Patients undergoing mechanical ventilation (MV) for COVID-19 exhibit an increased risk of ventilator-associated pneumonia (VAP). The occurrence of lung abscesses following VAP in these patients has been poorly studied. We aimed to describe the incidence, characteristics, risk factors and prognosis of lung abscesses complicating VAP after COVID-19. We conducted an observational, retrospective study in three French intensive care units. La défaillance rénale est la deuxième atteinte la plus fréquente après le syndrome de détresse respiratoire aiguë (SDRA), chez les patients en réanimation suite au COVID-19, et est fortement associée à la mortalité. L’objectif de cette étude multicentrique était d’analyser l’impact des traitements spécifiques du COVID-19 et du SDRA sur le risque d’IRA sévère chez les patients en réanimation pour COVID-19 sévère. Dans cette étude de cohorte, les données de patients consécutifs hospitalisés dans 6 unités de réanimation pour COVID-19 étaient collectées rétrospectivement. L’incidence et la sévérité de l’IRA étaient monitorées durant tout le séjour en réanimation. Les patients majeurs hospitalisés en réanimation pour SDRA secondaire à un COVID-19 sévère nécessitant la ventilation mécanique était inclus. Au total, 164 patients ont été inclus dans l’analyse finale, 97 (59,1 %) présentaient une IRA, dont 39 (23,8 %) une IRA sévère KDIGO 3 et 21 (12,8 %) nécessitant une épuration extrarénale (EER). En analyse univariée, l’IRA sévère était associée à l’exposition aux inhibiteurs de l’enzyme de conversion de l’angiotensine (IEC) (p = 0,016), à l’HTA (p = 0,029), au score de gravité APACHE-II (p = 0,004) et à la mortalité à j28 (p = 0,008), j60 (p < 0,001) et j90 (p < 0,001). En analyse multivariée, les facteurs associés à l’apparition d’une IRA sévère étaient : l’exposition aux IEC (OR : 4,238 (1,307–13,736), p = 0,016), le score APACHE-II (sans l’âge) (OR : 1,138 (1,044–1,241), p = 0,003) et le monoxyde d’azote (NO) inhalé (OR : 5,694 (1,953–16,606), p = 0,001). Les facteurs protecteurs étaient le décubitus ventral (OR : 0,234 (0,057–0,967), p = 0,045) et la dexaméthasone (OR : 0,194 (0,053–0,713), p = 0,014) (Figure 1). La dexaméthasone est associée à une prévention du risque d’IRA sévère et d’EER, et le NO inhalé est associé à un risque d’IRA sévère et d’EER chez les patients en réanimation suite à un COVID-19. Le NO inhalé doit être utilisé avec précaution au cours du SDRA du au COVID-19.
IntroductionLa protéinurie et l’insuffisance rénale aiguë sont fréquentes au cours du COVID-19, et associées à la morbi-mortalité, mais la pathogénicité directe du SARS-CoV-2 sur le rein reste débattue. Notre objectif était d’évaluer la prévalence et la valeur pronostique de la tubulopathie proximale chez les patients en SDRA COVID en réanimation.DescriptionDans cette étude de cohorte prospective multicentrique, étaient inclus les patients majeurs présentant une détresse respiratoire nécessitant la ventilation mécanique avec rapport PaO2/FiO2<300 dans les 24h de l’intubation.MéthodesNous réalisions une analyse détaillée des marqueurs de dysfonction rénale avec électrophorèse des protéines urinaires (EPU) dans les 24h de l’intubation chez 85 patients en réanimation pour un SDRA COVID.RésultatsSeulement 16 (18,8 %) des patients présentaient une IRA initiale. L’uricémie était abaissée (moyenne 212μmol/L), 87,1 % des patients présentaient une glycosurie. Une hématurie microscopique était retrouvée chez 74 (87,1 %) patients. Les rapports protéinurie/créatinine et albuminurie/créatinine étaient de 785 et 142 mg/g. L’EPU retrouvant un profil tubulaire chez 91 % des patients, avec une atteinte glomérulaire surajoutée dans chez 17 %. Il existait une fuite urinaire massive d’alpha-1-microglobuline (a1m, 96,1 % des patients), beta-2-microglobuline (b2m, 95,8 %), chaînes légères kappa et lambda (CLL) (100 % et 93,5 %) et Vitamine-D-Binding-Protein (87,5 %). Après un suivi moyen de 28jours, 37 patients (43,5 %) ont développé une insuffisance rénale aiguë (13 KDIGO3, 3 nécessitant la dialyse). Les patients ayant développé une IRA KDIGO3 avaient une urémie, une créatininémie, des rapports protéinurie/créatininurie, albuminurie/créatininurie et a1m/créatininurie significativement plus élevés et une bicarbonatémie plus basse. La dexamethasone semblait protéger de l’IRA. La survenue d’une IRA et des rapports b2m/créatininurie et CLLl/créatininurie plus élevés était associés à la mortalité (p=0,027, p=0,04, p=0,005, respectivement) (Fig. 1).ConclusionLes patients développant un SDRA au cours du COVID-19 présentent une dysfonction tubulaire proximale, précédant l’IRA, qui semble être un mécanisme important de la néphropathie COVID et pourrait être un marqueur de sévérité.
Le COVID-19 peut entraîner un syndrome de détresse respiratoire aiguë (SDRA). Chez les patients intubés atteints de SDRA sévère, des séances précoces, prolongées et répétées de décubitus ventral (DV) réduisent la mortalité. Le DV vigile en dehors de la réanimation est faisable, améliore l'oxygénation et pourrait prévenir une aggravation respiratoire [1]. L'objectif principal de l'étude était d'étudier l'augmentation du support ventilatoire chez les patients avec une pneumonie hypoxémiante à COVID-19 faisant du DV vigile. Nous avons réalisé une étude de cohorte rétrospective bicentrique exposée/non exposée avec score de propension chez les patients en dehors de la réanimation avec pneumonie hypoxémiante a COVID-19 entre le 20 mars et le 20 avril 2020, à l'hôpital d'Aix-en-Provence et l'hôpital Nord de Marseille. Les critères d'inclusion étaient : l'insuffisance respiratoire hypoxémique nécessitant une supplémentation en oxygène, soit par oxygénothérapie conventionnelle, soit oxygène à haut débit (OHD), et un COVID-19 confirmé par PCR. Deux stratégies de traitement ont été comparées : le DV vigile 3 heures par jour pendant trois jours consécutifs (groupe DV) et aucune instruction concernant le positionnement ou une mauvaise tolérance du DV (groupe non-DV). Sur les 414 patients confirmés en PCR, 168 patients étaient éligibles et 48 ont fait du DV 3 h par jour pendant trois jours contre 120 le groupe non-DV. Après score de propension, 96 patients ont été analysés (48 patients dans chaque groupe). Tous les patients recevant du DV ont été appariés avec succès avec un patient sans DV. Les patients ont eu du DV pendant 6,9 (± 5,2) jours. Parmi les 48 patients du groupe DV, 32 (67%) ont eu du DV pendant 3 à 8 heures par jour et 16 (32%) pendant plus de 8 heures par jour. Pour le critère de jugement principal, 15 patients (31,2%) du groupe DV ont eu une augmentation de leur support ventilatoire à j14, contre 30 (62,5%) dans le groupe sans DV (p = 0,002) avec un hazard ratio (HR) de 2,59 (IC95% : 1,39–4,84). Pas de différence retrouvée sur la mortalité ou sur les taux d'intubation. Pas d'évènement indésirable majeur. Le DV vigile 3 h par jour pendant 3 jours consécutifs pourrait prévenir l'augmentation du support ventilatoire chez les patients atteints d'une pneumonie hypoxémiante à COVID 19.
Prevention and treatment of ventilator-associated bacterial pneumonia resulted in numerous publications and guidelines. Respiratory viral “nosocomial” infections, consisting mainly in the reactivation of viruses from the herpes group, were described in the past two decades in the nonimmunocompromised intensive care unit (ICU) patients. The actual pathogenesis of these infections has often been discussed with the assumption that they were the marker of severity of patients’ illness rather than a real disease. To date, they are still underdiagnosed since rarely investigated in previously immunocompetent ICU patients. Several recent studies and meta-analyses suggested that they represent an independent risk factor for mortality and major morbidity to be considered as one separate disease entity by itself. Our purpose is to review respiratory viral infections occurring in the ICU focusing at the supposed pathophysiological mechanisms, risk factors, prognosis and ongoing studies regarding their management.
Objectives: Cytomegalovirus (CMV) reactivation in intensive care unit patients may increase mortality and favour bacterial pneumonia. We developed a murine model to compare the severity of staphylococcal pneumonia after CMV reactivation and in CMV-negative mice.Methods: Balb/c mice were primo-infected with murine cytomegalovirus (MCMV n = 90) or received saline (control n = 90). After latency, all mice underwent caecal ligation and puncture to trigger MCMV reactivation in MCMV primary-infected mice. Surviving animals received an intra-nasal inoculation with methicillin-susceptible Staphylococcus aureus (MSSA) to induce pneumonia. Mortality, lung bacterial count, histology and interferon-alpha and gamma serum levels were compared in MCMV reactivated and control mice 2, 5 and 15 days after pneumonia.Results: AfterMSSA pneumonia, MCMV mice showed a trend towards a higher mortality (9.4% versus 0%; p 0.09) and a higher weight loss (2.2 (0.6-4.1 g) versus 0.7 (-0.3 to 1.3 g); p 0.005). The lung bacterial countwas higher in MCMV mice 2 days (5 x 10(3) (10(3) to 3 x 10(5)) versus 10(2) (0 to 4 x 10(2)) CFU/lung; p 0.007) and 5 days (2.5 x 10(4) (1.6 x 10(4) to 6.5 x 10(5)) versus 15 (10-40) CFU/lung; p 0.005) after MSSA pneumonia. 8/40 (20%) MCMV mice developed lung abscesses compared to 0% in control (p 0.011). Interferon-alpha serum levels 2 days after staphylococcal pneumonia were higher in MCMV mice.Conclusions: MCMV reactivation decreased lung bacterial clearance and favoured the development of staphylococcal abscessing pneumonia. Background. Veno-venous extracorporeal membrane oxygenation (VV-ECMO) is an effective rescue therapy for improving oxygenation in selected severe acute respiratory distress syndrome (ARDS). Prone position (PP) is usually considered before vvECMO and few data are available on the association of PP during VV-ECMO. Thus, we investigated the effect on oxygenation and the safety of PP during vvECMO.Methods. During a two-year period, 15 patients with severe ARDS were turned into PP during VV-ECMO therapy for at least one of the three following conditions: severe hypoxemia (PaO2/FiO(2) ratio below 70) despite maximal oxygenation, injurious ventilation parameters with plateau pressure exceeding 32 cmH(2)O or failure of attempt to wean ECM after at least 10 days on ECMO support.Results. PP was considered after a median of 9 days of ECMO and applied for a median of 12 hours and an average of 1.4 sessions per patient resulting in a total of 21 procedures. We found significant improvement in PaO2/FiO(2) ratio at 6 hours (P=0.03) and 12 hours (P=0.007) after reversal. The improvement in oxygenation has still persisted 1 hour (P=0.017) and 6 hours (P=0.013) after back to the supine position. No change in PaCO2, respiratory system (RS) compliance was observed. ECMO flow was maintained constant during the procedure. No complication related to PP was detected.Conclusion. PP may be considered in selected patients difficult to wean or remaining very hypoxemic despite VV-ECMO support.
La préservation de la ventilation spontanée au cours du syndrome de détresse respiratoire aiguë (SDRA), que ce soit au moyen de la ventilation spontanée avec aide inspiratoire, la biphasic airway pressure (BIPAP® ou Bilevel), ou encore l’airway pressure release ventilation pourrait apporter certains bénéfices en termes d’hémodynamique, de prévention de la dysfonction diaphragmatique, de limitation de la durée de séjour en réanimation et de la durée de ventilation mécanique. Toutefois, chez les patients les plus sévères, à la phase aiguë du SDRA, des travaux récents ont montré une réduction de la mortalité liée à une curarisation de courte durée mais également au recours au décubitus ventral, plaidant pour une limitation, voire une abolition de la ventilation spontanée lors du pic de l’agression pulmonaire afin de favoriser les stratégies de ventilation protectrice. En s’appuyant sur les éléments disponibles dans la littérature, cette revue a pour but de montrer les avantages du maintien de la ventilation spontanée au cours du SDRA tout en insistant sur ses limites et ses risques pour le parenchyme pulmonaire. Nous définissons une stratégie séquentielle de prise en charge ventilatoire s’appuyant sur la gravité du SDRA et son évolution, à savoir une ventilation protectrice avec abolition de la ventilation spontanée par curarisation dans un premier temps suivie par une mise en jeu de la ventilation spontanée. Ces deux stratégies sont dès lors complémentaires dans le cadre de la prise en charge ventilatoire des patients présentant une forme sévère de SDRA.
The deformability of circulating leukocytes plays an important role in the physiopathology of several diseases like sepsis or acute respiratory distress syndrome (ARDS). We present here a microfluidic method for the passive testing, sorting and separating of non-adherent cell populations by deformability. It consists of microfluidic sieves in series with pore sizes decreasing from the upstream to the downstream. The method capabilities are demonstrated with monocytic cell lines (THP-1) treated by Jasplakinolide (a stabilizer of polymerized actin), LatrunculinA (an inhibitor of actin polymerization), and with the plasma of patients suffering from ARDS. Simple sample injection with standard syringes and pumps makes the method readily adapted for experimentation in hospitals.
Objective Cytomegalovirus (CMV) and herpes simplex virus (HSV) are common viruses that can affect critically ill patients who are not immunocompromised. The aim of this study was to determine whether the identification of CMV and/or HSV in mechanically ventilated critically ill patients suspected of having pneumonia was associated with an increased mortality. Design Prospective epidemiological study. Setting Medical intensive care unit of a tertiary medical center. Patients Ninety-three patients with suspected pneumonia. Interventions Patients with suspected pneumonia had bronchoalveolar lavage and blood samples taken to confirm the diagnosis. Antigenemia was used to detect CMV in the blood. Bronchoalveolar lavage samples were submitted to testing using quantitative real-time Polymerase Chain Reaction. Measurements and Main Results We identified 22 patients with a CMV infection, 26 patients with an HSV infection and 45 patients without CMV or HSV infection (control group). Mortality at day 60 was higher in patients with a CMV infection than in patients from the control group (55% vs. 20%, P<0.01). Mortality at day 60 was not significantly increased in the group with HSV infection. Duration of ICU stay and ICU mortality were significantly higher in patients with CMV infections when compared to patients from the control group, whereas ventilator free days were significantly lower in patients with CMV infections when compared to patients from the control group. Conclusions In critically ill patients, a CMV infection is associated with an increased mortality. Further interventional studies are needed to evaluate whether treatment could improve the prognosis.
