ZusammenfassungDiese Handlungsempfehlung soll eine Grundlage für die Qualitätssicherung der Radiosynoviorthese (RSO) bei entzündlichen (nichtinfektiösen) Gelenkerkrankungen schaffen. Sie wurde interdisziplinär erarbeitet und beschreibt die allgemeine Zielsetzung, Definitionen, klinische Hintergrundinformationen und Kontraindikationen dieser Radionuklidtherapie. Aufgeführt werden die Anforderungen, die an ein Behandlungszentrum gestellt werden, prätherapeutisch notwendige Untersuchungsbefunde sowie Empfehlungen, wie die Behandlung technisch und organisatorisch durchgeführt werden sollte. Ferner finden sich Angaben zur Kontrolle und Nachsorge der therapierten Patienten. Grundsätzlich sollen Behandlung und Nachsorge in Abstimmung und Zusammenarbeit der beteiligten Fachdisziplinen erfolgen.
Surgery remains the only curative treatment of pancreatic neuroendocrine neoplasms (pNEN). Here, we report the outcome after surgery for non-functional pNEN at a European Neuroendocrine Tumor Society (ENETS) center in Germany between 2000 and 2019; cases were analyzed for surgical (Clavien–Dindo classification; CDc) and oncological outcomes. Forty-nine patients (tumor grading G1 n = 25, G2 n = 22, G3 n = 2), with a median age of 56 years, were included. Severe complications (CDc ≥ grade 3b) occurred in 11 patients (22.4%) and type B/C pancreatic fistulas (POPFs) occurred in 5 patients (10.2%); in-hospital mortality was 2% (n = 1). Six of seven patients with tumor recurrence (14.3%) had G2 tumors in the pancreatic body/tail. The median survival was 5.7 years (68 months; [1–228 months]). Neither the occurrence (p = 0.683) nor the severity of complications had an influence on the relapse behavior (p = 0.086). This also applied for a POPF (≥B, p = 0.609). G2 pNEN patients (n = 22) with and without tumor recurrence had similar median tumor sizes (4 cm and 3.9 cm, respectively). Five of the six relapsed G2 patients (83.3%) had tumor-positive lymph nodes (N+); all G2 pNEN patients with recurrence had initially been treated with distal pancreatic resection. Pancreatic resections for pNEN are safe but associated with relevant postoperative morbidity. Future studies are needed to evaluate suitable resection strategies for G2 pNEN.
ZusammenfassungUnter dem Überbegriff neuroendokrine Neoplasien (NEN) wird eine molekular und biologisch sehr heterogene Gruppe von Tumoren zusammengefasst, deren gemeinsames Merkmal der Ursprung aus neuroendokrinen Zellen ist. Die ebenfalls sehr heterogene Subgruppe der gastroenteropankreatischen neuroendokrinen Neoplasien (GEP-NEN) ist am besten klassifiziert und untersucht. Der vorliegende Beitrag bietet eine Systematik zu aktueller Klassifikation, Diagnostik und zu Therapieoptionen von GEP-NEN. Zur besseren Übersicht wurde bewusst darauf verzichtet, nach der Primärlokalisation der Tumoren vorzugehen. Stattdessen wurde eine thematische Gliederung gemäß Klassifikation, klinischem Phänotyp, Diagnostik und Therapie gewählt.
The term neuroendocrine neoplasms (NEN) encompasses a molecularly and biologically very heterogeneous group of tumors, which have in common their origin in neuroendocrine cells. The also very heterogeneous subgroup of gastroenteropancreatic neuroendocrine neoplasms (GEP-NEN) is the best classified and investigated group. This article provides a systematic review of the current classification, diagnostics and treatment options of GEP-NEN. In order to achieve a better overview, it was consciously decided not to use an approach based on the primary localization. Instead, a thematic organization according to classification, clinical phenotype, diagnostics and treatment was chosen.
ZusammenfassungDie vorliegende Handlungsempfehlung dient als Grundlage zur Qualitätssicherung der Somatostatinrezeptor (SSTR-) PET/CT bei onkologischen Patienten. Die Leitlinie wurde interdisziplinär erarbeitet und enthält neben der Definition und Zielsetzung einen Konsens hinsichtlich der erforderlichen klinischen Hintergrundinformationen, Indikationsstellung, Durchführung, Auswertung, Dokumentation und Befunderstellung. Dabei werden die beiden gebräuchlichsten Radiotracer der SSTRPET/ CT (68Ga-DOTATOC und 68Ga-DOTATATE) vertieft dargestellt. Die SSTR-PET/CT erfordert eine enge Interaktion zwischen den zuweisenden und ausführenden Fachdisziplinen unter Berücksichtigung bereits vorhandener Leitlinien und Handlungsempfehlungen europäischer und deutscher Fachgesellschaften, namentlich der European Association of Nuclear Medicine (EANM), der Deutschen Gesellschaft für Endokrinologie (DGE), Deutschen Gesellschaft für Nuklearmedizin (DGN) und Deutschen Röntgengesellschaft (DRG).
Pheochromocytomas/paragangliomas (PPGLs) and their metastases are tumors that predominantly express somatostatin receptor 2 (SSR2). 68Ga-DOTA(0)-Tyr(3)-octreotate (68Ga-DOTATATE) is a PET radiopharmaceutical with both high and selective affinity for SSRs. The purpose of this study was to evaluate the utility of 68Ga-DOTATATE in comparison with other specific and nonspecific radiopharmaceuticals recommended in the current guidelines for the localization of metastatic sporadic PPGL by PET/CT.
This document describes the guideline for peptide receptor radionuclide therapy (PRRT) published by the German Society of Nuclear Medicine (DGN) and accepted by the Association of the Scientific Medical Societies in Germany (AWMF) to be included in the official AWMF Guideline Registry. These recommendations are a prerequisite for the quality management in the treatment of patients with somatostatin receptor expressing tumours using PRRT. They are aimed at guiding nuclear medicine specialists in selecting likely candidates to receive PRRT and to deliver the treatment in a safe and effective manner. The recommendations are based on an interdisciplinary consensus. The document contains background information and definitions and covers the rationale, indications and contraindications for PRRT. Essential topics are the requirements for institutions performing the therapy, e.g. presence of an expert for medical physics, intense cooperation with all colleagues involved in the treatment of a patient, and a certificate of instruction in radiochemical labelling and quality control are required. Furthermore, it is specified which patient data have to be available prior to performance of therapy and how treatment has to be carried out technically. Here, quality control and documentation of labelling are of great importance. After treatment, clinical quality control is mandatory (work-up of therapy data and follow-up of patients). Essential elements of follow-up are specified in detail. The complete treatment inclusive after-care has to be realised in close cooperation with the involved medical disciplines. Generally, the decision for PRRT should be undertaken within the framework of a multi-disciplinary tumour board.
Aim: The subject of our study is to investigate the accuracy of a typical PET/CT system applying F-18-FDG to determine activity of small, hot volumes (HV) by phantom measurements under the impact of reconstruction techniques (RT) with and without Time-of-flight-Mode (TOF), activity concentration ratios into the phantom (ACR), density variations and relevant analysis parameters. Methods: We investigated a body phantom containing up to 7 spheres (d = 6-37mm) on a topical PET/CT system (SIEMENS BIOGRAPH mCT40). Recovery coefficients (RC) of small HVs dependent on the ACR and several RTs (iterative default (IT), iterative high-definition including the system Point-Spread-Function (HD), HD+TOF (UHD), filtered backprojection (FBP) and FBP+TOF (FBPT) were analysed. In addition, we determined RCs with respect to the matrix size, number of iterations, filter settings and a phantom insert of lung and bone density equivalent material. Results: The application of UHD in particular reproduces true activities of small volumes best, followed by HD, IT, FBPT and FBP. Lung and bone equivalent density inserts reduce the RCs by up to 19%. Especially for small HVs the RC can be raised by increasing the matrix size and iterations. Conclusions: Novel technology and analysis have a strong impact on the quantification. Hence, new guidelines are required enabling comparisons to other, prior systems. In order to get a precise activity determination of small structures, we recommend the UHD algorithm, high resolution matrix and high number of iterations with respect to acquisition time and administered activity.
ZusammenfassungDiese Handlungsempfehlung soll eine Grundlage für die Qualitätssicherung der Peptid - rezeptor-Radionuklidtherapie (PRRT) von Patienten mit Somatostatinrezeptor-exprimierenden Tumoren schaffen, die zurzeit in Deutschland bei fehlenden gleichwertigen oder besseren Therapiealternativen im Rahmen eines individuellen Heilversuches erfolgt. Sie wurde interdisziplinär erarbeitet und enthält neben Definition, allgemeiner Zielsetzung und klinischen Hintergrundinformationen Angaben zu Indikationen und Kontraindikationen der PRRT. Im Mittelpunkt stehen die Anforderungen, die an das Behandlungszentrum gestellt werden, wie die enge Zusammenarbeit der an der Behandlung beteiligten Fachgebiete. Weiterhin wird spezifiziert, welche Untersuchungsbefunde vor der Therapie vorliegen müssen und wie die PRRT technisch und organisatorisch durchzuführen ist. Nach der Behandlung ist eine langfristige Nachsorge/Kontrolle der therapierten Patienten – u. a. zur Gewinnung onkologischer Qualitätsparameter – unabdingbar. Die gesamte Behandlung einschließlich der Nachsorge muss in enger Abstimmung und Zusammenarbeit der beteiligten Fachdisziplinen erfolgen, wobei in der Regel die Empfehlung zur PRRT durch ein multidisziplinäres Tumorboard erfolgen sollte.
Pheochromocytomas/paragangliomas overexpress somatostatin receptors, and recent studies have already shown excellent results in the localization of sympathetic succinate dehydrogenase complex, subunit B, mutation-related metastatic pheochromocytomas/paragangliomas using 68Ga-DOTATATE PET/CT. Therefore, the goal of our study was to assess the clinical utility of this functional imaging modality in parasympathetic head and neck paragangliomas (HNPGLs) compared with anatomic imaging with CT/MRI and other functional imaging modalities, including 18F-fluorohydroyphenylalanine (18F-FDOPA) PET/CT, currently the gold standard in the functional imaging of HNPGLs. Methods: 68Ga-DOTATATE PET/CT was prospectively performed in 20 patients with HNPGLs. All patients also underwent 18F-FDOPA PET/CT, 18F-FDG PET/CT, and CT/MRI, with 18 patients also undergoing 18F-fluorodopamine (18F-FDA) PET/CT. 18F-FDOPA PET/CT and CT/MRI served as the imaging comparators. Results: Thirty-eight lesions in 20 patients were detected, with 18F-FDOPA PET/CT identifying 37 of 38 and CT/MRI identifying 23 of 38 lesions (P < 0.01). All 38 and an additional 7 lesions (P = 0.016) were detected on 68Ga-DOTATATE PET/CT. Significantly fewer lesions were identified by 18F-FDG PET/CT (24/38, P < 0.01) and 18F-FDA PET/CT (10/34, P < 0.01). Conclusion: 68Ga-DOTATATE PET/CT identified more lesions than other imaging modalities. With the results of the present study, and the increasing availability and use of DOTA analogs in the therapy of neuroendocrine tumors, we expect that 68Ga-DOTATATE PET/CT will become the preferred functional imaging modality for HNPGLs in the near future.
Hintergrund: Neuroendokrine Neoplasien (NEN) sind seltene und heterogene Tumoren. Die mit unklarem Primarius (N-CUP) stellen eine Untergruppe mit schlechter Prognose dar. Die Resektion des Primarius ist die Voraussetzung für einen kurativen Ansatz, sodass sich die Frage nach dem sinnvollsten diagnostischen und therapeutischen Vorgehen bei N-CUP stellt. Patienten/Methoden: Die Datenbankauswertung von 03/1990 bis 03/2011 ergab 35 N-CUP aus 261 Fällen mit NEN. Es wurde unterteilt in Patienten, bei denen nach operativer Exploration ein Primarius gesichert wurde (Kollektiv 1 = K1, n = 10), durch operative Exploration kein Primarius gesichert wurde (K2, n = 10) und in Patienten, die nicht operativ exploriert wurden (K3, n = 13). Ergebnisse: Die N-CUP-Rate beträgt bei Erstvorstellung 13,4 % (35/261), nach erneuter Diagnostik im Zentrum 12,7 % (33/261) und nach operativer Exploration 8,8 % (23/261). Das Geschlechterverhältnis ist ausgeglichen, das mittlere Alter ist mit 63,8 Jahren (J) in der CUP-Gruppe signifikant höher als in der Gesamtgruppe (55,9 J, p = 0,004). Das 5-Jahres-Überleben (JÜL) ist 58 % (Gesamtgruppe: 72 %, n. s.). Operativ exploriert wurden 60,6 % (20/33), von denen 30 % (6/20) inoperabel waren. Bei 20 % (4/20) erfolgte die R0-Resektion von solitären Metastasen ohne Primariusdarstellung und bei 50 % (10/20) konnte der Primarius intraoperativ lokalisiert werden (8 × Dünndarm, 2 × Pankreas). Hier erfolgte in 70 % (7/10) eine R0-Resektion und in 30 % eine Primariusresektion mit Debulking von Lebermetastasen. In der nicht operierten Gruppe (39,4 %, 13/33) erhielten 69,2 % (9/13) eine Chemotherapie. Das mittlere Alter ist signifikant niedriger in K1 mit 54,9 J, im Vergleich zu K3 mit 68,3 J (p = 0,028), eine männliche Dominanz ist in K3 mit 3,3 : 1 (m : w, n. s.) zu sehen. Der mittlere Ki-67-Index beträgt 4,3, 23,8 und 53 % in K1, K2 und K3 und ist hochsignifikant zwischen (zw.) K1 und K3 (p < 0,0001) und signifikant zw. K2 und K3 (p = 0,035) unterschiedlich. Verstorben sind 20, 30 und 76,9 % in K1, K2 und K3. Schlussfolgerung: Primärtumoren des Dünndarms und seltener des Pankreas verbergen sich häufig hinter dem CUP-Syndrom bei gut differenzierten NEN. Eine R0-Resektion, auch von solitären Metastasen ohne Primariusnachweis, führt zu einem guten klinischen Ergebnis. Die offene Exploration wird nach Ausschöpfen diagnostischer Möglichkeiten einschließlich der Immunhistochemie (CDX-2, TTF-1) empfohlen, um eine Primariuslokalisation und damit Heilung bei N-CUP zu ermöglichen.
Background: Neuroendocrine neoplasia (NEN) are a rare and heterogenous tumour entity. The subgroup with unknown primary tumour (N-CUP) seems to have a worse prognosis as resection of the primary is necessary for cure. The diagnostics and therapeutic algorithms for N-CUP in a German single centre are presented. Patients/Methods: Analysis of the surgical databank showed 35 cases of N-CUP in 261 cases with NEN from gastroenteropancreatic and lung origin over 2 decades (03/1990-03/2011). Three groups were built: K1 - primary detection after operative exploration (n = 10), K2 - unknown primary after operative exploration (n = 10) and K3 - no operative exploration for various reasons (n = 13). Results: Initially 13.4% (35/261) of patients presented as N-CUP, after intensified diagnostics 12.7% (33/261) and after operative exploration 8.8% (23/261) remained with unknown primary tumour. The sex ratio was 1: 1, the median age is significantly higher in N-CUP [63.8 years (y) vs. 55.9 y, p = 0.004), the 5-year-survival is lower (58 vs. 72%, n.s.). compared to NEN with known primary. Operative exploration was performed in 60.6% (20/33), 30% (6/20) of them were found to have inoperable situations, in 20% (4/20) single site metastases were removed completely and in 50% (10/20) a primary tumour was detected (8 x midgut, 2 x pancreas) intraoperatively. In these cases 70% (7/10) got complete tumour resection (R0) and in 30% (3/10) primary tumour resection with debulking of liver metastasis was done. In K3 (39.4%, 13/33) most patients [69.2% (9/13)] were treated with chemotherapy. The median age in K1 was significantly lower than in K3 (54.9 y vs. 68.3 y, p = 0.028), male dominance was seen in K3 (3,3 : 1, n.s.). The average Ki-67 index was 4.3, 23.8 and 53% in K1, K2 and K3 (p < 0.0001 for K1 and K3 and p = 0.035 for K2 and K3), respectively.The death ratewas 20, 30 and 76.9% in K1, K2 and K3, respectively. Conclusion: Primary tumours of the midgut and pancreas are often found in the subset of well differentiated neuroendocrine CUP syndrome after open surgical exploration. A high rate of complete tumour resection and cure can be achieved in these cases. After common diagnostic tools (CT, MRI and somatostatin receptor scintigraphy), immunhistochemistry can give important hints (CDX-2 for midgut, TTF-1 for lung and thyroid) for a primary lesion. Also in single site metastasis without primary tumour detection a good clinical outcome is seen after complete resection.
Ziele: Untersucht wurde, ob die uHD-PET (HD-PET + time of flight) oder die HD-PET der iterativen Rekonstruktion (IR) in der Detektion hypermetaboler LL bei Patienten mit Bronchialkarzinom (BC)in der FDG-PET-CT auch in Abhängigkeit vom BMI überlegen ist. Methode: 60 Pat. mit BC wurden prospektiv untersucht (Biograph mCT, Siemens). Die Pat. erhielten 236 ± 21 MBq FDG i.v.; 80 ± 12 min p.i. begann die Akquisition von uHD, HD- und IR-Daten. Hypermetabole LL wurden visuell detektiert und anhand des CT verifiziert. SUVmax-Werte wurden mittels VOI-Analyse ermittelt. Bei Patienten mit LL >1 ml wurden zusätzlich SUVpeak-Werte ermittelt. Die Anzahl der hypermetabolen LL, SUVmax-, SUVpeak und SUVmax/Background (BG)-Ratio wurden für alle 3 Rekonstruktionsmodi (RM) verglichen. Der BG wurde bestimmt durch eine für alle 3 RM identische VOI in angrenzendem normalem Lungengewebe. Ferner wurde für alle Pat. der BMI erhoben. SUVmax-Werte adipöser Pat. (BMI >25 kg/m2) wurden verglichen mit denen normalgewichtiger Pat. (Signifikanzniveau <0,05). Ergebnis: 210 (100%) hypermetabole LL wurden mittels uHD detektiert und zu 100% mittels CT als richtig-positiv verifiziert, 193 (91,9%) mittels HD und 177 (84,3%) mittels IR (p<0,001). UHD- und HD-PET zeigten signifikant höhere SUVmax-Werte als die IR (p<0,001). Die SUVpeak-Werte waren nicht signifikant unterschiedlich. Die SUVmax/BG-Ratio war signifikant höher in uHD und HD als in der IR (p<0,001). 21 Pat. (40%) hatten einen BMI >25 kg/m2. Der BMI zeigte jedoch keinen Einfluss auf die Ergebnisse. Schlussfolgerung: UHD-PET ist der HD-PET und der IR in der Detektion maligner LL signifikant überlegen, unabhängig vom BMI. Es profitieren also auch dünne Pat. von der uHD-PET. Der SUVpeak verspricht die höchste Vergleichbarkeit der SUV-Werte, unabhängig vom RM. Wir schlussfolgern, dass UHD und SUVpeak in der täglichen Praxis eingesetzt werden sollten.
2349 Objectives The aim was to evaluate if ultra-HD-PET (HD-PET + time of flight (TOF)) and/or high definition reconstruction (HD-PET) is superior to iterative reconstruction (IR) in the detection of malignant lung lesions by FDG-PET/CT in patients with bronchial carcinoma (BC) and in consideration of the body mass index (BMI) >25 kg/m2. Methods 22 patients with BC were prospectively examined with FDG-PET/CT (Biograph mCT40, Siemens). A mean of 226±14 MBq FDG were applied i.v., and the acquisition was initiated 79±10 min p.i. Pulmonal lesions with hypermetabolism were visually detected and SUVmax-values were calculated by VOI analysis. The number of pulmonal lesions with hypermetabolism and SUVmax were compared for all 3 different reconstruction methods. SUVmax/background-ratios were calculated by drawing identical VOIs in the surrounding, physiological lung tissue and were used to define the proportion of radiotracer uptake. Furthermore, SUVmax of patients with a BMI > 25 kg/m2 were compared with normal weight patients. The significance level was defined as p ≤ 0,05. Results In our preliminary results, 89 (100%) lung lesions with glucose hypermetabolism were detected by ultra-HD-PET, 78 (87,6%) by HD-PET without TOF and 61 (69%) by IR (p 25kg/m2. However, the BMI did not significantly influence the findings in any reconstruction mode. Conclusions In malignant BC lesions, ultra-HD-PET is superior to HD-PET which is again superior to IR with regard to sensitivity, SUVmax-values and SUVmax/background-ratios. In our small patient collective, no significant influence of the BMI with a cut-off value of >25 kg/m2 was demonstrated between the different reconstruction algorithms
Ziele: Untersucht wurde, ob die high definition Rekonstruktion (HD-PET) und die ultra-HD-PET (HD-PET + Time of Flight (TOF)) der iterativen Rekonstruktion (IR) beim Nachweis glukosehypermetaboler pulmonaler Läsionen in der FDG-PET bei BC-Patienten (Pat.) überlegen sind. Methode: Es wurden 21 BC-Pat. mittels FDG-PET-CT (Biograph mCT40, Siemens) prospektiv untersucht. Die Pat. erhielten 228±14 MBq FDG i.v., Akquisitionsbeginn 78±10min p.i. Es wurden ultra-HD-, HD- und iterative PET-Daten akquiriert, glukosehypermetabole pulmonale Läsionen visuell detektiert und mittels VOI-Analyse SUVmax-Werte ermittelt. Die Rekonstruktionsmethoden wurden in Anzahl der Läsionen und SUVmax-Werten verglichen. Zur Ermittlung der SUVmax/BG-Ratio als Maß für das Anreicherungsverhältnis wurden für alle 3 Rekonstruktionsmodi identische Vergleichs-VOIs in normalem angrenzendem Lungengewebe gelegt. Die SUVmax-Werte von Pat. mit BMI>25kg/m2 wurden mit denen normalgewichtiger Pat. verglichen (Signifikanzniveau p<0,05). Ergebnis: Mittels ultra-HD-PET wurden 88 glukosehypermetabole Läsionen detektiert (100%), mittels HD-PET ohne TOF 77 (87,5%) und mittels IR 61 (69%) (p<0,001). Ultra-HD-PET (p=0,028) und HD-PET (p=0,009) zeigten in einem lesion-to-lesion-Vergleich signifikant höhere SUVmax-Werte. Die SUVmax/BG-Ratio war in der ultra-HD-PET signifikant höher als in der HD-PET und IR (p=0,009). 38% der Pat. hatten einen BMI>25kg/m2. Ein signifikanter Einfluss des BMI zeigte sich nicht. Schlussfolgerung: Die ultra-HD-PET zeigt ein signifikant besseres Anreicherungsverhältnis zum Lungengewebe als die HD-PET und die PET mit IR und ist sensitiver in der Detektion von BC-Läsionen. Ultra-HD-PET und HD-PET zeigen signifikant höhere SUVmax-Werte als die IR. Bei bisher geringer Pat.-Zahl und wenigen Pat. mit einem BMI >25kg/m2 (38%) konnte kein signifikanter Einfluss des BMI ermittelt werden.
