BACKGROUND There is a close relationship between cardiovascular risk factors and polycystic ovary syndrome (PCOS), and omega-3 fatty acids may have a key role in improving cardiovascular risk factors. We conducted the current systematic review and meta-analysis to evaluate the effect of omega-3 fatty acid supplementation on cardiovascular risk factors in patients with PCOS. METHODS We searched 4 databases including PubMed (MEDLINE), Cochrane Library, Embase, and Web of Science from inception to February 2021. We included randomized controlled trials (RCTs) that reported the effects of omega-3 fatty acid treatment for PCOS. According to the Cochrane system evaluation guide manual, 2 researchers independently assessed the methodological quality of the included studies. We pooled results using either a fixed effect model or random effect model. RESULTS We identified 314 articles, of which 10 met the criteria for inclusion, involving 778 participants. The pooled results suggested an association between the supplementation of omega-3 fatty acids and a reduction in serum insulin [-2.58 pmol/L, 95% confidence interval (CI): -3.34 to -1.82 pmol/L, P<0.00001, I2=0], homeostatic model assessment of insulin resistance (HOMA-IR) (-0.57, 95% CI: -0.75 to -0.40 L, P<0.00001, I2=2%), serum total cholesterol (TC) (-6.87 mg/dL, 95% CI: -10.28 to -3.47 mg/dL, P<0.0001, I2=95%), serum triglyceride (-4.03 mg/dL, 95% CI: -5.53 to -2.52 mg/dL, P<0.00001, I2=97%), serum low-density lipoprotein cholesterol (LDL-C) (-6.64 mg/dL, 95% CI: -11.58 to -1.70 mg/dL, P=0.008, I2=99%), serum very low-density lipoprotein cholesterol (VLDL-C) (-3.29 mg/L, 95% CI: -6.54 to -0.05 mg/L, P=0.05, I2=72%), serum high-sensitivity C-reactive protein (hs-CRP) (-8.97mg/dL, 95% CI: -17.66 to -0.28 mg/dL, P=0.04, I2=99%), an improvement in serum high-density lipoprotein cholesterol (HDL-C) (2.94 mg/dL, 95% CI: 0.63 to 5.26 mg/dL, P=0.01, I2=87%), but no effect on serum glucose (-0.76 mg/dL, 95% CI: -1.71 to 0.19 mg/dL, P=0.12, I2=73%) was found. DISCUSSION The current meta-analysis demonstrated that omega-3 fatty acid supplementation for women with PCOS resulted in a statistical improvement in insulin, HOMA-IR, TC, triglyceride, LDL-C, VLDL-C, and HDL-C, but did not affect serum glucose. The limitation of this paper is due to the lack of included research literature.
目的:建立迷你人源性肿瘤移植(miniPDX)动物模型,筛选敏感上皮性卵巢癌(EOC)患者化疗方案并评价效果,为实现EOC个体化药物治疗提供依据.方法:选取首次诊断为EOC并进行肿瘤细胞减灭术的患者22例作为药敏组,22例患者均同意进行miniPDX试验并签署相关同意书;另选取同期治疗并基本情况匹配的41例EOC患者作为对照组.药敏组采用患者肿瘤组织制成细胞悬液装入胶囊,并植入裸鼠皮下建立miniPDX模型.采用不同化疗药物治疗裸鼠后取出胶囊,CellTiter-Glo荧光细胞活力测定法测定胶囊中肿瘤细胞增殖率,选用细胞增殖率最低的方案对患者进行化疗.对照组采用紫杉醇+卡铂或紫杉醇+顺铂化疗方案.评价2组患者的疗效和不良反应发生率.将2组患者肿瘤组织病理切片进行CK7、WT-1、p16、p53、Ki-67和配对盒基因8抗原(PAX-8)免疫组织化学染色,分析其阳性结果与患者对含铂化疗敏感性之间的关系.结果:药敏组22例患者中,14例完全缓解(CR),4例部分缓解(PR),总缓解率(ORR)为81.8%(18/22);对照组41例患者中,13例CR,14例PR,ORR为65.9%(27/41),药敏组患者ORR高于对照组(P<0.05).2组患者的年龄、病理类型和分期等临床特征比较差异无统计学意义(P>0.05).药敏组和对照组患者不良反应发生率比较差异无统计学意义(P>0.05).免疫组织化学检测,p16、p53、Ki-67和PAX-8阳性铂敏感患者百分率高于p16、p53、Ki-67和PAX-8阴性铂敏感患者(P<0.05),CK7和WT-1阳性铂敏感患者百分率与CK7和WT-1阴性者铂敏感患者比较差异无统计学意义(P>0.05).结论:根据miniPDX动物模型筛选EOC化疗方案可提高EOC患者治疗效果;p16、p53、Ki-67和PAX-8阳性表达可预测EOC患者的铂敏感.
Objective To investigate the effects of long non-coding RNA DUXAP8 on the proliferation, cell cycle and apoptosis of ovarian cancer cells by regulating miR-4677-3p. Methods The expression of DUXAP8 and miR-4677-3p in ovarian cancer tissues and cell lines (SKOV3, OV90 and CaOV3) was detected by RT-qPCR. Then SKOV3 cells were divided into 6 groups, namely, si-NC group, si-DUXAP8 group, miR-NC group, miR-4677-3p group, si-DUXAP8+anti-miR-NC group and si-DUXAP8+anti-miR-4677-3p group. CCK-8 assay and clone formation assay were adopted to detect cell viability, flow cytometry to observe cell apoptosis and cycle, Western blotting to measure the expression levels of Cyclin D1, Bcl-2 and Bax, and dual luciferase reporter assay system to analyze the relationship of DUXAP8 and miR-4677-3p. Results When compared with adjacent tissues and normal ovarian epithelial cells, the expression level of DUXAP8 was significantly lower in ovarian cancer tissues (0.98±0.25 vs 4.14±0.72) and that of SKOV3, OV90, CaOV3 was obviously higher (1.03±0.08 vs 3.26±0.33、2.75±0.29、2.54±0.26, all P < 0.05). Interference with DUXAP8 or overexpression of miR-4677-3p inhibited the proliferation, reduced the rate of apoptosis, and arrested the cell cycle of ovarian cancer cells (P < 0.05). DUXAP8 regulated the expression of miR-4677-3p, and inhibiting miR-4677-3p partially restore and interfere with the effects of DUXAP8 on the proliferation, cycle and apoptosis of ovarian cancer cells (P < 0.05). Conclusion Interference with DUXAP8 can inhibit the proliferation of ovarian cancer cells, induce cell apoptosis, and block cell cycle, the mechanism may be related to the up-regulation of miR-4677-3p.
Background: Endometrial thickness (ET) is closely related to many gynecological symptoms. The measurement of ET is also an important tool for evaluating adverse symptoms such as bleeding in postmenopausal women. However, the significance of ET in asymptomatic women is still unclear. The purpose of this study was to determine the correlation between ET and the incidence of endometrial lesions in asymptomatic women after menopause, and to statistically analyze the correlation with a 5 mm cutoff value. Methods: A systematic literature search was conducted in May 2021 to screen out articles that reported that ET measurement was used to diagnose endometrial carcinoma (EC), endometrial hyperplasia (EH), and endometrial polyps (EP) in asymptomatic postmenopausal women who did not use hormone replacement therapy (HRT). The endometrial membrane was set at 5 mm as the cut-off, and using 5 mm as the cut-off of the ET, the relationship between the thickness of the endometrium and the prevalence of EC, EH, and EP was evaluated. Relative risk (RR) and standardized mean difference (SMD) were extrapolated with 95% confidence interval (CI). Results: After screening, 9 studies reported a total of 3,620 cases of asymptomatic postmenopausal women whose ET was measured. Among them, there were 1,758 cases of ET <5 mm, the probability of EC, EH, and EP were 0.284% (5/1,758), 0.398% (7/1,758), and 0.626% (11/1,758), respectively. In another 1,862 cases with ET ≥5 mm, the probabilities of EC, EH, and EP were 1.128% (21/1,862), 1.128% (21/1,862), and 1.557% (29/1,862), respectively. The results showed that ET can be used as a risk factor for predicting EC and other pathological changes. Discussion: The results of this meta-analysis show that when the ET is greater than 5 mm, the incidence of EC, EH, and EP increases significantly. It is reasonable to use ET as a screening test for EC and EH in asymptomatic postmenopausal women.
目的 比较卵巢癌体外药敏试验化疗方案与经验化疗方案的疗效差异.方法 采用ATP-TCA法对75例卵巢癌肿瘤新鲜组织标本进行体外培养及肿瘤药物敏感性试验,选用临床常用的8种化疗药物进行测试,将化疗效果与经验化疗方案进行对比,并随访患者比较两者的差异.结果 体外药敏试验结果提示75例卵巢癌标本对常见化疗药敏感性高;根据药敏结果选用中、高度敏感药物进行化疗,总有效率为77.33%,高于对照组.药敏组无进展生存期与中位生存期分别为20.72月及30.30月,均长于对照组,3年生存率也高于对照组,差异有统计学意义(P<0.05).结论 根据肿瘤药物敏感试验选用的化疗药物能有效治疗卵巢癌,对患者的个体化化疗具有一定的指导意义.
目的 探索长链非编码RNA(long non-coding RNA,lncRNA)H19对卵巢癌细胞运动及模型小鼠生存能力的影响.方法 RT-PCR检测H19和miR-29b-3p mRNA在不同癌组织、癌旁组织、正常卵巢上皮细胞及多种卵巢癌细胞中的表达差异.生物学信息预测两者的靶向关系并验证.检测体外培养细胞增殖、迁移、侵袭能力,移植瘤体内生长、裸鼠存活情况,及细胞凋亡和增殖相关蛋白表达.结果 H19和miR-29b-3p mRNA在不同癌组织、癌旁组织、正常卵巢上皮细胞及多种卵巢癌细胞中均存在差异表达.si-H19能显著逆转miR-29b-3p inhibitor对细胞增殖、Ki-67表达、划痕闭合率、侵袭细胞数及VEGF、MMP-2、MMP-9表达的影响.体内实验发现,si-H19能显著逆转miR-29b-3p inhibitor对移植瘤生长、荷瘤裸鼠存活率、细胞凋亡率、Ki-67和VEGF表达的影响.结论 H19通过靶向miR-29b-3p提高卵巢癌细胞运动能力,降低荷瘤裸鼠的生存能力.
Ovarian cancer (OC) is one of the most prevalent cancers with high fatality rate. In the present study, RT-PCR showed that the mRNA level of CDKN2B-AS1 was significantly upregulated while the miR-411-3p was downregulated in OC cell lines. In addition, the Sh-CDKN2B-AS1 resulted in the suppression of cell growth, invasion, migration and promotion of apoptosis, and miR-411-3p showed reversed results. Further studies demonstrated that CDKN2B-AS1 could directly interact with miR-411-3p, and that there was an inverse correlation between miR-411-3p and CDKN2B-AS1. Moreover, the in vivo experiments further demonstrated that Sh-CDKN2B-AS1 could inhibit the tumor growth. In addition, we examined the effect of CDKN2B-AS1 and miR-411-3p on HIF1a/VEGF/P38 axis. Consequently, Sh-CDKN2B-AS1 could suppress this pathway. In summary, our study demonstrated that the CDKN2B-AS1 interacted with miR-411-3p contributing to carcinogenesis in OC. Meanwhile, Sh-CDKN2B-AS1 showed anti-cancer role by promoting apoptosis and inhibiting cell growth, invasion and migration. Collectively, CDKN2B-AS1 modulated these activities possibly though miR-411-3p/HIF1a/VEGF/P38 pathway.
Objective To compare the clinical efficacy of high-dose density neoadjuvant chemotherapy (HDNC)and classic neoadjuvant chemotherapy (CNC)on locally advanced cervical carcinoma.Methods 76 patients with locally advanced cervical carcinoma were treated with operations and neoadjuvant chemotherapy between January of 2009 and December of 201 5 in Sichuan Provincial People′s Hospital and selected as research subjects in this study.The clinicopathological data were analyzed retrospectively.The patients were divided into the HDNC group,in which the patients received 2 cycles of intravenous drip of 60 mg/m2 cisplatin and 60 mg/m2 paclitaxel with 10-day intervals,and the CNC group,in which the patients received 2 cycles of intravenous drip of 70 mg/m2 cisplatin and 135 mg/m2 paclitaxel with 21-day intervals.The data were compared to analyze the differences of clinical efficacy between the two groups. Results After all the patients received neoadjuvant chemotherapy for 2 weeks,the clinical remission rate and the surgical rate of the HDNC group were 78.6% and 89.3% respectively,while those of the CNC group were 75% and 93.7% respectively,which showed no statistical significance between the two groups.The diameter of the tumors was reduced after neoadjuvant chemotherapy.The postoperative pathological results showed that there were no significant differences between the two groups in the positive rate of lymph node and the rate of the parametrial infiltration,the lymphovascular space invasion and the deep stromal invasion.The postoperative radiotherapy rate and the 3-year cumulative survival rate of the HDNC group were 60% and 76% respectively,while those of the CNC group were 55.6% and 82%,which showed no significant differences between the two groups.Conclusion The neoadjuvant chemotherapy with high-dose density of cisplatin and paclitaxel for the patients with locally advanced cervical carcinoma has the same clinical efficacy as the classic neoadjuvant chemotherapy,but it can shorten the chemotherapy time before operation.Therefore,it is an effective neoadjuvant chemotherapy for locally advanced cervical carcinoma.
目的 分析97例胎儿染色体非整倍体异常的临床资料,探讨产前诊断技术的临床应用.方法 产前诊断包括羊水穿刺及脐血穿刺的4076例孕妇,检出胎儿染色体非整倍体异常9 7例.对这9 7例孕妇的年龄、产前诊断孕周、产前诊断指征和超声检查异常表现等进行分析.产前诊断指征包括:1.唐氏筛查高风险(≤1/270);2.唐氏筛查临界风险(1/270-1/1000);3.唐氏筛查单项筛查指标mom值异常;4.高龄;5.孕早孕超声检查提示胎儿颈项透明层厚度NT增厚≥2.5mm;6.无创产前检测NIPT高风险;7.不良孕产史;8.超声提示胎儿结构异常或软指标异常.结果 孕妇平均年龄为31.1±6.59岁,21-三体65例占67%,18-三体10例占10.31%,9-三体2例占2.06%,20-三体且为嵌合体1例,占1.03%,47,NN,+mar2例,占2.06%,性染色体非整倍体1 7例,占17.53%;其中22 (22.68%)例孕妇孕期超声发现异常,4例B超提示软指标异常者中孕期唐筛均为低风险.结论 妊娠期多种产前筛查包括早期NT检查、血清学筛查以及NIPT等筛查手段能有效的帮助胎儿常染色体三体的诊断,而孕中晚期的超声检查在产前筛查中具有重要意义.
目的 探讨彩超检测胎儿颈项透明层(nuchal translucency,NT)厚度与胎儿染色体异常、结构异常及妊娠结局的关系并分析其在产前诊断中的价值.方法 应用彩色多普勒超声诊断仪对孕11~13+6周的3 248例孕妇行经腹超声检查,测量胎儿NT值,记录孕妇不良妊娠结局,并随访至胎儿出生后6个月.结果不同年龄孕妇的胎儿NT值(F=0.089,P=0.976)及NT增厚率(x2=0.432,P=0.934)比较,差异均无统计学意义;不同NT值组染色体异常检出率(x2 =559.89,P<0.001)及不良妊娠结局发生率(x2=494.27,P<0.001)比较,差异有统计学意义.NT增厚组染色体异常检出率(x2=323.28,P<0.001)及不良妊娠结局发生率(x2=270.66,P<0.001)明显高于胎儿NT正常组.NT≥3.5 mm组染色体异常检出率(x2=6.46,P=0.011)及不良妊娠结局发生率(x2=9.99,P=0.002)明显高于2.5~3.5 mm组.结论 NT增厚在产前诊断中具有重要筛查意义,随着NT增厚,胎儿的染色体异常率及不良妊娠结局发生率均增高.
