In 20 adult cleft patients, the influence of a Le-Fort-I-osteotomy and ventral-caudal advancement of the maxilla on nasal breathing was studied. An increase of nasal air flow in the majority of patients is due to an increase in the volume of nasal skeleton and/or in the nasolabial angle and alar nasal base. Rhinomanometric measurements and X-ray examination confirm the results.
17 unilateral and 6 bilateral choanal atresias in children were observed in the ENT-Dept. of the University Hospital Erlangen during 1975-1982. Bilateral choanal atresia requires immediate trepanation by using the CO2 or Argon laser technique. The surgical procedure in unilateral choanal atresia is postponed until the age of 4 years. Transnasal microsurgical approach is preferred in these cases because it involves minimal damage to the developing structures. The satisfying results are reported.
In the great majority of patients with hypoplasia of the middle third of the face, the surgical advancement or displacement of the maxilla results in a subjective improvement in nasal breathing, which can also be verified. This is due to an increase in the volume of the nasal skeleton and/or in the nasolabial angle, which improves nasal ventilation. These operations have virtually no effect on endonasal obstructions to flow, which, in order further to improve nasal breathing, should be dealt with separately.

Die Hämangiome im Säuglings- und Kleinkindalter stellen insofern ein interdiszi-plinäres Problem dar, da mehrere Fachdisziplinen die Kinder sehen und behandeln. Neben dem Pädiater, der die Kinder zuerst sieht, werden von den ratsuchenden Eltern der Dermatologe, der Röntgenologe, der Kieferchirurg, der plastische Chirurg und der Hals-Nasen-Ohrenarzt aufgesucht, die dann nicht selten sehr widersprüchliche Ratschläge erteilen. Aus diesem Grunde erscheint es sinnvoll, im Rahmen dieser Tagung das Problem der Säuglingshämangiome kurz zu besprechen.
Dieser Film dient der Gesundheitsaufklfirung und versucht aus der Sicht des Rhinologen das Symptom ,,Schnupfen", an dem jfihrlich Millionen Menschen leiden, in altgemein verstfindlicher Form unter Aufzeigen der pathophysiologischen, diagnostischen, therapeutischen und prophylaktischen Aspekte darzustellen. Die Auswirkung eines akuten oder chronischen Schnupfens auf das Allgemeinbefinden des Menschen und die m6gliche Mitbeteiligung der benachbarten Organbereiche kennzeichnen den Schnupfen als einen ernstzunehmenden Gegner. Besondere Bedeutung wird dabei der freien Nasenatmung beigemessen, sei es im akuten Stadium durch Applikation abschwellender Nasentropfen oder bei der chronisch rezidivierenden Form mit zu enger Nasenstruktur durch operative Magnahmen.
Der operative Verschluß von Septumperforationen (S.P.) ist auch heute noch nicht problemlos. Grundsätzlich sollten nur Patienten mit Beschwerden wie Bluten, Krusten, Pfeifen und Kopfschmerzen behandelt werden. Von 61 operierten S. P. waren mehr als ⅔ iatrogener Art. Der vollständige Verschluß der Perforation ist für die Beseitigung der Beschwerden nicht absolut erforderlich. Wichtiger erscheint die Protektion des Hinterrandes der S.P. Bei kleinen S.P. (bis zu 5 mm ∅) läßt sich eine Faszieninterposition kombiniert mit einem Verschiebelappen gut durchführen. Bei mittleren S.P. (bis zu 15 mm ∅) wird eine zweizeitige Deckung mit einem Vestibulum-Conchalappen empfohlen, der in 80% der operierten Fälle befriedigende Resultate brachte. Bei großen S. P. (> 15 mm ∅) ist die Versorgung mit einem Obturator vorzuziehen.
Die Merthiolat-konservierten, infektionsfreien Knorpellangzeitimplantate zeigen — unabhängig von der Verweildauer — ein unterschiedliches Ausmaß an Resorption. Die Resorption durch das wirtseigene, gefäßhaltige Granulationsgewebe findet teils circumscript, teils auf breiter Front statt. Die Folge ist eine permanente Aufzehrung des Knorpels und dessen Ersatz durch ein Narbengewebe. Die bakterielle Infektion führt zur eitrigen Einschmelzung oder zur beschleunigten Resorption. Neben dem Knorpelersatz durch Narbengewebe kann auch eine Osteogenese beobachtet werden.
Bei 49 Kaninchen, die in 5 Untersuchungsgruppen eingeteilt worden waren, wurde entsprechend dem operativen Vorgehen nach Cottle streng einseitig das Mucoperichondrium der Nasenscheidewand gelöst und das knorpelige Septum zusätzlich nach vorgegebenen Versuchsbedingungen vertikal bzw. horizontal durchschnitten oder geritzt.
Bei dem Septumabsceß handelt es sich zwar um ein seltenes, aber subjektiv und objektiv schweres Krankheitsbild. Es wird insbesondere deshalb gefürchtet, weil es bei ausbleibender oder unsachgemäßer Behandlung neben seltenen Komplikationen mit übergreifender Infektion auf extranasale Gebiete (Thrombose, Meningitis) häufig zu schweren Deformitäten im knorpeligen Nasenbereich (wie Sattelbildung, Hidden columella, Veränderungen der Naseneingänge, Ballooning-Phänomen) kommen kann (Abb. 1).
The histological findings following transplantation of crushed septal cartilage into the nasal septums of seven rabbits during septoplasties are reported. Living and devitalized chondrocytes were found together with loss of cartilage substance, its replacement by granulation tissue, and new cartilage and to a lesser extent new bone formation.
In 49 rabbits the mucoperichondrium of their nasal septa was strictly unilaterally separated according to the Cottle procedure. In addition, in some rabbits only the surface of the septal cartilage was incised and in others the cartilage was cut vertically or horizontally. Four months after the operation the following alterations of the septa were seen: The separation of the mucoperichondrium from the cartilage or superficial incision of the cartilage led to a deviation to the operated left side. If in addition parts of the cartilage septum were mobilized, thus swinging horizontally or vertically, a deviation to the other nonoperated side was seen. Similarly, if two parallel vertical stripes of cartilage were excised, the ends of the remaining cartilage plate bent and the cartilage plate itself deviated to the nonoperated side. The postoperative deviations are due to released unbalanced forces of the cartilage itself and to the disequilibrium of scar tension resulting from the unilateral separation of the mucoperichondrium. To avoid these postoperative deviations the bilateral tunneling of the mucoperichondrium is required.
There are still some problems of the surgical treatment of septal perforations (s.p.). Only patients with s.p. having complaints i.e. bleeding, crusting, whistling and headache should be treated. More than 60% of 61 operated patients had an iatrogenic perforation. The complete closure of the s.p. is not abosulutely necessary. More important seems to be the protection of the posterior circumference of the s.p. In small perforations (up to 5 mm) an interposition of fascia combined with a rotating mucosa flap is a safe technique. In middle size s. p. (up to 15 mm) a two stage technique is recommended by using a vestibular-conchal flap which gives satisfactory results in 80% of the operated patients. In great s. p. (more than 15 mm) a septal button is preferred.
Poor correction of the deformed septal cartilage in area No. 1 and 2. In the classical submucous resection the important framework of the septum remains uncorrected. Only a septoplasty will give satisfactory results and will restore free nasal breathing.
Summary A concept of surgical treatment of septum perforation (s.p.) is presented. This concept based on 61 operated cases and follow up examination after 6–18 months. A classification of s.p. divided in small, medium and large size is proposed requiring different surgical techniques.
Nasal obstruction in neonates can cause significant airway compromise and even be life-threatening. In general, otolaryngologists are familiar with management of the neonate born with choanal atresia but there have been few reports and there are no guidelines for management of the neonate or young infant who presents with nasal obstruction and airway compromise but who does not have choanal atresia (NOWCA). This study demonstrates the difficulty in diagnosis, variability in severity of airway compromise, and delineates the problematic aspects of management.
The submucous resection of septum by Killian is not appropriate as a primary correcting treatment, especially in case of traumatical septum deformities. Due to the loss of cartilage often form and function of the nose deteriorates in a typical way. These complaints demand an operative correction under complicated circumstances. Based on the patient clientèle of the Univ.-HNO-Klinik Erlangen it is demonstrated how by secondary operations with the special technique of displacement of the remaining cartilage a satisfying result finally can be achieved.
