目的 采用网状Meta分析方法系统评价不同抗骨质疏松症药物预防绝经后骨质疏松性骨折的有效性及安全性.方法 计算机检索中、英文数据库,收集相关随机对照试验,检索时限均从建库至2019年3月.文献筛选、评估偏倚风险后,采用R Studio、Stata进行数据分析.结果 纳入37个RCT,共计83907例患者,包括16种干预措施.网状Meta分析结果显示:各干预措施对预防椎体骨折的疗效均优于安慰剂,其中阿巴帕肽(OR=0.12,95%CI:0.04~0.35)、特立帕肽(OR=0.24,95%CI:0.16~0.35)、唑来膦酸(OR=0.28,95%CI:0.18~0.43)以及romosozumab(OR=0.30,95%CI:0.19~0.45)的效果最为显著;SUCRA排序结果提示这4种干预措施对于椎体骨折的预防由优到劣依次为:阿巴帕肽>特立帕肽>唑来膦酸>romosozumab.对于非椎体骨折的预防,阿巴帕肽(OR=0.46,95%CI:0.21~0.97)、特立帕肽(OR=0.54,95%CI:0.39~0.75)、romosozumab(OR=0.62,95%CI:0.44~0.90)、利塞膦酸钠(OR=0.70,95%CI:0.50~0.97)的疗效均优于安慰剂;SUCRA排序结果提示对于非椎体骨折的预防由优到劣依次为:阿巴帕肽>特立帕肽>romosozumab>利塞膦酸钠;除rhPTH(1-31)外,各干预措施间不良反应发生率与安慰剂相比差异均无统计学意义,安全性良好.结论 阿巴帕肽、特立帕肽、romosozumab和唑来膦酸对预防绝经后妇女骨质疏松性骨折更加有效安全.
目的:采用网状Meta分析方法系统评价长效β2受体激动药(LABA)/长效抗胆碱能拮抗药(LAMA)联合治疗中到极重度慢性阻塞性肺疾病(COPD)的有效性及安全性.方法:计算机检索PubMed、Embase、Cochrane Central Register of Controlled Trials(CENTRAL)、ClinicalTrail.gov、SinoMed、CNKI、WanFang Data数据库,搜集LABA/LAMA联合治疗中到极重度COPD的随机对照试验(RCT),检索时限均为建库至2018年7月.由两名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用Stata 14软件在频率学框架下进行数据处理分析.结果:纳入35个RCT,共31 758例患者,包括11种干预措施.网状Meta分析结果显示:格隆溴铵/茚达特罗(50/110 μg)[MD=0.255,95%CI(0.181,0.329)]、格隆溴铵/茚达特罗(15,6/27.5 μg)[MD=0.223,95%CI(0.195,0.252)]、噻托溴铵/茚达特罗[MD=0.209,95%CI(0.166,0.252)]对FEV1谷值的改善均优于其他干预措施;格隆溴铵/茚达特罗(50/110 μg)[MD=-4.719,95%CI(-6.161,-3.278)]、格隆溴铵/茚达特罗(15.6/27.5 μg)[MD=-4.928,95%CI(-6.783,-3.073)]、噻托溴铵/奥达特罗[MD=-4.202,95%CI(-5.387,-3.016)]能明显降低圣乔治呼吸问卷(SGRQ)评分;格隆溴铵/茚达特罗(50/110 μg)、噻托溴铵/奥达特罗、格隆溴铵/福莫特罗、芜地溴铵/维兰特罗、噻托溴铵治疗的患者急性加重发生率均低于安慰剂,差异有统计学意义,LABA/LAMA联合组间的差异均无统计学意义;各干预措施安全性相当,各组间严重药品不良反应发生率的差异无统计学意义;噻托溴铵/奥达特罗组患者依从性较好,退出率低于噻托溴铵、格隆溴铵/茚达特罗(50/110 μg)、格隆溴铵/福莫特罗、芜地溴铵/维兰特罗治疗组,差异有统计学意义.结论:格隆溴铵/茚达特罗(15.6/27.5 μg)、噻托溴铵/奥达特罗相对其他LABA/LAMA联合治疗措施对中到极重度COPD有更好的临床疗效和依从性.
Objective To evaluate the effectiveness and safety of umeclidinium/ inhaled corticosteroid/ longacting ?z-agonist (UMEC/ICS/LABA) in the treatment of chronic obstructive pulmonary disease(COPD). Meth叫S PubMed , Embase, Cochrane Central Register of Controlled Trials(CENTRAL) , ClinicalTrail. gov. CBM. VIP, CNKI and WanFang Data were electronically searched for randomized controlled trials (RCTs)about UMEC/ICS/LABA for COPD from inception to May 28 , 2018. Two reviewers independently screened literatures, extracted data and assessed the risk of bias of included studies. Then. meta-analysis was performed using RevMan 5.3 and R software. Results Seven RCTs involving 13 219 patients were included. UMEC 62. 5μg/ICS/LABA vs PBO/ICS/LABA: UMEC 62.5μg/ ICS/LABA ∞uld sigIùficantly improve the lung function (trough FEV. (2) UMEC 125μg/ICS/ LABA vs PBO/ICS/LABA : UMEC 125μg/ICS/LABA could significantly improve the lung function and the quality of life, reduce the use of rescue drugs (salbutamol). However there is no difference in the rate of COPD acute exacerbation [RR =0.57 ,95% CI (0.24 ,1. 37), P =0.21J and a hígher drug-relating adverse events rate than the control group [RR = 1. 44 , 95 % CI (1. 00, 2. 08) , P = O. 05 J . Conclusion Both UMEC 62. 5μg and 125μg in combination with ICS/LABA are superior to PBO/ICS/LABA in the treatment of COPD patients, which can significantly improve lung function and the quality of life. UMEC 62. 5μg is preferred because of the lower adverse events rate. However, due to the limitation of quantity of inc1uded studies, this conc1usion should be further confirmed by more high quality studies.
目的 系统评价依折麦布联合他汀对血脂不达标冠心病患者血脂水平的作用.方法 检索Cochrane Library、PubMed、Embase、CBM、CNKI、万方、维普数据库,搜集依折麦布联合他汀与双倍剂量他汀治疗血脂不达标冠心病患者的随机对照试验,检索时限均从建库至2017年6月1日.由2名评价员独立筛选文献、提取资料并评价纳入研究的方法学质量,采用RevMan 5.3和R 3.2.3软件进行数据分析.结果 共纳入17个RCT,共2377例患者.Meta分析结果显示,与双倍剂量他汀组相比,依折麦布联合他汀组能显著降低血脂不达标冠心病患者低密度脂蛋白胆固醇(LDL-C)(MD=-0.37,95%CI:-0.41~-0.34,P<0.00001)和总胆固醇(TC)(MD=-0.39,95%CI:-0.45~-0.33,P<0.00001),明显提高血脂不达标冠心病患者LDL-C达标率(OR=3.93,95%CI:2.47~6.25,P<0.00001),差异均具有统计学意义.对于高密度脂蛋白胆固醇(HDL-C)和三酰甘油(TG)水平,两组无显著性差异(MD=-0.00,95%CI:-0.02~0.02,P=0.83;MD=-0.04,95%CI:-0.08~0.01,P=0.12).结论 依折麦布联合他汀较双倍剂量他汀能更有效的降低血脂水平,明显提高降脂治疗达标率,但上述结果尚需要更多高质量、大样本随机对照试验进一步证实.
目的 系统评价质子泵抑制剂(PPI)治疗功能性消化不良的临床疗效和安全性.方法 计算机检索Pubmed、Embase、Central、中国期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)和万方数据库,收集PPI治疗功能性消化不良的随机对照试验(RCT),检索时限均从建库至2017年9月30日,并同时追溯纳入研究的参考文献.由两名研究者根据纳入和排除标准独立进行文献筛选、资料提取、质量评价并交叉核对后,采用RevMan 5.3和R软件进行Meta-分析.结果 最终纳入15个RCTs,共6 350例患者(试验组4266例,对照组2 084例).Meta-分析结果显示:PPI与安慰剂组相比,在综合疗效[RR=1.27,95%CI (1.12,1.44),P=0.000 2]和整体症状完全缓解[RR=1.42,95%CI(1.13,1.80),P=0.003]方面,两组差异有统计学意义,而在不良反应发生率[RR=1.18,95%CI (0.97,1.43),P=0.09]方面,差异无统计学意义.而PPI对亚洲国家功能性消化不良患者未显示出优势[RR=1.26,95%CI (0.80,1.97),P=0.32].结论 PPI能提高功能性消化不良的综合疗效和整体症状完全缓解率,与安慰剂比较安全性相当.受纳入研究样本量和质量的限制,PPI对亚洲国家功能性消化不良患者的疗效有待大样本、高质量的RCT进一步验证.
目的 系统评价依折麦布联合他汀类药物与单药应用双倍剂量他汀调血脂治疗中对主要心血管事件的影响.方法 检索CENTRAL、PubMed、EMBASE、中国生物医学文献数据库(CBM)、中国学术期刊全文数据库(CNKI)和万方等数据库,收集依折麦布联合他汀类药物与双倍剂量他汀类药物调脂治疗心血管不良事件的随机对照试验(RCT),检索时限均从建库至2018年4月1日.由2名评价员独立筛选文献、提取资料并评价纳入研究的方法学质量,采用RevMan 5.3和R3.3.1软件进行数据分析.结果 共纳入16个RCTs,共3 534例患者.Meta-分析结果显示与单药应用双倍剂量他汀组相比,依折麦布联合他汀组可降低心绞痛[RR=0.36,95%CI (0.21,0.63),P=0.000 3]、心肌梗死[RR=0.59,95%CI (0.36,0.95),P=0.03]和主要心血管不良事件(心源性死亡、心绞痛、心肌梗死、血运重建)[RR=0.58,95%CI (0.38,0.87),P=0.009]的发生风险,且具有统计学差异;两组血运重建、心源性死亡、卒中和全因死亡风险无统计学差异.结论 现有证据表明依折麦布联合他汀类药物比单药应用双倍剂量他汀能进一步降低不良心血管事件发生风险,但上述结果尚需更多多中心、大样本随机对照试验进一步证实.
Objective To compare the effectiveness of standard and double dose of proton pump inhibitors (PPIs) for moderate to severe reflux esophagitis and to offer a scientific basis for rational use of drug in clinical practice.Methods We electronically searched databases,including PubMed,Embase,CENTRAL,CBM,CNKI and WanFang Data from their establishment date to June 14,2016,to collect randomized controlled trials about standard and double dose of PPIs for moderate to severe reflux esophagitis.Relevant references were searched manually at the same time.Two reviewers independently screened literatures,extracted data,evaluated quality and cross checked according to strict inclusive and exclusive criteria.Meta-analysis of healing rate conformed by endoscope was conducted using RevMan 5.3 and Stata 14.0 software.Results A total of 20 RCTs involving 8 277 patients receiving 8 weeks of treatment were included,in which 7 different doses of PPIs were investigated.Network Meta-analysis yielded 11 direct and 10 indirect comparisons,among which 4 were demonstrated statistically significant.The healing rate of esomeprazole (40 mg) was higher than lansoprazole (30 mg),pantoprazole (40 mg) and omeprazole (20 mg):odds ratios OR =1.65,95% CI(1.22,2.24);OR =1.71,95 % CI(1.16,2.52);OR =1.87,95 % CI(1.37,2.55) respectively.The results of efficacy from high to low were esomeprazole (40 mg),dexlansoprazole (60 mg),pantoprazole (80 mg),omeprazole (40 mg),lansoprazole (30 mg),pantoprazole (40 mg) and omeprazole (20 mg).Conclusion For standard dose of PPIs,the effectiveness of esomeprazole is better than lansoprazole,pantoprazole and omeprazole.Due to the limitation of the present studies,the effectiveness comparison of esomeprazole (40 mg) with dexlansoprazole (60 mg) and other PPIs of double dose needs further investigation.
目的:基于医院信息系统(HIS)研究住院病例质子泵抑制剂(proton pump inhibitors,PPIs)使用情况并分析其合理性.方法:抽取陆军总医院2014年使用PPIs的出院病例,通过整理其费用、诊断、手术、医嘱、检验结果等资料,设计指标、确定算法,对PPIs使用情况进行分析.结果:纳入研究的病例共14 310例,其中26.31%为适应证治疗用药,16.79%为预防药物相关胃肠黏膜损害用药,19.47%为预防手术应激性溃疡用药,37.44%为无适应证用药.此外也发现0~~5岁患儿PPIs日剂量变异大,同时使用氯吡格雷和奥美拉唑的比例高等问题.结论:本研究利用HIS数据库实现对住院病例PPIs使用情况和合理性进行快速全面分析,临床药师应进一步结合循证依据实施PPIs合理用药点评.
Objective To evaluate the effectiveness and safety of salvianolate injection (SI) on motor and cognitive function of patients with acute cerebral infarction (ACI).Methods We electronically searched databases including PubMed, The Cochrane Library, EMBASE, ClinicalTrail.gov, CBM, CNKI and WanFang Data to collect randomized controlled trials (RCTs) focused on SI treating ACI up to February 29th, 2016. Two reviewers independently screened literatures, extracted data and assessed the risk of bias of included studies. Then, meta-analysis was performed using RevMan 5.3 and R 3.2.3 software.Results A total of 19 RCTs involving 2383 patients were included. The results of meta-analysis showed that compared with the conventional treatment (CT) group, the total effective rate of SI treatment group was higher (RR=1.23,95%CI:1.17~1.29,P<0.00001) in 10 RCTs;NIHSS score of SI treatment group was lower (MD=-2.42,95%CI:-2.86~-1.98,P<0.00001) and Barthel score of SI treatment group was higher (MD=7.68,95%CI:5.15~10.21) in 15 RCTs; and CRP level of SI treatment group was lower (MD=-2.14,95%CI:-2.63~-1.65,P<0.00001). There was no significant difference in adverse reaction rates between the two groups (P>0.05).Conclusion Salvianolate can promote recovery of the motor and cognitive function of patients with acute cerebral infarction. However, due to the limited quality and quantity of included studies, more high quality studies are needed to verify the above conclusion.
目的 系统评价五酯胶囊(片)对他克莫司(FK506)血药浓度及药动学参数的影响.方法 检索PubMed、中国生物医学文献数据库、中国知网、万方数据,五酯胶囊(片)与FK506合用治疗肝肾移植患者的随机对照试验,检索时限2004年1月 2014年1月.由2位评价员根据纳入与排除标准独立进行文献筛选、资料提取并质量评价后,采用RevMan 5.2软件进行Meta分析.结果 纳入10个临床随机对照试验,共491例患者.根据干预时间的长短进行亚组分析,Meta分析结果显示:(1)干预1个月后:与FK506组相比,合用组血药浓度显著增加[SMD =0.75,95% CI(0.54,0.97),P<0.001],FK506剂量减少[SMD=-1.52,95%CI(-1.78,-1.26),P<0.001];(2)干预3个月后:合用组血药浓度显著增加[SMD=0.44,95% CI(0.25,0.64),P<0.001],FK506剂量减少[SMD=-1.71,95%CI(-1.95,-1.47),P<0.001];(3)药代动力学主要参数:峰浓度Cmax[SMD=1.57,95%CI(0.26,2.09),P=0.002],药时曲线下面积(AUC0h)[SMD=1.66,95%CI(1.10,2.23),P<0.001]和达峰时间(tmax)[SMD=1.03,95%CI(0.52,1.55),P<0.001]有显著差异.结论 五酯胶囊(片)对FK506组的血药浓度有显著影响,可提高其血药浓度,降低其用量,提高生物利用度.受限于纳入研究数量,上述结论尚需今后开展更多大样本、高质量的临床试验加以验证.
Objective To improve the process of extraction and purification by establishing a new method for determining the content of naringin in Shutong capsules.Methods HPLC was used to assay the content of naringin in Shutong capsules.Extraction time and purification methods were studied by analyzing the yield rate of dehydrated extract and the content of naringin.Results There was a good linear relationship for naringin assay over the range of 50-1500 ng,with an average recovery of 99.90% and RSD of 1.71%.A maximum yield rate of dehydrated extract was obtained by two-time continuous water extraction,0.5 h each time.The ratio of materia to water was 1∶ 10.Conclusion The process of Shutong capsule can be improved by analyzing the yield rate of dehydrated extract and the content of naringin.
Objective :To study the efficacy of Shutong Capsules on promoting large intestine enterokinesia and gastrointestinal hormones of the slow transit constipation in rats.Methods :Sixty Wistar rats were randomly divided into normal control group(10 rats)and model group(STC,50 rats).Fifty STC rats were divided into 5 groups :STC model group,Shutong Capsules low dosage,middle dosage and high dosage(0.54,1.08,2.16 g·kg-1·d-1)groups,and Maren pills group.After 2-week treatment,the discharge time of the first black feces,the black feces' s total number at 5 hours and 12 hours were observed.Levels of plasma substance P(SP),nitric oxide(NO)and motilin(MTL)were determined.Results :Shutong Capsules middle and high dose groups' rats shortened discharge time of the first black feces and increased black discharge points(P<0.05,P<0.01).Shutong Capsules middle dosage and high dosage groups' plasma substance P(SP),nitric oxide(NO)and motilin(MTL)contents difference were statistically significant(P<0.05).Conclusion :Shutong Capsules can obviously promote facilitation function to the STC rats colon,raise SP and MTL levels,and reduce NO level of STC,which is probably due to its regulation of the gastro intestinal hormones levels.
目的探讨可舒胶囊对酒精所致大鼠急性肝损伤的保护作用及机制。方法采用酒精灌胃辅以高脂饲料法复制急性酒精性肝损伤小鼠模型,以血清谷酰转肽酶(GGT)、血清丙氨酸转氨酶(ALT)和天门冬氨酸转氨酶(AST)水平为模型参考指标。50只雄性昆明小鼠随机分为5组:正常对照组、肝损伤模型组、可舒胶囊低、中、高剂量(0.78、1.56、3.12 g/kg)干预组,每组10只。检测小鼠肝组织天冬氨酸特异酶切的半胱氨酸蛋白酶(Caspase)-3,-8活性。结果模型组小鼠血清GGT[(4.52±0.26)U/L]、ALT[(173.09±8.79)U/L]、AST[(141.90±7.82)U/L]水平较正常对照组显著升高(P<0.01);可舒胶囊高、中、低剂量干预组小鼠血清GGT水平分别为(1.42±0.10)、(2.65±0.41)、(3.25±0.56)U/L;ALT分别为(72.19±8.19)、(78.52±9.72)、(98.95±10.70)U/L;AST水平分别为(70.27±8.19)、(77.46±5.94)、(94.57±5.26)U/L;较模型对照组显著降低(P<0.01)。模型组小鼠肝组织Caspase-3活性为(6.54±0.43)U/mgpro,Caspase-8活性为(2.71±0.23)U/mgpro,较正常对照组显著升高(P<0.01);可舒胶囊高、中、低剂量组小鼠肝组织Caspase-3活性分别为(2.57±0.33)、(3.20±0.30)、(4.32±0.35)U/mgpro;Caspase-8活性分别为(1.13±0.25)、(1.43±0.29)、(1.98±0.31)U/mgpro,较模型组显著降低(P<0.01)。结论酒精肝损伤小鼠肝组织Caspase-3与Caspase-8活性显著升高,可舒胶囊对其具有明显抑制作用。

Objective Renal ischemia followed by reperfusion leads to acute renal failure,which is a complex pathophysiologic process involving hypoxia and free radical(FR) damage.We aimed to investigate the effect of oxymatrine on kidney ischemia/reperfusion(I/R) injury and the antioxidant effects of oxymatrine in rats.MethodsSD rats were randomly divided into 5 groups(I/R group,sham operation group and oxymatrine treatment groups).Rats in the I/R group were subjected to bilateral renal ischemia for 45 min and followed by reperfusion.Rats in the oxymatrine treatment groups received oxymatrine intraperitoneally(i.p.) at 3 different doses before I/R for 7 days.In some experiments,rats were killed and kidney function,tissue catalase(CAT),malondialdehyde(MDA) levels,superoxide dismutase(SOD) and glutathione peroxidase(GSH-Px) activities were determined.ResultsOxymatrine significantly prevented I/R-induced kidney injury as indicated by decreased serum creatinine(Scr) and blood urea nitrogen(BUN) levels compared to I/R group(P<0.05).Reduction of tissue CAT,SOD and GSH-Px activities after I/R were significantly improved by oxymatrine(P<0.05).Treatment with oxymatrine also resulted in significant reduction in tissue MDA that was increased by renal I/R injury(P<0.05).Conclusions Based on our results,it could therefore be concluded that oxymatrine protects the kidneys against I/R injury at least partly via its antioxidant effects.
Objective To screen the proper excipients and the preparation process for Shutong Sugar-free Particles.Methods By analyzing the resistance to moisture and moldability,the types and ratio of the excipients and the different preparation process were screened out.Results The most proper preparation was consisted of 95% of the extract of Shutong,4% of lactose,1% of mannit,and then adding 15% of phycite as flavor.Conclusion The optimized granules are characterized with a high resistance to moisture,solubility and moldability.
Objective To study the quality control of Gezhihugan Capsules.Methods Thin layer chromatography was used to determine Radix puerariae,Daidzein,Hesperidin and GinsenosideRg1/Re/Rb1,and high performance liquid chromatography system was used to determine the content of puerarin,the active compound,puerarin in the Gezhihugan capsules.Results The spot of Radix puerariae,Daidzein,Hesperidin and GinsenosideRg1/Re/Rb1 of the test sample was consistent with that of the control in the thin layer chromatography.The linear range of puerarln was both 5-80 μg/ml,r=0.9999,the average recovery was 100.66%,and RSD was 2.41%.Conclusion Thin layer chromatography and high performance liquid chromatography used in this study are simple and accurate for the quality control of Gezhihugan Capsules.
AIM:To investigate the dilation effect of isosorbide dinitrate(ISDN) on the rat urine bladder rings.METHODS: The values of the mean contraction tension,maximum amplitude and frequency of rat urine bladder rings were measured.The activity of nitric oxide synthase(NOS) and the content of nitric oxide(NO) were measured in the rat urine bladder tissue pre-incubated with ISDN(10-4 mol/L) for 1 h.RESULTS:ISDN(10-5 mol/L,10-4 mol/L) could inhibit the mean contractile tension,maximum amplitude and frequency of spontaneous rhythmic contractions of rat urine bladder pre-contracted with ACh(10-4 mol/L).However,there was no significant change to the rings pre-contracted with KCl(4×10-4 mol/L).And the activity of NOS and the content of NO were elevated in the rat urine bladder tissue pre-incubated with ISDN.CONCLUSION: ISDN can induce relaxation on rat isolated bladder detrusor muscle rings pre-contracted with ACh,and the mechanism is possibly related to the activation of the nitric oxide synthase and the elevation of NO level.
OBJECTIVE: To optimize the prescription of traditional Chinese medicine for motion sickness (MS).METHODS: Zingiber officinale,Herba pogostemonis,Aucklandia lappa were extracted respectively,and rotating-inducing MS mice were enrolled in uniform design.The prescription was optimized with MS index as the parameters.RESULTS: The optimal prescription was as follows:60 g Z.officinale,45 g Herba pogostemonis,5 g A.lappa.The extractive of prescription was significantly better than dimenhydrinate in the treatment of MS.CONCLUSION: MS index is a stable and sensitive parameters and it is suitable for screening and evaluation of anti-MS drugs.R.zingiberis,H.pogostemonis,A.lappq are potential drug for MS.
目的 观察健脑口服液对双侧颈总动脉结扎致痴呆大鼠学习记忆能力及脑组织中超氧化物歧化酶(SOD)、丙二醛(MDA)和乙酰胆碱酯酶(AchE)含量的影响.方法 大鼠双侧颈总动脉反复夹闭再灌注同时腹腔注射硝普钠降压法建立血管痴呆(VD)大鼠模型,考察痴呆大鼠在水迷宫中学习、记忆成绩,测定脑组织SOD、MDA和AchE活性.结果 与假手术组比较,痴呆模型组大鼠记忆成绩明显下降(P<0.01),脑组织SOD含量下降(P<0.01),MDA、AchE含量增加(P<0.05);与痴呆模型组大鼠比较,健脑口服液中、高剂量组学习记忆成绩明显上升,脑组织SOD含量升高,MDA、AchE含量下降(P<0.01).结论 健脑口服液升高VD大鼠脑组织SOD和AchE含量,降低MDA活性,增强VD大鼠学习记忆能力.
