Introduction L’immunomarquage de PRAME est un outil d’une grande sensibilité et spécificité pour différencier la nature bénigne/maligne des proliférations mélanocytaires. Il contribue à améliorer de manière significative la classification des lésions mélanocytaires de diagnostic histologique difficile.L’AFAQAP a organisé en 2023 un premier essai d’aptitude inter-laboratoires du marquage PRAME afin d’évaluer la qualité technique à l’échelon national. Objectif Identifier les protocoles inadaptés ou sub-optimaux, et proposer des recommandations si nécessaire. Méthode Les participants ont appliqué leur protocole de routine sur une lame blanche comportant 2 échantillons cutanés, l’un provenant d’un mélanome, l’autre d’un nævus, et 1 échantillon de testicule normal.Les résultats attendus étaient les suivants : marquage nucléaire intense dans 100 % des cellules tumorales du mélanome ; absence de marquage dans les cellules næviques et marquage cytoplasmique franc des cellules des glandes sébacées ; marquage nucléaire modéré à intense des spermatogonies, et faible à modéré de quelques spermatocytes.Les résultats étaient classés en 4 groupes : optimal, bon, moyen et insuffisant. Résultats Quarante-trois structures ont participé au test et 93 % d’entre elles ont obtenu un marquage adéquat (optimal ou bon) sur les 3 échantillons.Dans le mélanome, le taux de marquage adéquat était de 95 %. Seuls 2 participants (5 %) ont eu un résultat insuffisant lié à une intensité de marquage bien trop faible. Leur marquage était également de trop faible intensité sur les échantillons de nævus et de testicule. À l’inverse, un participant a obtenu un résultat moyen sur le nævus en raison d’un marquage positif aberrant de quelques noyaux de cellules naeviques.Trois clones ont été utilisés : le clone QR005 de Quartett/Diagomics (35 fois), le clone EPR20330 (7 fois), et le clone RBT-PRAME de BioSB/Diagomics (1 fois).Parmi les 35 utilisateurs du clone QR005, 32 ont eu un résultat adéquat sur plateforme Dako Omnis (5/5), Leica (7/7) et Roche-Ventana (20/22) ; 2 utilisateurs de la plateforme Roche-Ventana ont obtenu un résultat insuffisant sur le mélanome, et moyen sur le nævus et le testicule. Le seul utilisateur de ce clone sur plateforme Dako Autostainer a obtenu un résultat moyen sur le nævus.Le clone EPR20330 de Roche-Ventana (6 utilisateurs) ou d’Abcam (1 utilisateur), toujours utilisé sur plateforme Roche-Ventana, a donné 100 % de résultats adéquats.Le clone RBT-PRAME de BioSB/Diagomics (1 utilisateur sur Dako Omnis) a donné un résultat adéquat. Discussion/Conclusion L’essai d’aptitude IHC PRAME 2023 de l’AFAQAP a montré un marquage adéquat de PRAME chez 93 % des participants, moyen chez 2 %, et insuffisant chez 5 %. Le testicule normal est un très bon témoin pour calibrer la technique IHC PRAME. Les recommandations proposées aux 3 structures ayant obtenu un résultat moyen ou insuffisant devraient leur permettre d’améliorer leur technique.
Le cancer du poumon est le cancer le plus meurtrier au monde. L'immunothérapie par inhibiteurs de points de contrôle immunitaires (ICI) a récemment démontré son efficacité par rapport à la chimiothérapie dans le cancer bronchique non à petites cellules (CBNPC) métastatique, en 1re et 2e lignes thérapeutiques. Cette approche révolutionnaire vise à restaurer l'activité antitumorale du système immunitaire; il s'agit notamment des anticorps anti-PD1et anti-PDL1. Néanmoins, seuls 20% des patients présentent un bénéfice durable, sans possibilité actuellement de prédire la sensibilité à ce traitement coûteux. L'objectif de ce travail est d'identifier un ou des biomarqueurs prédictifs de l'évolution sous ICI. L'expression de 7 cibles biologiques (PDL1, FOXP3, CD3, CD4, CD8, CD23et CD56) a été évaluée par immunohistochimie sur les prélèvements histologiques de patients atteints d'un CBNPC et ayant bénéficié d'un traitement par ICI aux Hôpitaux Universitaires de Strasbourg entre 2015 et 2022, en monothérapie ou en association avec une chimiothérapie par sels de platine. L'expression de PDL1 a été évaluée sous forme d'un score de proportion tumorale, celle de CD23sous forme d'une valeur binaire (présence ou absence d'un marquage pertinent) et deux méthodes ont été comparées pour les marqueurs lymphocytaires (CD3, CD4, CD8, CD56, FOXP3): le pourcentage d'infiltration globale du stroma par la population lymphocytaire ciblée et le dénombrement dans un seul champ microscopique à magnification x400 (hotspot). Une cohorte de 130 patients avec un CBNPC a été constituée, dont 67 sujets sensibles aux ICI (caractérisés par une survie sans progression (SSP) ≥ 12 mois) et 63 sujets résistants (SSP ≤ 3 mois). La comparaison de ces groupes a mis en évidence des différences significatives dans l'expression de PD-L1, CD8, CD56et FOXP3. La combinaison de CD56et PDL1 permet d'améliorer la valeur prédictive négative de ce dernier: 77,42% (p = 0,0008) vs 65,08%. La combinaison de CD56, CD8, FOXP3 et PDL1 permet d'améliorer la valeur prédictive positive de ce dernier: 75,00% (p = 0,0225) vs 67,16% (Fig. 1). Des combinaisons de CD56, CD8et FOXP3 avec PD-L1 améliorent les performances prédictives de la sensibilité aux ICI par rapport à PDL1 seul. CD8identifie les cellules effectrices principales de l'immunité anti-tumorale. CD56témoigne de la robustesse de la réponse immunitaire innée [1]. FOXP3 identifie des cellules immunosuppressives dont la fonction pourrait être renversée par l'administration d'un ICI [2]. Nous poursuivons ces recherches afin d'améliorer davantage la prédiction avec des marqueurs supplémentaires.
e21064 Background: Testing for EGFR mutations is an essential recommendation in both the ESMO and ASCO guidelines for metastatic non-squamous NSCLC, and is considered mandatory in European countries. However, in practice, considerable challenges are often faced when performing routine biomarker testing, including access to testing, inadequate tissue samples, and long turnaround times (TATs). Methods: In order to evaluate the real-world EGFR testing practices of thoracic pathology laboratories across Europe, an online survey was set up and validated by The Pulmonary Pathology Working Group of the European Society of Pathology and distributed to 64 expert testing laboratories across Europe. The retrospective survey focused on the laboratory organisation and daily practice of pathologists and molecular biologists between 2018 and 2021, and covered the clinical circumstances in which EGFR testing was performed, sample types used, molecular biology techniques performed, average TATs for determining EGFR status, annual EGFR testing rates, laboratory accreditation, and external quality assessment. Results: TATs varied greatly depending on different parameters (notably relating to testing practices and methods used) both between and within countries. TATs were slightly (but non-significantly) faster for liquid biopsy samples (Mean: 7 days, IQR 4.5-12.5) when compared to tissue samples (mean 7 days, IQR 5-15) but were reported to take as long as 20 or 30 days, respectively. Discrepancies in TATs may be due in part to reflex testing practices, as 20% (11/54) of laboratories performed EGFR testing only at the request of the clinician. Many laboratories across Europe still favour targeted sequencing as a primary method of EGFR mutation identification; 32.7% indicated that they only used targeted techniques, and 45.1% indicated they used targeted sequencing followed by NGS, depending on the case. Reported testing volumes were consistent over time with no significant decrease in the number of EGFR tests performed observed in 2020, despite the increased pressure and difficulties faced by testing facilities during the COVID-19 pandemic. ISO 15189 accreditation was reported by 42% of molecular biology laboratories for targeted sequencing, and by 42.3% for NGS. 92% (49/53) of laboratories indicated they regularly participate in an EQA scheme. Conclusions: The results of the survey highlight the strong heterogeneity of EGFR testing that still occurs within thoracic pathology and molecular biology laboratories across Europe. Even among expert testing facilities there remains large variability in testing capabilities, reflex testing practice and laboratory accreditation, which stresses the urgent need to harmonize budgets and reimbursement in Europe, in addition to harmonization of the technologies and decision-making algorithms.
Les tumeurs mélanocytaires sont dans certains cas des lésions histologiquement équivoques et la détermination de leur caractère bénin/malin peut être difficile, même pour un pathologiste expérimenté. L’analyse de ces tumeurs nécessite fréquemment le recours à des examens complémentaires onéreux, peu disponibles et longs à mettre en place (FISH–CGH-array). L’immunomarquage de PRAME a récemment été décrit comme un outil d’une grande sensibilité et spécificité pour distinguer la nature bénigne/maligne des proliférations mélanocytaires non équivoques. L’intérêt potentiel de cet immunomarquage pour l’analyse des lésions mélanocytaires de diagnostic histologique difficile reste à ce jour peu étudié. L’objectif de notre travail était de déterminer les performances diagnostiques de l’immunomarquage de PRAME dans l’analyse des tumeurs mélanocytaires d’interprétation histopathologique difficile. Nous avons sélectionné dans la base de données du service de dermatopathologie des Hôpitaux Universitaires de Strasbourg tous les cas de tumeur mélanocytaire de diagnostic difficile reçus entre janvier 2016 et janvier 2021. Nous avons désarchivé et réexaminé l’ensemble des lames histologiques de ces cas, collecté dans les dossiers médicaux des patients les informations cliniques pertinentes concernant le suivi (survenue de métastases ou de récidive loco-régionale, ré-excision…) et récupéré, le cas échéant, les conclusions des analyses moléculaires et des relectures d’expert. L’ensemble de ces données a été intégré et chaque cas a été classé en « tumeur plutôt maligne » ou en « tumeur plutôt bénigne ». Pour chaque tumeur, l’expression immunohistochimique de PRAMEa été évaluée sur des recoupes faites à partir du bloc en paraffine le plus représentatif, indépendamment du diagnostic final retenu, puis corrélé à celui-ci. Comme proposé dans la littérature, seules les tumeurs présentant un marquage nucléaire dans plus de 75 % des cellules tumorales pour PRAME étaient considérées comme positives. De plus, pour les proliférations mélanocytaires survenant sur un nævus préexistant, seule l’immunoréactivité du contingent tumoral suspect était prise en compte. Cinquante-cinq cas de tumeur mélanocytaire ont été étudiées, dont 31 « tumeurs plutôt malignes » et 24 « tumeurs plutôt bénignes ». Parmi ces lésions, 31 avaient un marquage positif pour PRAME, dont 29 (93,5 %) des « tumeurs plutôt malignes » et 2 (8,3 %) des « tumeurs plutôt bénignes ». Dans notre cohorte de tumeurs mélanocytaires de diagnostic difficile, l’immunomarquage de PRAME présentait une spécificité de 91,7 % et une sensibilité de 93,5 % pour la distinction du caractère bénin/malin de la prolifération tumorale. Les résultats de notre étude soulignent l’intérêt de l’immunomarquage de PRAME dans l’analyse des lésions mélanocytaires de diagnostic histopathologique difficile. L’analyse doit cependant être globale, intégrant l’ensemble des données cliniques, histologiques et moléculaires. L’interprétation de l’immunomarquage de PRAME doit rester prudente puisque de rares tumeurs mélanocytaires bénignes peuvent également présenter un marquage positif diffus.
Introduction: Pulmonary hypertension in COPD (COPD-PH) is a common complication generally proportionate to the severity of bronchial obstruction. Severe PH is less common (1 to 10% of COPD patients) but has a major impact on morbi-mortality. Computed tomography (CT) and histopathological features of COPD-PH are still incompletely known. We aimed to describe CT and histopathological lesions in patients with end-stage COPD. Methods: 59 patients who received lung transplantation for terminal COPD were retrospectively included. We divided the population into 3 PH severity groups based on the 6th World Symposium on PH classification. We performed CT-Scan quantification of emphysema and measured pulmonary artery to ascending aorta ratio (PA/Ao). We analyzed the following histopathological characteristics in explanted lungs after transplantation: small pulmonary arteries layers, neo-muscularization of pre-capillary vessels and capillary density. Results: In the severe PH group compared to the moderate PH group, the PA/Ao ratio was significantly higher (p<0.00002); the media layer of small pulmonary arteries and the neo-muscularization of pre-capillary vessels were significantly increased (p=0.0004 and p<0.0001 respectively), and the capillary density was lower (p=0.01). No lesions suggestive of primary pulmonary arterial hypertension were found. Conclusion: The PA/Ao ratio could be an interesting noninvasive tool to detect severe COPD-PH. Severe COPD-PH is associated with microvascular remodeling and capillary pruning.
OBJECTIVES:Gastric cancer (GC) is an aggressive disease due to late diagnosis resulting from the lack of easy diagnostic tools, resistances toward immunotherapy (due to low PD-L1 expression), or chemotherapies (due to p53 mutations), and comorbidity factors, notably muscle atrophy. To improve our understanding of this complex pathology, we established patient-derived xenograft (PDX) models and characterized the tumor ecosystem using a morpho-functional approach combining high-resolution imaging with molecular analyses, regarding the expression of relevant therapeutic biomarkers and the presence of muscle atrophy.MATERIALS AND METHODS:GC tissues samples were implanted in nude mice. Established PDX, treated with cisplatin or not, were imaged by magnetic resonance imaging (MRI) and analyzed for the expression of relevant biomarkers (p53, PD-L1, PD-1, HER-2, CDX2, CAIX, CD31, a-SAM) and by transcriptomics.RESULTS:Three well-differentiated, one moderately and one poorly differentiated adenocarcinomas were established. All retained the architectural and histological features of their primary tumors. MRI allowed in-real-time evaluation of differences between PDX, in terms of substructure, post-therapeutic changes, and muscle atrophy. Immunohistochemistry showed differential expression of p53, HER-2, CDX2, a-SAM, PD-L1, PD-1, CAIX, and CD31 between models and upon cisplatin treatment. Transcriptomics revealed treatment-induced hypoxia and metabolic reprograming in the tumor microenvironment.CONCLUSION:Our PDX models are representative for the heterogeneity and complexity of human tumors, with differences in structure, histology, muscle atrophy, and the different biomarkers making them valuable for the analyses of the impact of platinum drugs or new therapies on the tumor and its microenvironment.
Background:COVID-19 acute respiratory distress syndrome (ARDS) shares the common histological hallmarks with other forms of ARDS. However, the chronology of the histological lesions has not been well established. Objective:To describe the chronological histopathological alterations in the lungs of patients with COVID-19 related ARDS. Design:A prospective cohort study was carried out. Setting:Intensive Care Unit of a tertiary hospital. Patients:The first 22 consecutive COVID-19 deaths. Measurements:Lung biopsies and histopathological analyses were performed in deceased patients with COVID-19 related ARDS. Clinical data and patient course were evaluated. Results:The median patient age was 66 [63-74] years; 73% were males. The median duration of mechanical ventilation was 17 [8-24] days. COVID-19 induced pulmonary injury was characterized by an exudative phase in the first week of the disease, followed by a proliferative/organizing phase in the second and third weeks, and finally an end-stage fibrosis phase after the third week. Viral RNA and proteins were detected in pneumocytes and macrophages in a very early stage of the disease, and were no longer detected after the second week. Limitation:Limited sample size. Conclusions:The chronological evolution of COVID-19 lung histopathological lesions seems to be similar to that seen in other forms of ARDS. In particular, lung lesions consistent with potentially corticosteroid-sensitive lesions are seen.
Background: This study evaluated the consequences in Europe of the COVID-19 outbreak on pathology laboratories orientated toward the diagnosis of thoracic diseases. Materials and methods: A survey was sent to 71 pathology laboratories from 21 European countries. The questionnaire requested information concerning the organization of biosafety, the clinical and molecular pathology, the biobanking, the workload, the associated research into COVID-19, and the organization of education and training during the COVID-19 crisis, from 15 March to 31 May 2020, compared with the same period in 2019. Results: Questionnaires were returned from 53/71 (75%) laboratories from 18 European countries. The biosafety procedures were heterogeneous. The workload in clinical and molecular pathology decreased dramatically by 31% (range, 3%-55%) and 26% (range, 7%-62%), respectively. According to the professional category, between 28% and 41% of the staff members were not present in the laboratories but did teleworking. A total of 70% of the laboratories developed virtual meetings for the training of residents and junior pathologists. During the period of study, none of the staff members with confirmed COVID-19 became infected as a result of handling samples. Conclusions: The COVID-19 pandemic has had a strong impact on most of the European pathology laboratories included in this study. Urgent implementation of several changes to the organization of most of these laboratories, notably to better harmonize biosafety procedures, was noted at the onset of the pandemic and maintained in the event of a new wave of infection occurring in Europe.
Background: Breast Implant-associated anaplastic large cell lymphoma (BI-ALCL) is a rare T-cell lymphoma arising in association with breast implant, particularly those with textured surfaces. We recently identified two histopathological BI-ALCL subtypes: in-situ and tumor-type which correlated with the seroma vs tumor mass clinical presentation, respectively. Although genetic events involving the JAK/STAT pathway have been reported and the putative role of local chronic inflammation has been suspected, BI-ALCL pathogenesis remains elusive. To further explore potential molecular mechanisms involved in the pathobiology of these two distinct BI-ALCL subtypes, we performed a genomic characterization of 34 such cases. Patient and Methods: Fifty-four BI-ALCL patients have been diagnosed through the Lymphopath network and registered in the Lymphoma Study Association Registry from 2010 to 2018. A total of 34 BI-ALCL samples have been analyzed by whole exome sequencing (WES) (n = 22 paired tumor/ germline DNA) and/or targeted next generation sequencing (NGS) (n = 24), across 406 genes to validate the findings. The average depth of sequencing was 300x for WES and 500x for targeted NGS. Results: Fifty nine percent of cases (20/34) showed mutations in at least one member of the JAK/STAT signaling pathway. STAT3(38%) and JAK1 (18%) were the most mutated genes whereas STAT5B (n = 1) and the negative regulators SOCS3 (n = 2), SOCS1 (n = 1) and PTPN1 (n = 1) were seldom mutated. Fifteen percent of cases harbored simultaneous mutations in two genes of this pathway. STAT3 mutations were more frequent in tumor-type than in-situ BI-ALCL (p = 0.0078). Recurrent mutations in epigenetic modifiers were seen in 70% of cases with KMT2C (26%), CHD2 (15%), CREBBP (15%) and KMT2D (9%) being the most frequently mutated genes. Loss of function mutations in TP53 (12%) have also been identified. Genomic alterations in genes involved in lymphocytes development such as EOMES and in PI3K-AKT and MAP cascades were also seen in 23.5%, 12% and 9% BI-ALCL cases, respectively. In addition, CNV analysis in 8 cases identified recurrent deletions on chromosomes 8q, 15p and 20. Furthermore, by this approach additional alterations in SOCS3 (2/8), SOCS1 (1/8), PTPN1 (6/8) and STAT5B (3/8) as well as LOH in several epigenetic regulators such as TET2 (3/8) and HDAC8 (2/8) were seen. Conclusions: This large series strongly reinforce the hypothesis that dysregulation of cytokine receptor signaling caused by recurrent mutations in the JAK/STAT pathway is a key event in BI-ALCL pathogenesis. Furthermore, our observation of frequent mutations in chromatin remodeling genes highlights the importance of epigenome and provides new insights into the complexity of BI-ALCL oncogenesis. Introduction. The natural history of orphan lung diseases is often unclear. We report the long-term follow-up of a case of bronchiectasis due to pulmonary non amyloid light chain deposition disease (LCDD). Case report. A 50-year-old woman who was a smoker, was diagnosed with diffuse thin walled bronchiectasis of uncertain origin after presenting with a respiratory tract infection. Ten years later, the combination of bronchiectasis, the appearance of pulmonary cysts and the identification of increased kappa free light chains evoked the diagnosis of pulmonary LCDD. The diagnosis was confirmed by lung biopsy. No immunoproliferative disorder was identified. During the 12 years follow-up, dyspnea worsened progressively and bronchiectasis and lung cysts extended leading to multicystic lung disease. Pulmonary function tests did not show any ventilatory defect but a small decrease in carbon monoxide transfer factor occurred. Conclusion. We describe the evolution of a rare presentation of isolated pulmonary LCDD, characterized by cystic diffuse atypical bronchiectasis with thin walls, associated with progressive cystic destruction of the lung parenchyma. The possibility of pulmonary LCDD should be considered in cases of atypical bronchiectasis of unknown etiology. (C) 2019 SPLF. Published by Elsevier Masson SAS. All rights reserved.
La transplantation pulmonaire est un traitement majeur de l’insuffisance respiratoire chronique. Son pronostic est altéré par la dysfonction chronique du greffon, caractérisée essentiellement par le syndrome de bronchiolite oblitérante (BOS). L’un des mécanismes responsable est la transition épithélio-mésenchymateuse (TEM), entraînant la transformation des cellules épithéliales en cellules mésenchymateuses. Le rôle des microparticules membranaires (MPs) dans la BOS n’est pas clairement connu. Ce sont des fragments de membrane plasmique émis par les cellules qui se comportent en effecteurs cellulaires. Des modifications de concentration ou de propriétés de MPs ont été décrites dans le compartiment aérien et dans le sang dans différentes pathologies pulmonaires. Notre hypothèse est que les MPs leucocytaires induisent la TEM responsable de la BOS. Nos objectifs sont : – de confirmer l’existence de la TEM au cours de la BOS sur des coupes de poumons explantés de patients ayant bénéficié d’une deuxième transplantation pour BOS ; – de caractériser et phénotyper les MPs circulantes chez des patients greffés avec ou sans BOS ; – d’étudier l’effet des MPs isolées de monocytes THP-1 stimulés par le LPS de P. Aeruginosa dans la TEM de cellules épithéliales bronchiques humaines en culture. Sur des coupes de poumons explantés pour BOS, nous avons observé par endroit une destruction de l’épithélium bronchique associée à une baisse de l’expression de l’E-Cadhérine et de la β-caténine (protéines membranaires épithéliales) et une majoration de l’expression de la Vimentine (phénotype mésenchymateux). L’immunophénotypage des MPs circulantes montrait que la concentration totale de MPs mesurée dans le groupe BO était de 5,46 ± 1,2 nM PhtdSer et de 8,34 ± 1,6 nM PhtdSer dans le groupe témoin (p : 0,2). La concentration des MPs porteuses de E-cadhérine était inférieure dans le groupe BO (0,02 ± 0,02 nM PhtdSer) comparé au groupe témoin (0,12 ± 0,02) (p : 0,007). L’étude de notre modèle cellulaire a montré que les MPs de THP-1 entraînaient une diminution significative de l’expression de l’E-cadhérine (p < 0,05), et une augmentation de celle de la Vimentine (p : 0,06). Cela s’accompagnait également de modifications morphologiques des cellules BEAS-B, ainsi que d’une majoration des capacités de cicatrisation évaluées par Scratch test. Les MPs de monocytes pourraient contribuer à la TEM des cellules épithéliales bronchiques en modifiant l’expression des protéines membranaires.
Antibody-mediated rejection (AMR) is emerging as an important cause of solid organ transplantation failure. The role of AMR in lung allografts is a source of ongoing investigation, as well as the identification of more sensitive and accurate diagnostic features. The mammalian target of rapamycin (mTOR) pathway, in particular phosphorylated P70 S6 kinase (S6K) and S6 ribosomal protein (S6RP), is reported to be associated with AMR in heart and kidney transplantation. The main goal of the study was to evaluate S6K and S6RP expression in lung transplanted (LT) patients with AMR from different LT centers.
Hyposplenism is associated with autoimmune diseases, inflammatory bowel disease, severe celiac disease, autoimmune thyroiditis, untreated HIV infection and chronic graft-versus-host disease. The aim of this study was to review the existing data on hyposplenism associated with celiac disease and Hashimoto's autoimmune thyroiditis. Our research was based on a clinical case concerning a 41-year-old female who presented with asthenia, fatigue, dyspepsia and chronic diarrhea. The medical history revealed autoimmune Hashimoto's thyroiditis, type 2 diabetes, fatty liver disease, chronic gastritis and thrombocytosis. Multiple investigations showed hyposplenism and complex autoimmune dysfunction with positive serum markers for celiac disease and type 1 autoimmune hepatitis along with minor symptomatology. The intestinal symptomatology of celiac disease is often hid by hypothyroidism-associated autoimmune thyroiditis. Asymptomatic or minimally symptomatic celiac disease associated with Hashimoto's autoimmune thyroiditis is diagnosed by biomarkers. Hyposplenism in celiac disease can occur regardless of the disease stage, latent or symptomatic.
