BackgroundHabituation to repetitive noxious stimuli is a well-known phenomenon. We investigated brain correlates of habituation to pain in a transdermal electrical pain model using functional magnetic resonance imaging (fMRI).MethodsElectrical painful stimulation with 1Hz was applied to the volar forearm of 48 healthy subjects for 45min. Before and after conditioning stimulation, psychophysical testing and fMRI were performed. During fMRI sessions, the subjects underwent blockwise painful electrical stimulation with a fixed percept-adapted current intensity. After fMRI 1 and fMRI 2 subjects rated the individual pain intensity of the electrical stimulus.ResultsSubstantial habituation occurred during conditioning electrical stimulation. Accordingly, areas typically involved in pain processing showed decreased activity after conditioning stimulation. The blood oxygen level-dependent signal of the subgenual anterior cingulate gyrus, the superior parietal lobule and the supplemental motor area correlated positively with habituation. In contrast, activity in the periaqueductal grey, thalamus and insula correlated negatively. The results of the correlation analyses did not survive correction for multiple comparisons.ConclusionsWith this study, we identified central components associated with habituation to repetitive painful stimuli. The results suggest that an increase in tonic inhibitory activity in cortical pain processing areas is a major mechanism contributing to habituation to phasic noxious stimuli. Moreover, areas involved in descending pain modulation were differentially modulated. This may hint at a simultaneous activation of facilitating and inhibiting nociceptive systems that are both altered in the transdermal electrical pain model.
BACKGROUND AND PURPOSE:Noninvasive imaging of cerebral aneurysms is still considered inferior to conventional angiography. The purpose of this study was to evaluate the diagnostic accuracy of ivACT in the assessment of intracranial aneurysms compared with 3D-DSA.MATERIALS AND METHODS:We included 13 patients with 15 incidental unruptured saccular aneurysms scheduled for diagnostic angiographic work-up in our study. In each patient, we performed an ivACT and a conventional angiography including a 3D rotational run. During postprocessing, MPR images were generated for each technique. Maximal aneurysm diameter, neck diameter, aneurysm height, maximum width, bulge height, parent artery diameter, and angle between the parent artery and aneurysm apex were measured for each aneurysm.RESULTS:3D-DSA and ivACT both provided images of high quality without artificial disturbances (ie, motion artifacts). Measurements of all parameters resulted in comparable values for both modalities with a strong correlation (P ≤ .001).CONCLUSIONS:ivACT is feasible for the noninvasive visualization of saccular cerebral aneurysms and may provide reliable diagnostic information for the assessment of aneurysm size and geometry comparable with conventional intra-arterial 3D rotational angiography. These preliminary results might be a first promising step to replacing conventional angiography in preinterventional aneurysm imaging.
Ziele: Flow Diverter (FD) und Stents werden zunehmend zur Therapie von Aneurysmen eingesetzt. Eine objektive Messmethode zur Darstellung der hämodynamischen Auswirkungen eines FD oder Stents wäre wünschenswert. Methode: Bei 10 Kaninchen wurden Aneurysmen (Elastase Modell) induziert und mit Flow Divertern (Pipeline n=5) oder einem konventionellen Aneurysma Stent (Neuroform n=3) behandelt. Zwei Tiere dienten als Kontrollen. Vor und nach Implantation wurden Angiographieserien mit einer Bildfrequenz von 30 Bildern/Sekunde angefertigt. Mit einer neuen Software zur Analyse von DSA-Serien (prototypische Weiterentwicklung syngo iFlow, Siemens AG, Healthcare Sector, Forchheim) wurden Zeit-Kontrast Kurven (ZKK) erstellt. Es wurde das Time to Peak (TTP, maximaler Kontrast im Aneurysma), die volle Halbwertsbreite (VHB) sowie die durchschnittliche Steigung der Kurve (KM-Dichteänderung/Zeit) zur Darstellung der Durchschnittliche Ein- und Ausstromrate (ESÄ, ASÄ) ermittelt. Vor, unmittelbar nach Implantation und im Verlauf nach 8 Monaten wurden Kontrollangiographien durchgeführt. Ergebnis: Bei allen Versuchstieren konnten DSA-Serien und ZKK erstellt werden. Die Mittelwerte vor Therapie waren TTP=0,6sec, VHB=0,9sec, ESÄ=216,1 und ASÄ=81,5. Nach FD war das TTP mit 1,4sec und die VHB mit 14,6sec deutlich länger. Die ESÄ war mit 60,7 und die ASÄ mit 6,2 deutlich verzögert. Die Werte nach Stent Therapie (TTP=0,6sec, VHB=0,8sec, ESÄ=198, ASÄ=81,3) unterschieden sich nicht relevant von den Mittelwerten über alle Tiere vor Therapie. Nach 8 Monaten waren alle mit FD versorgten Aneurysmen verschlossen, die der Neuroform- und Kontroll-Gruppe waren weiterhin perfundiert. Schlussfolgerung: Mittels geeigneter Software zur DSA-Analyse können im Tiermodell ZKK erstellt und der hämodynamische Effekt eines Implantates quantitativ beurteilt werden. Der hämodynamische Effekt ist bei einem FD stärker als bei einem konventionellen Stent. Die von uns dargestellten Messwerte könnten als objektive Parameter und als und prädiktive Marker dienen.
Patients suffering from neuropathic pain frequently present with a puzzling clinical picture including spontaneous and evoked pain. During the last years detailed insights were gained into the processing of neuropathic pain in the human brain. Studies using functional neuroimaging techniques provided evidence for activity changes in central nociceptive networks and additional central reorganization phenomena in the central somatosensory and motor system. Moreover, it has been shown that chronic neuropathic pain influences the structural and neurochemical configuration of the brain. Future neurorehabilitation strategies may benefit from these findings.
ZusammenfassungPatienten mit neuropathischen Schmerzen zeigen eine heterogene klinische Manifestation mit spontanen und stimulusevozierbaren Schmerzen. In den vergangenen Jahren ge-lang es, detaillierte Einblicke in die Verarbeitung von neuropathischen Schmerzen im menschlichen Gehirn zu gewinnen. Mittels funktionell-bildgebenden Methoden konnten veränderte Aktivitätsmuster in zentralen schmerzverarbeitenden Netzwerken detektiert werden. Des Weiteren fanden sich kortikale Reorganisationsphänomene mit Verschiebungen im Bereich somatotopisch gegliederter Hirnareale des somatosensorischen und motorischen Systems. Der Nachweis neuroplastischer Veränderungen gelang nicht nur auf funktioneller Ebene, auch die strukturelle und neuronal-chemische Konstitution des Gehirns scheint bei chronischen Schmerzen beeinflusst zu sein. Im Zuge einer erfolgreichen Therapie zeigten sich die zentralen reorganisatorischen Vorgänge rückläufig. Eine Implementierung dieser Erkenntnisse in zukünftige neurorehabilitative Strategien erscheint denkbar.
Ziele: Vergleich der intravenösen Flachdetektor-CTA (FD-CTA) mit der konventionellen DSA zur minimalinvasiven Verlaufskontrolle von Patienten mit intrakraniellen Stents bei Stenosen zerebraler Gefäße. Methode: 15 Patienten wurden wegen einer symptomatischen Stenose (MCA n=7, VA n=8) mit einem intrakraniellen Stent behandelt. Zur Verlaufskontrolle 6 Monate nach Implantation wurde ein optimiertes intravenöses FD-CTA Programm und eine konventionelle DSA durchgeführt. Aus der FD-CTA wurden multiplane (MPR) und subtrahierte (MIP) Rekonstruktionen angefertigt. Die anonymisierten Bilder wurden von zwei Neuroradiologen beurteilt und der residuelle bzw. Rezidiv-Stenosegrad in Prozent gemessen. Ergebnis: Alle intravenösen FD-CTA Untersuchungen waren von sehr guter Qualität. Es zeigten sich Artefakte durch die Stent Marker. Das perfundierte Lumen der Stents konnte gut beurteilt werden. Bei 7 Patienten war keine Stenose feststellbar. Bei 6 Patienten zeigten sich residuelle Stenosen mit einem mittleren Stenosegrad von 38% +/- 5,5%. In zwei Fällen konnte, insbesondere in den MPR-Rekonstruktionen, eine In-Stent-Stenose durch eine Intimahyperplasie (mittlerer Stenosegrad 52,5% +/- 3,5%) nachgewiesen werden. Es zeigte sich eine hohe Korrelation der Messerte in DSA/FD-CT MIP (r=0,91), DSA/FD-CT MPR (r=0,82) und FD-CT MIP/FD-CT MPR (r=0,8). Es ergab sich im Wilcoxon Test keinen signifikanter Unterschied zwischen den Stenosegraden in der FD-CTA und DSA (p>0.05). Schlussfolgerung: Die intravenöse FD-CTA ermöglicht eine nichtinvasive Darstellung des Lumens intrakranieller Stents und könnte die konventionelle DSA als Monitoring Modalität ersetzten. In den MPR-Rekonstruktionen war das Stentlumen gut sichtbar, so dass In-Stent-Veränderungen zuverlässig erfasst werden können.
Intracranial stenosis as late sequela after cerebral radiation therapy for medulloblastoma is an exceptionally rare finding. We report for the first time a case of 31-year-old patient with a history of medulloblastoma 25 years ago and now presenting with radiation-induced high-grade basilar stenosis and recurrent ischemic symptoms despite best medical therapy. The stenosis was treated successfully with angioplasty followed by stenting using a self-expandable microstent. Additionally, MRI revealed multiple cavernomas, illustrating that radiation-induced microbleeds may occur.
Ziele: Untersuchung der Wertigkeit der intravenösen Flachdetektor-CTA (ivFD-CTA) in der Darstellung endovaskulär versorgter experimenteller Aneuysmen. Methode: Bei je fünf Kaninchen wurden experimentell induzierte Aneurysmen (Elastase Modell) mit Coils, Stents oder kombiniert Coils und Stent behandelt. Anschließend wurden intraarterielle DSA-Serien, native und angiographische (iv Kontrastmittel) Datensätze mit Mehrzeilen-CT (MS-CTA) und eine angiographische FD-CT nach intravenöser KM-Gabe erstellt. Zwei Neuroradiologen beurteilten im Consensus-Reading die anonymisierten DSA-Serien, MIP- und MPR-Rekonstruktionen der MS-CT und FD-CT bezüglich Aneurysma-Größe und Okklusionsrate sowie die Stentgröße und das Stentlumen nativ und nach iv-Kontrast. Ergebnis: Bezüglich der Aneurysmagröße ergab sich bei den nur mit Stent behandelten Aneurysmen kein signifikanter Unterschied in allen drei Modalitäten. Bei den nur mit Coils versorgten Aneurysmen zeigte sich in der MSCT gegenüber der DSA eine Überschätzung der Coilpaket-/Aneurysmagröße um 56% (p=0,005), in der ivFD-CT um 16% (p=0,047). Eine Restperfusion am Aneurysmahals konnte in der ivFD-CT wesentlich besser abgegrenzt werden als in der ivMSCT. Der Außendurchmesser der Stents war in der ivFD-CT identisch zur DSA, in der ivMSCT jedoch signifikant größer (p=0,005). Auch gelang mit der ivFD-CT eine bessere Beurteilung der Stents und Darstellung des Stentlumens. In der ivMSCT wurde das Stentinnenlumen signifikant unterschätzt (p<0,05). Schlussfolgerung: Die ivFD-CT zeigt aufgrund der höheren Auflösung und geringeren Artefaktanfälligkeit eine sehr gute Korrelation zur DSA und ist geeignet zur Darstellung von implantiertem Material. Insbesondere kann das Stentlumen beurteilt werden. Die FD-CT könnte sich damit zu einer minimal-invasiven Monitoring-Modalität entwickeln und möglicherweise die DSA bei bestimmten Indikationen ersetzten.
Aims: Prior studies have shown a high rate of cerebral embolic lesions after transcatheter aortic valve implantation (TAVI). The frequency of similar events in older patients undergoing conventional surgical aortic valve replacement is unknown. We therefore compared the incidence of new ischemic brain lesions in pre-and post-procedural magnetic resconance imaging of patients undergoing TAVI and conventional surgical aortic valve replacement.
Ziele: Eine neue Flach-Detektor-CT (FD-CT) Anwendung ermöglicht die Messung des zerebralen Blutvolumens (CBV) unmittelbar im Angiographie Raum. Unsere Hypothese war, das CBV Läsionen nach endovaskulärer Schlaganfalltherapie mit dieser neuen Anwendung erkannt werden können und mit dem definitiven Infarktvolumen korrelieren. Methode: Bei 12 Patienten mit akutem Mediainfarkt wurde initial eine CT mit CT Perfusion (CTP), direkt im Anschluss eine mechanische intra-arterielle Rekanalisation mit abschließender FD-CT zur CBV Messung sowie ein Kontroll CT mit CTP nach 24 Stunden durchgeführt. Die Volumina der CBV Läsionen sowie das Volumen der Infarkt Demarkierung (24 Stunden Konrolle)wurden in den Untersuchungen bestimmt. Zudem wurden die absoluten CBV Werte vergleichend gemessen. Ergebnis: Bei allen Patienten war in der CBV Messung mit FD-CT eine Läsion erkennbar. Die Größe der Läsionen nahm im Vergleich mit der initialen PCT und FD-CT CBV deutlich zu. Wir fanden eine hohe Korrelation des Schlaganfallvolumens im FD-CT CBV mit dem 24 Stunden Kontroll CT (r=0,97, p<0,01). Absolute CBV Messungen der CBV Läsionen im FD-CT und PCT zeigten vergleichbare Werte mit guter Korrelation (r=0,81, p<0,01). Schlussfolgerung: Die neue FD-CT Anwendung ermöglicht die Darstellung von CBV Läsionen während neuro-endovaskuläre Eingriffe. Dabei korreliert das Volumen der FD-CT CBV sehr gut mit dem demarkierten Infarktvolumen. Absolute CBV Messwerte korrelieren gut. Diese neue Option zur CBV Messung könnte das Management während des Eingriffes und das Outcome verbessern, da ein Monitoring und damit eine verbesserte Patientenselektion möglich wird.
Background: Epistaxis is one of the most common emergencies of ENT surgery and can be managed conservatively in most cases. However, transarterial embolization is an accepted treatment option for intractable epistaxis, if conservative management fails. But often, direct detection of the bleeding point by obvious contrast extravasation is not possible in conventional subtracted angiographic series (DSA). Then the suspected bleeding point is treated by endovascular embolization based on the clinical suspicion.Case Report: We here present the case of a young woman with intractable epistaxis where hemorrhage with contrast extravasation was only faintly visible. We used the new imaging modality of flat detector computed tomography (FD-CT) to visualize acute hemorrhage and treatment effect accurately.Conclusion: FD-CT was helpful to visualize both irregular branches of the bleeding mucosa and active hemorrhage and also to monitor an effective embolization within the angio suite. Acquisition of FD-CT imaging in addition to conventional 2-D imaging with the same system enhances the possibility to a better understanding of the individual patient's anatomy and could be beneficial in the accurate depiction of active bleeding, and it might also improve surgical management, if endovascular attempts should fail.
Das klinische Bild eines Schlaganfalles kann durch sehr unterschiedliche Erkrankungen verursacht sein. Grundlage einer spezifischen und effektiven Therapie stellt die sofortige Durchführung einer bildgebenden Diagnostik dar, um die Ursache zu klären. Die Computertomographie (CT) ist die am weitesten verfügbare Methode zur bildgebenden Diagnostik. Ihre entscheidende Bedeutung liegt in der schnellen und einfachen Durchführbarkeit sowie in der schnellen Darstellung einer intrakraniellen Blutung oder einer Ischämie. Die MRT mit Diffusions- und Perfusionsbildgebung ist ausreichend sensitiv in der Darstellung einer intrakraniellen Blutung und liefert schnell den Nachweis einer Ischämie, da die diffusionsgewichtete Bildgebung eine extrem sensitive Methode für früheste ischämische Veränderungen darstellt. Gerade die Diffusion ermöglicht auch die Darstellung kleiner Schlaganfälle, die mit der CT nicht oder nur ungenügend nachweisbar sind. Sowohl die CT als auch die MR-Angiographie lassen Gefäßstenosen oder Verschlüsse im zervikalen oder intrakraniellen Bereich sicher erkennen. Die Einführung der Perfusionsbildgebung hat in den letzten Jahren das Verständnis der Pathophysiologie der zerebralen Ischämie deutlich verbessert und neue diagnostische und therapeutische Möglichkeiten eröffnet. Somit könnte Perfusionsbildgebung, die sowohl mit der CT als auch mit der MRT möglich ist, in Zukunft wesentlich zur Indikationsstellung einer Lysetherapie beitragen.
Ziele: Stents zur Behandlung von intrakraniellen Stenosen müssen mittels Bildgebung kontrolliert werden, da Re-Stenosen in bis zu 30% der Fälle möglich sind. Idealerweise sollte ein nicht invasives Verfahren zur Verfügung stehen. Wir verglichen die Abbildung von intrakraniellen Stents mit dem neuen Verfahren der Flach Detektor CT (FD-CTA) mit Multislice CTA (MSCTA) und konventioneller DSA. Methode: Bei 12 New Zealand White Rabbits wurden 12 selbstexpandierbare Stents (Wingspan, Ø 3,5mm, Länge 15–20mm) in die linke A. carotis communis implantiert. Die Stents wurden dann mit intra-arterieller DSA sowie der FD-CTA und MSCTA, jeweils nach intravenöser Kontrastmittel Gabe, dargestellt. Die DSA und MPR Bilder der FD-CTA und MSCTA (axiale und saggitale Rekonstruktionen) wurden durch zwei Neuroradiologen ausgewertet. Es wurde der proximale und distale Außen-und Innendurchmesser der Stents, zudem die Fläche gemessen. Ergebnis: Technisch konnte bei allen Kaninchen ein Stent platziert werden. Es zeigte sich, dass der Außendurchmesser (proximal und distal) in der CTA am größten, gefolgt von FD-CTA und DSA (p<0,05) war. Der Innendurchmesser war in der FD-CTA signifikant größer als in der MSCTA (p<0,05). Die Messwerte der FD-CT korrelierte besser mit den Werten der DSA als die CTA. Die Messung der Fläche war in der FD-CTA signifikant größer als in der MSCTA. Schlussfolgerung: In der MSCTA werden die Dimensionen eines Stents, am ehesten durch „Blooming“, nicht korrekt abgebildet. Der Außendurchmesser wird übertrieben groß, das Lumen übertrieben klein abgebildet. Die Messungen der FD-CT waren der DSA am ähnlichsten. Die FD-CTA scheint damit eine korrektere Abbildung des Lumens als die MSCTA zu ermöglichen. Die FD-CT nach iv Kontrastmittelgabe könnte eine neue minimal invasive Alternative zur DSA zur Visualisierung intrakranieller Stents darstellen.
The polyphosphoinositide PtdIns(4,5)P2, best known as a substrate for phospholipase C isozymes, has recently been recognized to be involved in a variety of other cellular processes. The aim of this study was to examine whether the cellular levels of this versatile phospholipid are controlled by tyrosine phosphorylation. The studies were performed in human embryonic kidney (HEK)-293 cells stably expressing the M3 muscarinic acetylcholine receptor. Inhibition of tyrosine phosphatases by pervanadate induced an up-to-approx.-2. 5-fold increase in the total cellular level of PtdIns(4,5)P2, which was both time- and concentration-dependent. In contrast, the tyrosine kinase inhibitors, genistein and tyrphostin 23, caused a rapid and specific fall in the cellular PtdIns(4,5)P2 level and prevented the stimulatory effect of pervanadate on PtdIns(4,5)P2 formation. Inactivation of Rho proteins by Clostridium difficile toxin B caused a similar fall in the HEK-293 cell PtdIns(4,5)P2 level, which was not altered by additional genistein treatment. Furthermore, toxin B treatment abolished the pervanadate-induced increase in PtdIns(4,5)P2 levels. As PtdIns(4,5)P2 is an essential stimulatory cofactor for phospholipase D (PLD) enzymes, we finally examined the effects of the agents regulating PtdIns(4,5)P2 levels on PLD activity in HEK-293 cells. Inhibition of tyrosine phosphatases by pervanadate caused an increase in PLD activity, which was susceptible to genistein and tyrphostin 23, and which was abolished by prior treatment with toxin B. In conclusion, the data presented indicate that the cellular level of the multifunctional phospholipid, PtdIns(4,5)P2, in HEK-293 cells is controlled by a tyrosine-kinase-dependent mechanism and that this process apparently involves Rho proteins, as found similarly for tyrosine-phosphorylation-induced PLD activation.
Receptor signalling to phospholipase D (PLD) in human embryonic kidney (HEK) cells stably expressing the m3 muscarinic acetylcholine receptor apparently involves Rho proteins. Since phosphatidylinositol 4,5-bisphosphate [PtdIns(4,5)P2] has been recognized as an essential cofactor for PLD activity and since activated Rho proteins have been reported to stimulate the synthesis of PtdIns(4,5)P2, we studied whether in HEK cells PLD activity is regulated by PtdIns(4,5)P2 and, in particular, whether PtdIns(4,5)P2 can restore PLD activity inhibited by Clostridium difficile toxin B, which inactivates Rho proteins. Addition of MgATP to permeabilized HEK cells increased basal PLD activity and potentiated PLD stimulation by the stable GTP analogue, guanosine 5'-[gamma-thio]triphosphate (GTP[S]), concomitant with a large increase in PtdIns(4,5)P2. On the other hand, neomycin, which binds to PtdIns(4,5)P2, inhibited basal and GTP[S]-stimulated PLD activities. Addition of PtdIns(4,5)P2 increased PLD activity in HEK cell membranes by 2-3-fold, whereas various other phospholipids were ineffective. Prior treatment of HEK cells with toxin B reduced the level of PtdIns(4,5)P2, measured either in intact cells or in membrane preparations, by about 40%. In membranes of toxin-B-treated cells, basal and GTP[S]-stimulated PLD activities were reduced, when measured with exogenous phosphatidylcholine as enzyme substrate. Inclusion of PtdIns(4,5)P2 with phosphatidylcholine in the substrate vesicles or addition of PtdIns(4,5)P2 fully restored basal and GTP[S]-stimulated PLD activities in membranes of toxin-B-treated cells. In conclusion, the data indicate that PtdIns(4,5)P2 is an essential cofactor for PLD activity in HEK cells and that inhibition of PLD activity by the Rho-inactivating toxin B is apparently caused by depletion of the PLD cofactor, PtdIns(4,5)P2.
