PURPOSE:The MR image quality after intracranial aneurysm clipping is often impaired because of artifacts induced by metal implants. The purpose of the present study was to evaluate the benefit of a new WARP sequence with slice-encoding metal artifact correction (SEMAC) and view-angle tilting (VAT) MR imaging as novel artifact reduction techniques.MATERIALS AND METHODS:A new WARP TSE (a work-in-progress software package provided by Siemens Healthcare) sequence was implemented for cranial applications based on a turbo spin echo (TSE) sequence. T1- and T2-weighted images with standard and WARP TSE sequences were acquired from 6 patients with 11 clipping sites, and the images were compared based on artifact size and general image quality.RESULTS:T2- and T1-weighted WARP TSE sequences resulted in a highly significant reduction of metal artifacts compared with standard sequences (T2w- WARP TSE: 89.8 ± 1.4 %; T1w- WARP TSE: 84.9 ± 2.9 %; p < 0.001) without a substantial loss of image quality.CONCLUSION:The use of a new WARP TSE sequence after aneurysm clipping is highly beneficial for increasing the diagnostic MR image quality due to a striking reduction of metal artifacts.
Das Labrum glenoidale ist ein wichtiger Stabilisator des Glenohumeralgelenks. Verletzungen des Labrums beeinträchtigen in Abhängigkeit von der Lokalisation, der Ausdehnung und den beteiligten Strukturen die Stabilität des Schultergelenks in unterschiedlichem Maße. Die komplexe Anatomie und die hohe anatomische Variabilität erfordern detaillierte Kenntnisse und eine hochaufgelöste bildgebende Darstellung mit intraartikulärer Kontrastmittelgabe, um pathologische Befunde von Normvarianten zu unterscheiden. Die MR-Arthrographie, alternativ auch die CT-Arthrographie, haben sich als bildgebende Verfahren der Wahl etabliert. In dieser Übersichtsarbeit werden MR-arthrographische Befunde von Normvarianten und labralen Verletzungsmustern gegenübergestellt.
The glenoid labrum is an important stabilizer of the glenohumeral joint. Labral injuries affect the stability of the shoulder joint to varying degrees depending on the localization, the extension and the structures involved. Because of the complex anatomy and the high variability of the glenoid labrum, in-depth knowledge concerning normal variations and labral injury patterns is required as well as high-resolution imaging with intra-articular contrast in order to discriminate anatomical variants from pathological findings. Therefore, magnetic resonance (MR) and alternatively computed tomography (CT) arthrography have become established as the imaging methods of choice. In this review MR arthrographic findings of normal labral variants and different labral injury patterns are portrayed.
In diesem Refresherkurs wird die technische Durchführung (Punktionstechnik und Untersuchungsprotokolle) einschließlich potenzieller Komplikationen von MR- und CT-Arthrografien des Ellbogen-, Knie- und Sprunggelenkes erläutert. Etablierte Indikationen für Arthrografien der drei Gelenkregionen werden vorgestellt und es wird die diagnostische Effizienz der arthrografischen Verfahren dargestellt. Durch die intraartikuläre Kontrastmittelapplikation können zusätzliche Informationen im Vergleich zur konventionellen MRT gewonnen werden, da sowohl die Gelenkdistension als auch der verbesserte Kontrast eine bessere Visualisierung intraartikulärer Strukturen ermöglichen. Beide Methoden (MR- und CT-Arthrografie) weisen im direkten Vergleich unterschiedliche Stärken und Schwächen auf, die in diesem Kurs herausgearbeitet werden sollen.

Osteoporosis is classified as a public health problem due to its increased risk for fragility fractures. Osteoporotic fractures, in particular spine and hip fractures, are associated with a high morbidity and mortality, and generate immense financial cost. The World Health Organisation (WHO) based the diagnosis of osteoporosis on the measurement of bone mineral density (BMD) using dual-energy X-ray absorptiometry (DXA). However, BMD values of subjects with versus without osteoporotic fractures overlap. Furthermore, it was reported that the anti-fracture effects of drugs could be only partially explained by their effects on BMD. Bone strength reflects the integration of BMD and bone quality. The later can be partly determined by measurements of bone microstructure. Therefore, substantial research efforts have been undertaken to assess bone microstructure by using high-resolution imaging techniques, including high-resolution peripheral quantitative computed tomography (hr-pQCT), high-resolution multi-detector computed tomography (MDCT), and high-resolution magnetic resonance imaging (MRI). Clinical MDCT and MRI systems are broadly available and allow an adequate depiction of the bone microstructure at the clinically most important fracture sites, i.e. radius, spine and hip. Bone microstructure parameters and finite element models can be computed in high-resolution MDCT and MR images. These measurements improved the prediction of bone strength beyond the DXA-derived BMD and revealed pharmacotherapy effects, which are partly not captured by BMD. Therefore, high-resolution bone imaging using clinical MDCT and MRI may be beneficial for osteoporosis diagnostics and allow a highly sensitive monitoring of drug treatment, which plays an important role in the prevention of fragility fractures.
Ziele: Retrospektive Evaluation der diagnostischen Wertigkeit der MR-Arthrographie (MR-A) der Schulter in der Detektion von Läsionen des Pulley-Systems der langen Bizepssehne (LBS) und Evaluation etablierter und neuer diagnostischer Kriterien. Methode: 80 MR-Arthrographien der Schulter (mittleres Patientenalter: 34,2 Jahre; m: 53; w: 27) mit arthroskopisch bestätigten oder ausgeschlossener Pulley-Läsion wurden durch drei Radiologen unabhängig und geblindet evaluiert. Zu beurteilende diagnostische Kriterien waren: Verlagerung der LBS relativ zur Subscapularissehne auf parasagittalen Aufnahmen (sog. “displacement sign”); mediale (Sub-)Luxation der LBS auf transversalen Aufnahmen; Diskontinuität des superioren glenohumeralen Ligaments (SGHL) auf parasagittalen Aufnahmen; Zeichen einer Tendinose der LBS auf transversalen oder parasagittalen Aufnahmen; Rupturen der Rotatorenmanschette angrenzend an das Rotatorenintervall. Ergebnis: In der Arthroskopie zeigten 28/80 Patienten eine Pulley-Läsion. Die MR-A wies eine Sensitivität von 89,3%/85,7%/82,1% (Auswerter 1/Auswerter 2/Auswerter 3) und eine Spezifität von 96,2%/98,1%/86,5% für die Detektion einer Pulley-Läsion auf. Die Interrater-Reliabilität war hoch (κ=0,74). Die Diskontinuität des SGHL zeigte eine Sensitivität von 78,6%/89,3%/87,6% bei einer Spezifität von 82,7%/78,9%/75,0%. Die Sensitivität für das “displacement sign” lag bei 85,7%/82,1%/75,0% mit einer Spezifität von 96,2%/98,1%/90,4%). Die Tendinose der LBS hatte eine Sensitivität von 92,9%/82,1%/64,3% bei einer Spezifität von 80,8%/96,2%/84,6%. Subluxationen der Bizepssehne zeigten eine niedrige Sensitivität (35,7%/50,0%/64,3%) bei hoher Spezifität (100,0%/98,1%/96,2%). Schlussfolgerung: Die MR-Arthrographie der Schulter weist eine hohe diagnostische Genauigkeit in der Beurteilung von Pulley-Läsionen auf, wobei das “displacement sign”, ein diskontinuierlich dargestelltes SGHL und Zeichen einer LBS-Tendinose auf parasagittalen Aufnahmen die aussagekräftigsten diagnostischen Kriterien sind.
Carpal tunnel syndrome (CTS) is a neuropathy caused by compression of the median nerve leading to paresthesia of the fingers in the median nerve distribution, weakness of the thenar muscles and pain. The underlying cause may be idiopathic, but can include tenosynovitis, ganglion cysts and soft tissue masses. In very rare cases, thrombosis of a persistent median artery in the carpal tunnel has been reported as a cause for such compression neuropathy (Kele H et al. J Neurosurg 2002; 97: 471 – 473).
Patellofemoral instability remains a diagnostic and therapeutic challenge due to its multifactorial genesis. The purpose of imaging is to systematically analyze predisposing factors, such as trochlear dysplasia, patella alta, tibial tuberosity-trochlear groove (TT-TG) distance, rotational deformities of the lower limb and patellar tilt. In order to evaluate anatomical abnormalities with a sufficient diagnostic accuracy, standardized measurement methods and implementation of various imaging modalities are necessary.Diagnosis of acute and often overlooked lateral patellar dislocation can be established with magnetic resonance imaging (MRI) because of its characteristic patterns of injury. Damage to the medial patellofemoral ligament (MPFL) has a significance just as high as the predisposing risk factors in relation to the cause of chronic instability.
Axis and torsion malalignment of the femur has been widely recognized as a primary reason for patellofemoral instability and pain. In this article we explain the current concepts of biomechanics and describe the radiological findings in computed tomography (CT) examination. We describe the technique of a biplanar varus and/or external rotation distal femoral osteotomy in detail. Existing clinical studies describe this technique as part of a multimodal treatment concept with good to excellent results. We present our current technique and clinical results.
Achsen- und Torsionsfehlstellungen des Femurs sind als Ursachen von Patellainstabilität und patellofemoralem Schmerz anerkannt. Die zugrunde liegende Biomechanik wird erklärt und die radiologische Diagnostik mittels Computertomographie (CT) beschreiben. Die Technik der biplanaren varisierenden bzw. derotierenden distalen Femurosteotomie wird im Detail beschrieben und wir berichten über die bisherigen Behandlungsverläufe. In der Literatur findet dieses Vorgehen als Teilaspekt multimodaler Behandlungskonzepte mit guten bis sehr guten klinischen Ergebnissen Erwähnung.
Die Bildgebung folgender benigner chondrogener Tumoren soll in diesem Refresherkurs erläutert werden: Enchondrom (solitär und Enchondromatose), Osteochondrom (solitär, epiphyseal und multiple kartilaginäre Exostosen), Chondroblastom, periostales Chondrom und Chondromyxoidfibrom.
Das Chondroblastom ist ein seltener, gutartiger Knochentumor, der weniger als 1 % aller primärer Knochentumoren ausmacht und sich aus unreifen Chondroblasten und einer kargen chondroiden Matrix zusammensetzt. Die am häufigsten betroffenen Skelettelemente sind der proximale Humerus, die proximale Tibia und das distale Femur. Das Fußskelett und die platten Knochen von Scapula, Schädel und Becken werden nur selten befallen.
Typischerweise ist die Glenolabral-articular-disruption- (GLAD-)Läsion nicht mit einer Instabilität kombiniert und weist neben dem Knorpeldefekt lediglich oberflächliche Einrisse am anteroinferioren Labrum auf. Im vorgestellten Fall wird eine seltene Kombinationsverletzung beschrieben, bei der es durch eine chronische Überbelastung bei bestehender GLAD-Läsion zu einer symptomatischen Instabilität kam. Therapeutisch konnten der kraniale Labrumkomplex unter Erhalt der langen Bizepssehne refixiert und der kaudale Labrumkomplex mittels Interpositionsarthroplastik augmentiert werden. Ein begleitender Knorpelschaden im Bereich des Humeruskopfs wurde nicht therapeutisch angegangen. Die Weiterentwicklung der arthroskopischen Techniken erlaubt die Behandlung einer Vielzahl instabilitätsassoziierter pathoanatomischer Befunde.
Shoulder instability may veil various patho-anatomic findings. A case with a rare combination of patho-anatomic lesions is presented. In a patient with antero-inferior instability arthroscopy confirmed a superior labrum anterior-to-posterior (SLAP IV) lesion and a glenolabral articular disruption (GLAD) as well as a concomitant chondral injury to the apex of the humeral head. Stabilization of the labral complex was achieved arthroscopically. The osteochondral defect of the glenoid was ad dressed by interposition arthroplasty. Arthroscopic treatment of shoulder instability offers an individualized approach to a broad variety of patho-anatomic findings.
Während Osteonekrosen per definitionem die epiphysären und apophysären Knochenabschnitte betreffen (unter Einbeziehung der Kortikalis und des subchondralen Knochens), stellen Knochenmarkinfarkte die Spongiosa und das Fettmark betreffende Nekrosen dar, die typischerweise metaphysär oder metadiaphysär lokalisiert sind (ohne Beteiligung des kortikalen Knochens). Die klinische Manifestation eines Knocheninfarkts reicht von völliger Beschwerdefreiheit, über Dauerschmerzen bis hin zu plötzlich einschießenden Schmerzen. In der Bildgebung zeigen ältere Infarkte ein pathognomonisches Bild. In dem Refresherkurs sollen die typischen Charakteristika dieser Infarkte herausgestellt werden. Es sollen aber auch seltenere Lokalisationen und atypische Darstellung insbesondere von akuten und subakuten Knochenmarksinfarkte erläutert werden.
