Aims: Fungal periprosthetic joint infections (fPJIs) are rare complications, constituting only 1% of all PJIs. Neither a uniform definition for fPJI has been established, nor a standardized treatment regimen. Compared to bacterial PJI, there is little evidence for fPJI in the literature with divergent results. Hence, we implemented a novel treatment algorithm based on three-stage revision arthroplasty, with local and systemic antifungal therapy to optimize treatment for fPJI. Methods: From 2015 to 2018, a total of 18 patients with fPJI were included in a prospective, single-centre study (DKRS-ID 00020409). The diagnosis of PJI is based on the European Bone and Joint Infection Society definition of periprosthetic joint infections. The baseline parameters (age, sex, and BMI) and additional data (previous surgeries, pathogen spectrum, and Charlson Comorbidity Index) were recorded. A therapy protocol with three-stage revision, including a scheduled spacer exchange, was implemented. Systemic antifungal medication was administered throughout the entire treatment period and continued for six months after reimplantation. A minimum follow-up of 24 months was defined. Results: Eradication of infection was achieved in 16 out of 18 patients (88.8%), with a mean follow-up of 35 months (25 to 54). Mixed bacterial and fungal infections were present in seven cases (39%). The interval period, defined as the period of time from explantation to reimplantation, was 119 days (55 to 202). In five patients, a salvage procedure was performed (three cementless modular knee arthrodesis, and two Girdlestone procedures). Conclusion: Therapy for fPJI is complex, with low cure rates according to the literature. No uniform treatment recommendations presently exist for fPJI. Three-stage revision arthroplasty with prolonged systemic antifungal therapy showed promising results. Cite this article: Bone Jt Open 2021;2(8):671–678.
Fragestellung Major-Amputationen der oberen Extremität sind seltene Verletzungen, gehen aber mit erheblichen Einschränkungen für die betroffenen Patienten sowie einer Mehrbelastung der Gegenseite durch den höheren Einsatz einher. Durch diese Mehrbelastung können Beschwerden und potentiell ein frühzeitiger Verschleiß der Gegenseite durch Überlastung entstehen. Ziel der Studie war es, vermehrt vorhandene einseitige degenerative Veränderungen der Schulter nach kontralateraler Major-Amputation zu untersuchen.
Fragestellung Monteggia-like-lesions werden als Frakturen des Ulnaschaftes mit Luxation des Radiuskopfes und vorhandenen Begleitverletzungen definiert. Heutzutage wird meist eine osteosynthetische Versorgung der Ulna empfohlen. Die Behandlung der Begleitverletzungen wird weiterhin kontrovers diskutiert. Ziel der Untersuchung war es, fehlgeschlagene Primärversorgungen einer Monteggia-like-lesion zu untersuchen um möglicherweise vermeidbare Probleme in der Primärversorgung dieser Verletzungen zu erfassen.
Background The high demands that fracture-related infections put on patients, physicians and the healthcare system have led to the establishment of a international group of experts called the Fracture-Related Infection (FRI) Consensus Group, whose aim is to develop evidence-based treatment recommendations. Diagnosis Fracture-related infections are classified according to the time of occurrence, extent and treatment options. The diagnostic algorithm distinguishes between confirmatory and suggestive diagnostic criteria. If there are indications of an infection, tissue biopsy with microbiological and histological workup is recommended to confirm the diagnosis. Therapy The primary objective of FRI treatment is to achieve fracture consolidation, while avoiding osteomyelitis. Therapeutic options are removal of the implant, eradication of the infection with implant retention or suppression of FRI. A multidisciplinary team is recommended to develop a patient-specific, optimized surgical and antimicrobial therapy.
Die hohen Anforderungen, die implantatassoziierte Infektionen an Patienten, Ärzte und Gesundheitssystem stellen, haben zur Gründung der internationalen Expertengruppe FRI („fracture-related infection“) Consensus Group geführt. Ihr Ziel ist die Formulierung evidenzbasierter Behandlungsempfehlungen. Implantatassoziierte Infektionen werden nach dem zeitlichen Auftreten, der Ausdehnung und der Therapiemöglichkeit klassifiziert. Der diagnostische Algorithmus unterscheidet dabei zwischen beweisenden und hinweisenden Diagnosekriterien. Bei Hinweisen auf eine Infektion wird die frühzeitige Gewebebiopsie mit mikrobiologischer und histologischer Aufarbeitung zur Diagnosesicherung empfohlen. Primäres Ziel der Behandlung einer implantatassoziierten Infektion ist die Konsolidierung der Fraktur unter Vermeidung einer Osteomyelitis. Therapieoptionen sind die Implantatentfernung, die Infekteradikation bei einliegendem Implantat oder die Infektsuppression bis zur knöchernen Konsolidierung. Ein multidisziplinäres Team ist notwendig, um die patientenadaptierte chirurgische und antimikrobielle Therapie optimal aufeinander abzustimmen.
Im Rahmen eines zweizeitigen septischen Knieprothesenwechsels wird bei ausgedehnten knöchernen Defekten nach Prothesenexplantation häufig eine temporäre Arthrodese mithilfe eines antibiotikahaltigen Polymethylmethacrylat v(PMMA)-Spacer vorgenommen. Eine gängige Methode stellt die Verwendung von konventionellen Stahl- oder Kohlefaserstangen dar, die über einen „Tube-to-tube“-Konnektor verbunden und anschließend gelenknah mit Knochenzement armiert werden.

BACKGROUND:In two-stage septic revision arthroplasty of the knee, a temporary intramedullary fixation with an antibiotic-containing polymethyl methacrylate (PMMA) spacer is often performed in cases of extensive bone defects after endoprosthesis explantation. A common method is the use of conventional steel or carbon fiber rods, which are connected via a tube-to-tube connector and finally reinforced near the joint with bone cement. OBJECTIVE:As the surface of foreign materials plays a critical role in the colonization and biofilm formation in the treatment of periprosthetic joint infections (PJI), the steel and carbon fiber rods were examined and compared with respect to bacterial surface adhesions. MATERIAL AND METHODS:Carbon fiber and steel rods of external fixator systems were used for this experimental study. The sample material was placed in a substrate enriched with S. aureus. The adherent bacteria were examined both by fluorescence microscopy and quantitatively after ultrasonic detachment (sonication) in a smear preparation. In addition, scanning electron micrograph (SEM) images were taken to analyze the topography of bacterial adhesions. RESULTS:The fluorescence microscopy revealed a uniform surface distribution for both materials. The observation of the SEM images showed that for carbon fiber rods the growth of bacteria ran in unison with the direction of the fiber, while for the steel rods an arbitrary arrangement was found. With the help of sonication a significant difference in the number of adherent micro-organisms between the two materials could not be determined using the Wilcoxon test (significance level p < 0.05). CONCLUSION:Both materials can be used to perform PMMA-reinforced intramedullary fixation without fear of sacrificing therapeutic success. From an economic point of view, the use of steel rods seems reasonable as the material costs are significantly lower.
Abstract Background Knee joint infections with pronounced joint destruction, soft tissue and bone involvement are serious diseases in which not only the joint but also the entire extremity are directly endangered. What results can be achieved with the Ilizarov external fixator for septic knee arthrodesis? Patients and Methods Between 2005 and 2017, 13 patients (4 women and 9 men, mean age 46 years) with acute joint and concomitant bone and soft-tissue infections were treated with Ilizarov knee joint arthrodesis. In addition to demographic data, the time spent in the fixator, major and minor complications were retrospectively evaluated. Results The average time spent in the Ilizarov external fixator was 27 weeks (min. 13, max. 68). Arthrodesis and infection repair were primarily achieved in 12 out of 13 patients. In one patient, a delayed healing of the arthrodesis zone could be cured by iliac crest bone grafting, so that eventually a bony knee arthrodesis was achieved in all patients. Pin infects as a minor complication during the time spent in the fixator were common and could be treated by stab incision and antibiotics. Major complications occurred in 3 cases (2 femoral fractures peri-implant, 1 nonunion in the arthrodesis area), which were treated by dilatation of the Ilizarov external fixator and finally plate osteosynthesis or by resection of the pseudarthrosis and iliac crest bone grafting. Conclusion The knee joint arthrodesis with the Ilizarov external fixator is a salvage procedure for limb preservation in fulminant knee joint and concomitant soft tissue infections. The advantages lie in the possibility of osteosynthesis in the acute infection and/or chronic osteomyelitis and the direct full load capacity. Disadvantages are long and complicated treatment times in the fixator with limited patient comfort. Zusammenfassung Hintergrund Kniegelenksinfekte mit ausgeprägter Gelenkdestruktion, Weichteil- und Knochenbeteiligung sind schwerwiegende Erkrankungen, bei denen nicht nur das Gelenk, sondern auch die gesamte Extremität unmittelbar gefährdet ist. Welche Ergebnisse lassen sich mit dem Ilizarov-Ringfixateur zur septischen Kniegelenksarthrodese erzielen? Patienten und Methoden Zwischen 2005 und 2017 wurden 13 Patienten (4 Frauen und 9 Männer, Durchschnittsalter 46 Jahre) mit akut floriden Kniegelenks- und begleitenden Knochen- und Weichteilinfekten mit einer Ilizarov-Arthrodese des Kniegelenkes behandelt. Neben demografischen Daten wurden retrospektiv die Fixateurtragedauer, die Major- und Minorkomplikationen ausgewertet. Ergebnisse Die durchschnittliche Tragezeit des Ilizarov-Ringfixateurs betrug 27 Wochen (min. 13, max. 68). Die knöcherne Arthrodese und Infektsanierung konnte bei 12 von 13 Patienten primär erzielt werden. Bei einem Patienten konnte eine verzögerte Heilung der Arthrodesezone durch 2-malige Spongiosaanlagerung zur Ausheilung gebracht werden, sodass schließlich bei allen Patienten eine knöcherne Kniegelenksarthrodese erreicht wurde. Pininfekte als Minorkomplikation während der Ringfixateurbehandlung traten häufig auf und konnten durch Stichinzision und Antibiose behandelt werden. Majorkomplikationen traten in 3 Fällen auf (2 Femurfrakturen periimplantär, 1 Pseudarthrose im Arthrodesebereich), die durch Erweiterung des Ilizarov-Ringfixateurs und schließlich Plattenosteosynthese oder durch Pseudarthrosenresektion und Spongiosaanlagerung ausbehandelt wurden. Schlussfolgerung Die Kniegelenksarthrodese mit dem Ilizarov-Ringfixateur ist ein Salvage-Verfahren zum Extremitätenerhalt bei fulminanten Kniegelenks- und begleitenden Weichteilinfekten. Die Vorteile liegen in der Osteosynthesemöglichkeit im akut floriden Infekt und/oder chronischer Osteomyelitis sowie der direkten Vollbelastbarkeit. Nachteile sind lange und komplikationsreiche Behandlungszeiten im Fixateur bei eingeschränktem Patientenkomfort.
Ein Kniegelenkinfekt gilt als chirurgischer Notfall, der unbehandelt zur irreversiblen Gelenkdestruktion mit schlechten funktionellen Resultaten führen kann. Ätiologisch werden endogene und exogene Ursachen sowie ein Früh- und Spätinfekt unterschieden. Die Inzidenz ist gering und nimmt mit dem Anteil von eingebrachten Fremdmaterialien zu. Durch die Invasion von Bakterien mit der Freisetzung von körpereigenen Zytokinen und lysosomalen Proteinen kommt es zur Knorpelschädigung bereits nach 48 h. Die Kombination aus Klinik, Labor, Histopathologie und Mikrobiologie sowie Bildgebung führt zur Diagnosestellung. Die Therapie besteht aus der operativen Infektsanierung in Kombination mit der Gabe von Antibiotika. Infekte des nativen Kniegelenkes werden in der Regel arthroskopisch versorgt, wobei ein einzeitiger Eingriff zumeist ausreichend ist. Ein Infekt nach Kreuzbandtransplantation wird ebenfalls arthroskopisch behandelt, nicht selten mit mehrzeitigem Vorgehen. Die Transplantate können erhalten bleiben, wenn sie intraoperativ stabil sind. Therapie der Wahl beim periimplantären Infekt bei konsolidierter Fraktur ist die arthroskopische Sanierung mit Metallentfernung. Bei nicht konsolidierter Fraktur und bestehendem Frühinfekt kann ein Erhalt der Osteosynthese durch eine Revisionsoperation erzielt werden. Beim Spätinfekt wird im Regelfall ein zweizeitiges Vorgehen notwendig. Die initial kalkulierte Antibiotikatherapie wird nach Erhalt des Mikrobiologiebefundes resistenzgerecht angepasst. Die Dauer der Antibiotikatherapie sollte beim nativen Kniegelenkinfekt 6 Wochen und beim periimplantären Infekt 12 Wochen betragen.
Background Glenohumeral combination fractures are severe shoulder injuries for which, however, few data are available in the literature. The aim of the study was to analyze the incidence and numeric distribution of glenohumeral combination fractures using a classification system. Furthermore, treatment methods and complications are discussed. Methods This retrospective study is based on the data evaluation of a level I trauma center between 1998 and 2016. Glenohumeral combination fractures are classified into four types, and the incidence, treatment method, and complications are evaluated. Additionally, the fracture morphology and displacement of the different fracture types are analyzed. Results In total, 52 patients with a glenohumeral combination fracture were identified. There were 26 (50%) type 1 fractures (anterior glenoid rim fracture/greater tuberosity fracture), 20 (38%) type 2 fractures (anterior glenoid rim fractures and proximal humerus fractures), four (8%) type 3 fractures (glenoid fossa fractures and greater tuberosity fractures), and two (4%) type 4 fractures (glenoid fossa fractures and proximal humerus fractures). In the majority of patients ( n = 25), both lesions of the combined fractures were treated operatively, in 15 both fractures were treated nonoperatively, and in 12 patients only one of the two lesions was treated surgically. Complications were encountered in 21% of cases, particularly on the humeral side. Conclusion Glenohumeral combination fractures are rare. Type 1 and type 2 fractures are the most frequent combination fractures. The indications for treatment are based on the displacement of the respective fragments, which can be addressed during the same operation if surgery is indicated. Complications are more frequently found on the humeral side.
Abstract Background A key factor in the successful treatment of periprosthetic joint infection is the consistent antimicrobial therapy. Depending on the pathogen antimicrobial susceptibility, intravenous (i. v.) antibiotics may not be switched to oral medication at patient discharge, without risking a treatment failure. The implementation of an outpatient i. v. therapy using peripherally inserted central catheters (PICCs) allows for early discharge with continuation of adequate antimicrobial therapy. Methods We prospectively evaluated the systematic use of PICC in 11 patients with periprosthetic joint infection. Inclusion criterion was the detection of a pathogen that cannot be optimally treated with oral antibiotics according to the current treatment concept (Trampuz et al.). Results A total of 12 PICCs in 11 patients were analysed. An error or mechanical nerve irritation was not observed. The occurrence of a catheter-related bloodstream infection (CRBSI) was not recorded. None of our patients had PICC-associated infections, thrombophlebitis or thrombosis. One patient stopped therapy due to a lack of compliance. Ten patients successfully completed their treatment. At a mean follow-up of 29 months, successful infection eradication as measured by the Delphi criteria was achieved in 100%. Conclusion Although the number of patients included in the study was small, our short-term results suggest a good applicability of PICCs in the outpatient i. v. antimicrobial therapy for treatment of periprosthetic joint infection. Catheter-associated complications could not be observed in our collective. The successful infection control in all cases despite proven difficult-to-treat infection is promising. Zusammenfassung Hintergrund Ein maßgeblicher Faktor in der erfolgreichen Behandlung periprothetischer Infektionen ist die korrekte antimikrobielle Therapie. In Abhängigkeit vom Antibiogramm kann die intravenöse Antibiotikatherapie zur Entlassung nicht immer ohne Risiko eines Behandlungsversagens oralisiert werden. Die Durchführung einer ambulanten i. v. Therapie unter Verwendung von peripher eingesetzten zentralen Venenkathetern ermöglicht die Fortführung einer resistenzgerechten, optimierten Antibiotikatherapie. Methode In die prospektive, klinische Studie wurden 11 Patienten eingeschlossen. Einschlusskriterium war der Nachweis eines Erregers, der nach zugrunde gelegtem Behandlungskonzept (Trampuz et al.) mittels oraler Antibiotika nicht adäquat zu therapieren war. Die Infusion der Antibiotika erfolgte in Eigenregie durch die Patienten oder Angehörige. Eine Schulung durch spezialisiertes Personal eines Dienstleisters für komplexe ambulante Arzneimitteltherapie war vorausgegangen. Ergebnisse Insgesamt wurden 12 PICCs (peripherally inserted central catheters) bei 11 Patienten analysiert. Katheterassoziierte Komplikationen wie Infektionen der Blutbahn (catheter-related bloodstream infection – CRBSI) sind nicht aufgetreten. Eine Fehlanlage oder mechanische Nervenreizung wurde nicht beobachtet. Es wurden keine PICC-Line-assoziierten Thrombophlebitiden oder Thrombosen festgestellt. Eine Patientin beendete bei fehlender Compliance vorzeitig die Therapie. Zehn Patienten konnten ihre Behandlung erfolgreich abschließen und sind in das Follow-up eingeschlossen worden. Bei einem mittleren Nachuntersuchungszeitraum von 29 Monaten liegt die anhand der Delphi-Kriterien gemessene Infekteradikation bei 100%. Schlussfolgerung Die Verwendung von PICC Lines zur ambulanten i. v. Antibiotikatherapie bei periprothetischer Infektion mit Difficult-to-treat-Erregern zeigt bei der Limitierung durch die geringe Patientenzahl vielversprechende kurzfristige Ergebnisse. Neben der kontinuierlichen Umsetzung einer resistogrammgerechten antimikrobiellen Therapie im ambulanten Bereich ist eine Reduktion der Behandlungskosten durch verkürzte Liegezeiten ein weiterer Vorteil.
Radiuskopffrakturen sind die häufigsten Frakturen am Ellenbogengelenk des Erwachsenen. Gute klinische Ergebnisse werden bei Radiuskopffrakturen Typ Mason II ohne Begleitverletzungen sowohl mit konservativer als auch operativer Therapie erzielt. Daten bezüglich der Instabilität des Ellenbogengelenks nach konservativ versorgten Radiuskopffrakturen im Vergleich zu operativ versorgten Radiuskopffrakturen liegen nicht vor.
BACKGROUND:Radial head fractures are the most frequent fractures of the elbow joint in adults. For Mason type II fractures without concomitant injuries favorable results have been shown with operative and conservative management. There is insufficient evidence concerning elbow joint stability after conservative treatment compared to open reduction and internal fixation (ORIF). MATERIALS AND METHODS:All patients with isolated Mason type II (two part fracture displaced >2 mm and <5 mm) radial head fractures between 1 January 2003 and 1 April 2013 were retrospectively reviewed. Exclusion criteria were age <18 years, associated fractures of the ipsilateral extremity or elbow luxation. A total of 50 patients (mean age 44.2 years, range 19-71 years) who received either ORIF (n = 31) or conservative treatment (n = 19) were included. The mean follow-up was 43.2 months (range 9-61 months). Patients were evaluated using the Disability of the Arm, Shoulder and Hand (DASH) score, Mayo Elbow Performance score (MEPS), and Oxford Elbow score (OES). Joint stability (varus and valgus stress) was assessed under fluoroscopy and a distinction was made between slight instability (angulation <10°), moderate instability (angulation ≥10°) and gross instability (elbow dislocation). RESULTS:Residual fracture displacement (conservative: 2.7 mm, ORIF: 1.4 mm, p < 0.042) and varus/valgus joint stability (3% ORIF vs. 26% conservative, p = 0.031) showed significant differences. The stability as tested by a radiological dynamic procedure showed an instability after ORIF in 3% of the joints compared to 26% after conservative treatment; however, this did not influence the short to mid-term clinical outcome: No significant differences were found in the DASH score (conservative 33 points, ORIF 36 points), MEPS (conservative 76 points, ORIF 78 points) and OES (conservative 41 points, ORIF 43 points). DISCUSSION:Both conservative management and operative treatment had a good functional outcome. Operative treatment showed a positive tendency concerning radiological and functional outcome without statistical significance.
Rezidivierenden Schulterinstabilitäten liegt häufig ein Randdefekt der anterioren Cavitas glenoidalis scapulae (Glenoid) zugrunde. Grund hierfür können rezidivierende Luxationen mit Erosion des Glenoids oder Frakturen des anterioren Glenoidrands sein. Behandlungskonzepte für diese Entitäten unterscheiden sich maßgeblich. Während in der akuten Fraktursituation konservativ oder operativ therapiert werden kann, sollte bei fehlendem Fragment, insbesondere bei einer Defektgröße über 15–25 % der Glenoidfläche, die Möglichkeit einer Augmentation zur Stabilisierung der Schulter bedacht werden. Ziel des vorliegenden Beitrags ist die Zusammenfassung von Diagnostik und Therapie der akuten Fraktursituation sowie der chronischen Instabilität bei anteriorem Glenoidranddefekt.
Abstract Introduction Bilateral acute proximal humerus fractures are rare. There are no data available about these bilateral injuries. The aim of the study was to analyse bilateral proximal humerus fractures retrospectively in terms of incidence, complications and revisions. Methods All bilateral proximal humerus fractures were evaluated retrospectively using the institutionʼs database, with the focus on cause of the injury, fracture severity and the clinical course compared to published information on monolateral proximal humerus fractures. Bilateral posterior dislocation fractures were excluded, because these fractures are a separate entity. Results Between 2005 and 2016, n = 17 patients were primarily treated within our hospital for an acute proximal humerus fracture on both sides (n = 12 female, n = 5 male, average age: 68 years; overall 34 proximal humerus fractures). The general trauma mechanism was a fall on both arms (82% [18% polytrauma]). There were 65% displaced 3-/4-part proximal humerus fractures. Angle-stable plate osteosynthesis was performed predominantly (64%), followed by fracture prosthesis (18%; tension wiring: 3%; non-operatively: 15%). Overall, n = 10 patients (59%) or n = 18 (53%) proximal humerus fractures developed a complication, primarily with loss of reduction or implant loosening (44%). In n = 14 (78%) of the complications further operations were necessary. Alcohol abuse was increasingly found in 29% of the cases within the bilateral patient cohort compared to patients with monolateral fractures. Conclusion Bilateral proximal humerus fractures are mainly associated with comminuted displaced fractures and a higher complication rate in comparison to monolateral fractures after surgical treatment. Zusammenfassung Einleitung Die akute beidseitige proximale Humerusfraktur ist eine sehr seltene Verletzungskombination. Derzeit liegen innerhalb der Literatur keine Daten über diese bilateralen Verletzungen vor. Das Ziel war es, die vorhandenen Fälle beidseitiger Humeruskopffrakturen retrospektiv hinsichtlich Inzidenz, Komplikations- und Revisionsrate zu analysieren. Methodik Sämtliche Datensätze beidseitiger proximaler Humerusfrakturen wurden retrospektiv ausgewertet. Dabei wurde der Fokus auf Ursache der Verletzung, Frakturschwere und den klinischen Verlauf gelegt und mit monolateralen proximalen Humerusfrakturen innerhalb der Literatur verglichen. Ausgeschlossen wurden beidseitige posteriore Humeruskopfluxationsfrakturen, da diese Frakturen eine eigene Entität darstellen. Ergebnisse Insgesamt wurden im Zeitraum zwischen 2005 und 2016 n = 17 Patienten mit einer beidseitigen frischen proximalen Humerusfraktur in unserem Hause primär behandelt (n = 12 Frauen, n = 5 Männer, Durchschnittsalter: 68 Jahre; insgesamt 34 proximale Humerusfrakturen). Der übliche Traumamechanismus war ein Sturz auf beide Arme (82% [18% Polytrauma]). Dabei lagen zu 65% dislozierte proximale 3-/4-Part-Humerusfrakturen vor. Primär wurden überwiegend winkelstabile Plattenosteosynthesen durchgeführt (64%), gefolgt von Frakturprothesen (18%; Zuggurtung: 3%; konservativ: 15%). Insgesamt kam es in n = 10 Patienten (59%) bzw. bei n = 18 (53%) proximalen Humerusfrakturen zu einer Komplikation vorwiegend mit Osteosyntheseversagen (44%). In n = 14 (78%) der Komplikationen waren Folgeoperationen notwendig. Ein C2-Abusus war mit 29% vermehrt innerhalb des bilateralen Kollektivs im Vergleich zu Patienten mit monolateralen Frakturen nachweisbar. Schlussfolgerung Im Vergleich zu monolateralen proximalen Humerusfrakturen gehen beidseitige proximale Humerusfrakturen überwiegend mit mehrfragmentären dislozierten Frakturen einher. Auch die Komplikationsrate nach operativer Versorgung liegt dabei höher als die der Versorgungen bei den monolateralen Frakturen.

Die humeralen und/oder glenoidalen Defekte können durch aseptische und septische Prothesenlockerungen und im Rahmen des Ausbaus der Prothesenkomponenten entstehen.
Background: How the shape of the glenoid defect being reconstructed influences stability in reversed shoulder arthroplasty has never been evaluated. The purpose of this study was to compare the reconstruction of two different shaped defects in reversed shoulder arthroplasty. Methods: Two groups (ten Sawbone scapulae each) of oblique- and rectangular-shaped glenoid defects were tested biomechanically. On the anterior half of the glenoid, bony defects (rectangular and oblique shaped) were prepared and reconstructed subsequently with a graft and reversed shoulder arthroplasty. As a control group, Sawbones without glenoid deficiency were used. In addition, these tests were reproduced in cadavers. Findings: In Sawbones, no significant difference in initial stability was found between the two groups (p > 0.05). Additionally, in the cadaver tests no significant difference was found between the groups with different defects (p > 0.05). During the preparation, macroscopic loosening of the oblique bone grafts was found in three cases after the performance of the reversed shoulder arthroplasty due to the lack of medial support. The localization of the highest micromotion were measured primarily between the scapula bone and the graft compared to the measured micromotions between glenoid implant and the graft. Interpretation: If the oblique-shaped bone graft was secured under the baseplate, the rectangular defect preparation did not show a significantly higher primary stability. However, the advantage of medial support in rectangular defects leads to more stability while placing the bone graft and baseplate during the surgical technique and should therefore be considered a preferable option.
