The student accident insurance has been part of the German Statutory Accident Insurance (Deutsche Gesetzliche Unfallversicherung, DGUV) for 50 years. In order to assess the reduction in working capacity (Minderung der Erwerbsfahigkeit, MdE) in the event of permanent consequences of accidents and injuries, the recommendation to treat the affected child or adolescent "as an average adult" is currently still valid. The present work deals with the everyday practice of the MdE assessment in children and adolescents and their weaknesses through the transfer of the principles from adulthood. In addition, proposals for the adaptation of the assessment principles for the growth age are drawn up.
While many finger conditions in climbers have been studied extensively, no data exist on the treatment of rock climber's finger flexor tenosynovitis. The purpose of this study was to evaluate the outcomes after corticosteroid injection. The study included rock climbing athletes suffering from chronic (longer than 6 weeks) finger flexor tenosynovitis who were seen at our clinic in 2017. All 42 patients received two corticosteroid injections within a 7-10 day period. Thirty-one climbers (73.8%) were pain free after the second injection and a mean of 20.9 +/- 23.1 days. The climbers reported an 84.2% decrease in pain level and no complications. The positive outcome after corticosteroid injection therapy and the absence of complications justifies this invasive approach in rock climbing athletes. (C) 2019 SFCM. Die ambulante orthopädische Rehabilitation hat sich in Deutschland seit der Jahrtausendwende rasant entwickelt sowohl hinsichtlich der Anzahl der Einrichtungen und behandelter Patienten als auch bezüglich der Qualität der abgegebenen Therapie. Im gleichen Zeitraum zeigte sich in der muskuloskeletalen Rehabilitation ein neuer und zunehmend bedeutender Behandlungsschwerpunkt im Bereich der arbeitsplatzspezifischen Therapie. Vonseiten der Deutschen Gesetzlichen Unfallversicherung (DGUV) wurde deshalb die arbeitsplatzbezogene muskuloskeletale Rehabilitation etabliert. Die vorliegende Studie dokumentiert anhand von 38 Fällen im Beobachtungszeitraum 2014 bis 2016 deskriptiv deutliche Verbesserungen im Rehabilitationsverlauf für die verwendeten Assessments Funktionsfragebogen-Hannover-Rücken, DASH (Disability of Arm, Hand and Shoulder)- und WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index)-Score. Das Ziel der Wiederherstellung der Leistungsfähigkeit für die Wiederaufnahme der Arbeitstätigkeit oder für die Durchführung einer stufenweisen Wiedereingliederung wurde in der weit überwiegenden Anzahl der Fälle erreicht. Somit erbringt die Studie einen Beitrag zum Wirksamkeitsnachweis der arbeitsplatzorientierten muskuloskeletalen Rehabilitation in einem ambulanten orthopädischen Rehabilitationszentrum.

The Enchondroma is a common, benign, cartilage forming tumour. They usually occur as a single, asymptomatic lesion. Occasionally patients present with multiple enchondromas which is generally defined as enchondromatosis. This entity encompasses several different subtypes including Ollier disease and Maffucci syndrome (enchondromatosis associated with soft tissue haemangiomas) as the most commons. Some of them have a complicated clinical course when malignant transformation occurs. This malignant progression is a well known fact especially in enchondromatosis, but up to now there is still a lack of recommendations concerning the follow up. The aim of this article is to review the clinical and imaging features of patients with solitary enchondroma and enchondromatosis focusing on the development of secondary chondrosarcoma and the follow up.
Ziele: Evaluation posttraumatischer Knochenmarksläsionen mittels Dual-Energy CT basierter “virtual noncalcium technique“. Methode: In dieser prospektiven Studie wurden 21 Patienten mit einem akuten Knietrauma, die eine Dual-Energy CT und MRT erhalten hatten, eingeschlossen. Frakturen wurden klassifiziert. Aus den CT-Bildern wurde Calcium mittels einer Softwareapplikation virtuell subtrahiert. Das Vorhandensein einer Knochenmarksläsion wurde anhand einer 4-Punkt Skala für jeweils 6 femorale und tibiale Regionen von 2 Radiologen bewertet. Für jede Region wurde eine Dichtemessung (HU) durchgeführt. Das MRT diente als Referenzstandard. Für die Bildbewertung und die Dichtewertmessung wurden ROC-Analysen durchgeführt. Ergebnis: Nach Ausschluss von 16 Regionen aufgrund von Artefakten, fanden sich mit der MRT 59 Knochenmarksläsionen in den verbleibenden 236 Regionen (19 von 114 femoral, 40 von 122 tibial). Bei 8 Patienten wurden Frakturen festgestellt. Die visuelle Beurteilung zeigte für die beiden Auswerter AUC-Werte von 0,886 and 0,897 für den Femur sowie 0,974 und 0,953 für die Tibia. Für die Dichtewertmessung ergaben sich AUC Werte von 0,922 respektive 0,974. Wurden milde und kräftige Knochenmarksläsionen (Skala 1 und 2) als positiv gewertet, erzielten Auswerter 1 und 2 Sensitivitäten von 86,4% und 86,4% sowie Spezifitäten von 94,4% and 95,5%. Die Kappa Statistik zeigte eine gute bis exzellente Übereinstimmung (Femur, kappa=0,78; Tibia, kappa=0,87). Schlussfolgerung: Die Dual-Energy CT basierte “virtual noncalcium technique“ ermöglicht die Beurteilung des Knochenmarks und macht die Detektion posttraumatischer Knochenmarksläsionen des Knies mit der CT potentiell möglich.
Fractures of the proximal humerus are common, particularly seen in elderly, female patients. Using open reduction and internal fixation good clinical results can be achieved in general. But even today not every problem has been solved in the treatment of proximal humeral fractures. Varus displaced fractures are particularly challenging, especially when the medial column is destroyed. Anatomical reduction of the humeral head and medial bone contact are crucial for a good surgical outcome. Otherwise a secondary varus collapse and/or an implant failure are predictable. Further challenges are the intra-articular fracture patterns, as well as fractures with an initial ischemic humeral head. The indications for prosthetic replacement are always present if an initially stable internal fixation could not be achieved. The reverse fracture prostheses represent an increasingly common treatment option; however, the indication should be reserved for the elderly over 75 years.
Die Humeruskopffraktur ist eine häufige Verletzung, insbesondere des älteren weiblichen Patienten. Im Rahmen der Osteosynthese lassen sich in der Regel gute klinische Ergebnisse erzielen. Probleme werden jedoch weiterhin bei den die varisch dislozierten Humeruskopffrakturen, insbesondere wenn die mediale Säule zerstört ist, beobachtet. Hier sind eine anatomische Reposition des Kalottenfragments sowie ein medialer Knochenkontakt für ein gutes Operationsergebnis unentbehrlich. Ansonsten lässt sich ein sekundärer Varuskollaps und/oder ein Implantatversagen vorhersehen. Weitere Herausforderungen bilden die intraartikulären Frakturformen sowie die initial ischämen Humeruskopffrakturen. Die Indikation zur Frakturprothese ist immer dann gegeben, wenn keine stabile Osteosynthese in adäquater Reposition erzielt werden kann. Inverse Frakturprothesen stellen eine zunehmend verbreitete Therapieoption dar. Sie sollten jedoch den älteren Patienten >75 Jahren vorbehalten bleiben.
Ziel des Artikels ist das Vermitteln von Behandlungskonzepten für offene Frakturen und Weichteilschäden unterschiedlicher Schweregrade. Als Basis effektiver Therapiekonzepte werden die gängigen Klassifikationen vorgestellt und erklärt. Beginnend mit der primären (auch präklinischen) Versorgung werden diagnostische und therapeutische Arbeitsschritte von der Antibiotikatherapie über die Wundreinigung, die Frakturstabilisierung sowie den Möglichkeiten der konventionellen und plastischen Weichteildeckung diskutiert. Als Inhalte werden sowohl evidenzbasierte Behandlungskonzepte als auch übliche Verfahren und persönliche Vorgehensweisen vorgestellt und diskutiert. Die Behandlung von offenen Frakturen und Weichteilschäden ist eine Herausforderung für den behandelnden Chirurgen und muss von Beginn an bis zur erfolgreichen Heilung mit großer Sorgfalt durchgeführt werden. Bewährt hat sich ein frühzeitiger, interdisziplinärer Therapieplan. Durch sorgfältige und bedachte Therapie können auch schwere Verletzungen mit guten funktionellen Ergebnissen und wenig Komplikationen zur Ausheilung gebracht werden.
Über somatopsychische Belastungsreaktionen von Kindern und Jugendlichen auf Reizgasunfälle liegen kaum systematische Erhebungen vor. Nach einer nicht akzidentellen Exposition mit CS-Aerosol wurden 35 Schüler mit Affektionen der Schleimhäute und des Respirationstraktes nach Erstversorgung im Rahmen eines rettungsdienstlichen Großeinsatzes in ein geeignetes Schwerpunktkrankenhaus aufgenommen und nach vorgegebenen Organisations- und Behandlungsschemata unter Notfallbedingungen untersucht, behandelt und erforderlichenfalls stationär aufgenommen. Zwei Monate später wurden die Betroffenen hinsichtlich ihrer Belastungsreaktionen evaluiert. 16 (46%) der 35 Schüler beteiligten sich an der anonymisierten Erhebung. Die somatopsychische Belastung durch den Reizgasunfall wurde überwiegend als gering angegeben. An einzelnen somatischen Beschwerden wurden gehäuft Bauch- und Kopfschmerzen sowie Atembeschwerden berichtet. Fünf Schüler wiesen übereinstimmend in mehreren Kategorien klinisch bedeutsame Werte für somatische und psychische Belastungen aus Eigen- oder Elternsicht auf. Indirekt bestätigt sich diese stärkere Belastung durch häufigere ärztliche Konsultationen im Beobachtungszeitraum. Mit einer Auftretenswahrscheinlichkeit von 14–42% sind posttraumatische Stressreaktionen sind bei Kindern und Jugendlichen nicht zu unterschätzen. Mit den vorgestellten symptomorientierten Erhebungsinstrumenten lassen sich besonders belastete Kinder und Jugendliche frühzeitig identifizieren, sodass wir deren prospektiven Einsatz in der medizinischen Nachsorge von Großschadenslagen mit Kindern und Jugendlichen vorschlagen.
The aim of the study was to evaluate the application of a locked internal fixator in complex fractures of the proximal femur, in which the internal fixation with standard implants was not possible due to poor quality of bone or already failed internal fixation in the past.Ten patients suffering from a pertrochanteric (n = 5), periprosthetic (n = 1) or subtrochanteric (n = 4) femural fracture between 2003 and 2008 were prospectively registered, underwent open reduction and internal fixation with an "upside-down" femur LISS (less invasive stabilisation system) and were followed up. In all these patients a primary internal fixation had failed or the local bone situation and circulation were poor. The mean follow-up was 14 +/- 25 months. X-ray images and a clinical examination were performed at each appointment.All fractures reached a primarily stable fixation with the locked internal plate fixator. Seven patients showed a complete bone healing after 3 months of follow-up and could bear full body weight afterwards. Three patients with preoperatively existing vascular disease or chronic osteomyelitis showed a deep wound infection postoperatively, which led to the explantation of the implant.The "reversed" locked internal fixator could be a successful alternative implant for stabilisation of proximal femur fractures which could not be fixated by standard implants due to poor bone quality and circulation. It can also be used as a salvage procedure after internal failed fixation in proximal femur fractures.
Purpose: The treatment of paediatric polytrauma patients in the emergency room is not common. The knowledge of specific injuries in consideration of the age-specific characteristics is of particular importance for precise diagnostics and therapy. The goal of this study is the aquisition of the frequency, the localisation and the severity of paediatric polytrauma (age: 0-16years) in comparison with adults. Patients and Methods: In the period 7/01 to 5/04 the localisation and injury severity of 23 paediatric polytrauma patients (age: 2-16 years) were compared retrospectivly with those of 324 adults (age: 17-88 years). In the paediatric group (ISS: 31) the lethality was 17 % and so much higher than that in the grown-up population (ISS: 33) with 10 % at comparable injury severity. The cause of accident and the injury severity of the affected body region were analysed. The severity of the diffferent body regions were classified by the Abbreviated Injury Severity Score (AIS). The results were discussed with regard to the current literature. Results: With 65 %, more than every second child suffered from severe head injuries (AIS > 2), whereas only 37 % of the adults were affected in this way. The different types of intracranial bleedings were analysed and compared. Heavy injuries of the thorax (AIS > 2) were the result of an accident in 61 % of the children and in 54 % of the adults. The incidence of children with injuries to the abdomen was 30 % compared to 31 % in the grown-up collective. Lesions of the spleen and liver had a frequency of 13 to 16 %. Injuries of the spine could be only found in 4 % of the children compared to 40 % of the adult group. The frequencies of pelvic injuries were similar at 22 % for children and 28 % for adults. With 13 % for the upper extremities and 17 % for the lower extremities, children were much less injured in these body regions. In the group of adults 43 % had injuries to the upper extremities and 33 % injuries to the lower extremities. Conclusions: Taking the results into account with consdieration of the literature data, the authors recommend that the emergency room management for adults and, especially, the radiolgical diagnostic chain with CT scans should also be applied to poly-traumatised children. The main reasons for this are the extremely high incidence of intracranial injuries and the high sensitivity of CT scans also for abdominal trauma and pelvic injuries.
