BACKGROUND:Six thoracic pathologists reviewed 259 lung neuroendocrine tumours (LNETs) from the lungNENomics project, with 171 of them having associated survival data. This cohort presents a unique opportunity to assess the strengths and limitations of current World Health Organization (WHO) classification criteria and to evaluate the utility of emerging markers. PATIENTS AND METHODS:Patients were diagnosed based on the 2021 WHO criteria, with atypical carcinoids (ACs) defined by the presence of focal necrosis and/or 2-10 mitoses per 2 mm2. We investigated two markers of tumour proliferation: the Ki-67 index and phospho-histone H3 (PHH3) protein expression, quantified by pathologists and automatically via deep learning. Additionally, an unsupervised deep learning algorithm was trained to uncover previously unnoticed morphological features with diagnostic value. RESULTS:The accuracy in distinguishing typical from ACs is hampered by interobserver variability in mitotic counting and the limitations of morphological criteria in identifying aggressive cases. Our study reveals that different Ki-67 cut-offs can categorise LNETs similarly to current WHO criteria. Counting mitoses in PHH3+ areas does not improve diagnosis, while providing a similar prognostic value to the current criteria. With the advantage of being time efficient, automated assessment of these markers leads to similar conclusions. Lastly, state-of-the-art deep learning modelling does not uncover undisclosed morphological features with diagnostic value. CONCLUSIONS:This study suggests that the mitotic criteria can be complemented by manual or automated assessment of Ki-67 or PHH3 protein expression, but these markers do not significantly improve the prognostic value of the current classification, as the AC group remains highly unspecific for aggressive cases. Therefore, we may have exhausted the potential of morphological features in classifying and prognosticating LNETs. Our study suggests that it might be time to shift the research focus towards investigating molecular markers that could contribute to a more clinically relevant morpho-molecular classification.
L’objectif de cette étude rétrospective était de comparer la survie et la tolérance des inhibiteurs de tyrosine kinase (ITK) de première et de seconde génération chez des patients atteints d’adénocarcinomes bronchiques métastatiques mutés EGFR non sélectionnés rencontrés en pratique courante. Dans cette étude multicentrique, les données de tous les patients atteints d’adénocarcinome de stade IV muté EGFR et traités par ITK (1re ou 2e génération) entre 2010 et 2018 ont été revues. Les patients ont été assignés en 2 groupes : ITK de 1re génération (erlotinib et gefitinib) vs ITK de 2e génération (afatinib). La survie a été évaluée à l’aide d’un test du Log-rank et du modèle à risque proportionnel de Cox. Deux cent soixante-huit patients ont été inclus dont 221 (82,5 %) traités par ITK de 1re génération et 47 (17,5 %) par afatinib. Dans les 2 groupes, l’âge médian des patients était similaire (68,40 ans [IQR : 8,28] vs 68,43 ans [IQR : 6,54], p = 0,89), de même que la proportion de femmes (82,4 % vs 87,2 %, p = 0,42). Les patients du groupe afatinib étaient plus altérés sur le plan général (PS ≥ 2 : 21,3 % vs 13,6 %, p = 0,004), avaient une maladie plus avancée (stade M1b ou M1c : 80,9 % vs 63,3 %, p = 0,004) avec plus de métastases cérébrales, et étaient porteurs de mutations plus rares (25,5 % vs 5,5 %, p < 0,0001). En analyse univariée, la médiane de survie globale sous erlotinib était de 24 mois [IC95 % : 22,1–25,9]. Elle était significativement meilleure en comparaison au gefitinib et à l’afatinib ; respectivement 21 mois [IC95 % : 17,7–24,3] et 14 mois [IC95 % : 11,9–16,0], p = 0,03. Cependant, aucune différence significative n’a été observée entre les groupes 1re et 2nde génération, p = 0,46. En analyse multivariée, après ajustement sur les facteurs pronostiques identifiés en analyse univariée, on retrouve un impact pronostique de l’erlotinib comparativement à l’afatinib, HR = 3,76 [IC95 % : 1,97–7,21 ; p < 0,0001]. Il y a eu plus d’effets secondaires dans le groupe 2nde génération par rapport au groupe 1re génération : toxicité cutanée (68,1 % vs 41,6 %, p = 0,001) et digestive (61,7 % vs 34,8 %, p = 0,0001). Dans cette étude, il n’a pas été mis en évidence de bénéfice de l’afatinib, comparativement aux ITK de 1re génération en termes de survie chez les patients fragiles.
L’immunothérapie est le standard de traitement en 2e intention des carcinomes bronchiques non à petites cellules. L’objectif de ce travail est d’évaluer si les différentes substitutions d’acide aminé de Kirsten rat sarcoma viral oncogene homolog (KRAS) sont des facteurs pronostiques de réponse à l’immunothérapie chez les patients porteurs d’un adénocarcinome pulmonaire en récidive. Étude rétrospective, multicentrique, de patients porteurs d’adénocarcinomes pulmonaires de stade III ou IV, en récidive, dont le statut mutationnel était connu, et traités par immunothérapie entre 2015 et 2018. Les critères de jugement étaient la survie globale (SG), la survie sans progression (SSP), la réponse tumorale selon les critères RECIST, et la tolérance du traitement. Parmi les 674 patients, 248 (36,8 %) avaient une mutation KRAS, essentiellement du codon 12 (n = 240, 96,8 %), et 426 (63,2 %) étaient KRAS wild type (WT). Les 2 groupes étaient comparables en termes d’âge (âge médian de 64,04 ans) et de sexe (65,9 % d’hommes). Le groupe KRAS présentait moins de cancers de stade IV (n = 212, 85,5 %) et de métastases cérébrales (n = 52, 21 %) que le groupe WT (stade IV : n = 402, 94,4 %, p < 0,001 ; métastases cérébrales : n = 156, 36,6 %, p < 0,001), mais avait un Performans Status plus élevé (p = 0,001). Les SG et la SSP globales de la cohorte étaient de 15 et 12 mois respectivement. Les SG et SSP médianes étaient meilleures dans le groupe KRAS (SG : 21 mois ; SSP : 15 mois) comparé au groupe WT (SG : 13 mois, p = 0,009 ; SSP : 3 mois, p = 0,01). Les patients avec les mutations KRAS G12 V (n = 60, 24,2 %) et G12 C (n = 112, 45,2 %) avaient de meilleures SG et SSP (G12 V : SG : 28 mois, SSP : 17 mois ; G12 C : SG : 23 mois, SSP : 11 mois) que les patients WT (SG : 13 mois, p < 0,0001 ; SSP : 3 mois, p < 0,0001), et que les patients mutés KRAS G13X (SG : 1 mois, p < 0,0001 ; SSP : 0 mois, p < 0,0001). La réponse à l’immunothérapie était meilleure dans le groupe KRAS comparé au groupe WT (27,9 % vs 20,9 %, p = 0,05), notamment pour les mutants KRAS G12 V (OR : 5,75 [IC95 % : 2,29–8,30], p = 0,02) et G12 C (OR : 4,33 [IC95 % : 2,23–8,80], p = 0,01). La tolérance de l’immunothérapie était similaire chez les patients KRAS et WT. Les transversions G12 V et G12D de KRAS seraient un facteur prédictif de bonne réponse à l’immunothérapie chez les patients suivis pour un adénocarcinome pulmonaire en récidive.
The worldwide incidence of pulmonary carcinoids is increasing, but little is known about their molecular characteristics. Through machine learning and multi-omics factor analysis, we compare and contrast the genomic profiles of 116 pulmonary carcinoids (including 35 atypical), 75 large-cell neuroendocrine carcinomas (LCNEC), and 66 small-cell lung cancers. Here we report that the integrative analyses on 257 lung neuroendocrine neoplasms stratify atypical carcinoids into two prognostic groups with a 10-year overall survival of 88% and 27%, respectively. We identify therapeutically relevant molecular groups of pulmonary carcinoids, suggesting DLL3 and the immune system as candidate therapeutic targets; we confirm the value of OTP expression levels for the prognosis and diagnosis of these diseases, and we unveil the group of supra-carcinoids. This group comprises samples with carcinoid-like morphology yet the molecular and clinical features of the deadly LCNEC, further supporting the previously proposed molecular link between the low-and high-grade lung neuroendocrine neoplasms.
Context.— Pleural mesothelioma is a rare cancer with an often-challenging diagnosis because of its potential to be a great mimicker of many other tumors. Among them, primary lung and breast cancers are the 2 main causes of pleural metastasis. The development and application of targeted therapeutic agents have made it even more important to achieve an accurate diagnosis. In this setting, international guidelines have recommended the use of 2 positive and 2 negative immunohistochemical biomarkers. Objectives.— To define the most highly specific and sensitive minimum set of antibodies for routine practice to use for the separation of epithelioid malignant mesothelioma from lung and breast metastasis and to determine the most relevant expression cutoff. Design.— To provide information at different levels of expression of 16 mesothelial and epithelial biomarkers, we performed a systematic review of articles published between 1979 and 2017, and we compared those data to results from the Mesothelioma Telepathology Network (MESOPATH) of the standardized panel used in routine practice database since 1998. Results.— Our results indicate that the following panel of markers—calretinin (poly)/thyroid transcription factor 1 (TTF-1; clone 8G7G3/1) and calretinin (poly)/estrogen receptor-α (ER-α; clone EP1)—should be recommended; ultimately, based on the MESOPATH database, we highlight their relevance which are the most sensitive and specific panel useful to the differential diagnosis at 10% cutoff. Conclusions.— Highlighted by their relevance in the large cohort reported, we recommend 2 useful panels to the differential diagnosis at 10% cutoff.
INTRODUCTION:The 2015 WHO classification of tumors categorized malignant mesothelioma into epithelioid, biphasic (BMM), and sarcomatoid (SMM) for prognostic relevance and treatment decisions. The survival of BMM is suspected to correlate with the amount of the sarcomatoid component. The criteria for a sarcomatoid component and the interobserver variability between pathologists for identifying this component are not well described. In ambiguous cases, a "transitional" (TMM) subtype has been proposed but was not accepted as a specific subtype in the 2015 WHO classification. The aims of this study were to evaluate the interobserver agreement in the diagnosis of BMM, to determine the nature and the significance of TMM subtype, and to relate the percentage of sarcomatoid component with survival. The value of staining for BRCA-1-associated protein (BAP1) and CDKN2A(p16) fluorescence in situ hybridization (FISH) were also assessed with respect to each of the tumoral components. METHODS:The study was conducted by the International Mesothelioma Panel supported by the French National Cancer Institute, the network of rare cancer (EURACAN) and in collaboration with the International Association for the Study of Lung Cancer (IASLC). The patient cases include a random group of 42 surgical biopsy samples diagnosed as BMM with evaluation of SMM component by the French Panel of MESOPATH experts was selected from the total series of 971 BMM cases collected from 1998 to 2016. Fourteen international pathologists with expertise in mesothelioma reviewed digitally scanned slides (hematoxylin and eosin - stained and pan-cytokeratin) without knowledge of prior diagnosis or outcome. Cases with at least 7 of 14 pathologists recognizing TMM features were selected as a TMM group. Demographic, clinical, histopathologic, treatment, and follow-up data were retrieved from the MESOBANK database. BAP1 (clone C-4) loss and CDKN2A(p16) homozygous deletion (HD) were assessed by immunohistochemistry (IHC) and FISH, respectively. Kappa statistics were applied for interobserver agreement and multivariate analysis with Cox regression adjusted for age and gender was performed for survival analysis. RESULTS:The 14 panelists recorded a total of 544 diagnoses. The interobserver correlation was moderate (weighted Kappa = 0.45). Of the cases originally classified as BMM by MESOPATH, the reviewers agreed in 71% of cases (385 of 544 opinions), with cases classified as pure epithelioid in 17% (93 of 544), and pure sarcomatoid in 12% (66 of 544 opinions). Diagnosis of BMM was made on morphology or IHC alone in 23% of the cases and with additional assessment of IHC in 77% (402 of 544). The median overall survival (OS) of the 42 BMM cases was 8 months. The OS for BMM was significantly different from SMM and epithelioid malignant mesothelioma (p < 0.0001). In BMM, a sarcomatoid component of less than 80% correlated with a better survival (p = 0.02). There was a significant difference in survival between BMM with TMM showing a median survival at 6 months compared to 12 months for those without TMM (p < 0.0001). BAP1 loss was observed in 50% (21 of 42) of the total cases and in both components in 26%. We also compared the TMM group to that of more aggressive patterns of epithelioid subtypes of mesothelioma (solid and pleomorphic of our large MESOPATH cohort). The curve of transitional type was persistently close to the OS curve of the sarcomatoid component. The group of sarcomatoid, transitional, and pleomorphic mesothelioma were very close to each other. We then considered the contribution of BAP1 immunostaining and loss of CDKN2A(p16) by FISH. BAP1 loss was observed in 50% (21 of 41) of the total cases and in both component in 27% of the cases (11 of 41). There was no significant difference in BAP1 loss between the TMM and non-TMM groups. HD CDKN2A(p16) was detected in 74% of the total cases with no significant difference between the TMM and non-TMM groups. In multivariate analysis, TMM morphology was an indicator of poor prognosis with a hazard ratio = 3.2; 95% confidence interval: 1.6 - 8.0; and p = 0.003 even when compared to the presence of HD CDKN2A(p16) on sarcomatoid component (hazard ratio = 4.5; 95% confidence interval: 1.2 - 16.3, p = 0.02). CONCLUSIONS:The interobserver concordance among the international mesothelioma and French mesothelioma panel suggests clinical utility for an updated definition of biphasic mesothelioma that allows better stratification of patients into risk groups for treatment decisions, systemic anticancer therapy, or selection for surgery or palliation. We also have shown the usefulness of FISH detection of CDKN2A(p16) HD compared to BAP1 loss on the spindle cell component for the separation in ambiguous cases between benign florid stromal reaction from true sarcomatoid component of biphasic mesothelioma. Taken together our results further validate the concept of transitional pattern as a poor prognostic indicator.
Purpose. - Prophylactic radiotherapy to prevent procedure-tracts metastases from malignant pleural mesothelioma remains controversial and clinical practice varies. The purpose was to assess the efficacy of local radiotherapy in a single fraction of 10 Gy in preventing malignant seeding at intervention pleural site in patients with malignant pleural mesothelioma. Material and methods. - This is a retrospective cohort study, including patients with histological confirmed malignant pleural mesothelioma treated by prophylactic irradiation to prevent interventional site metastases with a unique fraction of 10 Gy with 6 to 18 MeV, from January 1990 to December 2013 in the institut de cancerologie de Lorraine (Nancy, France). Results. - Ninety-one patients were treated by irradiation in intervention site, involving 120 intervention pleural sites, 91 thoracoscopies, 17 thoracotomies with chest drain and 12 CT or ultrasound guided needle biopsies. The median follow-up was 7 months (interquartile between 3 and 15 months). The overall survival was 43.5% at 12 months. The local progression free survival was 43.7% at 12 month. The incidence of local recurrence was 8% at 12 months. The median interval from radiotherapy to local recurrence was 4 months (2; 32). No grade II or higher toxicity was observed. Conclusion. - Irradiation of pleural intervention sites with a single fraction of 10 Gy is effective, well tolerated, simple, fast and cost effective. L’influence de l’âge sur le délai de prise en charge du cancer bronchique (CB) est controversée. L’objectif principal de cette étude était de déterminer la relation entre le délai de prise en charge du CB et l’âge des patients dans notre institution. Une étude rétrospective monocentrique a été réalisée chez les patients ayant reçu un premier traitement spécifique pour un CB entre le 1er novembre 2014 et le 31 octobre 2015. Le délai de prise en charge était défini par le délai entre la première imagerie pathologique et le premier traitement reçu (chirurgie, radiothérapie, thérapie ciblée, chimiothérapie). Les patients ont été répartis en 3 tranches d’âges : moins de 60 ans, 60 à 70 ans et plus de 70 ans. Les tests du Chi2 de Pearson pour comparer les variables qualitatives et Anova pour les variables quantitatives ont été effectués. Le seuil de significativité des tests était fixé à 5 %. Deux cent quarante-six patients ont été inclus. L’âge moyen était de 64,9 ans. Les délais médian et moyen de prise en charge du CB étaient de 69 et 96,8 jours. Le délai moyen de prise en charge des patients de plus de 70 ans était plus long que pour celui des moins de 60 ans et de 60 à 70 ans. (116,2 jours vs 83,6 jours et 92,1 jours, p = 0,04). Les délais médian et moyen de prise en charge du CB étaient de 69 et 96,8 jours. Le stade de la maladie et le type de traitement reçu n’étaient pas différents selon les classes d’âges (p = 0,238 ; p = 0,511). Les CB à petites cellules étaient pris en charge plus rapidement que les carcinomes épidermoïdes et les adénocarcinomes (42,8 jours vs 99,3 jours et 99 jours, p = 0,019). Les CB de stade IV étaient pris en charge plus rapidement que ceux de stades I, II et III (65,2 jours vs 118,4 jours, 92,3 jours et 102,5 jours, p = 0,001). Le délai de prise en charge n’était pas différent selon le type de traitement reçu (p = 0,087). Cette étude confirme l’allongement du délai de prise en charge chez les personnes âgées déjà décrit dans la littérature. Selon les recommandations internationales une réduction du délai de mise en œuvre des traitements est nécessaire dans notre filière de soins.
Doege-Potter syndrome is a paraneoplastic syndrome characterized by non-islet cell tumor hypoglycemia secondary to a solitary fibrous tumor. These tumors are rare and usually asymptomatic. The syndrome of hypoglycemia is seen in less than 5% of the cases, and the associated tumors are large with a high mitotic rate. The cause of hypoglycemia is related to insulin-like growth factors produced by these tumors called "big" IGF-2. Several biological tests can demonstrate the increase of "big" IGF-2 plasma levels confirming the diagnosis of non-islet cell tumor induced hypoglycemia. The diagnosis is suggested by imaging but diagnostic confirmation is provided by the surgery, which remains the treatment of choice. Resection in many cases is the cure leading to hypoglycemia resolution. Recurrences and malignant transformations are possible which imposes a long-term monitoring. We report a case with relapsed malignant pleural fibrous tumor for which the pathophysiological mechanism of hypoglycemia could be documented as a paraneoplastic syndrome. L’histiocytose langerhansienne (HL) est liée à une infiltration spécifique d’organe de cellules présentant les mêmes caractéristiques morphologiques et immunohistochimiques que les cellules de Langerhans. La maladie peut être divisée en 2 catégories : les formes monotissulaires (diffuses ou localisées), et les formes systémiques. Nous rapportons un cas d’HL avec une atteinte génitale isolée. Un jeune homme de 14 ans était adressé en consultation pour une curieuse éruption génitale évoluant depuis 1 an. À l’examen clinique, il présentait une dizaine de lésions papuleuses, non ombiliquées, jaunâtres, mesurant 3 à 4 mm, parfois ulcérées, localisées au niveau du sillon balano-préputial et du fourreau de la verge. Les hypothèses évoquées étaient : molluscum contagiosum ou xanthomes éruptifs. L’analyse histologique révélait un infiltrat inflammatoire granulomateux polymorphe associant histiocytes et polynucléaires neutrophiles et l’immunohistochimie mettait en évidence des marqueurs CD1a et PS100 positifs. Le diagnostic d’HL monotissulaire localisée était posé. Une consultation de suivi 2 mois plus tard révélait une disparition de la plupart des lésions tandis que quelques lésions persistantes prenaient un aspect ulcéré. L’HL est une affection rare qui concerne principalement les enfants avec une prévalence d’environ 3 à 5 cas/millions d’enfants. Les histiocytoses langerhansiennes monotissulaires peuvent être purement cutanées et sont alors plus fréquentes chez le grand enfant et l’adulte. Les signes cutanés peuvent correspondre aux formes classiques d’histiocytose langerhansienne avec des lésions papulo-croûteuses des plis ou se présenter sous des formes trompeuses. Environ 25 % des HL ont une atteinte cutanée et/ou muqueuse, cependant une atteinte muqueuse isolée du pénis est exceptionnelle. Dans la littérature, seulement 7 cas d’atteinte du pénis sont rapportés dont 4 concernent des enfants. Le pronostic des formes monotissulaires est relativement bon avec l’observation de rémissions spontanées en quelques mois à plusieurs années. Cependant, elles peuvent être la présentation initiale d’une forme systémique de moins bon pronostic avec la possibilité d’atteintes multiviscérales. Les organes atteints peuvent être les os, la peau, les ganglions lymphatiques, les poumons et le système nerveux central. La surveillance clinique des lésions localisées devra être prolongée afin de s’assurer du caractère purement cutané de l’affection et surveiller l’absence d’évolution systémique. Nous présentons un cas atypique d’histiocytose langerhansienne monotissulaire du pénis. Malgré le faible nombre de cas rapporté dans la littérature, il est important de ne pas méconnaître cette présentation rare de la maladie et de procéder à un suivi prolongé du patient.
Les réarrangements du gène ALK dans les adénocarcinomes pulmonaires (ADK) induisent une activation constitutive de la kinase ALK, ciblable par le crizotinib [1], [2]. Bien que la FISH (hybridation in situ fluorescente) soit la technique de référence pour le diagnostic de ces réarrangements, l’immunohistochimie (IHC) pourrait être utile en tant qu’outil de pré-screening. Comparer les techniques d’IHC, FISH et RT-qPCR ALK réalisées par 14 plateformes de génétique moléculaire des cancers (PGMC). Pour cette étude méthodologique, 547 ADK, enrichis en prélèvements chirurgicaux et en cas positifs, ont été analysés, en utilisant les anticorps 5A4 et D5F3 (IHC), 2 sondes break-apart (FISH), et des kits TaqMan (RT-qPCR), et les données clinicopathologiques des patients renseignées. Au total, 140 tumeurs présentaient un réarrangement du gène ALK par FISH (≥ 15 % de cellules réarrangées) et 400 étaient négatives (< 15 %). La FISH était ininterprétable pour 7 cas. Les patients ALK-positifs étaient plutôt jeunes (p = 0,003), en majorité des femmes (p = 0,007) et petits- ou non-fumeurs (p < 0,0001). Treize cas FISH-positifs étaient négatifs par IHC, dont 6 avec une FISH entre 15 et 20 %. Huit cas avaient une IHC positive, avec une FISH entre 10 et 14 %. Huit sur 13 cas IHC/FISH discordants venaient d’une même PGMC. La sensibilité et la spécificité des anticorps 5A4 et D5F3 était de 87 % et 92 %, et 89 % et 76 %, respectivement. Le pourcentage de faux-négatifs de l’IHC, observés dans 2,4 % des cas, s’abaissait à 1,3 % pour les cas avec FISH >20 %. Aucun variant de transcription n’a été détecté dans 36 % des cas ALK-positifs. L’analyse des résultats a mis en évidence que les discordances IHC/FISH sont plus fréquentes pour les cas « borderline » (FISH entre 10 et 20 %), et qu’elles sont PGMC-dépendantes. L’IHC est donc un outil de préscreening fiable pour la détection des réarrangements du gène ALK.
Les thérapeutiques ayant pour cible les protéines des signalisations cellulaires contrôlant la division cellulaire (mitose) et l’angiogenèse sont apparues dans l’arsenal thérapeutique du cancer bronchique non à petites cellules (CBNPC) au cours des cinq dernières années, avec des résultats déjà spectaculaires dans certains sous-groupes de patients, annonçant pour l’avenir des progrès thérapeutiques encore plus importants. Les molécules inhibant le récepteur à l’EGF ont fait l’objet de plusieurs essais de phase 2 et 3, ayant inclus plusieurs milliers de patients. Leur efficacité est désormais établie en deuxième et troisième ligne, après chimiothérapie à base de sel de platine, dans les adénocarcinomes non mucineux, chez les non-fumeurs, les femmes et les patients asiatiques, avec des taux de réponse en monothérapie allant de 10 % pour les patients caucasiens non sélectionnés, à 40 % pour les sujets asiatiques non fumeurs, et des survies prolongées. Le ciblage thérapeutique augmente les chances de succès, qu’il repose sur la recherche d’amplification génique de l’EGFR par FISH, ou la recherche de mutations du domaine tyrosine kinase de l’EGFR. Dans ce dernier cas, des kits commerciaux permettront dans un proche avenir un diagnostic moléculaire de routine, et un ciblage moléculaire venant affiner le ciblage clinique. Les inhibiteurs de l’angiogenèse, et particulièrement l’anticorps monoclonal anti-VEGF, le bevacizumab, connaissent aussi un développement spectaculaire. Le bevacizumab, associé à une chimiothérapie classique, a permis, chez des patients sélectionnés (cancers non épidermoïdes, absence d’antécédents cardiovasculaires), de franchir le cap des douze mois de médiane de survie au prix d’une toxicité acceptable. Déjà, d’autres molécules se profilent, ayant à la fois une activité anti-angiogénique et une activité anti-EGFR, l’avenir étant aux molécules inhibant plusieurs signalisations cellulaires simultanément.Therapies targeted on cell signal pathways that control cell division and tumor angiogenesis have been developed over the last five years for non small cell lung cancer (NSCLC) with some amazing results, in subgroups of selected patients, predicting more significant success in the upcoming years. Compounds targeted on EGF tyrosine kinase receptor have been tested in large clinical phase 2 and 3 trials including thousands of patients. Their efficacy has been proved, in second and third line trials, after first line cisplatin-based chemotherapy for non-mucinous adenocarcinoma in non-smokers, women and Asian patients. Response rates vary from 10% in non selected Caucasian patients to 40% in non-smoking Asian patients with long survivals. Therapeutic targeting improves success rates, either relying on EGFR gene amplification detection by FISH, or search for EGFR tyrosine kinase domain mutations. Commercial kits are available for routine molecular diagnosis of domain mutations potentially enabling molecular targeting in addition to clinical targeting. Angiogenesis inhibitors, especially monoclonal antibody to VEGF, bevacizumab, have also been developed in the last few years. Bevacizumab associated with classical cytotoxic chemotherapy led, in selected patients (with non squamous cell lung cancer and no past history of cardiovascular disease) to an increase of median survival to more than 12 months with tolerable toxicity. Other drugs that have both anti-EGFR activity and anti-angiogenic properties will be soon developed, since future bioactive anti-cancer drugs will probably be multi-targeted drugs.
Ces dernières années, nous assistons chez les patients porteurs de carcinome bronchique non à petites cellules à un développement des thérapeutiques ciblant des voies de signalisations anormalement activées. Les mutations d’EGFR sont retrouvées dans environ 10 % des cancers bronchiques, particulièrement dans les adénocarcinomes, chez les femmes et les non-fumeurs. À ce jour, trois inhibiteurs de tyrosine kinase (ITK) d’EGFR, l’erlotinib, le gefitinib et l’afatinib ont une AMM européenne pour un traitement dès la 1re ligne dans le CBNPC muté EGFR avec une efficacité supérieure à celle de la chimiothérapie. Les ITK de 3e génération (rociletinib, AZD9291) sont efficaces chez les patients ayant développé une mutation de résistance secondaire de l’EGFR notamment la mutation T790M ; ils ont obtenu une ATU en 2015. La présence d’une translocation EML4-ALK est rare et retrouvée dans environ 5 % des CBNPC de type adénocarcinome et plus particulièrement chez les sujets jeunes non-fumeurs. Le crizotinib est un nouveau standard thérapeutique dans les CBNPC ALK+ en 2e ligne. Le ceritinib est un ITK d’ALK de 2e génération a l’AMM depuis 2015 pour les patients ALK+ ayant progressé sous crizotinib. L’INCa soutient le programme ACSé évaluant l’efficacité du crizotinib dans les cancers bronchiques amplifiés pour MET ou transloqués pour ROS1 et le programme ACSé évaluant l’efficacité du vemurafenib chez les patients présentant une mutation BRAF V600E dans des tumeurs hors mélanome. La place d’autres biomarqueurs tels que les mutations des gènes RET, KRAS, HER2 et PI3KCA reste à déterminer dans le cancer bronchique non à petites cellules.Recently, developments of therapies that target abnormally activated signaling pathways are increasing for patients with non-small cell lung cancer. EGFR mutations are found in about 10% of lung cancers, especially in adenocarcinoma, women and non-smokers. Three EGFR inhibitors (erlotinib, gefitinib and afatinib) received a European marketing authorization for up to first line treatment of EGFR mutated NSCLC. Effectiveness of EGFR inhibitors is higher than conventional chemotherapy. Third generation EGFR inhibitors (rociletinib, AZD9291) are effective for patients who develop a resistance mutation such as T790M resistance mutation; they obtained temporary authorization for use in France in 2015. The EML4-ALK translocation is found in about 5% of NSCLC and more particularly in adenocarcinoma of young non-smoking patients. Crizotinib is a new therapeutic standard in ALK translocated NSCLC in second line. Ceritinib is a 2nd generation ALK inhibitor which received a European marketing authorization for up to treatment of ALK translocated NSCLC after progression with crizotinib. INCA supports ACSé program evaluating the efficacy of crizotinib in NSCLC amplified for MET or translocated for ROS1 and ACSé program evaluating the efficacy of vemurafenib in tumors non melanoma mutated V600E BRAF. The role of other biomarkers such as KRAS, BRAF, HER2 and PI3KCA mutations remains to be defined in NSCLC.
BACKGROUND:Eccrine spiradenoma (ES) is a benign adnexal tumor predominantly located in the head and neck regions. Multiple neoplasms located on the scalp have been reported but never with a zosteriform configuration on the first trigeminal area. CASE REPORT:We describe an original case report of a 75-year-old Caucasian man presenting multiple subcutaneous blue and purple nodules disseminated on the first left trigeminal dermatome. All the nodules appeared gradually on a one-year period. Biopsy revealed a nodular adnexal tumor in the dermis without malignant eccrine spiradenoma (MES) transformation. The surgical procedure was performed in a manner to protect the galea aponeurotica in the upper half on the first left trigeminal area. The frontalis muscle was raised with the surgical specimen in the lower half of the first trigeminal area. A split-thickness skin graft was applied on the surgical defect. Histological examination revealed multilobular well-defined tumors located in the dermis. CONCLUSION:The presence of multiple subcutaneous nodules in a trigeminal pattern should suggest a multiple localized zosteriform ES. The diagnosis is focused on clinical findings and the treatment is based on a large surgical excision. The histological examination is essential for not to fail a MES transformation.
Mesothelioma is a rare disease less than 0.3% of cancers in France, very aggressive and resistant to the majority of conventional therapies. Asbestos exposure is nearly the only recognized cause of mesothelioma in men observed in 80% of case. In 1990, the projections based on mortality predicted a raise of incidence in mesothelioma for the next three decades. Nowadays, the diagnosis of this cancer is based on pathology, but the histological presentation frequently heterogeneous, is responsible for numerous pitfalls and major problems of early detection toward effective therapy. Facing such a diagnostic, epidemiological and medico-legal context, a national and international multidisciplinary network has been progressively set up in order to answer to epidemiological survey, translational or academic research questions. Moreover, in response to the action of the French Cancer Program (action 23.1) a network of pathologists was organized for expert pathological second opinion using a standardized procedure of certification for mesothelioma diagnosis. We describe the network organization and show the results during this last 15years period of time from 1998-2013. These results show the major impact on patient's management, and confirm the interest of this second opinion to provide accuracy of epidemiological data, quality of medico-legal acknowledgement and accuracy of clinical diagnostic for the benefit of patients. We also show the impact of these collaborative efforts for creating a high quality clinicobiological, epidemiological and therapeutic data collection for improvement of the knowledge of this dramatic disease.
Introduction. - Bronchiolo-alveolar carcinoma is a primary pulmonary adenocarcinoma developing in the terminal respiratory unit.Case report. - An 84-year-old non-smoker woman with a history of untreated acute myeloid leukaemia was referred to the intensive care unit for pneumonia and acute respiratory failure. The patient reported dyspnoea and a productive cough for 3 months, treated by antibiotics and steroids without improvement. Thoracic CT-scan showed alveolar condensations and multiple nodular lesions. All microbiological samples were negative and the evolution was fatal within 72 hours despite empirical antibiotic therapy and noninvasive ventilation. Post-mortem lung biopsies gave a diagnosis of mucinous and non-mucinous bronchiolo-alveolar carcinoma with typical lepidic growth pattern of tumor cells and discrete septal thickening but no fibrosis, inflammation or local invasion.Conclusion. - Bronchiolo-alveolar carcinoma is an alternative diagnosis in alveolar condensations associated with pulmonary nodules even in a patient with immunosupression. BACKGROUND:Non-small-cell lung carcinoma (NSCLC) patients with a BRAF(V600E) mutation benefit from targeted therapy. The usefulness of immunohistochemistry (IHC) as an alternative approach for the detection of BRAF(V600E) in NSCLC patients has not been evaluated until now. This study compared the specificity and sensitivity of IHC with other methods for the detection of BRAF(V600E) in primary lung adenocarcinoma.PATIENTS AND METHODS:BRAF mutations were analysed by DNA sequencing of a Caucasian subpopulation of selected 450 of 1509 (30%) EGFR, KRAS, PI3KA, Her2 and EML4-ALK wild-type (wt) primary lung adenocarcinomas. Detection of the BRAF(V600E) mutation was carried out by IHC using the VE1 clone antibody and compared with the results of other molecular methodologies.RESULTS:Of 450 (9%) of tumours, 40 harboured a BRAF mutation, which corresponded to either a BRAF(V600E) or a non-BRAF(V600E) mutation in 21 of 450 (5%) and 19 of 450 (4%) cases, respectively. The IHC VE1 assay was positive in 19 of 21 (90%) BRAF(V600E)-mutated tumours and negative in all BRAF(nonV600E)-mutated tumours.CONCLUSION:IHC using the VE1 clone is a specific and sensitive method for the detection of BRAF(V600E) and may be an alternative to molecular biology for the detection of mutations in NSCLC.
Organizations responding to the 2014–2016 Ebola epidemic in Sierra Leone collected information from multiple sources and kept it in separate databases, including distinct data systems for Ebola hot line calls, patient information collected by field surveillance officers, laboratory testing results, clinical information from Ebola treatment and isolation facilities, and burial team records.After the conclusion of the epidemic, the Sierra Leone Ministry of Health and Sanitation and the U.S. Centers for Disease Control and Prevention partnered to collect these disparate records and consolidate them in the Sierra Leone Ebola Database.The Sierra Leone Ebola Database data are providing a lasting resource for postepidemic data analysis and epidemiologic research, including identifying best strategies in outbreak response, and are used to help families locate the graves of family members who died during the epidemic.This report describes the Ministry of Health and Sanitation and Centers for Disease Control and Prevention processes to safeguard Ebola records while making the data available for public health research.
