Zu diesem Termin hatte die Hautarztpraxis Am Schlossgarten in Kiel im 116. Jahr ihres Bestehens in Memoriam Dr. med. Carl Georg Schirren, 1892 bis 1989, in Zusammenarbeit mit der Akademie für medizinische Fortbildung der Ärztekammer Schleswig-Holstein eingeladen ([Abb. 1]).
Background Patients with actinic cheilitis should be treated to prevent transformation into invasive squamous cell carcinoma. Treatment options comprise, for instance, destructive approaches, such as vermilionectomy and carbon dioxide laser ablation. Photodynamic therapy (PDT) has demonstrated high efficacy in patients with epithelial skin cancers, but there is only one report on PDT with free d-aminolevulinic acid (ALA) and noncoherent light in the treatment of actinic cheilitis. Objective We report the treatment of three patients with actinic cheilitis with PDT using the new photosensitizing agent methylaminolevulinate (methyl-5-amino-4-oxopenthanoate [MAOP]) combined with red light. Methods MAOP was topically applied on the lower lip 3 hours before treatment with red light. Two consecutive treatments, 1 week apart, were administered. Clinical assessment was performed up to 13 months after the initial treatment. Results A good clinical response with an excellent cosmetic outcome was observed in all three patients. Moderate to severe pain was associated with the application of the red light, and mild inflammation with edema of the lower lip occurred. Conclusion This is the first use of PDT with MAOP and 634 mm wavelength red light in patients with actinic cheilitis. The clinical results demonstrate that PDT might be an alternative for patients who refuse surgical procedures. However, further studies are needed to compare the efficacy and cosmetic outcome with conventional treatment modalities.
Actinic cheilitis is a common disease mostly of the lower lip. While actinic cheilitis is usually classified as a pre-cancerous lesion, some authors describe this disease as in situ squamous cell carcinoma (SCC). Among several treatment options, the most common is surgical excision, followed by cryotherapy, CO2-laser ablation and topical treatment with 5-fluorouracil, trichloroacetic acid or retinoids (Dufrene and Curlin 1997). Recently, photodynamic therapy (PDT) has been introduced as a therapeutical option for epithelial skin tumors, actinic keratoses and superficial basal cell carcinomas (Szeimies and Landthaler 2002). Here we report three patients suffering from actinic cheilitis treated by PDT using the new photosensitizing agent methyl aminolevulinate (Methyl-5-amino-4-oxopenthanoate, MAOP, Metvix™). Before treatment, Metvix™ Creme containing 160mg/g of methyl aminolevulinate (Foley 2003) was applied using an occlusive plastic wrap. After three hours, these occlusive dressings were removed and the treated skin was exposed to red light exposure (Actilite® CL128, Galderma, Düsseldorf, Germany) for 7 minutes and 50 seconds. This procedure was repeated one week after the first PDT. In order to determine the degree of pain associated with this procedure, the patients were asked to rate the pain experienced on a scale from 0 (no pain) to 10 (obliterative pain) according to Grappengiesser et al (2002). The clinical and cosmetic results in all patients were excellent. Because actinic cheilitis may eventually transform into invasive SCC, early treatment is highly recommended. Surgical excision (vermillionectomy) of the affected skin is the established standard therapy. The effectiveness of PDT for the treatment of actinic keratoses and superficial basal cell carcinomas (BCC) with complete remission in 80 to 90% of all cases is documented in several clinical studies. For the last two years, MAOP (Metvix® Cream) is approved for PDT in Germany for the treatment of actinic keratoses and BCC. To the best of our knowledge, only one report concerning the treatment of actinic cheilitis using PDT has been published before. In 1996, Stender and Wulf treated three patients with 5-aminolevulinic acid and non-coherent light. During a monitoring period of up to 12 months no further relapse was observed. We consider PDT an appropriate alternative treatment option for actinic cheilitis particularly when surgical treatment is difficult, e.g. because of accompanying health conditions. However, based on the lack of reports concerning the long-term outcome of this treatment, the rate of recurrences from actinic cheilitis following PDT treatment compared to conventional vermillionectomy remains to be investigated in larger patient cohorts.
Die vollständige Exzision des malignen Melanoms stellt die Therapie der ersten Wahl bei Primärtumoren dar. Die Bestimmung des geeigneten Sicherheitsabstands um den Primärtumor war fast 70 Jahre lang von anekdotischen Fallberichten und Dogmen geprägt, die erst durch Ergebnisse kontrollierter klinischer (prospektiv-randomisierter) Studien reduziert wurden. Die Entscheidung für oder gegen ein weniger radikales Operationsregime beeinflusst neben der Morbidität der Patienten nachweislich auch das Ausmaß der Behandlungskosten. So konnte gezeigt werden, dass mit einem risikoadaptierten Vorgehen bei der Wahl des Sicherheitsabstands ein Teil von stationären Behandlungen vermieden oder die Dauer des stationären Aufenthalts signifikant verkürzt werden kann.
Eine 69-jährige Patientin stellte sich in unserer Klinik mit multiplen vor allem an den Extremitäten aber auch vereinzelt am Stamm bestehenden erythematösen Plaques und rötlich-lividen Knoten vor. Zusätzlich hatte sich am Malleolus lateralis ein ca. 10×5x2cm großer, ulzerierter Tumor mit fötider Sekretion gebildet. Die histologische Aufarbeitung von Probebiopsien ergab die Diagnose einer Mycosis fungoides im Tumorstadium. In den Staginguntersuchungen zeigte sich keine Generalisation des T-Zell-Lymphoms. Unter der eingeleiteten PUVA-Therapie sowie der intravenösen Applikation von pegyliertem, liposomal verkapseltem Doxorubicin (Caelyx®) alle 2 Wochen (20mg/m2 KO, 8 Zyklen) kam es zu einer fast vollständigen Remission. Nach einer 3-monatigen Therapiepause entwickelten sich erneut Effloreszenzen am rechten Unterschenkel, die unter einer topischen Therapie mit Aldara-Creme (5% Imiquimod) über 5 Wochen eine partielle Remission zeigten. 2 Monate später trat plötzlich an der rechten Wade eine ca. hühnereigroße, tief subkutan gelegene, derb palpable Raumforderung auf. Die pathologische Begutachtung zeigte ein „subkutanes Panniculitis-like T-Zell-Lymphom“ ohne Bezug zur vorbestehenden Mycosis fungoides. Unter einer erneuten systemischen Therapie mit 8 Zyklen Caelyx® entwickelte sich ein Progress des Lokalbefundes am Unterschenkel mit ungewöhnlich raschem Größenwachstum und Infiltration der Muskulatur. Während der daraufhin eingeleiteten perkutanen Radiatio des rechten Unterschenkels kam es nach 2 Wochen zu einer akuten Verschlechterung des Allgemeinzustandes. Unsere Patientin verstarb nach wenigen Tagen im Multiorganversagen. Die Obduktion bestätigte die blastoide Variante des subkutanen T-Zell-Lymphoms mit Befall von Pankreas und Lunge. Das subkutane Panniculitis-like T-Zell-Lymphom stellt eine seltene Form der kutanen T-Zell-Lymphome mit aggressivem klinischen Verlauf dar und ist in der neuen Klassifikation der EORTC für kutane Lymphome als eigenständige Entität aufgeführt. Eine allgemein gültige Therapieempfehlung besteht nicht, komplette Remissionen nach (Poly-) Chemotherapie und/oder Radiatio wurden jedoch beschrieben. Über fulminante Verlaufsformen wurde bei Patienten mit assoziiertem hämophagozytischem Syndrom berichtet.
In der Dermatologie und insbesondere in der Dermatoonkologie werden zahlreiche Immunmodulatoren therapeutisch oder im Nebenwirkungsmanagement eingesetzt. Zu den am häufigsten verwendeten Zytokinen gehören die Interferone und hier vor allem das Interferon-α (INF-α). Interferone besitzen aufgrund ihrer pleiotropen Wirkung, zu der antiproliferative, antivirale aber auch immunmodulierende Effekte gehören, prinzipiell ein breites Indikationsgebiet. Die klassischen, zugelassenen Interferone werden im Allgemeinen subkutan oder intravenös appliziert. Mit dem Wirkstoff Imiquimod ist seit kurzem eine völlig neue Substanzklasse der Immune Response Modifier (IRM) auf dem Markt, die auch in der topischen Therapie verschiedener Dermatosen Verwendung finden kann. Imiquimod stimuliert über „Toll-like-Rezeptoren” die Induktion, Synthese und Freisetzung von INF-α, Interleukin 1, 6 und 12, sowie TNF-α aus Monozyten bzw. dendritischen Zellen. Dabei wird die zellvermittelte spezifische Immunantwort (TH 1-Antwort) moduliert. Aufgrund der beschriebenen Eigenschaften liegt der klinische Einsatz von Imiquimod bei viral induzierten, aber auch bei neoplastischen Hauterkrankungen nahe. Bis heute am intensivsten untersucht ist die Behandlung von externen Condylomata acuminata mit Imiquimod (Aldara 5 % Creme). Bei mehr als der Hälfte der behandelten Patienten kommt es zum vollständigen Abheilen der behandelten Genitalwarzen. Die Domäne des derzeitigen Interesses an Imiquimod liegt sicherlich in der Behandlung von präkanzerösen, semimalignen und malignen Hauttumoren. Es liegen umfangreiche Erfahrungen in der Behandlung von oberflächlichen Basalzellkarzinomen vor, die eine hohe Effektivität dieser topischen Therapie mit einer Heilungsrate von etwa 70 - 80 % belegen. Darüber hinaus wurde Imiquimod zur Therapie aktinischer Keratosen und des Morbus Bowen mit Erfolg eingesetzt. Der Artikel soll eine aktuelle Übersicht des Wirkmechanismus und der bisher untersuchten Hauterkrankungen, die mit Imiquimod behandelt wurden, darstellen.
Excisional biopsy is recommended as the procedure of choice whenever there is suspicion of malignant melanoma. There are only few indications for incisional biopsies, which – in contrast to former opinions – do not worsen the prognosis. For nearly 70 years the debate about the optimal resection safety margin around the primary tumor was influenced by historical case reports and paradigms. Recently, controlled clinical studies provided new insights. Accumulating evidence over the past two decades showed that narrower surgical margins do not have any influence on the rate of advanced metastatic disease. Local recurrence is rare (approximately 0.1%) when primary tumors are thin and is seen more often (approximately 10%) in primary tumors of greater thickness (>4 mm). Analysis of the overall survival in randomized trials shows equal prognosis for malignant melanoma for narrow and wide resection margins. Due to these findings in-toto excisional biopsy for in-situ melanoma, a resection margin of 1 cm for primary tumors with a tumor thickness up to 2 mm and a resection margin of 2 cm for primary tumors greater than 2 mm appears sufficient. By this procedure primary closure of wounds will be possible in nearly all cases, morbidity and costs of surgical approaches will be reduced. For a long time it has been discussed whether prophylactic removal of lymph nodes ('elective lymph node dissection') is of benefit for melanoma patients. More recently 'selective' lymphadenectomy ('sentinel node biopsy', SNB) has been proposed to evaluate the status of the first draining lymph node ('sentinel node') of the regional basin. Several studies now demonstrate that the sentinel node evaluation for underlying metastatic disease reflects the status of the entire lymph node region and is therefore a useful prognostic factor superior to measurement of tumor thickness in primary melanoma. However, it is unclear whether sentinel node biopsy is of benefit for a better survival in affected patients.
We describe a 50-year-old female patient who developed extensive lymphomatous infiltrates on her forehead, scalp and face within a few months. Histology and immunohistochemistry of skin tumours revealed a CD20 positive follicular B-cell lymphoma. Subsequently, extracutaneous manifestations were detected by computed tomography scans and bone marrow biopsy. The patient suffered from a primary nodular malignant lymphoma with extraordinary cutaneous infiltration of the head. Therefore, combination treatment with a monoclonal antibody against the CD20 antigen, rituximab, and polychemotherapy (CHOP scheme) was administered every 3 weeks. After the second course of treatment a complete regression of cutaneous infiltrates was noticed. Follow-up biopsies on the forehead showed no evidence of CD20 positive lymphoma cells, now. Despite mild leucocytopaenia therapy was well tolerated.
PURPOSE:Low-dose interferon alfa (IFNalpha) has been shown to have limited effects in the adjuvant treatment of patients with intermediate- and high-risk primary melanoma. We hypothesized that a combination regimen with low-dose interleukin-2 (IL-2) may improve survival prospects in these patients.PATIENTS AND METHODS:After wide excision of primary melanoma without clinically detectable lymph node metastasis (pT3 to 4, cN0, M0), 225 patients from 10 participating centers were randomly assigned to receive either subcutaneous low-dose IFNalpha2b (3 million international units [MU]/m2/d, days 1 to 7, week 1; three times weekly, weeks 3 to 6, repeated all 6 weeks) plus IL-2 (9 MU/m2/d, days 1 to 4, week 2 of each cycle) for 48 weeks, or observation alone. The primary end point was prolongation of a relapse-free interval.RESULTS:Of the 225 enrolled patients, 223 were found to be eligible. Median follow-up time was 79 months. All evaluated prognostic factors were well balanced between the two arms of the study. Relapses were noticed in 36 of 113 patients treated with IFNalpha2b plus IL-2 and in 34 of 110 patients with observation alone. Five-year disease-free survival of those who had routine surgery supplemented by IFNalpha2b and IL-2 treatment was 70.1% (95% confidence interval [CI], 61.3% to 78.9%), compared with 69.9% in those receiving surgery and observation alone (95% CI, 60.7% to 79.1%) in the intention-to-treat analysis. Evaluation of the overall survival did not show any difference between treated and untreated melanoma patients (P =.93).CONCLUSION:Adjuvant treatment of intermediate- and high-risk melanoma patients with low-dose IFNalpha2b and IL-2 is safe and well tolerated by most patients, but it does not improve disease-free or overall survival.
Die Schildwächterlymphknoten-Exstirpation und die adjuvante Interferon-α-Therapie gehören in vielen Ländern zum Standard in der operativen Behandlung des malignen Melanoms. Wir berichten über eine 36-jährige Patientin, bei der während einer nicht bekannten Frühschwangerschaft sowohl verschiedene diagnostische Prozeduren als auch eine Interferon-Therapie durchgeführt wurden. Bei der 36-jährigen Patientin wurde ein malignes Melanom an der rechten Wange (Level IV; 2,4mm) diagnostiziert. Präoperativ führten wir eine Computertomographie von Thorax und Abdomen durch, nachdem die Patientin explizit nach einer möglichen Schwangerschaft befragt wurde. Die Untersuchung ergab bis auf „zwei kleine Uteruszysten“ keine pathologischen Befunde. So wurde in Allgemeinanästhesie der Primärtumor an der Wange exzidiert und gleichzeitig die Exstirpation des Schildwächterlymphknotens unter Zuhilfenahme von 99mTc durchgeführt. Nach mündlicher und schriftlicher Aufklärung auch über die Kontraindikation im Falle einer Schwangerschaft leiteten wir eine adjuvante Interferon α2b-Therapie mit einer Dosierung von 3×3 Mio. IE pro Woche ein. Sechs Wochen später stellte sich heraus, dass sich die Patientin mittlerweile schon in der zwölften Woche einer Zwillingsschwangerschaft befand. Die Patientin lehnte einen Schwangerschaftsabbruch ab und führte die Interferon-Therapie gegen ärztlichen Rat weiter. Vier Wochen vor dem Entbindungstermin entwickelte die Patientin Halslymphknotenmetastasen. Die Geburt wurde vorzeitig eingeleitet, und die Patientin brachte gesunde Zwillinge zur Welt. Sechs Monate später wurden erstmalig multiple Lungenfiliae festgestellt. Trotz mehrerer Chemotherapien verstarb die Patientin 1 Jahr später.
Cerebriform giant melanocytic nevus of the scalp is an extremely rare malformation. Clinical appearance with maceration and fetor within the crypts and the risk of malignant transformation may require surgical therapy. We report two cases with different methods of surgical management. A 27-year-old woman noticed a swelling of the parietal scalp that developed over a period of 4 years into a gyrus-like tumor measuring 12 cm x 18 cm. The crypts between the gyri could not be inspected. Serial excisions under subcutaneous infusion anesthesia were performed to reduce the size of the nevus and to flatten the surface of the scalp. The second patient, a 26-year-old man, demonstrated a giant 10 cm x 10 cm cerebriform nevus on the occiput. The nevus also contained areas of fetid maceration. After implanting a tissue expander under general anesthesia the nevus was excised. The defect was closed using a rotation flap.
BACKGROUND AND OBJECTIVE:Stage IV melanoma patients with a very advanced disease are usually excluded from clinical trials. We treated 25 stage IV patients with temozolomide - a cytostatic drug with 100% oral bioavailability and considerable penetration of CNS tissue.PATIENTS/METHODS:25 patients (17 female, 8 male) between 24 and 82 years (mean: 55.5 years) were included in this retrospective study. 19 patients had received at least one and up to four previous chemotherapy regimens during the course of stage IV disease. 11 (44%) patients showed cerebral metastases prior to therapy with temozolomide. 200 mg/m2 temozolomide were given orally at home on day 1 to 5 in week 1 and in week 5, respectively.RESULTS:Out of 23 evaluable patients 2 (8.7%) showed a partial remission, 2 (8,7%) a minor response, 6 (26.1%) had stable disease, 1 (4,3%) a mixed response, and 12 (52.1%) patients experienced disease progression. Sites of remission included brain, lung, liver, lymph nodes and muscle. Two patients interrupted therapy due to severe leuko- and thrombocytopenia (WHO grade 3 and 4). All other patients tolerated treatment with temozolomide well and no dose reduction was necessary. The median overall survival was 7 months (2-28+ months) since beginning of therapy and 15 months (4-63+ months) since onset of stage IV disease.CONCLUSION:Temozolomide represents a safe treatment option in patients with metastatic melanoma and poor prognosis.
Die Diskussion um die optimale Versorgung von Patienten mit Melanom der Haut wird kontrovers geführt. Mittels TED wurden zu Schlüsselfragen der Versorgung von Melanompatienten den Teilnehmern eines Symposiums fünf Multiple-Choice-Fragen gestellt und ausgewertet. Dies geschah vor und nach einem Vortrag zu diesem Thema. Somit konnte der Einfluss des Vortragenden auf das Meinungsbild des Auditoriums überprüft werden.
We report on a 49 year-old female patient with a cherry-sized tumor of the lower lip, which over several years had steadily increased in size. The patient was psychologically affected by the appearance of the tumor and had difficulties to eat. We excised the tumor in local anaesthesia. Histology confirmed the clinical suspicion of lipoma. A total of four lipomas at this location have been reported world-wide.
Hintergrund und Fragestellung. Melanompatienten mit weit fortgeschrittener Fernmetastasierung nach erfolglosen Therapieversuchen werden üblicherweise von klinischen Studien ausgeschlossen. Wir berichten über die Verwendung von Temozolomid (Temodal®) als einem oral verfügbaren und liquorgängigen Zytostatikum bei 25 Melanompatienten mit Fernmetastasierung.
Wir berichten über eine 49-jährige Frau mit einem seit 15 Jahren bestehenden ca. kirschgroßen Tumor im Bereich der Unterlippe. Dieser Tumor hatte stetig an Größe zugenommen und führte zu einer starken psychischen Beeinträchtigung der Patientin. Zusätzlich erschwerte er aufgrund seiner Größe die Nahrungsaufnahme. Wir exstirpierten den Tumor in Lokalanästhesie, die histologische Aufarbeitung ergab ein Lipom. Lipome im Bereich der Lippe wurden bisher in der deutschsprachigen Literatur nicht beschrieben. Auch weltweit liegen nur wenige Fallberichte vor.
