. Conclusion: MRI can contribute to the diagnosis of TOS. Specificity is sufficiently high to provide guidance for planning the surgical procedure. Sensitivity, however, is too low for MRI to be useful as a screening test. MRI should be used in combination with the clinical assessment and other investigations to assist in the diagnosis of TOS. Level of evidence: IV, retrospective cohort study. 2019 Elsevier Masson SAS. Le diagnostic de syndrome du défilé cervico-thoraco-brachial (SDCTB) est difficile de par la faible sensibilité ou spécificité de l’examen clinique et paraclinique. L’IRM associée à des manœuvres dynamiques permet d’éliminer une cause tumorale et de rechercher des anomalies morphologiques potentiellement compressives. L’objectif de cette étude était de comparer les informations recueillies par l’IRM et les constatations peropératoires afin de préciser la sensibilité et la spécificité de l’IRM dans l’analyse des éléments anatomiques compressifs du défilé. Notre hypothèse était que l’IRM permettait de faire le diagnostic d’éléments compressifs, notamment au sein du triangle interscalénique et du défilé de l’appareil suspenseur de la plèvre. Une étude a été menée sur 48 cas opérés d’un SDCTB et ayant eu une IRM lors de leur bilan préopératoire (29 IRM 1,5 T et 19 IRM 3 T). Les IRM ont été analysées rétrospectivement par un radiologue spécialisé, en aveugle des constatations peropératoires. Les données chirurgicales et de l’imagerie ont été comparées afin de déterminer la sensibilité et la spécificité de L’IRM. L’IRM a permis de mettre en évidence une lésion potentiellement responsable du SDCTB dans 71 % des cas. Il y avait 29 % de faux négatifs. La sensibilité de l’IRM au niveau de l’appareil suspenseur de la plèvre était de 28 %, de 81 % pour diagnostiquer une hypertrophie du muscle scalène antérieur, de 50 % pour diagnostiquer un muscle scalène accessoire. La performance diagnostique de l’IRM était excellente pour faire le diagnostic des côtes cervicale (Se et Sp de 100 %). L’IRM peut être un examen contributif pour aider au diagnostic de SDCTB. Avec une bonne spécificité, il peut guider le chirurgien dans la planification de son intervention mais sa sensibilité ne permet pas d’en faire un examen de dépistage. Elle doit s’associer aux autres outils cliniques paracliniques pour aider au diagnostic du SDCTB. IV, série de cas.
Le traitement orthopédique après une première luxation gléno-humérale antérieure (LGHA) est associé à un taux de récidive élevé chez les patients jeunes. L'objectif principal était de comparer la réinsertion capsulo-labrale arthroscopique d'emblée et le traitement orthopédique sur l'instabilité à 2 ans d'une première LGHA chez des patients de moins de 25 ans. Une étude prospective randomisée a été menée sur des patients âgés de 18 à 25 ans ayant une première LGHA. Le premier groupe bénéficiait d'une réinsertion capsulo-labrale arthroscopique réalisée dans les 15 jours post-traumatiques, le deuxième groupe était traité orthopédiquement. Les patients des deux groupes étaient immobilisés en rotation interne pendant 3 semaines et suivaient le même protocole de rééducation. Une radiographie et un scanner étaient réalisés à l'inclusion après réduction. Les patients étaient suivis cliniquement à 3, 6, 12 et 24 mois. Le critère de jugement principal était la survenue d'une instabilité récurrente définie comme une récidive avec réduction par une tierce personne, une subluxation ou une appréhension à la manœuvre de l'armer. Les critères de jugement secondaires étaient les scores fonctionnels (Quick-DASH, Walch-Duplay, WOSI), les mobilités articulaires et la reprise du sport. 40 patients étaient inclus (20 dans le groupe chirurgical et 20 dans le groupe immobilisation). L'âge moyen était de 21 ± 1,8 ans avec 33 hommes (82,5 %) et 7 femmes (17,5 %). Les patients du groupe chirurgical étaient opérés à un délai moyen de 4,3 ± 2,4 jours. Dans le groupe chirurgical, l'instabilité à 2 ans était significativement moins importante que dans le groupe immobilisation (2 (10 %) contre 14 (70 %) ; p = 0,0001). Moins de patients avaient présenté de récidive (0 contre 6 (30 %)), de subluxation (2 (10 %) contre 13 (65 %) ; p = 0,003), et d'appréhension à l'armer (1 (5 %) contre 11 (58 %) ; p = 0,0005). Les scores fonctionnels Quick-DASH (6,5 contre 11,2 points), Walch-Duplay (88,4 contre 70,3 points ; p = 0,007) et WOSI (11,5 contre 17,7 points ; p = 0,035) étaient meilleurs dans le groupe chirurgical à 2 ans. 95 % avaient repris le sport à 2 ans dans le groupe chirurgical contre 68 % dans le groupe immobilisation. Aucune complication per ou post-opératoire n'était relevée. Il n'existait pas de différence significative sur les mobilités articulaires. Après une première LGHA chez le patient jeune, la réinsertion capsulo-labrale arthroscopique diminuait l'instabilité récurrente à 2 ans et offrait une meilleure récupération fonctionnelle que le traitement orthopédique.
Extra-articular fractures of the distal radius with posterior displacement are typically treated with volar locking plates. However, this fixation method does not address the posterior comminution, which seems to have no impact on the final result. The purpose of this study was to determine the fate of the posterior comminution. This was a retrospective study of 22 patients over 50 years old with a distal radius fracture. A preoperative computed tomography (CT) scan was performed to evaluate the comminution. All fractures were fixed with a volar locking plate. All patients underwent a bone density scan. Patients were reviewed at 6 months post-operative to determine their clinical, radiological and functional outcomes. The CT scan was performed again to determine the fate of the comminution. At 6 months post-operative, 82% of patients had an oval metaphyseal defect. The mean volume of this defect was 1.86 mL. The contents of this defect most closely resembled fat. There was no statistical link between the defect's volume and the various parameters studied. On the other hand, the defect's density was positively related to the functional outcome and negatively related to the patients' body mass index. Because of the compression experienced by the cancellous bone, a distal metaphyseal defect often persists after consolidation in dorsally displaced distal radius fractures. The posterior comminution is ultimately of little consequence.
La tendinopathie des fibulaires est une pathologie sous-diagnostiquée dont le principal facteur déclenchant est l’entorse de cheville. L’échec du traitement médical conduit fréquemment la réalisation d’un geste chirurgical dont les modalités varient en fonction du type lésionnel. L’objectif de ce travail était d’évaluer les résultats cliniques et fonctionnels de la réparation tendineuse et d’analyser l’influence de l’axe de l’arrière-pied sur la qualité des résultats. Notre hypothèse était qu’un varus de l’arrière-pied altérait les résultats à moyen terme. Nous avons analysé une série rétrospective de 30 patients opérés d’une réparation des tendons fibulaires par réparation ou ténodèse au recul moyen de 20 mois. Tous les patients bénéficiaient d’une échographie ou d’une IRMN préopératoire. L’évaluation clinique faisait appel au score AOFAS et au score FAAM. L’axe de l’arrière-pied était chiffré sur les clichés en incidence de Méary en distinguant un groupe valgus (n = 11) et un groupe varus (n = 17). Le score AOFAS moyen de l’ensemble de la cohorte progressait de 74,5 ± 11,2 points avant l’intervention à 86,7 ± 9,4 points à la révision. Il existait une différence significative du score AOFAS lors de la révision (p = 0,0003) entre les patients du groupe valgus (AOFAS = 93/100) et le sous-groupe de patients présentant un varus de l’arrière-pied (AOFAS = 82/100). Le score FAAM ne révélait aucune différence significative entre les deux groupes. Le délai de prise en charge, le type lésionnel et la technique chirurgicale utilisée étaient sans influence sur le résultat clinique. Les résultats cliniques des réparations chirurgicales des tendons fibulaires sont satisfaisants et conformes aux données de la littérature mais la présence d’un varus de l’arrière-pied conduit à un résultat moins favorable pouvant faire discuter la nécessité d’une correction chirurgicale par ostéotomie de valgisation calcanéenne. Lateral epicondylitis is a condition whose pathophysiology is poorly understood and whose optimal treatment divides physicians. Arthroscopy has proven its worth, with results similar to or better than open surgery. The purpose of this study was to look for prognostic factors for the outcomes of arthroscopic treatment of lateral epicondylitis. This was a retrospective, single-center study involving 39 cases (36 patients). Patients were split into two groups based on the Mayo Elbow Performance score (MEPS). A good result was defined as a score of 75 or better. The variables of interest were gender, age, body mass index, smoking habits, recognition as an occupational disease, duration of absence from work, cartilage involvement, presence of a capsular lesion and length of follow-up. The mean follow-up at review was 37.2 months (± 17.4). The average MEPS was 77.44 (± 15.51). Smoking was statistically related to a poor result (P = 0.0422) and a longer follow-up was statistically related to a good result (P = 0.0396). We identified two prognostic factors for lateral epicondylitis treated by arthroscopy. Smoking has a negative effect and ideally, patients should quit before surgical treatment. Time has a positive effect; thus patients should be informed that it will take several months or even years for their ailment to improve. Taking these factors into consideration will improve the functional outcomes of this surgery.
Introduction: Peroneal tendinopathy is an under-diagnosed pathology, mainly triggered by ankle sprain. Failure of medical treatment often leads to surgery, with modalities adapted to the lesion type. The present study aimed to assess clinical and functional results of tendon repair and to analyze the influence of hindfoot alignment on quality of outcome. The study hypothesis was that hindfoot varus impairs medium-term results. Materials and methods: A retrospective series of 30 patients undergoing peroneal tendon repair or tenodesis was analyzed at a mean 20 months' follow-up. All patients had preoperative ultrasound scan or NMRI. Clinical assessment was based on AOFAS and FAAM scores. Hindfoot alignment was assessed on Wary view; 2 groups were distinguished: valgus (n = 11) and varus (n = 17). Results: Mean AOFAS score improved, from 74.5 +/- 11.2 preoperatively to 86.7 +/- 9.4 at follow-up. There was a significant intergroup difference (p = 0.0003) in AOFAS at follow-up: valgus, 93/100; varus, 82/100. There was no significant intergroup difference in FAAM score. Time to surgery, lesion type and surgical technique did not influence clinical results. Discussion/conclusion: Clinical results for surgical repair of peroneal tendons were satisfactory and in line with literature reports. However, hindfoot varus was associated with poorer results, raising the possibility of correction by opening wedge calcaneal osteotomy. (C) 2018 Published by Elsevier Masson SAS.
Les fractures extra-articulaires du radius distal à déplacement postérieur sont traitées par plaque antérieure verrouillée par la plupart des chirurgiens. Ce mode d’ostéosynthèse néglige la comminution de la corticale postérieure sans se soucier de sa bonne reconstitution. Ce travail avait pour but d’étudier le devenir de la comminution postérieure dans ce type de fracture. Il s’agit d’une étude prospective, monocentrique menée de mars à décembre 2014. Tous les patients consultant aux urgences et présentant une fracture extra-articulaire du radius distal à déplacement postérieur étaient inclus. Vingt patients ont pu bénéficié d’un scanner préopératoire permettant d’évaluer au mieux la comminution postérieure. Toutes les fractures étaient ostéosynthésées par une plaque antérieure verrouillée. Les patients étaient revus à 1, 3 et 6 mois pour une évaluation clinique, radiologiques et fonctionnelle. Entre le 1er et le 3e mois postopératoire, un bilan ostéoporotique était réalisé. Il comprenait le dosage de la vitamine D, de la calcémie et la réalisation d’une ostéodensitométrie osseuse. À 6 mois postopératoires, un scanner était réalisé. Vingt-neuf patients ont été inclus. Tous présentaient une comminution corticale postérieure. À 6 mois postopératoires, il a été trouvé chez 23 patients (79,3 %) un défect métaphysaire postérieur et distal de forme ellipsoïdale à grand axe transversal. Le volume moyen de ce défect était de 1,76 mL ± 1,52. Il est situé en moyenne à 5,47 mm ± 2,67 de l’interligne radio-carpienne. Le contenu de ce défect avait une tonalité proche de celle de la graisse. La corticale postérieure du radius était restituée ad integrum. Il n’a pas été mis en évidence de lien statistique entre le volume du défect et les différents paramètres étudiés (l’âge, les taux de calcium et de vitamine D, les données ostéodensitométriques, l’importance du déplacement initial et les caractéristiques de la comminution postérieure). Par contre, la densité du défect était liée négativement à l’IMC et positivement aux résultats fonctionnels. Dans les fractures extra-articulaires du radius distal à déplacement postérieur, persiste après la consolidation radiologique, un défect métaphysaire distal lié à la compression de l’os spongieux lors du traumatisme. La présence de ce défect ne semble pas être à l’origine d’une fragilité accrue de la métaphyse. La comminution postérieure, très fréquente dans ce type de fracture, guérit complètement sans lésion résiduelle.
IntroductionLa prise en charge du syndrome du défilé cervico-thoraco-brachial (SDCTB) repose sur le traitement rééducatif en première intention et sur le traitement chirurgical. Le but de cette étude était d’analyser les facteurs prédictifs de réponse à ces deux thérapeutiques et d’en rapporter les résultats.MatérielNous avons réalisé une étude prospective, les patients inclus présentaient un SDCTB confirmé sur différents examens paracliniques.MéthodeL’évaluation reposait sur un auto-questionnaire, un score DASH et un bilan clinique, initialement puis après 6 mois de kinésithérapie et à 6 mois postopératoires. Une kinésithérapie selon le protocole de Prost était proposée. La chirurgie était réalisée en cas d’échec de la rééducation. Nos critères de jugements principaux étaient la variation du score DASH et la satisfaction.RésultatsNous avons inclus 74 SDCTB chez 59 patients. Dans 69 cas un traitement rééducatif a été fait. Le score DASH est passé de 51,57 (±19,46) à 47,62 (±22,14) (p=0,01). Nous avons constaté une amélioration jugée suffisante dans 18 cas (26 %). Les facteurs péjoratifs significatifs sur les résultats de la kinésithérapie étaient un score DASH initial élevé (p=0,003), un nombre élevé de manœuvres initialement positives (p=0,034), une EVA douleur initialement élevée (p=0,04), un arrêt de travail lors de la consultation initiale (p=0,007), une profession impliquant des mouvements répétés en abduction (p=0,004), un antécédent traumatique (p=0,004). Sur les 47 cas opérés, nous avons constaté 70,2 % de bons et très bons résultats, 19,15 % de résultats moyens. L’amélioration de tous les symptômes était significative, le score DASH moyen est passé de 56,78 (±20,69) à 34,7 (±24,08) (p<0,001). Le facteur prédictif d’une meilleure satisfaction était la présence d’une côte cervicale (p=0,007). Les facteurs associés à un meilleur score DASH postopératoire étaient l’âge jeune (p=0,0046), une EVA douleur initialement basse (p<0,0001), et une absence d’arrêt de travail lors de la consultation initiale (p=0,0002).DiscussionNous avons démontré que plus le patient était symptomatique initialement, moins la kinésithérapie était bénéfique. Le traitement chirurgical reste le traitement de choix des formes compliquées ou résistantes au traitement conservateur.ConclusionLa kinésithérapie a été efficace dans 26 % des cas et doit être proposée en première intention. Nous avons obtenu 70 % de bons résultats postopératoires chez les patients en échec de kinésithérapie.
