Osteoporosis-associated fractures represent a growing challenge in the treatment of orthopedic patients. In November 2014 a new revision of the guidelines on osteoporosis by the German Osteology Society (Dachverband Osteologie DVO) was adopted, in which additional risk factors for fractures and further treatment options have been included. On the one hand the existing model used to diagnose osteoporosis and estimate a high fracture risk as a guidance for the use of specific anti-osteoporotic therapy in patients without a fragility fracture was maintained and further refined. On the other hand the guideline includes the option to initiate a specific osteoporosis therapy without a prior bone densitometry in patients with typical radiographs of a proximal femur fracture and higher grade vertebral fractures, suspicious for osteoporosis, depending on the overall clinical context. This may reduce the treatment gap of osteoporosis in Germany. In this paper the changes in the DVO guidelines 2014 on osteoporosis are summarized, focusing on the most important changes with practical relevance for orthopedic surgeons.
Geeignete diagnostische Strategien zur Identifizierung von Personen mit einem hohen Frakturrisiko, die über allgemeine Maßnahmen der Frakturprophylaxe hinaus am meisten von einer spezifischen Therapie profitieren, sind für eine effiziente Therapie der Osteoporose von großer Bedeutung. Für den deutschsprachigen Raum gibt der Dachverband Osteologie (DVO) seit dem Jahr 2003 Leitlinienempfehlungen für die diagnostische Abklärung und Therapie einer Osteoporose heraus. Die letzte Aktualisierung war im November 2014. Die Leitlinie gibt differenzierte Empfehlungen für die Indikation zu einer Osteoporosediagnostik unter Einbeziehung des Lebensalters, des Geschlechts, und des Vorliegens und der Stärke klinischer Frakturrisikofaktoren. Die Zahl der Frakturrisikofaktoren, die die diagnostischen und therapeutischen Empfehlungen in der DVO-Leitlinie 2014 bestimmen, ist gegenüber der Vorversion größer geworden durch die zusätzliche Einbeziehung einer monoklonalen Gammopathie unklarer Signifikanz, der Spondylitis ankylosans, der COPD, der Herzinsuffizienz, der Zöliakie, des Diabetes mellitus Typ 2, der chronischen Therapie mit Protonenpumpeninhibitoren und der hochdosierten Therapie mit inhalativen Glukokortikoiden. Die DVO-Leitlinie 2014 empfiehlt allen Personen mit einem erhöhten Frakturrisiko eine sogenannte Basisdiagnostik, bestehend aus Anamnese, klinischer Untersuchung einschließlich der Erfassung des Sturzrisikos, DXA-Knochendichtemessung an der Lendenwirbelsäule, dem Schenkelhals und dem proximalen Gesamtfemur, Laboruntersuchung und ggf. einer zusätzlichen bildgebenden Diagnostik. Der Trabecular Bone Score bietet eine neue optionale diagnostische Möglichkeit der Frakturprädiktion.
SummaryThe Biomechanically founded individualised osteoporosis Assessment and treatment (BioAsset) consortium pursues experimental and clinical studies in the context of skeletal effects of bisphosphonate treatment. Here, first results using newly developed diagnostic methods in a set of vertebral bone specimen obtained from donors with documented bisphosphonate history ranging from 0 to more than 5 years of treatment are presented. A new thoracolumbar quantitative computed tomography (QCT) protocol covering T6 to L4 plus high-resolution QCT (HRQCT) assessment of T12 were compared with high-resolution peripheral QCT (HRpQCT) and micro-CT scans of excised specimens serving as gold standard techniques. Finite element (FE) modelling was performed. Material, ultrastructural, and micromechanical properties were tested on a set of single trabeculae obtained from the donor specimens. A newly developed quantitative ultrasound (QUS) device for measuring the anisotropy of cortical material properties at the tibia was designed and built. The thoracolumbar QCT protocol permitted in situ imaging with good image quality and automated segmentation of vertebral bodies in the whole range from T6 to L4. The duration of bisphosphonate treatment was significantly associated with increased levels of mineralization and this effect could be measured with HRQCT performed on excised specimens. Microstructural parameters contributed to vertebral bone strength modelled by FE analysis independently of bone mineral density. The new QUS tibia scanner permitted measuring the acoustical anisotropy of reference materials. Taken together, these results document that new methods developed in BioAsset permit a more comprehensive assessment of bone fragility. The set of donor specimens with a documented history of bisphosphonate treatment allows for the assessment of the effects of long-term treatment from the organ down to the tissue and material level. These results will ultimately be linked to the parallel clinical study to provide guidance for determining the optimum duration of bisphosphonate treatment to reduce the incidence of osteoporotic fractures.
Fragestellung und Methoden: Die Prävalenz einer chronischen Hyponatriämie in der älteren Bevölkerung ist hoch. Häufige Ursachen sind die Einnahme von Diuretika und das Syndrom der inappropriaten antidiuretischen Hormonsekretion (SIADH). Verbalis et al. haben in einem Rattenmodell für SIADH und in der Dritten Nationalen Gesundheitsund Ernährungsuntersuchungserhebung in den USA (NHANESIII) die Auswirkungen einer Hyponatriämie auf den Knochenstoffwechsel untersucht. Ergebnisse: Im Tiermodell führte eine 3-monatige Hyponatriämie zu einer signifikanten 30%igen Reduktion der DXAKnochendichte am Femur im Vergleich zu normonatriämischen Kontroll-Tieren. Ergänzende Untersuchungen mittels hochauflösendem CT zeigten eine ausgeprägte Reduktion sowohl der trabekulären als auch der kortikalen Knochenmasse. Histomorphometrisch war die Knochenresorption vermehrt und die Knochenneubildung vermindert. In der NHANES-IIIErhebung war eine milde Hyponatriämie in linearen Regressionsmodellen auch nach Adjustierung für Alter, Geschlecht, Rasse, BMI, physikalische Aktivität, DiuretikaGebrauch, Rauchen und Serum-25-Hydroxy-Vitamin-D-Konzentration mit einer erhöhten Wahrscheinlichkeit einer Osteoporose am proximalen Femur nach der WHO-Definition der Osteoporose (DXA-T-Wert ≤ -2,5) verbunden (Odds Ratio 2,85; 95 % KI 1,03–7,86, p < 0,01). Schlussfolgerung: Die Autoren folgern, dass eine chronische Hyponatriämie mit einer Reduktion der Knochenmasse einhergeht. Eine Hyponatriämie ist ein Risikofaktor für Frakturen
Insulin-like growth factor-I (IGF-I) is a well documented bone-active growth factor. Clinical studies reported that circulating hormones may affect serum IGF-I levels, with potential consequences on bone remodeling. However, no data on bone matrix concentrations of IGF-I in subjects with endocrine dysfunction is available in humans. Bone mineral density and cancellous bone matrix IGF-I levels were assessed in iliac crest biopsies from 38 patients with low bone mass related to glucocorticoid- (n=10), parathyroid- (n=14) or thyroid (n=14) hormones excess. Results were compared to those of sex- and age-matched patients with primary osteoporosis. Bone matrix extraction was performed based on a guanidine-chlorhidric acid/ethylendiamine-tetraacetic acid method. Long-term glucocorticoid therapy (> or =24 months) led to significantly lower cancellous bone matrix IGF-I levels in comparison to age-matched controls (p=0.03). Although higher trabecular bone IGF-I levels were seen in hyperparathyroid subjects, the difference was not significant in comparison to controls (p=0.24). Likewise, no difference was noticed in cancellous bone matrix IGF-I concentrations between subjects with low bone mass and sub-clinical or overt thyrotoxicosis and euthyroid controls. Neither parathyroid hormone (PTH) nor thyroxin (T (4)) concentrations were associated with bone matrix IGF-I levels. To conclude, our study documented that in vivo long-term corticotherapy is associated with low trabecular human bone matrix IGF-I. In contrast, no influence of increased circulating parathyroid- or thyroid hormones levels on human iliac crest skeletal IGF-I concentrations was observed.

Unser Beitrag zeigt, wie Sie mit spezifischen Problemen von Osteoporosepatienten umgehen und was Sie tun können, um Frakturen im Alter zu verhindern.
Ziel der Osteoporosetherapie ist die Prävention osteoporotischer Knochenbrüche und die Minimierung frakturassoziierter Schmerzen und funktioneller Einschränkungen. Je nach Höhe des geschätzten 10-Jahres-Frakturrisikos reichen die Therapieoptionen von einer knochengesunden Lebensweise bis zu einer medikamentösen Langzeittherapie. Zu den wichtigsten nichtpharmakologischen Möglichkeiten der Fraktursenkung zählen Muskeltraining und Koordinationstraining, eine kalorisch ausreichende und kalziumreiche Ernährung, eine ausreichende Versorgung mit Vitamin D und ein umsichtiger Umgang mit sturz- bzw. osteoporosefördernden Medikamenten. Bei der medikamentösen Therapie der postmenopausalen Frau stehen mit den Bisphosphonaten, Raloxifen, Strontiumranelat und Parathormon mehrere bezüglich ihrer fraktursenkenden Wirkung gut evaluierte Substanzen als Monotherapie zur Verfügung. Alendronat, Risedronat und Parathormon sind auch für die männliche Osteoporose zugelassen. Offene Fragen gibt es noch bezüglich der optimalen Therapiedauer.
Treatment of osteoporosis aims to reduce osteoporotic fractures and to minimize fracture-related pain and functional impairment. Components of non-drug therapy include improvement of muscle strength and coordination, treatment of modifiable causes of falls, a diet rich in calcium and sufficient in calories, adequate supply of vitamin D and an individual assessment of drugs known to increase falls or osteoporosis. Bisphosphonates, raloxifene, strontium ranelate and parathyroid hormone have been shown to reduce osteoporotic fractures in postmenopausal women. Alendronate, risedronate and parathyroid hormone are also licensed for the treatment of male osteoporosis. One of the still open questions is the optimal duration of pharmacological treatment.

Zu den allgemeinen Maßnahmen zählen die Verbesserung von Muskelkraft und Koordination, die Therapie vermeidbarer Sturzursachen, eine kalorisch ausreichende und kalziumreiche Ernährung, eine ausreichende Versorgung mit Vitamin D und ein umsichtiger Umgang mit sturz- bzw. osteoporosefördernden Medikamenten. Die aktualisierte Leitlinie des DVO zur Behandlung der Osteoporose empfiehlt als Interventionsschwelle für eine medikamentöse Therapie eine 30%ige 10-Jahres-Wahrscheinlichkeit für das Auftreten osteoporotischer Hüft- oder Wirbelkörperfrakturen. Anhand epidemiologischer Frakturdaten aus Mitteleuropa ermöglicht die Leitlinie eine Abschätzung des absoluten 10-Jahres-Frakturrisikos, basierend auf einer Kombination aus Alter, Geschlecht, prävalenten Frakturen, DXA-Messungen an der Gesamtlendenwirbelsäule und dem gesamten proximalen Femur (Hüfte) sowie weiteren Risikofaktoren.

Zu den allgemeinen Maßnahmen zählen die Verbesserung von Muskelkraft und Koordination, die Therapie vermeidbarer Sturzursachen, eine kalorisch ausreichende und kalziumreiche Ernährung, eine ausreichende Versorgung mit Vitamin D und ein umsichtiger Umgang mit Sturz- bzw. osteoporosefördernden Medikamenten. Anhand klinischer Risikofaktoren lassen sich in Kombination mit einer Knochendichtemessung Personen mit einem besonders hohen Frakturrisiko identifizieren, die zusätzlich einer spezifischen medikamentösen Therapie bedürfen. Zu den Personen mit einem besonders hohen Risiko zählen Patienten mit manifesten osteoporotischen Frakturen und Patienten unter einer Langzeittherapie mit oralen Glukokortikoiden.
Previous studies have shown a link between low serum insulin-like growth factor-I (IGF-I) and decreased bone mass of patients with osteoporosis. However, whether serum levels are representative for the growth factor concentration or activity available in human bone tissue is controversial. In the present study, IGF-I was assessed in serum and bone matrix extracts from the iliac crest in 19 eugonadal women with idiopathic osteoporosis and in 38 age-matched controls. In addition, the relationship between the skeletal levels of IGF-I and bone mineral density (BMD) or the susceptibility to osteoporotic fractures in women with osteoporosis was examined. Bone matrix extraction was performed based on a guanidine-HCL/ethylendiamine-tetraacetic acid (EDTA) method. No significant difference in both serum and bone matrix IGF-I levels between groups was observed. Serum IGF-I concentrations failed to be associated with bone matrix IGF-I levels in osteoporotic patients. However, in premenopausal women with idiopathic osteoporosis, skeletal IGF-I positively correlated with BMD at the lumbar spine (r = + 0.58, p = 0.01). In contrast, neither femoral neck BMD nor Ward's triangle BMD was associated with bone matrix IGF-I concentrations. A tendency towards lower levels of bone matrix IGF-I in subjects with vertebral fractures as compared to those without fractures was observed in age-adjusted analyses, however the difference failed to remain statistically significant after adjustment for bone mineral density. These data provide no clear evidence for low bone matrix IGF-I as a determinant factor of age-unrelated osteoporosis. However, low skeletal IGF-I concentrations may aggravate osteoporosis in these women.
OBJECTIVE Data from cell culture experiments suggest that local growth factors (GFs) may mediate the effects of estrogens, calcitonin or fluor ions on the skeleton. To assess the in vivo relevance of the in vitro reports, the effect of fluor salts, hormone replacement therapy (HRT) and calcitonin on the concentrations of IGF-I, IGF-II and transforming growth factor (TGF)-beta 1 in bone matrix extracts from osteoporotic patients was evaluated. DESIGN Iliac crest bone biopsies were obtained from 170 patients (76 men and 94 women) with primary osteoporosis aged 55.5+/-0.8 Years. METHODS Bone matrix extraction was performed based on a guanidine-HCl/ethylendiamine-tetra-acetic acid method. RESULTS In comparison with age- and body mass index (BMI)-matched controls, no influence of long-term therapy with fluor ions (n=41) or calcitonin (n=16) on the bone matrix concentration of GFs was noticed. Postmenopausal women with osteoporosis on HRT (n=39) had lower skeletal IGF-I but not IGF-II levels as compared with age- and BMI-matched non-users. However, the lower rate of bone turnover in women with HRT may account for this difference, since the significance was lost after adjustment for alkaline phosphatase. Likewise, a tendency for lower TGF-beta 1 levels was observed in HRT users as compared with non-users but was lost after adjustment for bone turnover. None of the therapies influenced the serum levels of GFs when patients receiving continuous therapy for at least 1 Year before bone biopsy were considered. CONCLUSIONS Our data suggest no direct effect of fluor therapy on skeletal GFs levels. At the concentrations used, neither HRT nor calcitonin appeared to exert any significant influence on serum or bone matrix GF levels.
The expression of thyroid hormones receptors in osteoblasts and osteoclasts has involved these cells as direct targets for triiodothyronine (T3), but thyroid hormones may also interact with other hormones or local growth factors to exert their actions on bone cells. Among these, growth hormone (GH) is recognised as participating in the acquisition and maintenance of bone mass and exerting stimulatory effects on human osteoblastic cells. The aim of this study was to investigate T3 effects on primary human adult osteoblast-like cells (HOB) as well as to test for possible interactions between T3 and GH on bone cell metabolism. Primary human bone cell cultures were obtained by outgrowth from trabecular bone fragments from the hip and knee. Dose-response studies demonstrated enhanced [3H]-thymidine incorporation for T3 at 10(-9), 10(-8), 10(-7) and 20(-7) M, with a maximal response of 162.81 +/- 12.97 % with T3 10(-8) M, compared to vehicle (p < 0.001). Time-course studies showed an increased osteoblast-like cell proliferation after 24 h, followed by a decrease of cell proliferation by 48 h and 72 h of culture, respectively, when compared to control cells, with a maximal response after 72 h (T3 10(-10) M: 45.21 +/- 6.97 %, p < 0.01). In addition, T3 markedly increased specific alkaline phosphatase (AP) activity in HOB (10(-10) M: 169.86 +/- 12.14 % vs. control, p < 0.001), but no significant influence on type I procollagen propeptide (PICP) production was observed. At 10(-9) - 10(-7) g/ml, GH significantly enhanced HOB proliferation (p < 0.001) however, GH effects were not dose-dependent. Triiodothyronine, at a high concentration (10(-7) M), stimulated GH-receptor (GHR) mRNA levels by 165.20 +/- 16.54 % after 24 h (p < 0.05). Correspondingly, a synergistic effect of T 3 with the same concentration and GH on cell proliferation in human adult osteoblast-like cells was found.
Zielsetzung: Das kürzlich beschriebene humane Carboxylterminale LIM-Domäne-Protein (CLP36) bindet in aktivierten Thrombozyten sowie Endothelzellen während der Reorganisation des Zytoskeletts an Actinin und trägt damit zur Lösung der fokal bestehenden Inter-Actin-Kontakte bei. Da die Reorganisation des Actins sowie dessen konsekutive Freisetzung typische Merkmale der Apoptose darstellen und zudem erhöhte Apoptoseraten in Plazenten von Patientinnen mit Präeklampsie (PE) in der Vergangenheit beschrieben worden sind, hatte die vorliegende Studie zum Ziel, die CLP36-Expression in Plazenten mit schwerer [s] PE±HELLP-Syndrom [HELLP] zu untersuchen.
