Introduction Les infections de l'aorte et ses branches sont des maladies rares à l'origine d'une forte morbi-mortalité. Elles peuvent survenir sur tissu natif ou prothétique. L'objectif de ce projet est de décrire les caractéristiques des patients de ce registre puis d'identifier les facteurs pronostiques de la mortalité à un an. Matériels et méthodes Repia est un registre monocentrique, observationnel et rétrospectif conduit de 2014 à 2022. Les patients inclus avaient plus de 18 ans et souffraient d'une infection de l'aorte et/ou de ses branches, définie par la présence d'au moins deux des trois critères suivants :- 1/ Clinique : signe(s) infectieux et/ou constatation per opératoire d'une infection au contact de la paroi de l'aorte ou d'une de ses collatérales- 2/ Indirecte : syndrome inflammatoire et/ou imagerie (scanner et/ou 18-PET-FDG et/ou scintigraphie aux leucocytes marqués) en faveur du diagnostic.- 3/ Microbiologique : culture positive (hémocultures et/ou prélèvements peropératoires) et/ou par sérologie (Coxiella burnetii) et/ou via la biologie moléculaire de prélèvements chirurgicaux (PCR ARN 16S).L'ensemble de la cohorte a été décrite avant de comparer deux sous-groupes : infection vasculaire native (IVN) et infection de prothèse vasculaire (IPV). La survie à un an a été décrite par une courbe de Kaplan-Meier, puis comparée entre les deux sous-groupes (test de Log Rank). Enfin, des modèles de Cox bi- et multivariés ont été réalisés afin d'identifier les facteurs pronostiques de mortalité à un an. Résultats Quatre-vingt-sept patients ont été inclus : 47 (54,0%) IVN et 40 (46.0%) IPV. Il s'agissait d'hommes dans 74,7% des cas (65/87 patients) avec un âge médian de 69,9 ans (IQR 62,8-75,8ans). L'aorte sous rénale était la zone la plus souvent infectée (49,4% de l'ensemble des patients (43/87)). Staphylococcus aureus était le germe le plus souvent isolé (33,3% de l'ensemble de la population (29/87)). La mortalité intra-hospitalière étaient de 23,0% (20 patients) et la mortalité à un an de 37,9% (33 patients).Les patients du groupe IVN présentaient plus d'antécédents d'immunodépression (19,1% des patients [9/47] vs 5,0% des patients [2/40]) ou de maladies auto-immunes (17,0% des patients (8/47) vs 0,0% des patients (0/40)). A l'inverse, les pathologies cardiovasculaires et insuffisances chroniques d'organes étaient plus représentées dans le groupe IPV. Cependant, l'index de comorbidité de Charlson n'apparaissait pas comme différent entre les groupes (médiane à 2 [IQR 1-4] dans le groupe IVN vs 2,5 [IQR 1-4] dans le groupe IPV). L'atteinte thoracique était plus fréquente dans le groupe IVN (31 patients vs 9). La mortalité à un an n'était pas significativement différente entre les deux groupes (42,6% dans le groupe IVN soit 20/47 patients vs 32,5% dans le groupe IPV soit 13/40 patients ; Test de Log Rank, p=0,4803).L'absence de chirurgie était le seul facteur associé à la mortalité à un an en analyse multivariée (p= 0,0125, HR 2,8, IC 95% [1,2-6,3]). Conclusion Les données du registre Repia confirme la forte mortalité de cette pathologie. Malgré la prise en charge pluridisciplinaire, par une équipe expérimentée, la morbi-mortalité de cette infection reste élevée avec un rôle central de la chirurgie.Aucun lien d'intérêt
Clostridium haemolyticum is a sporulating Gram-positive anaerobic rod that is considered to be one of the most fastidious and oxygen-sensitive anaerobes. It is a well-known animal pathogen and the cause of bacillary hemoglobinuria primarily in cattle. To date, human infections caused by C. haemolyticum have been reported in three patients with malignant underlying diseases. We present herein the case of a 30-year-old obese woman with no significant past medical history who developed bacteremia caused by C. haemolyticum with massive intravascular hemolysis associated with bone marrow necrosis and acute renal failure. Because of subculture failure, the diagnosis was made on the basis of 16S rDNA sequencing and next-generation sequencing. The patient, who had been afebrile for 20 days after a 17-day-course of antibiotics, experienced a second bacteremic episode caused by C. haemolyticum. After having been successfully treated for 42 days with clindamycin and amoxicillin-clavulanic acid, the patient developed acute myeloid leukemia as a result of bone marrow regeneration. Although uncommon in humans, infections caused by C. haemolyticum are severe and should be considered in a febrile patient who has severe hemolytic anemia. This case also highlights the importance of using molecular techniques for the identification of this fastidious anaerobic organism.
L'endocardite infectieuse (EI) est une infection rare (incidence 30 à 35 cas/millions d'habitants/an en France). Staphylococcus aureus (SA) est le premier agent infectieux responsable d'EI. Par rapport aux EI à streptocoques et entérocoques, il est associé à une mortalité plus importante (20 à 30 % de mortalité intra-hospitalière) et à des complications plus fréquentes (12 à 35 % d'emboles symptomatiques, 25 à 50 % de chocs septiques, 15 à 45 % d'indications de chirurgie valvulaire). Certains facteurs pronostiques des EISA sont connus (âge, gravité de l'infection, comorbidités, …). Malgré des progrès dans l'identification et la prise en charge de ces facteurs pronostiques, mais aussi dans le diagnostic et la prise en charge des EISA, la morbi-mortalité de cette infection reste élevée. Il est donc crucial d'identifier de nouveaux facteurs pronostiques afin de proposer des prises en charge innovantes permettant d'améliorer la survie de ces patients. Notre objectif était d'identifier de nouveaux facteurs pronostiques de l'EISA. Une approche mixte combinant des méthodes qualitatives à une approche quantitative plus classique a été privilégiée. Nous avons utilisé les données de la cohorte multicentrique française EI2008 conduite dans sept régions françaises. Les patients inclus étaient adultes et présentaient une EI possible ou certaine selon la classification de Duke et Li. Ils ont été suivis durant un an. Les caractéristiques sociodémographiques, cliniques et paracliniques ont été recueillies sur cahier d'observation (CRF) à partir des dossiers médicaux. Nous avons sélectionné les EISA certaines. Pour l'approche qualitative, nous nous sommes inspirés des revues de morbi-mortalités pour identifier de nouveau facteurs pronostiques potentiels. L'approche consistait en une lecture complète approfondie et sans a priori des dossiers médicaux de 30 patients tirés au sort (15 survivants et 15 décédés) afin d'identifier les facteurs (non recueillis dans le CRF et non décrits dans la littérature) ayant pu avoir un rôle favorable chez les survivants et défavorable chez les décédés. Pour l'approche quantitative, il s'agissait de recueillir les données relatives aux facteurs identifiés par l'approche qualitative sur l'ensemble des patients avec EISA certaine. Puis nous avons estimé la valeur pronostique de chaque facteur par analyse statistique utilisant des modèles de Cox. Le seuil de significativité était de 0,05. Parmi les 134 EISA certaines inclus, 64 sont décédés durant la première année. Par l'approche qualitative, nous avons identifié 36 facteurs supposés défavorables et 22 facteurs supposés favorables réparties en cinq catégories : terrain du patient, gravité du patient à la prise en charge, éléments liés à la prise en charge, éléments organisationnels et éléments liés à l'évolution durant l'hospitalisation. En analyse multivariée, un facteur supposé favorable a une influence significative sur la survie (« diminution de taille végétation/abcès au cours de la prise en charge » HR 0,34 [0,12-0,96] p = 0,0424) contre deux facteurs supposés défavorables (« Traitement par AINS » HR 2,91 IC95[1,35-6,26] p = 0,0063 et « Indication chirurgicale non respectée » HR 1,90 [1,05-3,44] p = 0,0328). Cette méthode, combinant une approche qualitative inspirée des revues de morbi-mortalité à une approche quantitative classique, pourrait permettre d'identifier de nouveaux facteurs pronostiques.
Whether cefazolin is as effective and safer than antistaphylococcal penicillins (ASPs) for the treatment of methicillin-susceptible Staphylococcus aureus (MSSA) infective endocarditis (IE) is still debated in the absence of a randomized controlled trial. In this quasi-experimental study, we aimed to assess the effectiveness and safety of these two treatments in MSSA-IE, using the ASPs nationwide shortage in April 2016 as a unique opportunity to overcome the indication bias associated with observational studies. In this single-centre study, we compared patients with Duke-Li definite MSSA-IE treated with ASPs from January 2015 to March 2016 versus those treated with cefazolin from April 2016 to December 2018, when ASPs were not available. Effectiveness outcome was 90-day all-cause mortality. Safety outcomes included significant decrease in GFR and significant increase in serum liver enzymes. Logrank test was used to compare survival rates. Of 73 patients with MSSA-IE, 35 and 38 were treated with ASPs and cefazolin, respectively. Baseline patients’ characteristics (demography, native or prosthetic valve IE, clinical characteristics, cardiac and septic complications) were similar between groups. Ninety-day all-cause mortality was 28.6% and 21.1%, in patients treated with ASPs and cefazolin, respectively (logrank p = 0.5727). There was no difference between groups for incident renal or liver toxicity events: acute kidney injury 45.7% vs. 44.7% (p = 0.933), increased ALT 5.7% vs. 13.2% (p = 0.432), bilirubin increase 5.7% vs. 10.5% (p = 0.676), in ASPs vs. cefazolin groups, respectively. In this quasi-experimental, effectiveness and safety did not statistically differ between ASPs and cefazolin for MSSA-IE treatment.
Background and importance The emergence of bacterial resistance and the proper use of antibiotics are major public health issues. In 2011, the European Conference on Infections in Leukaemia (ECIL) published recommendations for the management of febrile neutropenia. In this context, a university hospital wanted to evaluate follow-up of these recommendations. Aim and objectives To evaluate conformity for duration of antibiotic therapy in patients with febrile neutropenia, hospitalised in the haematology department. Material and methods The study was monocentric, retrospective, observational and conducted over a 6 month period in the haematology department. Data collection was carried out via a collection form. Two algorithms, created with the ECIL guidelines, were used to evaluate febrile neutropenia episodes. Duration of the prescription was considered to conform if it satisfied the following criteria: for undocumented infection, discontinuation of probabilistic antibiotic therapy at 72 hours of apyrexia; for documented infection, continuation of documented antibiotic therapy, according to the recommendations of the local antibiotic guidelines. Results Ninety infectious episodes were studied. The study population comprised 49 men (54%) and 41 women (46%). Average age was 56 years. Cefepime or piperacillin/tazobactam were systematically introduced as probabilistic therapy. If the infection was undocumented (n=61/90), the duration of probabilistic antibiotic therapy conformed in 41% of cases (n=25/61). For clinical documentation (n=6/90), the conformity rate was 67% (n=4/6). For microbiological documentation (n=23/90), compliance rate was 74% (n=17/23). Conclusion and relevance For most undocumented infections, probabilistic antibiotic therapy was prescribed for too long. This may be explained by the fragility of haematology patients and the fear of being confronted with recurrence of infection. For documented infections, conformity was very satisfying, as haematologists have extensive knowledge of infectiology. In order to harmonise prescription duration and continue to prevent the emergence of bacterial resistance, a guide for correct use of antibiotics and a second prospective study should be considered. Background and importance There is an urgent need to establish the proper systemic use of antifungals because of drug resistance and a limited therapeutic arsenal. In June 2018, we created two operational multidisciplinary teams, each comprising a pharmacy resident and an infectious diseases specialist. With prescription assistant software and a data gathering document, residents analysed and validated prescriptions daily. They reappraised each case with the infectious diseases specialist once a week. Aim and objectives To produce a summarised report of the analyses on antifungal prescriptions. This report allowed us to measure the performance of the newly created operational multidisciplinary teams. Material and methods All antifungal prescriptions given to adults, oral and intravenous, were analysed in a prospective way from 18 June 2018 to 1 March 2019. The data gathered were patient identity, antifungal prescriptions (molecule, start date, posology and administration route), antifungal indication, patient biological check-up, and clinical and biological proofs. For each prescription, we evaluated the relevance of the indication and the overall compliance with the prescription. Results A total of 653 prescriptions were analysed for this study, relating to 383 patients. On average, residents analysed 64 prescriptions a month and 59 were appraised by the operational multidisciplinary teams. Haematology was the most prescribing unit (49.8%). Caspofungin (35%), using the intravenous route, or posaconazole (35%), using the oral route, were the most prescribed antifungals. Indications were probabilistic 35% of the time, prophylactic 34% of the time and documented 30% of the time. Documented infections were mainly invasive candidiasis (57%) and pulmonary aspergillosis (32%). Among the 653 prescriptions, 96 were the subject of a pharmaceutical opinion, mainly for improper dosage (50%) or missing a loading dose (29.2%); 84% of prescriptions were re-evaluated by the infectious diseases specialist. Opinions were mainly about switching molecules (32%) and stopping therapy (28%). A total of 75.8% of prescriptions were successfully updated. Comparing our results with those obtained in 2015 in our hospital, the global conformity of the prescription (indication, molecule choice, posology, treatment length, lack of therapeutic alternatives) was up from 81.5% to 87%. Conclusion and relevance Implementation of operational multidisciplinary teams helped reduce the number of issues and thus contributed to improvement in the quality of prescriptions. Despite group A streptococci being an infrequent cause of pharyngitis in adult outpatients, sore throat remains a common indication for antibiotic prescription. This prospective multicentre non-randomised study describes a community pharmacy-based antimicrobial stewardship intervention consisting in the implementation of rapid antigen testing (RAT) for the management of adults with sore throat. Trained pharmacists triaged patients presenting with symptoms of pharyngitis using the modified Centor score. Those at risk for streptococcal infection were tested with RAT. Patients with a positive RAT were invited to consult a physician, whereas others were offered a symptomatic treatment. All patients received educational leaflets and were asked to fill in a follow-up form 7 days later. Ninety-eight pharmacies in one French region participated, and 559 patients were included over 6 months. RAT was proposed in 367 (65.7%) cases, and it was positive in 28 (8.3%). The follow-up form was returned by 140 (38.5%) participants. Of these, 10/10 patients with positive RAT further consulted a physician and were prescribed an antibiotic treatment, whereas 96.5% (110/114) of patients with negative results and not having any other reason to seek for doctor’s advice did not consult. All participants found the intervention useful. Pharmacists spent 6–15 min to perform the intervention, and 98.6% (73/74) of pharmacists giving a feedback declared to be ready to implement this intervention in daily practice, if endorsed and reimbursed. Our results suggest that a pharmacy-based programme for the management of sore throat is feasible and could increase adherence to guidelines.
Ceftolozane tazobactam (CT) est l’association antibiotique d’une nouvelle céphalosporine et d’un inhibiteur de béta lactamase qui a une activité sur Pseudomonas aeruginosa (PSA) y compris les souches présentant des résistances microbiologiques. Nous nous sommes intéressés à son efficacité sur les cas d’infections à PSA extrêmement résistants. Étude rétrospective nationale des infections à PSA XDR traitées par CT identifiées avant commercialisation. Donnés anamnestiques, cliniques, microbiologiques, et évolution recueillies par le clinicien en charge du patient à l’aide d’un questionnaire standardisé. Le caractère XDR était classiquement défini par la résistance à au moins un des agents dans toutes les classes d’antibiotiques anti-PSA sauf 2 ou moins. L’évolution était évaluée en in de traitement et à J28 ou à la sortie de l’hôpital. Au total 15 patients ont été traités par CT (14 adultes et 1 enfant de 3 ans) dans 7 hôpitaux du 3 mars au 11 novembre 2016 ; 10 d’entre eux étaient immunodéprimés. Le CT a été prescrit en réanimation pour 8 patients, en service de médecine pour 6 et en chirurgie pour 1. Concernant les patients de réanimation le score SOFA moyen était de 7,6 ± 4,0. Au total, 6 patients étaient sous ventilation mécanique et 5 recevaient des amines vasopressives Le point de départ de l’infection était une pneumonie (n = 7) dont 6 acquises sous ventilation mécanique, une infection urinaire (n = 3), une infection digestives (n = 2), 1 méningite, 1 infection de prothèse vasculaire et 1 infection ostéo articulaire. L’infection était polymicrobienne dans 8 cas associant : une entérobactéries (n = 3), Stenotrophomonas maltophilia (n = 1), enterococcus faecalis (n = 1), citrobacter koseri (n = 1), une autre souche de PSA (n = 1), et staphylococcus epidermidis (n = 1). Le CT était prescrit en moyenne après 2,5 lignes d’antibiothérpie préalables (0–6) et la durée moyenne de traitement était de 19,3 jours avec une posologie moyenne de 5,0 g/j. Le C/T était prescrit en monotherapie chez 5 patients et 10 recevaient une autre antibiothérapie (colistine : 6, aminoglycosides : 2, fluoroquinolones : 1). À la fin du traitement par CT une guérison était observée chez 10/15 patients (66 %), la mortalité tout cause confondue était de 4/15 patients dont un en échec clinique. La guérison microbiologique était obtenue chez 6/8 patients ; 7 patients n’ont pas eu de nouveaux prélèvements microbiologiques et 2 ont été classes en échec microbiologique. À J28 et/ou à la sortie de l’hôpital, 8/9 patients étaient considérés comme guéris et 1 avait présenté une récidive septique. Le CT semble une option efficace et peu toxique lors des infections sévères à PSA XDR.
Ceftolozane/tazobactam (C/T) is a novel β-lactam/β-lactamase inhibitor combination targeting Enterobacteriaceae and Pseudomonas aeruginosa (PA). It is approved in adult patients for complicated urinary tract infections (cUTIs) and complicated intra-abdominal infections (cIAIs) as well as for nosocomial pneumonia. It displays excellent activity against PA, even multidrug-resistant (MDR) and extensively drug-resistant (XDR) strains. The aim of this systematic review (PROSPERO protocol no. CRD42019117350) was to summarise the available evidence from observational studies regarding the efficacy and safety of off-label use of C/T when administered to treat MDR- or XDR-PA infections. The MEDLINE and Embase databases were screened from inception up to 30 June 2019. Studies were deemed eligible if they described real-life use of C/T in the case of MDR- or XDR-PA infections for non-approved indications. Exclusion criteria were cIAIs, cUTIs, pneumonia (unless occurring in a paediatric population) and infections by non-MDR/XDR-PA. Thirty articles fulfilled the inclusion criteria. In total, 130 cases of MDR- or XDR-PA infections treated with C/T in 128 patients were described. The most relevant off-label uses were skin and soft-tissue infection (49/30; 37.7%), bone and joint infection (42/130; 32.3%) and bloodstream infection (23/130; 17.7%). Five cases involved paediatric patients. The overall clinical success rate was 76.2%. The most common adverse event was hypokalaemia (4.2%, in 48 evaluable cases). C/T may be a useful therapeutic option for difficult-to-treat infections by PA even outside the framework of approved indications. Further studies are necessary to better define new indications for the drug.
Selective reporting of antibiotic susceptibility test (AST) results is a potential intervention for laboratory-based antibiotic stewardship. The aim of this study was to assess the impact of AST reporting on the appropriateness of antibiotics selected by French general practitioners for urinary tract infections (UTIs). A randomised controlled case-vignette study in a region of northeast France surveyed general practitioners between July and October 2015 on treatment of four clinical cases of community-acquired Escherichia coli UTIs (two cases of complicated cystitis, one of acute pyelonephritis and one male UTI). In Group A, selective reporting of AST results was used for the first two cases and complete reporting for the other two cases; these were reversed in Group B. The overall participation rate was 131/198 (66.2%). Provision of selective AST results significantly increased the rate of adherence to national guidelines for first-line antibiotic treatment in Cases 1, 3 and 4 by 22.4% (55.2% vs. 32.8%, P = 0.01), 67.5% (75.0% vs. 7.5%, P < 0.001) and 36.3% (45.3% vs. 9.0%, P < 0.001), respectively. The improvement in compliance was not significant for Case 2. Prescriptions of amoxicillin-clavulanic acid, fluoroquinolones and cephalosporins decreased by 25.0% to 45.0%, depending on the clinical vignette. Most (106/131, 81.0%) participants favoured the routine use of selective reporting of AST results. In conclusion, selective reporting of AST results seems to improve antibiotic prescribing practices in primary care, and may be considered a key element of antimicrobial stewardship programmes. Introduction : L’antibioresistance est un enjeu mondial de sante publique, en particulier la lutte contre l’emergence d’enterobacteries productrices de betalactamases a spectre etendu et de carbapenemases. L’antibiogramme cible, qui consiste a ne rendre que quelques molecules aux cliniciens, est une piste d’amelioration de juste prescription des antibiotiques en medecine generale. L’objectif de l’etude est d’evaluer l’impact des antibiogrammes cibles sur la conformite des antibiotherapies d’infections urinaires communautaires avec les recommandations de la Societe de Pathologie Infectieuse de Langue Francaise (2014) par des medecins generalistes a l’echelle d’une region francaise. Materiels et methodes : Etude prospective, comparative, randomisee et interventionnelle a l’aide de questionnaires comprenant 4 cas-cliniques (CC) fictifs (2 cystites a risque de complication a Escherichia. coli multisensible CC1 et secreteur d’une penicillinase de haut niveau CC2, 1 pyelonephrite aigue a E. coli secreteur d’une penicillinase de bas niveau CC3, 1 infection urinaire masculine a E. coli resistant aux fluoroquinolones CC4), menee aupres de medecins generalistes entre juillet et octobre 2015. Dans un groupe A, un antibiogramme cible (intervention) est rendu pour les deux premiers CC et un antibiogramme complet (controle) pour les deux autres CC et inversement dans l’autre groupe B. Resultats : Le taux de participation global etait de 70% (131/187). Les medecins connaissaient l’antibiotherapie de premiere intention recommandee lors d’une cystite aigue simple et d’une pyelonephrite aigue simple aux taux respectifs de 87,7% et 45%. L’antibiogramme cible ameliorait significativement le taux de conformite des antibiotherapies dans l’ensemble des CC : CC1 + 22,4% (55,2% vs 32,8% - p = 0,01), CC2 + 21,8% (35,9% vs 14,1% - p = 0,004), CC3 + 67,5% (75% vs 7,5% - p < 0,001), CC4 + 36,3% (45,3% vs 9% - p < 0,001). Les antibiogrammes cibles permettaient de favoriser la prescription d’antibiotiques a spectre plus etroit. Les participants se declaraient a l’aise avec l’utilisation des antibiogrammes actuels pour 80,1% voire tres a l’aise pour 13,7%, pourtant 80,8% souhaitent que le mode de rendu cible remplace les antibiogrammes complets. Conclusion : La lutte contre la selection de bacteries multiresistantes repose sur un ensemble de mesures de bon usage des antibiotiques. L’utilisation d’antibiogrammes cibles semble etre une methode efficace et bien acceptee par les prescripteurs de ville.
Background. - The increased use of new costly antifungal agents has led to a considerable increase in pharmaceutical expenditure. In December 2011, the Lorraine Regional Health Agency commissioned the Antibiolor network to evaluate costly antifungal agent stewardship using as reference regional, French, and international recommendations.Methods. - We performed a regional retrospective multicenter study. The criteria for evaluation were the appropriateness of the indication for treatment, the choice of the agent or of a combination, compliance with dose and treatment duration, and the absence of any alternative.Results. - One hundred and fourteen prescriptions were analyzed, in 7 intensive care units, 4 hematology units, and 1 infectious diseases unit. The indication for costly antifungal treatment was appropriate in 110 cases (96.5%), the choice of the antifungal agent in 102 cases (93%), the dose in 98 cases (89%), treatment duration in 102 cases (93%), and an alternative antifungal treatment was possible in 10 cases (9%). Eighty-two prescriptions (74.5%) complied with the marketing authorization, 19(17%) were related to a protocol for temporary use, and 9(8%) were considered as inappropriate.Conclusion. - Our results show a high rate of appropriate prescriptions. The easily accessible and regularly updated local recommendations probably resulted in the standardization and optimization of costly antifungal agent prescriptions. (C) 2015 Elsevier Masson SAS. Multidrug resistant pulmonary tuberculosis was diagnosed to a 32-year-old man. An AA-amyloidosis was subsequently diagnosed on the renal biopsy performed for nephrotic syndrome and macroscopic hematuria. A 6-drug antibiotic treatment was delivered quickly after first results of genotypic antibiogram given the renal failure, and was secondarily adapted to the phenotypic antibiogram. Multidrug therapy was fairly well tolerated. Clinical and biological improving were slow. Although tuberculosis is a classic cause of amyloidosis, this is the first case reporting an association between a multidrug resistant case and an amyloidosis in adults. This case also raises the question of MDR probabilistic treatments in situations whether a vital organ prognosis is engaged.
L’espérance de vie des personnes séropositives pour le VIH (PVVIH) se rapproche de celle de la population générale. Il persiste néanmoins une surmortalité prématurée en rapport avec des caractéristiques individuelles (histoire de l’infection VIH, comorbidités, facteurs de risque cardiovasculaires ou de cancers (FDR), situation socio-économique). Décrire les décès survenant à un âge supérieur à 50 ans permet d’approcher la distribution des causes de mortalité les plus à même de survenir à l’ère de l’infection VIH considérée comme une maladie chronique. Les causes initiales de décès des PVVIH ont été documentées sur une période d’un an au sein de 90 hôpitaux français représentant une file active de 82 000 patients. Les caractéristiques biologiques relatives à l’infection VIH (nombre de lymphocytes T-CD4+/mm3, copies d’ARN VIH/mL plasma) sont celles relevées lors du dernier contrôle biologique précédant le décès. Parmi les 728 décès enregistrés, 374 (51 %) concernaient des PVVIH de plus de 50 ans. Leur âge médian au moment du décès était de 58 ans (interquartiles 25–75 % : 53–64) et 79 % était des hommes. La contamination était hétérosexuelle 43 %, homo/bisexuelle 25 % ou liée à une toxicomanie IV 19 %. Le diagnostic d’infection VIH datait en médiane de 14 ans. Seuls 21 % des décès étaient dus à un évènement SIDA (vs.29 % chez les PVVIH décédés < 50 ans). Les autres causes de décès étaient les cancers (principalement broncho-pulmonaire puis digestif, ORL, urogénital et hémopathie) non liés au SIDA ou aux hépatites virales 27 % (vs. 17 % si < 50 ans), les maladies cardiovasculaires (principalement infarctus du myocarde puis AVC et insuffisance cardiaque) 13 % (vs. 7 % si décès < 50 ans), les infections non liées au SIDA 10 % (vs. 8 % si < 50 ans), les maladies liées aux virus des hépatites 7 % (dont une majorité d’hépatocarcinomes) (vs. 14 % si < 50 ans), les morts subites inexpliquées 4 %, et les suicides 3 % (vs. 7 % si décès < 50 ans). Le total des décès par néoplasies (liées ou non au sida ou aux virus des hépatites) représente 39 % des causes de mortalité. Douze pour cent des patients décédaient d’un panel de causes diverses (neurologiques non vasculaires, hématologiques, broncho-pulmonaires non tumorales, rénales, métaboliques, accidents). La cause restait inconnue chez 3 %. Dans 60 % des cas (vs. 51 % si < 50 ans), les lymphocytes CD4+ étaient > 200/mm3, et dans 61 % (vs. 51 % si < 50 ans) l’ARN VIH plasmatique était indétectable (< 50 copies/mL). On identifiait HTA, dyslipidémie et diabète, dans respectivement 25 %, 19 % et 16 % des cas (vs. 8, 8 et 4 chez < 50 ans). Une co-infection VHC concernait 22 % des PVVIH décédées (vs. 33 % si < 50 ans). Les évènements liés au sida représentent désormais une part minoritaire des causes de mortalité des PVVIH dont le décès survient le plus souvent en situation immuno-virologique contrôlée : ce phénomène est encore plus marqué chez les « seniors ». Les causes de décès des PVVIH de plus de 50 ans sont très diversifiées et se rapprochent de celles de la population générale. De façon encore plus marquée que pour l’ensemble de la population des PVVIH, la nécessité d’une approche polyvalente (généraliste, interniste) et/ou pluridisciplinaire s’impose pour les PVVIH âgés de plus de 50 ans afin d’optimiser leur prise en charge. La place d’un suivi oncologique doit être importante (prévention, dépistage et traitement).
Objectives The current study aimed at describing the distribution and characteristics of malignancy related deaths in human immunodeficiency virus (HIV) infected patients in 2010 and at comparing them to those obtained in 2000 and 2005. Methods Data were obtained from three national surveys conducted in France in 2010, 2005 and 2000. The underlying cause of death was documented using a standardized questionnaire fulfilled in French hospital wards involved in the management of HIV infection. Results Among the 728 deaths reported in 2010, 262 were cancer-related (36%). After a significant increase from 28% in 2000 to 33% in 2005 and 36% in 2010, cancers represent the leading cause of mortality in HIV infected patients. The proportion of deaths attributed to non-AIDS/non-hepatitis-related cancers significantly increased from 2000 to 2010 (11% of the deaths in 2000, 17% in 2005 and 22% in 2010, p<0.001), while those attributed to AIDS-defining cancers decreased during the same period (16% in 2000, 13% in 2005 and 9% in 2010, p = 0.024). Particularly, the proportion of respiratory cancers significantly increased from 5% in 2000 to 6% in 2005 and 11% in 2010 (p = 0.004). Lung cancer was the most common cancer-related cause of death in 2010 (instead of non-Hodgkin lymphoma so far) and represented the leading cause of death in people living with HIV overall. Conclusions Cancer prevention (especially smoking cessation), screening strategies and therapeutic management need to be optimized in HIV-infected patients in order to reduce mortality, particularly in the field of respiratory cancers.
