Eine Serie von 182 Patienten mit Schwannomen des N. vestibularis wurde im Hinblick auf den langfristigen Hörverlauf untersucht. Bei allen Patienten erfolgte die Entfernung des Tumors über den transtemporalen Zugang. Außerdem war bei allen Personen der Hörverlauf im Schnitt 3,8 Jahre (1,5 Jahre–10 Jahre) postoperativ nachvollziehbar. Innerhalb der ersten drei postoperativen Monate zeigten 124 Patienten ein Absinken der Hörschwelle im Tonaudiogramm im Vergleich zum Befund kurz nach der Operation. Bei 83 dieser Personen kam es in den darauffolgenden Monaten und Jahren zu einer vollständigen Wiederherstellung der Hörfunktion auf das Niveau kurz nach der Operation. Ein Jahr nach der Operation konnte bei 52 Patienten (28%) in der Tonaudiometrie eine Verschlechterung in Relation zum Befund direkt postoperativ festgestellt werden. Davon unberührt blieb die Sprachdiskrimination, die auch bei diesen 52 Patienten nahezu konstant blieb. Bei der Analyse der langfristigen Verschlechterung des Tonaudiogramms zeigte sich eine Korrelation mit abgelaufenen Hörstürzen vor der Operation und eine Häufung bei Tumoren, die ihren Ausgang von N. vestibularis inferior nahmen. Im Langzeitverlauf kann es zu einem Absinken der Schwelle im Tonaudiogramm kommen. Die Sprachdiskrimination bleibt jedoch in fast allen Fällen konstant, so dass nach der transtemporalen Chirurgie des Vestibularisschwannoms von stabilen Hörverläufen ausgegangen werden kann.
BACKGROUND:Schwannoma of the vestibular nerve is the most common tumor found in the inner auditory canal. If a schwannoma of the facial nerve is found unexpectedly during surgery, the original surgical plan may have to be changed. In our series, we describe the symptoms, diagnostic procedures and therapy of this infrequent, benign tumor. In particular, we want to analyse the influence of diagnostic techniques, such as magnetic resonance tomography and electrophysiology, on the diagnosis of these tumors.PATIENTS:Between 1975 and 2001, we diagnosed 17 cases of facial nerve schwannoma in the inner auditory canal or the geniculate ganglion. All patients were treated surgically. In 16 cases, we used the enlarged middle fossa approach and in the remaining patient the translabyrinthine approach was used.RESULTS:In 11 cases, the facial nerve could be saved but in six the nerve had to be rebuilt with a transplant. During the first years of our study, palsy of the facial nerve was a frequent finding. In the later years, a hearing impairment and tinnitus became the most frequent symptoms.CONCLUSIONS:Advances in magnetic resonance imaging have, in many cases, facilitated the differentiation between schwannomas of the facial nerve and the vestibular nerve over the last years. Signal enhancement at the geniculate ganglion is important for the diagnosis of facial nerve schwannoma. In individual cases, we found significant findings using electrophysiological procedures. In three cases, the temporal delay was elongated. Based on these findings, the therapeutic procedures can be discussed together with the patients.
Über die physiologische Bedeutung der Nasennebenhöhlen gibt es bisher keine gesicherten Erkenntnisse. Auch die Frage einer möglichen Beeinflussung der Stimmresonanz durch die Nasennebenhöhlen wird in der Literatur unterschiedlich beurteilt.
Bei der Behandlung der Polyposis nasi et sinuum bietet die endoskopisch kontrollierte Lasertherapie insbesondere bei der Rezidivpolyposis und Solitärpolypen eine ambulant durchführbare und nahezu unblutige Methode zur Wiederherstellung von Ventilation und Drainage der anhängenden Nasennebenhöhlensysteme. Von Vorteil ist hier der kontinuierliche Einblick in das Operationsgebiet und die Möglichkeit, in engen anatomischen Arealen wie z.B. Siebbein- oder voroperierten Kieferhöhlenostien dosiert und gezielt Gewebe abzutragen.
Malignant tumors of the anterior cranial skull base are still a challenge for radical surgical treatment. Several different techniques and approaches have been developed over the years and the results, with mortality rates over 50%, are still not encouraging. Here we present our results of an interdisciplinary, one-stage, neurohino transfronto-transbasal surgical approach in twelve patients with such tumors. The long-term survival rate in our patients is now 83% with a mean postoperative follow-up of 19.3 months. Two patients died due to early recurrencies and metastasis after 10 and 13 months after initial treatment and postoperative irradiation. Another five patients with a follow-up of 34.2 months have had tumor recurrencies diagnosed at 19.5 months postoperatively. These patients have survived their recurrent tumor for 14.7 months. Five patients, 41% of the whole group, have been living without evidence of tumor recurrence for between 2 and 35 months.
Im Rahmen der endonasalen Chirurgie der Nasennebenhöhlen läßt sich die Keilbeinhöhle transnasal oder transethmoidal resp. transseptal erreichen und eröffnen. Im Falle einer ausgedehnten entzündlich-hyperplastischen Schleimhauterkrankung mit Verwischung der Landmarken werden in der Literatur verschiedene anatomische Hilfslinien angegeben. Dixon (1983) empfiehlt das Eröffnen der Keilbeinhöhle über eine posteriore Siebbeinzelle. Mosher (1929) warnt vor diesem Vorgehen, gibt aber als Orientierungshilfe an, die obere Muschel teile die Keilbeinhöhlenvorderwand in eine Pars ethmoidalis sowie Pars nasalis im Verhältnis 2:1. Wigand (1989) beschreibt einen Hilfspunkt 1 cm oberhalb der Choane paraseptal. Naumann (1980) sucht zur Eröffnung der Keilbeinhöhle zunächst deren Ostium auf und erweitert dieses.
Die im Innenohr vereinigten Sinnesorgane für Gehör und Gleichgewicht stellen eine morphologisch-funktionelle Einheit dar, so daß destruierende Prozesse oder chirurgische Maßnahmen am Gleichgewichtsorgan meistens irreparable Schäden am ganzen Gehörorgan setzen. Es gibt jedoch klinische Beobachtungen aus der Felsenbeinchirurgie über eine Gehörerhaltung trotz partieller oder kompletter Zerstörung des vestibulären Labyrinths.
We present an animal model in rabbits for the investigation of normal wound healing in the paranasal sinuses. The blood and lymphatic stream and the mucociliary pathways are demonstrated on the medial maxillary sinus wall to study the influence on wound geometry. Standardized and circular wounds are created in the same area exposing the underlying bone of the maxillary sinus. Using a computer aided system for 3-D-reconstruction, it can be shown that respiratory wound closure is independent of the stream of blood, lymph or mucus. The circular wound diminishes in size concentrically. The standardized wound model may be suitable for comparison of different therapeutic methods of wound care.
A model of wound healing in the maxillary antrum of the rabbit was developed. The regeneration of standardized circular mucosal defects 4 mm in diameter was studied using conventional histology and serial sections as well as three-dimensional histomorphological analysis. Wound closure started by migration of epithelium; 24 h later mitoses were seen. 120 h after creation of the wound the granulation tissue covering the wound surface started to become hyperplastic, and bone apposition with formation of osteoid occurred. Systemic application of prednisolone i.m. (2 mg/kg per day) and topical administration of dexpanthenol-ointment (5%) resulted in an acceleration of the late epithelial wound closure together with a reduction of hyperplastic granulation tissue. Epidermal growth factor, applied locally, had no significant effect. The proposed animal model permits additional quantitative evaluation of postoperative treatment modalities.
Twenty two patients underwent standardized endoscopic follow-up for a period 3 months after a partial or complete intranasal ethmoidectomy. 370 endoscopic photographs were taken from the healing ethmoidal wound. After precise documentation of the site of removal, 157 biopsies of the regenerating mucosa were made. The endoscopic appearance of the wound was compared with the histological structure. Endoscopy showed that the process of wound healing of the respiratory mucosa could be subdivided into 4 phases: 1. Hemorrhagic crusting lasting up to 10 days 2. Obstructive lymphatic edema lasting up to 30 days 3. Mesenchymal tissue reaction lasting up to 3 months 4. Scar formation beginning after 3 months. The subdivision of wound healing phases enables the endoscopist to recognize retardation of healing and may lead to a staging of the postoperative therapeutic regime.
Nach einer vollständigen oder teilweisen Siebbeinausräumung wurden bei 22 Patienten engmaschige Kontrolluntersuchungen mit vielfacher photographischer Photodokumentation ausgeführt.
Hemiglossal paralysis due to lesions of the peripheral hypoglossal nerve leads to marked muscle atrophy with disturbed functioning in a minority of cases. Reinnervation from the unparalyzed, contralateral side may then be desired. In animal experiments on cats, a Z-plasty of the midportion of the tongue was carried out by transposing a portion of the normal tongue musculature into the opposite side following denervation 1 month previously by resection of the hypoglossal nerve. Electromyography was performed 13 months later. The tongue was then examined histochemically in serial sections in search of the motor endplates together with the nerves fibers. However, there was no evidence for reinnervation in the specimens examined. Signs of successful “reanimation” of the tongue by Z-plasty may be caused by mechanical synkinesia due to scar formation rather than by actual reneurotization.
Lung function tests of 13 patients suffering from manifest asthma and of 4 patients showing only bronchial hyperreactivity were performed before and on an average of 12 months after endonasal surgery of the paranasal sinuses. In the four subjects, bronchial hyperreactivity was no longer detectable postoperatively by provocation with carbachol. Five asthmatics could stop medication of 1 to 3 of their drugs, five others were able to reduce the dosage of one of their drugs to 50% or more. Lung function and medication was unchanged in two asthmatic patients, one patient had to add a drug to his medication while showing an unchanged lung function. In patients with asthma and chronic paranasal sinusitis, endoscopic endonasal sinus surgery (together with flanking measures, e.g. septal correction) is able to improve antiasthmatic therapy in a high percentage.
Due to the fact that the temporal bone has broad segments of bone anterior to and behind the internal auditory canal which are empty of functionally important structures, a broad exposure of the cerebellopontine angle may be obtained from above. This technique is adequate for the management of acoustic neurinomas up to 3 cm in diameter. A low postoperative morbidity and a high percentage of hearing preservation with total tumor removal were obtained in a series of 190 cases. This report underlines the advantages of the procedure and points to various other indications.
Bei 17 Patienten wurde meist über drei Monate nach einer endonasalen Siebbeinoperation und wöchentlich eine standardisierte endoskopische Photodokumentation des Operationsgebietes vorgenommen. Aus der sich schließenden Wunde der respiratorischen Schleimhaut wurden nach genauer Dokumentation des Entnahmeortes über 125 Biopsien gewonnen und in histologischen Routineschnitten sowie -Färbungen ausgewertet. Diese Untersuchung belegt, daß der Prozeß der Wundheilung in den Nasennebenhöhlen einen komplexen Vorgang von beträchtlicher Dauer darstellt. Verschiedene Phänomene, wie das Aufschießen von Ödemen aus belassenen Schleimhautresten unmittelbar postoperativ, die Mitreaktion der Kieferhöhlenschleimhaut bei Siebbeineingriffen sowie die Verklebung-Verwachsung flottierender Reste der mittleren Muschel und ihre erfolgreiche Therapie lassen sich durch die Diaserien belegen. Der Epithelschluß und die Reifung der Lamina propria unterliegt einer Vielzahl von Einflüssen der individuellen Disposition des Patienten und evtl. seiner Mikroanatomie. Diese Wundheilung bedarf der besonderen Aufmerksamkeit und therapeutischen Steuerung durch den Operateur oder einen erfahrenen HNO-Arzt unter Einsatz des Endoskopes.
Ziel der chirurgischen Stimmrehabilitation ist es, die Lungen als Flowspender, neu angelegte pharyngeale Stimmlippen als Tongeber und den Pharynx als Sprachmodulator zu nutzen, um bei oder nach Laryngektomie regelmäßig die Stimmfunktion wiederherzustellen.
In an attempt to overcome the complete auditory deprivation of young patients with bilateral acoustic nerve schwannomas (Morbus v. Recklinghausen) a planned partial resection of the tumour was carried out in 11 hearing ears. The cerebello-pontine angle was exposed by the enlarged middle fossa approach (Wigandet al., HNO 1985). A strip of tumour was left in contiguity to the facial and cochlear nerves. Hearing was initially preserved in all cases. Four of them, however, developed total deafness after 3–39 months. Recurrent tumour growth was checked by CT scans. During the observation time one resting tumour became invisible, five remained unchanged. Two grew larger. Three cases dropped out of the followup. With regard to the high value of preserved auditory function during the first decades of life the planned partial resection may be considered preferable for cases of bilateral acoustic neuromas.
