Introduction : La tuberculose et le VIH constituent un couple mortel, qui représente un problème majeur de santé publique. Chez les personnes vivant avec le VIH, la tuberculose augmente la réplication du VIH et l'hétérogénéité virale. L’objectif de ce travail était d’identifier les facteurs liés aux décès chez les patients coinfectés par la tuberculose et le VIH, hospitalisés dans le service des maladies infectieuses et tropicales du CHU du Point G. Méthodologie : Il s’agissait d’une étude descriptive et analytique à collecte rétrospective réalisée dans le service des maladies infectieuses et tropicales sur une période de 3 ans allant de 2020-2022. Les dossiers de tous les patients infectés par le VIH hospitalisés dans le service atteints de tuberculose confirmée bactériologiquement, répondant aux critères d’inclusion ont été inclus. Les données ont été saisies et analysées sur le logiciel SPSS IBM statistique version 22. Le test statistique de Khi2 a été utilisé pour la comparaison des facteurs avec un seuil de significativité p≤0,05. Résultats : La prévalence hospitalière de la coïnfection VIH/TB était de 7,5 %. A l’issue du séjour hospitalier, 28 patients étaient survivants (35 %) et 52 décédés (65%). En analyse bivariée, les facteurs prédictifs de létalité étaient l’IMC < 16kg/m2 ; la PAM < 65 mmHg ; le stade clinique IV OMS ; le taux Hb < 7 g/dl ; le taux CD4 ≤ 200 c/mm3 ; la présence de pathologies infectieuses associées et le délai de décès court en hospitalisation <7 jours. Conclusion : La tuberculose est la première cause de mortalité chez les PvVIH. Mots-clés : VIH, Tuberculose, Décès, Bamako, Mali.
Introduction : L’infection à VIH/SIDA demeure un problème de santé publique au niveau mondial. Elle est une cause importante de morbi-mortalité en Afrique subsaharienne. La plupart des études sur l’infection à VIH en Afrique ont été réalisées à une époque où l’accès des PVVIH aux ARV était limité par les directives OMS. Le but de cette étude était d’actualiser certaines données chez les PVVIH dans un contexte d’accès facilité à la trithérapie antirétrovirale par la recommandation OMS « dépister et traiter ». Méthodologie : Il s’agissait d’une étude transversale descriptive à recrutement prospectif menée chez les PVVIH âgées de plus de 15 ans, ayant été hospitalisées dans le service des maladies infectieuses et tropicales du CHU du Point G, du 01 décembre 2020 au 31 mai 2021. Résultats : Nous savons inclus 108 patients dans l’étude dont 73 étaient de sexe féminin (67,6%), le sex-ratio était de H/F était de 0,5. L’âge moyen des patients était de 40,9±11,4 ans avec des extrêmes de 17 et 70 ans. Selon la classification clinique OMS, 98,2% des patients étaient en phase sida. Quant à la classification CDC d’Atlanta 1993, nos patients appartenaient à la catégorie C dans 74,1% des cas. Les principales infections opportunistes étaient la candidose buccale et œsophagienne (60,2%), la pneumopathie bactérienne non tuberculeuse (40,7%), la tuberculose pulmonaire (17,6%) et la toxoplasmose cérébrale probable (29,6%). Le VIH de type I prédominait avec 95,4% des cas. L’immunodépression était sévère (CD4<200 cellules/mm3) chez 82,4% de nos patients. La charge virale VIH a été réalisée chez 84 patients, dont 73 patients (86,9%) avaient une charge virale élevée (>1000 copies/ml). Les patients ayant une immunodépression sévère ont fait plus d’infections opportunistes (IO) que ceux ayant une numération CD4 ≥ 200/mm3 (P<0,05). Les pathologies non classant sida étaient prévalences chez les patients sévèrement immunodéprimés (P>0,05). Conclusion Le profil des patients de cette série n’est pas différent de celui décrit dans la littérature avant cette ère. Les patients consultent encore à un stade avancé de la pathologie. L’atteinte de l’objectif 95-95-95 permettrait de renverser la tendance. Mots-clés : Profil, Personnes vivant, VIH/SIDA, CHU du Point G.
Le pyothorax est définie par la présence dans l’espace pleural d’un liquide purulent. Elle peut être primaire, secondaire d’une pathologie générale dont la pneumonie représente une part majoritaire, ou faire suite à un traumatisme. Nous rapportons le cas d’un homme de 32 ans, immunocompétent au VIH, tabagique actif non sevré, présentant une cellulite cervicale d’origine dentaire compliquée de pleurésie purulente. Le diagnostic a été posé sur la base clinique et d’imagerie (radiographie pulmonaire et l’échographie abdominale). La prise en charge pluridisciplinaire consistait en un drainage, une avulsion des dents affectées, un curetage alvéolaire et une bi antibiothérapie adaptée. L’évolution a été favorable avec régression totale des symptômes respiratoires et le retour du poumon à la paroi. Mots-clés : dent, cellulite, pyothorax.
Objectifs de cette étude était de déterminer la prévalence hospitalière des infections nosocomiales. Patients et méthodes : Il s’agissait d’une étude prospective transversale descriptive sur une période de six (6) semaines. Résultats : 463 patients ont séjourné pendant au moins 48 heures. Parmi eux, 57 patients inclus soit une prévalence de 12,3%. L’âge moyen était de 45,4 ± 20,8 ans. La durée moyenne du séjour était de 21,7 ± 12,7 jours. Les patients provenaient de la réanimation (17,5%), la médecine interne et la neurologie dans 15,8% chacune. L’immunodépression au VIH a été retrouvée dans 14%. Un dispositif invasif était présent dans 93% des cas. Les prélèvements microbiologiques réalisés étaient : un examen cytobactériologique des urines (36 cas), un prélèvement de pus (19 cas), une hémoculture (23 cas). Les infections étaient urinaires (30 cas), du site opératoire (16 cas), une bactériémie (15 cas), une pneumopathie acquise sous ventilation mécanique (2 cas). Un germe était isolé dans ces prélèvements dans 94,4%. Les germes retrouvés étaient des entérobactéries (33 cas), des bacilles à Gram négatif non fermentant (12 cas), des cocci à Gram positif (6 cas) et des levures (3 cas). L’écologie était dominée par une résistance élevée des germes impliqués dans ces infections. Conclusion : Ce travail a montré une prévalence élevée des infections nosocomiales avec un profil de résistance varié des germes impliqués. Il est important d'élargir ce travail afin d'en tirer des recommandations fortes.
Toxoplasmosis is defined as a cosmopolitan protozoan disease caused by an obligate intracellular coccidia, Toxoplasma gondii. The advent of HIV infection has made cerebral toxoplasmosis one of the most widespread neurological opportunistic infections.METHOD:We conducted a descriptive cross-sectional study with retrospective review of files of cerebral toxoplasmosis on HIV infected patients who had been hospitalized in the infectious diseases department of Point G University Hospital between January 1st, 2014 and September 30th, 2019.RESULTS:During the study period, the frequency of cerebral toxoplasmosis was 10.1% and in 46.4% of the patients, the diagnosis led to the discovery of HIV co-infection. The clinical features were characterized by fever, headaches, and motor deficit at 86.6%, 84.5% and 69.1% respectively. Roundel image on computed tomography was most represented and was found in 24.4% of patients. Anti-toxoplasma treatment based on trimethoprim /sulfamethoxazole (TMP/SMX) associated with folinic acid was initiated in 78 patients out of 90, but 19 patients had a contraindication or adverse effects to this combination and were treated with clindamycin. HAART was initiated in 31 patients out of 45 (68.9%) newly diagnosed. The overall prognosis was limited with a mortality rate of 42%.CONCLUSION:The prevalence of cerebral toxoplasmosis was high in our study, 10.1%. To reduce this prevalence, chemoprophylaxis should be initiated in all HIV-infected patients with a CD4 count below 200 cells/mm3.
INTRODUCTION:The rapid diagnostic capacities of laboratories in Mali have been an essential element in the response to COVID-19. The University Clinical Research center (UCRC) diagnosed the first cases of Mali COVID-19.OBJECTIVE:The objective was to describe the contribution of the UCRC in the diagnosis of Covid-19 and to clinically and epidemiologically characterize the patients tested in the UCRC laboratory.MATERIALS AND METHODS:A cross-sectional study was conducted during eight months of intense activity. The samples were sent from the National Institute of Public Health (INSP) to the UCRC.RESULTS:The UCRC tested 12,406 contacts and suspected samples and confirmed the diagnosis in 1091 patients, or 9%. The most common symptoms were cough (48.78%), headache (34.14%), fatigue / weakness (34.14%), while (33.33%) of the patients were asymptomatic. The sample positivity rate among new cases decreased from May to September 2020, despite almost 230% of the number of samples tested.CONCLUSION:The laboratory played a major role in the response and there may be a low transmission of the virus in the Malian community.
RESUME Objectif. Determiner la prevalence du VIH, des hepatites virales B et C, et du Treponeme pâle chez les donneurs de sang a Segou ainsi que les facteurs sociodemographiques associes. Population et methodes. Il s’agit d’une etude retrospective dont les donnees ont ete recueillies a partir des dossiers patients et du registre de don de sang a l’aide d’une fiche d’enquete. Resultats. Au total, 4060 personnes ont fait un don de sang durant la periode. Les hommes etaient majoritaires (95%). Nous avons trouve 905 donneurs infectes par au moins des pathogenes recherches. Les dons de compensations ont represente 96% des cas. La tranche d’âge de 18-35 ans a represente 71%. La moyenne d’âge a ete de 31,10 ans. Le groupe sanguin le plus retrouve a ete le groupe O (40.81%). Le rhesus dominant a ete le positif (93%). Les prevalences etaient les suivantes : hepatite B (10.84%), syphilis (7.36%), virus de l’hepatite C (4.06%) et VIH (3.13%). Une coinfection a ete trouvee chez 114 donneurs. La coinfection la plus rencontree a ete celle de la syphilis et de l’hepatite B. La prevalence de la triple infection hepatite B, hepatite C et la syphilis a ete de 0.12%. Toutes ces serologies ont ete trouvees positives chez un donneur. Conclusion. Les infections etudiees sont cosmopolites. L’hepatite B est la plus frequente. La fidelisation des donneurs pourrait etre une solution pour garantir un bon approvisionnement en quantite et en qualite de produits sanguins.ABSTRACTObjective. To determine the prevalence of HIV, Hepatitis B and C viruses and Treponema pallidum among blood donors associated socio-demographic factors. Population and Methods. This was a cross sectional retrospective study in which data were collected from patient records and the blood donation register using a survey form. Results. A total of 4060 people donated blood during the period. The great majority of them were men (95%). We collected 905 donors who were infected with at least one of the pathogens tested for. Compensatory donations accounted for 96% of the cases. The age range of 18-35 years was 71%. The average age was 31.10 years. The most common blood type was group O (40.81%). The dominant rhesus was positive (93%). The prevalence of hepatitis B, syphilis, hepatitis C and HIV was 10.84%, 7.36%, 4.06% and 3.13% respectively. At least one co-infection was found in 114 donors. The most common co-infection was syphilis - hepatitis B. The prevalence of triple infection with hepatitis B, hepatitis C and syphilis was 0.12%. All his serologies were found positive in one donor. Conclusion. The infections studied are cosmopolitan. Hepatitis B is the most frequent. Donor loyalty could be a solution to guarantee a good supply in quantity and quality of blood products.
ABSTRACT Objectif. Decrire les troubles electrolytiques dans un groupe de patients hospitalises infectes par le VIH dans la ville de Bamako. Patients et methodes Nous avons mene une etude transversale analytique retrospective au service des maladies infectieuses du CHU du Point G, du 1er janvier 2011 au 31 decembre 2016. Nos parametres d’etude etaient les resultats de l’ionogramme au cours de l’hospitalisation. Resultats Cent vingt et six patients vivant avec le VIH ayant realise un ionogramme sanguin au cours de leur hospitalisation ont ete inclus. La valeur moyenne de la natremie etait de 133,06±12,16 mmol/l et celle de la kaliemie de 3,89±1,15 mmol/l avec comme extremes respectivement 83,00 et 166,00 mmol/l ; 1,60 et 9,00 mmol/l. Les troubles electrolytiques retrouves etaient l’hyponatremie (41,2%), l’hypokaliemie (12,7%), l’hypernatremie (7,8%), l’hyperkaliemie (2,9%) et les troubles associes (35,3%). Il existait une association statistiquement significative entre les dysnatremies et la tuberculose (p=0,018) d’une part et son traitement (p=0,016) d’autre part. Une liaison statistiquement significative a ete aussi observee entre les dyskaliemies et la toxoplasmose cerebrale (p=0,002) ; la candidose buccale (p=0,008) ; la diarrhee (p=0,023) ; l’etat nutritionnel (p=0,023) ; la prise des ARV (p=0,020). Enfin, la mortalite etait liee au niveau d’immunodepression (p=0,012) ; a l’etat d’hydratation (p=0,010) et a la presence d’une toxoplasmose cerebrale (p=0,007). Conclusion. Les desordres electrolytiques sont frequents chez les patients vivant avec le VIH. Des associations statistiquement significatives ont ete trouvees entre ces troubles et certains evenements de survenue frequente chez les patients infectes par le VIH. Des etudes ulterieures de type prospectif avec un plus grand echantillon sont necessaires pour confirmer ces resultats. RESUME Objective. To describe electrolyte disorders in a group of HIV infected patients hospitalized in a reference treatment center of Bamako. Methods. This was a cross sectional retrospective analytic study has been conducted in the department of infectious diseases at the University hospital of Point G from 01 January 2011 to 31 December 2016. Our study variables were the ionogram findings. Results. One hundred and twenty-six HIV positive patients who did serum sodium, potassium and chloride dosage during their hospitalization were studied. The mean value was for the serum sodium 133,06±12,16 mmol/l, serum potassium 3,89±1,15 mmol/l with as extreme values respectively 83,00 and 166,00 mmol/l; 1,60 and 9,00 mmol/l. The main electrolyte disorders were hyponatremia (41.2%); hypokaliemia (12,7%); hypernatremia (7,8%); hyperkaliemia (2,9%) and associated disorders (35,3%). A statistically significant association was found between dysnatremia and tuberculosis (p=0,018)on one hand, and its treatment (p=0,016) on the other hand. A significant statistic association was also found between dyskaliemia and cerebral toxoplasmosis (p=0,002); oral candidosis (p=0,008); diarrhea (p=0,023); nutritional status (p=0,023); HAART usage (p=0,020). Moreover, mortality was associated to immunosuppression level (p=0,012); hydratation status (p=0,010) and presence of cerebral toxoplasmosis (p=0,007). Conclusion. Electrolyte disorders are common in HIV infected patients. We found statistically significant associations between electrolyte disorders and some common clinical occurrences in HIV infected patients. Further prospective studies are needed to confirm our results.
Background:The external quality assessment (EQA) or external quality control is an evaluation conducted by a certified external organization to inquire about the quality of the results provided by a laboratory. The primary role of EQA is to verify the accuracy of laboratory results. This is essential in research because research data should be published in international peer-reviewed journals, and laboratory results must be repeatable. In 2007, the University Clinical Research Center (UCRC's) biosafety level 3 (BSL-3) laboratory joined the EQA program with the College of American Pathologists in acid-fast staining and culture and identification of mycobacteria as per laboratory accreditation preparedness. Thus, after 11 years of participation, the goal of our study was to evaluate the performance of our laboratory during the different interlaboratory surveys. Methods:We conducted a descriptive retrospective study to evaluate the results of UCRC mycobacteriology laboratory from surveys conducted during 2007 and 2017. Results:Of the 22 evaluations, the laboratory had satisfactory (100% of concordance results) in 18 (81.8%) and good (80% of concordance results) in 4 (18.2%). Overall, the laboratory was above the commended/accepted limits of 75%. Conclusion:So far, UCRC's BSL-3 performed well during the first 11 years of survey participation, and efforts should be deployed to maintain this high quality in the preparedness for laboratory accreditation and support to clinical trials.
BACKGROUND: Non-conversion on auramine smear microscopy indicates a lack of treatment response, possibly associated with initial rifampicin-resistant tuberculosis (RR-TB). However, dead bacteria still stain positive and may be detected. Fluorescein diacetate smear microscopy (FDA) shows live mycobacteria only. Therefore, we studied the potential of 2-month (2M) FDA for the identification of initial RR-TB.METHODS: Between 2015 and 2018, we enrolled new smear-positive pulmonary TB patients from five local centres in Bamako, Mali. After baseline screening, sputum samples were collected at 1M, 2M, 5M and 18M. We used rpoB sequencing to identify initial RR-TB.RESULTS: Of 1359 patients enrolled, 1019 (75%) had rpoB sequencing results. Twenty-six (2.6%, 95%CI: 1.7-3.7) had mutations conferring rifampicin resistance. Most frequent rpoB mutations were located at the codons Asp435Val (42.4%) and Ser450Leu (34.7%). Among patients with initial RR-TB, 72.2% were FDA-negative at 2M (P = 0.2). The positive and negative predictive value of 5M FDA for culture-based failure was respectively 20.0% and 94.7%.CONCLUSION: FDA did not identify the majority of patients with initial RR-TB or culture-based failure. As the full spectrum of mutations identified on sequencing was identified using Xpert, our data support its rapid universal implementation in Mali.
RESUME Introduction. Les atteintes meningees d’origine bacterienne demeurent un probleme de sante publique dans le monde et particulierement en Afrique dans la ceinture meningitique de Lapeyssonnie. Notre etude avait pour but d’identifier les etiologies des atteintes meningees dans un service de maladies infectieuses au Mali. Materiels et methodes. Il s’agit d’une etude transversale descriptive retrospective qui a concerne les malades hospitalises dans le service de maladies infectieuses et tropicales du CHU du Point G entre le 01 Janvier 2011 et 31 Decembre 2014. Les variables etudiees etaient les donnees sociodemographiques, les aspects cliniques, l’analyse bacteriologique du liquide cephalorachidien, la serologie HIV et les lesions scanographiques. Resultats. Sur un total de 1697 hospitalisations durant la periode d’etude, nous avons recense 122 cas de meningite (7,2%). Un antecedent d’infection ORL a ete retrouve chez 28 patients (23%). Une infection a VIH etait diagnostiquee chez 45,9% des patients. La notion de prise d’antibiotiques avant la ponction lombaire a ete retrouvee dans 58,2% des cas. Le LCR avait un aspect trouble dans 63,1% des cas. Les etiologies retrouvees etaient bacteriennes dans 33,6% de cas, fongiques de 13,1% de cas, mycobacteriennes dans 3,3% des etiologies bacteriennes et non determinees dans 53,3% de cas. Neisseria meningitidis serogroupe W135 a represente 67,6% des etiologies bacteriennes. La serologie VIH positive et un taux de CD4 ≤ 100 cellules/mm3 a ete observee dans les meningites a Cryptocoque. Le taux de guerison global etait de 76%. Le taux de mortalite etait de 24%. Conclusion. Les bacteries sont la premiere cause de meningites et le meningocoque serogroupe W135 est le germe le plus retrouve. En relation avec l’immunodepression a VIH, la cryptococcose est egalement frequente. ABSTRACT Introduction. Bacterial meningitis is a public health issue in Africa in the \"meningitis belt\" of Lapeyssonnie. The aim of our study was to describe the etiologies of meningitis in a hospital setting in Mali. Materials and methods. This was a transversal retrospective study carried out in patients admitted in infectious Diseases department of the Point G University Teaching Hospital, between January 1st 2011 and December 31st 2014. Our study variables were sociodemographic data, clinical presentation, CSF bacteriology, HIV serology and CT findings. Results. Out of 1697 inpatients during the study period, 122 cases (7.2%) of meningitis were recorded. A past history of ENT infection was found in 28 patients (23%). HIV infection was diagnosed in 45.9% of patients. History of absorption of antibiotics prior to lumbar puncture was found in 58.2% of cases. In 63.1% of cases, the CSF was cloudy. The etiology of meningitis was bacterial in 33.6% of cases, fungal in 13.1% of cases, mycobacterium in 3.3% of cases and unknown in 53.3% of cases. Neisseria meningitidis (serogroup W135) represented 67.6% of the bacterial etiologies. Positive HIV serology and a CD4 cell count ≤ 100 cells/mm3 were present in all cryptococcal meningitis. The overall cure rate was 76% and the mortality rate was 24%. Conclusion. Bacteria (mostly N. meningitidis W135) are the main causes of meningitis in the infectious diseases department of Point G teaching Hospital. Cryptoccoccal meningitis is also frequent, and is always related to severe immunosuppression by HIV.
Introduction Le diagnostic de primo-infection reste insuffisamment évoqué compte tenu des moyens de diagnostic limités dans le contexte des pays à ressources limitées. Ce travail a pour but d’identifier les cas d’infection aigüe à VIH dans le district de Bamako au Mali. Méthodologie Il s’agit d’une étude prospective transversale et descriptive réalisée chez les tout-venants en consultation dans 12 centres de santé, de conseil et de dépistage volontaire de Bamako. La démarche du diagnostic consistait à utiliser une approche clinique combinée à un counseling permettant d’obtenir les symptômes et les comportements à risque et une approche biologique consistant à réaliser les tests de diagnostic du VIH. Les marqueurs biologiques recherchés et la détermination des stades de l’infection aigüe ont été faits comme décrits par Fiebig et al. Résultats Sur 29130 sujets dépistés dans les 12 centres tous cas confondus, 19 avaient une infection aigue avec une anomalie hématologique associée (thrombopénie, lymphopénie ou neutropénie). La charge virale médiane était de 58108 copies/ml (4,8Log10), le CD4 médian de 339 cellules/mm3 et le CD8 médian de 772 cellules/mm3. Les stades d’infection aigues étaient les suivants : stade 1 (5 cas), stade II (1 cas), stade III (6 cas) et stade IV (7 cas). Le facteur d’exposition le plus fréquent était le rapport sexuel non protégé retrouvé dans 14 cas. Conclusion Dans le renforcement du dépistage, un accent particulier devrait être mis sur l’identification des infections aigues avec un renforcement du counseling dans les groupes à risque d’exposition élevé
RESUMEIntroduction. Les infections urinaires sont frequentes sur les terrains d’immunodepression. Le but de notre etude etait de decrire les aspects epidemiologiques, cliniques et bacteriologiques des infections urinaires d’origine bacteriennes chez les PVVIH dans le service des maladies infectieuses du CHU de Point ‘‘G’’, du 1er janvier 2014 au 31decembre 2016. Methodes. Le diagnostic de l’infection urinaire a ete retenu sur la base de la presence d’une leucocyturie superieure ou egale a 10000 leucocytes/ml avec ou sans hematurie et/ou l’isolement d’une bacterie a l’uroculture. Resultats. La prevalence des infections urinaires bacteriennes a ete de 4,30% (n=48). Les principaux signes cliniques ont ete les brulures mictionnelles (n=5), la pollakiurie (n=2) et la pyurie (n=1). Escherichia coli a ete trouve dans 59,52% (n=25) des cas et avaient une sensibilite de 90,09% (n=22) a l’imipeneme, 88% (n=25) a la colistine, 69,23% (n=13) au cefotaxime. La resistance etait superieure a 65% pour les antibiotiques utilises en routine comme la doxycycline 84,61% (n=13), association amoxicilline acide clavulanique 73,33% (n=15), la gentamicine 68,75% (n=16), la ciprofloxacine 66,66% (n=15) et le ceftriaxone 66,66% (n=15). Dans 100% des cas, nos souches etaient resistantes au trimethoprime. Conclusion. Devant cette augmentation croissante de germes multi resistants, l’antibiotherapie de premiere intention en matiere d’infection urinaire merite d’etre redefinie apres une etude nationale sur l’ecologie et la sensibilite bacterienne.ABSTRACTObjective. Escherichia coli was found in 59.52% (n = 25) of the cases and had a sensitivity of 90.09% (n = 22) to imipenem, 88% (n = 25) to colistin, 69.23 % (n = 13) to cefotaxime. However, the resistance exceeded 65% with antibiotics used routinely as doxycycline 84.61% (n = 13), amoxicillin combination clavulanic acid 73.33% (n = 15), gentamicin 68.75% (n = 16), ciprofloxacin 66.66% (n = 15) and ceftriaxone 66.66% (n = 15). In 100% of cases our strains were resistant to trimethoprim. Conclusion. In view of this growing increase of multi-resistant germ, first-line antibiotic therapy for urinary tract infection deserves to be redefined after a national study on ecology and bacterial sensitivity.
RESUMELa cryptococcose neuromeningee est une infection fongique frequente chez les patients immunodeprimes au VIH. Sa survenue sur terrain immunocompetent au VIH doit faire rechercher systematiquement d’autres facteurs favorisants. Nous rapportons un cas de cryptococcose neuromeningee chez une patiente de 32 ans immunocompetente au VIH, sans aucun facteur d’immunodepression connu et sans antecedents medicaux notables qui a presente un tableau de meningo-encephalite subaigu et des troubles de la vision a type de diplopie. Le test au VIH etait negatif avec un taux de CD4 egal a 1367 cellules/mm3.La culture bacterienne du liquide cerebrospinal etait egalement sterile. Les virus responsables d’infection meningee n’ont pas ete recherches a cause de non disponibilite de ces analyses. L’evolution ete favorable sous fluconazole pendant la phase d’attaque du traitement puis les symptomes ont repris en debut de la phase d’entretien. Elle est decedee le 16eme jour du traitement. Le contexte de meningite bacterienne ou virale ne doit pas faire occulter la possibilite d’une meningite a cryptocoque quelque soit l’etat immunitaireABSTRACTNeuromeningeal cryptococcosis is a common fungal infection in HIV-immunocompromised patients. Its occurrence on immunocompetent patients to HIV should prompt to seek other predisposing factors. We report a case of neuromeningeal cryptococcosis in an immunocompetent 32-year-old female patient, without any known immunosuppressive factor and without a notable medical history. She presented with subacute meningoencephalitis and diplopia-type vision disorder syndrome. HIV serology was negative and the CD4 count was 1367 cells / mm3. The bacterial culture of the cerebrospinal fluid was sterile. Diagnostic tests for other virus responsible for meningeal infection were unavailable. Clinical course during the early days of initiation treatment with fluconazole was good. But, the symptoms and signs relapsed at the beginning of the maintenance phase. She died on the 16th day. A clinical picture of bacterial or viral meningitis should not overshadow the possibility of cryptococcal meningitis whatever the immune status.
RESUMEIntroduction/ but. La maladie a virus Ebola se manifeste par une fievre associee a des signes divers non specifiques. En periode d’epidemie, tout signe associe a la fievre pourrait conduire un patient dans un centre de triage. L’objectif de ce travail etait de decrire la distribution des motifs d’admission des patients dans le centre de traitement Ebola (CTE) de Police training School II (PTSII) de Freetown en Sierra Leone. Methodes. Il s’agissait d’une etude transversale retrospective qui consistait en une analyse des donnees relatives aux motifs d’admission entre decembre 2014 et fevrier 2015. La PCR etait utilisee pour la confirmation des cas de la maladie a virus Ebola. Un test de diagnostic rapide du paludisme etait systematiquement realise chez tous les patients. Resultats. Nous avons enregistre 152 admissions dont 93 hommes et 59 femmes. 52 patients soit 34,21% des admissions avaient une PCR positive. La moyenne d’âge des patients etait de 28,37 ans avec des extremes de 2 et 72 ans. Les signes les plus frequents chez les sujets avec PCR positive etaient dans l’ordre : manque appetit, fievre, douleur abdominale, vomissement, conjonctivite, dysphagie + dyspnee, fatigue intense, diarrhee, cephalees, hoquet. Conclusion. La maladie a virus Ebola n’a pas de caracteristiques symptomatologiques specifiques. La similitude symptomatologique de la maladie a virus Ebola avec les pathologies infectieuses endemiques en Afrique explique l’engouement des populations vers les centres de traitement Ebola en periode d’epidemie.ABSTRACTIntroduction/Purpose. Ebola virus disease (EVD) is manifested by fever associated with various non-specific signs. During an epidemic, any sign associated with fever could lead a patient to an Ebola treatment center. The objective of this work was to describe the distribution of reasons for admission of patients to the Ebola Treatment Center Police Training School (PTSII) in Freetown, Sierra Leone. Methods. This was a cross sectional retrospective study which consisted of an analysis of data on the reasons for admission of patients between December 2014 and February 2015. The PCR was used for confirmation of cases of Ebola virus. A rapid diagnostic test for malaria was routinely performed in all patients. Results. We recorded 152 admissions, including 93 men and 59 women. The PCR result was positive in 52 patients (34.21%) of admissions. The average age of patients was 28.37 years with extremes of 2 and 72 years. The most common reasons of admission of positive PCR patients were by decreasing order of frequency: anorexia, fever, abdominal pain, vomiting, conjunctivitis, dysphagia + dyspnea, intense fatigue, diarrhea, headache and hiccup. Conclusion. We found no specific combination of signs and symptoms for EVD. Similarity of signs and symptoms between EVD and other common infectious tropical diseases may explain why many patients consult Ebola treatment centers during epidemics.
RESUME Introduction. L’infection par le VIH et la tuberculose constitue un probleme majeur de sante publique en Afrique. Le but de ce travail etait d’etudier les aspects epidemio-cliniques de cette coinfection chez les patients suivis dans le service de maladies infectieuses (SMI) du Centre Hospitalier et Universitaire (CHU) du Point‘‘G’’. Materiels et methodes. Il s’agit d’une etude retrospective de trois ans, qui a eu lieu entre le 1er janvier 2014 et le 31 decembre 2016. Elle a porte sur les patients atteints de tuberculose toutes formes confondues et infectes par le VIH, dont l’etat clinique a necessite une hospitalisation dans le SMI du CHU du Point‘‘G’’. Resultats. La prevalence de la coinfection par le VIH/Tuberculose etait de 4,30%. Le sexe masculin etait le plus touche avec un sexe-ratio de 1,53. Les maries etaient majoritaires avec 68,75%. La tranche d’âge 35-50 ans etait dominante avec 75%. La fievre et l’amaigrissement etaient presentes respectivement dans 91,67% et 77,08% des cas. La tuberculose pulmonaire isolee etait predominante (43,75%). Le VIH1 etait plus represente dans 93,75%. Le Taux lymphocytaire CD4 moyen etait 71,51/mm3. le taux de deces etait plus represente dans 35,42% suivi du taux de guerison dans 22,92%. Le schema ARV TDF+3TC+EFV a ete plus prescrit avec 73,81%. L'evolution a ete globalement favorable chez nos patients. Conclusion. Ce travail souligne une predoinance du sexe masculin.Les signes majoritaires etaient la fievre et l’amaigrissement La recherche de la Tuberculose doit etre systematique chez toute personne vivant avec le VIH et vice versa. ABSTRACT Introduction. HIV infection and tuberculosis is a major public health problem in Africa. The aim of this work was to study the epidemiological and clinical aspects of this co-infection in patients followed in the Department of Infectious Diseases (IMS) of the Center Hospitalier et Universitaire (CHU) of Point ''G ''. Materials And Methods. This was a 3-year retrospective study, January 1, 2014 to December 31, 2016. It included patients with all forms of tuberculosis and HIV-infected patients, whose clinical condition required hospitalization. the SMI of the Point''G 'CHU. Results. The prevalence of HIV / TB co-infection was 4.30%. The male sex was the most affected with a sex ratio of 1.53. The married were in the majority with 68.75%. The 35-50 age group was dominant with 75%. Fever and weight loss were present in 91.67% and 77.08% of cases, respectively. Isolated pulmonary tuberculosis was predominant (43.75%). HIV1 was more represented in 93.75%. The mean CD4 lymphocyte rate was 71.51 / mm3. the death rate was more prevalent in 35.42% followed by the cure rate in 22.92%. The ARV regimen TDF + 3TC + EFV was more prescribed with 73.81%. The evolution has been generally favorable for our patients. Conclusion. This work underlines a prediction of the male sex. Majority signs were fever and weight loss. The search for tuberculosis should be systematic in all people living with HIV and vice versa.
La prescription de médicaments chez la femme enceinte est une problématique fréquente en Afrique. Le but de cette étude est de décrire la prescription médicamenteuse chez les femmes enceintes au deuxième niveau de la pyramide sanitaire au Mali. Nous avons mené une étude des prescriptions médicamenteuses par l’observation sur 200 femmes enceintes au centre de santé de référence de Koutiala entre le 1er juillet 2015 et le 29 février 2016 en utilisant un questionnaire. Les patientes étaient de niveau d’instruction et de profession divers avec 7 % de niveau d’instruction supérieur. L’âge des gestantes se situait majoritairement entre 15 et 24 ans (54,5 %). Les multi-gestes étaient les plus présentes soit 56,5 %. Les multipares ont été les plus nombreuses avec 38 %. Les pathologies non gynéco-obstétriques étaient importantes (46 %). La consultation prénatale a été le motif de consultation le plus fréquent (54,5 %). Les ordonnances de qualité non conforme ont été les plus représentées (84,5 %). Les médicaments antianémiques/vitamines étaient les plus prescrits (30 %), suivis des antipaludéens (20,4 %). Plus de 85,5 % des médicaments prescrits l’étaient en nom de spécialité. Les ordonnances dont le prix variait entre 1,500 à 3,000 FCFA (2,30 à 4,60 euros), étaient le plus représentées soit (39 % des ordonnances), suivies de celles de plus de 3,100 FCFA (4,75 euros) (33,5 %). Les ordonnances comportant en moyenne 3 à 6 médicaments représentaient 51,5 % des cas. Les médicaments potentiellement dangereux en cas de grossesse concernaient 18 cas. Une petite moitié des prescriptions (38,5 %) n’était pas rédigée par des médecins. Le respect des exigences déontologiques de la bonne prescription chez la femme enceinte assurerait une sécurité à la femme et au futur nourrisson, surtout au Mali où il existe des agents non autorisés dans la prescription médicale. Cette étude montre que les femmes prennent différents types de médicaments pendant leur grossesse. La qualité très moyenne de ces prescriptions nécessite une évolution de la réglementation avec formation des infirmières obstétriciennes au Mali. Drug prescription for pregnant women is a frequent issue in Africa. The purpose of this study was to describe drug prescription for pregnant women at the second level of the health pyramid in Mali. A descriptive study observed 200 pregnant women in the Koutiala Reference Health Center, Mali, from July 1, 2015 to February 29, 2016, using a questionnaire. Patients were of varying educational levels and occupations, 7% with higher education. Age was mainly between 15 and 24 years (54.5%). Among, 56.5% were multigestational and 38% multiparous. Non-gynecological obstetric pathologies were dominant (46%), and prenatal consultation was the most frequent reason for consultation (54.5%). Among 84.5% of prescriptions were non-compliant. 30% of prescriptions were for antianemic drugs and vitamins, followed by antimalarials (20.4%). More than 85.5% of prescribed drugs were brand name. Prescription costs were 1,500 to 3,000 FCFA (€ 2.30–4.60) in 39% of cases, followed by > 3,100 FCFA (> € 4.75) in 33.5%. Among, 51.5% of prescriptions comprised 3-6 drugs. In 18 cases, the prescribed drugs were potentially dangerous in case of pregnancy. Among, 38.5% of prescriptions were not written by physicians. Conformity to good prescription practices for pregnant women would improve safety for the woman and future infant, especially in Mali where there are non-authorized agents involved in the medical prescription process. This study showed that women took various medications during pregnancy. Given the relatively poor quality of prescriptions, we recommend changes in the regulations, with training for obstetric nurses in Mali.
Despite access to ARVs, HIV-infected patients are not immune to opportunistic infections. The aim of this study was to identify the causes of the deaths of people with HIV on ARVs, hospitalized in the Sikasso hospital, in order to improve the preventive and curative management of these opportunistic infections. From 1 July 2014 to 31 December 2015, we conducted a retrospective and descriptive study of 35 patients on antiretroviral treatment who died at the Sikasso hospital. Type 1 HIV infection was predominant with 94.2% of cases. Low socioeconomic status, poor adherence to ARVs and severe immunosuppression were the contributing factors in the development of opportunistic infections. Chronic cough and altered general status were the most incriminating reasons for hospitalization, with 20% of cases each. All patients received primary chemoprophylaxis with cotrimoxazole. Tuberculosis was the most opportunistic infection associated with deaths with 57.1% of cases in its multifocal form and 2.9% of cases in its pulmonary form, followed by mycotic esophagitis, cerebral toxoplasmosis and gastroenteritis (Cryptosporidiosis and isosporosis) with respectively: 17.1%; 11.2% and 8.6% of cases. The CD4 cell counts of the patients were much collapsed with a death rate of 100% at less than 50 cells / mm3. The combination of zidovudine + lamivudine + efavirenz was the most widely used ARV regimen in 37.1% of cases. Tuberculosis remains the first infection, causing deaths of patients living with HIV on ARVs, and good adherence to therapeutics will help reduce the death rate. Accidental exposure to blood (AEB) constitutes a risk of transmission of blood-borne viruses including HIV, hepatitis B and hepatitis C virus especially amongst health workers. This study aims to describe accidental exposure to blood and post exposure management five years after it begin at the Hospital of Gao, since 2012 there is insurrection in this area making difficult to maintain health care system. This study will help to identify gaps to fill in order to improve the prevention of blood borne pathogen infection in this hospital. All cases of AEB declared at the Hospital from January 1st 2007 to December 31, 2011 have been enrolled. The sociodemographic profile of victims, the circumstances of the accident, the post exposure management and the follow up of subject have been registered. Data were entered and analyzed with the SPSS software. Chi square test was used with the level of significance p<0.05. During the 60 months of the study, an average of 124 workers were employed at the Hospital of Gao and 15 cases of accidental blood exposure were declared equal to an incidence density of 4.2 cases per 100 persons per year. Males were the most affected (60%). Nurses and lab technicians represent the majority of victims (53.3%). The most frequent type of accident was the injury by cannula needle sticks during intravenous injections (73.3%). Most of the cases received High Activity Antiretroviral Therapy (HAART) within a mean delay of 25 Hours from the accident, and the regiment (AZT/3TC/LPV/r) is predominantly used. HIV serology was done in 53.3% of subjects and revealed 1 positive case. Initial blood tests in all victims were normal. The HAART observance was worst among medical doctors (p < 0.001). No contaminations by HIV were detected after 6 months of follow up. The frequency of declared accidental exposures to blood at the Hospital of Gao is low. The post exposure management is facing a problem of observance of the treatment whenever it shows efficacy in all cases.
