Yam is a versatile economic crop that serves both medicinal and dietary purposes. Dehua County, located in Fujian Province, China, is renowned as one of the major yam production areas, with a cultivation history spanning over 600 years. It has successfully cultivated Qingfeng yam and Ziyu yam, both of which have been recognized with China’s “Geographical Indications for Agricultural Products.” However, no comprehensive studies have been conducted to evaluate their quality. This study meticulously utilized the authentic medicinal material “Iron yam” as a benchmark, employing advanced techniques such as high-performance liquid chromatography (HPLC), ultraviolet spectrophotometry, and flame atomic absorption spectrometry to systematically analyze the nutritional and hypoglycemic active components of three distinct yam varieties. In order to interpret the data, descriptive statistics, correlation analysis, principal component analysis (PCA), partial least squares discriminant analysis (PLS-DA), cluster analysis and multiple linear regression analysis were systematically applied. The results revealed significant variations in the concentrations of various indicators across the three yam types. Correlation analysis identified 65 pairs of indicators with exceptionally strong correlations and 39 pairs with statistically significant associations. Additionally, the principal component analysis demonstrated that Iron yam exhibited the most favorable overall quality. Notably, Ziyu yam, characterized by its high concentration of hypoglycemic active compounds, emerged as a promising raw material for the production of hypoglycemic products, showcasing significant potential in this field.
目的:检测前期研究筛选与验证出的胎盘差异表达蛋白在妊娠期糖尿病(GDM)和正常孕妇血清中的表达情况,筛选GDM的候选生物标志物,为生物标志物和新治疗靶点的探索提供方向.方法:收集30例GDM和30例正常孕妇常规孕检资料和产前空腹肘静脉血标本,采用酶联免疫吸附测定法(ELISA)检测外血小板溶素1(EPPK1)、冷诱导RNA结合蛋白(CIRBP)、不均一核糖核蛋白A2/B1(HNRNPA2B1)、不均一核糖核蛋白AB(HNRNPAB)、不均一核糖核蛋白L(HNRNPL)、RAS癌基因家族成员RAB21(RAB21)和RAS癌基因家族成员RAB3B(RAB3B)7个蛋白在两组孕妇血清中的表达水平.用二元Logistic回归分析寻找GDM发生的影响因素,并通过绘制受试者工作特征曲线(ROC)分析其对GDM的诊断效力.结果:GDM组EPPK1、CIRBP、HNRNPAB和RAB3B表达水平均高于对照组,差异具有统计学意义(P<0.05),HNRNPA2B1、HNRNPL、RAB21在GDM组和对照组血清中的表达量差异无统计学意义(P>0.05).二元Logistic回归分析结果显示,EPPK1和RAB3B高表达是GDM发生的影响因素.ROC曲线分析显示,当EPPK1截断值为550.0 pg/mL时,其对GDM检验效能的灵敏度为0.931,特异度为0.733,曲线下面积(AUC)为0.885;当RAB3B截断值为333.1 pg/mL时,RAB3B对GDM检验效能的灵敏度为0.966,特异度为0.633,AUC为0.779.结论:EPPK1、CIRBP、HNRNPAB和RAB3B在GDM孕妇血清中高表达,EPPK1与RAB3B对GDM具有较好的诊断效力,可作为GDM的候选生物标志物进行后续验证研究.
INTRODUCTION:Gestational diabetes mellitus (GDM) refers to abnormal glucose tolerance that occurs or is firstly diagnosed during pregnancy. GDM is related to various adverse pregnancy outcomes, but GDM pathogeny has not been fully elucidated. Nevertheless, previous studies have observed that many proteins in the placentas of patients with GDM are dysregulated. The present study aimed to establish a novel differentially expressed protein (DEP) landscape of GDM and normal maternal placentas and to explore the possible connection between DEPs and GDM pathogenesis. This study provides new insights into the mechanism of GDM and should make an important contribution to the development of biomarkers. METHODS:The morphological characteristics of the placenta were observed on 30 GDM and normal maternal placental tissues stained with haematoxylin and eosin. Isobaric tags for relative and absolute quantitation (iTRAQ) was used in the proteomics screening of the DEPs of the normal and GDM maternal placentas. Bioinformatics analysis was performed on the DEPs, and parallel reaction monitoring (PRM) was performed to verify the DEPs. Finally, the quantitative analysis of iTRAQ and PRM was verified by immunohistochemical assay. RESULTS:A total of 68 DEPs in the GDM placenta were identified with iTRAQ proteomics experiment, comprising 21 up-regulated and 47 down-regulated DEPs. Bioinformatics analysis showed that the regulation of transport, catabolic process of non-coding RNA, cytoskeleton and cell binding were the most abundant Gene Ontology terms, and RNA degradation was an important pathway for significant enrichment. Protein-protein interaction network analysis showed that heterogeneous nuclear ribonucleoproteins A2/B1 (HNRNPA2B1), heterogeneous nuclear ribonucleoprotein A/B (HNRNPAB), heterogeneous nuclear ribonucleoprotein L (HNRNPL) and heterogeneous nuclear ribonucleoprotein A3 (HNRNPA3) were the cores of the up-regulated proteins. Band 3 anion transport protein (SLC4A1), spectrin beta chain erythrocytic (SPTB), ankyrin-1 (ANK1), spectrin beta chain non-erythrocytic 2 (SPTBN2), D-3-phosphoglycerate dehydrogenase (PHGDH) and exosome complex component RRP42 (EXOSC7) were the cores of the down-regulated proteins. These proteins are involved in the binding, splicing, processing, transport and degradation of RNA and in the formation and maintenance of the cytoskeleton. PRM verification results showed that seven proteins, namely, epiplakin (EPPK1), cold-inducible RNA-binding protein (CIRBP), HNRNPA2B1, HNRNPAB, HNRNPL, Ras-related protein Rab-21 (RAB21) and Ras-related protein Rab-3B (RAB3B), were up-regulated, whereas SPTB and SLC4A1 were down-regulated. The results of immunohistochemical assay also showed that the expression of five proteins, namely EPPK1, HNRNPA2B1, HNRNPAB, CIRBP and RAB21, were significantly higher in GDM placental tissues (P < 0.01). The GDM placentas showed changes in the morphological evaluation, including poor villous maturation, obvious increase in the number of syncytiotrophoblast nodules, thickening of the wall of dry villous arterioles with lumen stenosis, increased fibrinous exudation and excessive filling of villous interstitial vessels. DISCUSSION:Differentially expressed proteins related to a variety of biological processes in the GDM placenta were found. Fourteen proteins, namely, HNRNPA2B1, HNRNPAB, HNRNPL, HNRNPA3, EPPK1, CIRBP, RAB21, RAB3B, SLC4A1, SPTB, ANK1, SPTBN2, PHGDH and EXOSC7, which were differentially expressed in the placenta, may play an important role in regulating the occurrence and development of gestational diabetes through multi-channel and multi-link regulation.
ObjectiveTo screen out the candidate biomarkers of GDM by detecting the expression of placental differentially expressed proteins screened and verified by a previous study, which provided a direction for the exploration of biomarkers and new treatment targets.MethodsThe data of routine pregnancy examination and prenatal fasting blood samples from elbow were collected from 30 women with GDM and 30 normal pregnant women.The expression levels of Epiplakin (EPPK1),cold⁃inducible RNA⁃binding protein (CIRBP),heterogeneous nuclear ribonucleoproteins A2/B1 (HNRNPA2B1),heterogeneous ribonucleoprotein A/B(HNRNPAB),heterogeneous ribonucleoprotein L(HNRNPL),ras⁃related protein rab⁃21(RAB21) and ras⁃related protein rab⁃3b(RAB3B) in the serum of the two groups were detected by enzyme⁃linked immunosorbent assay(ELISA).Binary Logistic regression analysis was used to find out the factors affecting the occurrence of GDM,and its effectiveness in the diagnosis of GDM was analyzed by drawing the receiver operating characteristic curve (ROC).ResultsThe expression levels of EPPK1,CIRBP,HNRNPAB and RAB3B in the GDM group were significantly higher than those in the control group,the digfferences were all statistically significant(P<0.05).And there was no significant difference in the expression of HNRNPA2B1,HNRNPL and RAB21 between the GDM group and the control group(P<0.05).The results of binary Logistic regression analysis showed that the high expression of EPPK1 and RAB3B were the influencing factors of GDM.ROC curve analysis showed that,when the tangent value of EPPK1 was 550.0 pg/mL,the sensitivity,specificity and AUC of GDM were 0.931,0.733 and 0.885,respectively,and when the cut⁃off value of RAB3B was 333.1 pg/mL,the sensitivity,specificity and AUC of GDM were 0.966,0.633 and 0.779,respectively.ConclusionsEPPK1,CIRBP,HNRNPAB and RAB3B were highly expressed in the serum of women with GDM.EPPK1 and RAB3B had good diagnostic efficacy for GDM and could be used as candidate biomarkers of GDM for further verification.
Objective To investigate the perceptions of pelvic floor dysfunction (PFD) and rehabilitation care amongst women after radical hysterectomy and to explore ways to improve quality of care. Methods Thirty-six women who underwent radical hysterectomy at a hospital in southeast China were enrolled via purposive sampling. Semi-structured in-depth interviews were conducted. The texts were analysed via qualitative content analysis. Results Four themes were obtained: serious lack of knowledge, heavy psychological burden, different coping strategies and great eagerness to receive multiparty support on PFD rehabilitation care. Conclusion The society and professional staff should strengthen health education on PFD. Professionals should offer education before and after surgery and actively provide rehabilitation consultation to promote the availability of rehabilitation to support women with PFD rehabilitation care. In addition, family-centred care is an important way to support women to return to normal life, and women's need for family support should be more actively expressed. Moreover, knowledge of medical insurance should be popularised, especially in rural areas in China.
目的 构建中医智能健康护理管理需求模式问卷,并进行信效度检验.方法 通过理论分析、半结构式访谈形成问卷的初始条目池,通过德尔菲专家函询法形成问卷初稿.对30名患者进行预调查,并进行语言逻辑修订.对412例患者进行正式调查,通过方差分析对一般资料进行分析,通过条目分析修订条目,采用SPSS、Amos软件对问卷进行信效度分析.结果 2轮函询专家的积极性系数分别为90.90%和75.00%,专家权威系数分别为0.89和0.86,肯德尔协调系数分别为0.150和0.231.通过专家函询和预调查,形成正式版问卷,包含4个维度共29个条目.正式调查共发放425份问卷,有效问卷412份,有效率96.94%.除慢性病之外(P=0.008);其余项目的单因素方差分析显示,F为0.122~2.048(P>0.05);条目分析中相关系数r为0.725~0.833(P<0.001),区分度系数为13.629~21.024(P<0.001);各条目内容效度为0.867~1.000,总问卷的内容效度为0.956;卡方比自由度为4.720、比较拟合优度指数为0.879、Tucker-Lewis指数为0.867、近似误差均方和平方根为0.095、均方根残差为0.021、递增拟合指数为0.879、简约良性拟合指数为0.654、简约规范拟合指数为0.778;问卷的Cronbach's a系数为0.974,分半信度系数为0.937.结论 中医智能健康护理管理需求模式问卷具有良好信效度,可用来测试慢性病患者对于此种健康管理模式的需求.
目的 系统评价妊娠糖尿病(gestational diabetesmellitus,GDM)胎盘蛋白表达水平改变及其与母体胰岛素抵抗(insulin resistance,IR)的相关性.方法 计算机检索 PubMed、Cochrane Library、Scoups、Web of Science、Embase、中国知网数据库、维普数据库、万方数据库和中国生物医学文献数据库中有关GDM胎盘蛋白表达改变及与IR相关性的病例对照研究.检索时限均为2009年1月-2021年11月.由2名评价者独立筛选文献、提取资料及评价文献质量.采用RevMan 5.4软件进行meta分析,对不能合并分析的数据进行描述性分析.结果 共纳入19篇文献,包括2 012例患者.Meta分析结果示:GDM胎盘上视黄醇结合蛋白4表达水平升高[标准化均数差=2.11,95%置信区间(confidence interval,CI)(1.64,2.58),P<0.000 01],蛋白酪氨酸磷酸酶1B表达阳性率升高[相对危险度(relative risk,RR)=1.56,95%CI(1.29,1.88),P<0.000 01],胰岛素受体底物1表达阳性率降低[RR=0.69,95%CI(0.60,0.78),P<0.000 01];且视黄醇结合蛋白4(P<0.000 01)、蛋白酪氨酸磷酸酶1B(P<0.000 01)蛋白表达与胰岛素抵抗指数(homeostasis model assessment of insulin resistance,HOMA-IR)呈正相关,胰岛素受体底物1(P<0.00001)和脂联素(P=0.002)蛋白表达均与HOMA-IR呈负相关,葡萄糖转运蛋白4与HOMA-IR不相关(P=0.79).描述性分析发现,脂肪细胞因子(瘦素、抵抗素)、氧化应激标志物(黄嘌呤氧化酶、丙二醛、8-异前列腺素)、炎症因子(肿瘤坏死因子-α、Toll样受体4、半乳糖凝集素3、半乳糖凝集素2、巨噬细胞移动抑制因子)、胎球蛋白A、叉形头转录因子O亚型1、叉形头转录因子O亚型3a、雌激素受体α在GDM胎盘中的表达水平或阳性率上调,且均与HOMA-IR呈正相关.胰岛素抵抗信号通路蛋白(磷脂酰肌醇3-激酶、蛋白激酶B、磷酸化蛋白激酶B、葡萄糖转运蛋白4)表达水平或阳性率下调,且磷脂酰肌醇3-激酶、蛋白激酶B与HOMA-IR呈负相关,磷酸化蛋白激酶B与HOMA-IR无相关性.结论 胎盘蛋白表达失调可能介导母体IR加重,从而促进GDM及其并发症的发生发展,但因果关系和调控机制尚不明确,有待进一步深入研究.
目的:探讨热敏灸对H型高血压患者脑卒中及其危险因素的影响.方法:选取福建省人民医院2018年9月-2019年6月收治的H型高血压患者140例,采用随机数字表法分为观察组和对照组,每组70例.对照组行常规治疗,观察组行热敏灸治疗,记录两组干预前、4、8周后血压、Hcy水平、血脂水平、新增脑卒中及脑卒中后并发症的发生率.结果:随访3个月内,观察组患者新增脑卒中及脑卒中后并发症的发生率均低于对照组(P<0.05),干预4、8周后两组患者危险因素(血压、Hcy水平)均有下降,观察组低于对照组,差异均有统计学意义(P<0.05);干预4周后,两组血脂水平差异均无统计学意义(P>0.05),干预8周后,观察组血脂水平均优于对照组,差异均有统计学意义(P<0.05).结论:热敏灸治疗H型高血压患者,有利于降低脑卒中发生率,并影响其危险因素.
目的 确定妊娠SD大鼠空腹血糖(FBG)和空腹胰岛素(FINS)指标的参考值上限.方法 检索中国知网、万方、维普、Web of Science、The Cochrane Library、Scopus、PubMed、EMBase和MEDLINE共9个中英文数据库中与妊娠SD大鼠血糖有关文献,检索时限为2014年1月—2020年12月.严格按照纳入和排除标准,对检索到的文献进行筛选和数据提取.采用SYRCLE动物实验偏倚风险评估工具进行文献质量评价,以加权均数差(WMD)和95%可信区间(CI)作为合并统计量,应用STATA 15.0统计软件进行单组连续变量Meta分析.结果 纳入37篇文献,包含428只大鼠.系统评价和Meta结果显示妊娠大鼠FBG和FINS的参考值上限分别为妊娠第0周的5.51 mmol/L(FBG)及无统计数据(FINS);第1周6.92 mmol/L(FBG)和15.66 mIU/L(FINS)、 第2周的6.64 mmol/L(FBG)和15.98 mIU/L(FINS)及第3周的7.04 mmol/L(FBG)和15.63 mIU/L(FINS).结论 通过Meta分析确定妊娠SD大鼠FBG和FINS的参考值上限,为相关实验提供依据,以期减少实验误差.
目的 系统评价妊娠期糖尿病胎盘差异表达蛋白对胎儿的影响.方法 于2019年6月计算机检索PubMed、Web of Science、Cochrane Library、EMBase、Medline (Ovid)、Scopus、中国生物医学文献数据库、中国知网、万方数据知识服务平台、维普中与妊娠期糖尿病胎盘蛋白表达有关的文献,检索时限为2010年1月-2019年6月.采用主题词与自由词结合的方式进行检索,同时手工检索相关文献的参考文献.运用纽卡斯尔渥太华量表评价纳入研究的方法学质量,进行描述性分析.结果 最终纳入15篇文献,均为病例对照研究,共包含844名研究对象.系统评价结果显示:胎盘差异表达蛋白通过干扰葡萄糖、脂质代谢与转运,影响氨基酸转运和蛋白质合成,调整胎盘屏障,增加水、甘油和铁转运4个途径影响胎儿生长发育和妊娠结局.结论 胎盘蛋白表达失调可能介导胎盘营养转运能力的改变和影响胎盘屏障,参与胎儿宫内生长发育和增加不良胎儿结局的风险.但其因果关系及调控机制尚不明确,有待进一步深入研究.

目的:系统评价妊娠期糖尿病(GDM)胎盘差异表达蛋白对胎盘生长发育的影响.方法:计算机检索PubMed、Web of Science、Cochrane Library、EMBase、Medline(Ov-id)、Scopus、中国生物医学文献数据库、中国知网、万方数据知识服务平台和维普数据库中与GDM胎盘蛋白差异表达对胎盘生长发育影响的有关文献,检索时限为2009年6月至2019年6月.由两名研究者独立进行文献筛选、数据提取及质量评价.运用Review Manager 5.3软件对胎盘蛋白表达水平进行Meta分析;不适合Meta分析的数据进行描述性分析,包括蛋白表达水平改变、差异表达蛋白对胎盘的影响、蛋白定位、胎盘病理.结果:最终纳入20篇文献,共1210例研究对象.Meta分析结果示,GDM组胎盘VCAM-1表达水平和HIF-1α阳性率升高,PTEN表达水平降低.描述性分析发现:(1)GDM胎盘差异表达蛋白通过影响滋养细胞融合、增殖、分化、侵袭及凋亡,干扰胎盘血管生成与损伤,加强胎盘局部氧化应激,引起胎盘生长发育异常,改变胎盘结构与功能;(2)胎盘差异蛋白主要免疫定位在滋养细胞、和(或)血管内皮细胞、和(或)绒毛间质细胞中;(3)GDM胎盘病理改变报道最多的是绒毛成熟不良和血管壁增厚、管腔狭窄.结论:胎盘蛋白表达失调可能介导胎盘结构和功能改变,从而促进GDM和其它妊娠期并发症的发生发展,其因果关系及调控机制尚不明确,有待进一步深入研究.
目的 探讨脑卒中后认知功能障碍患者使用热敏灸的效果研究.方法 于2018年6月至2019年12月便利抽取福建中医药大学附属人民医院符合纳入排除标准的研究对象86例,用随机数字表法分为观察组和对照组各43例,观察组在采用常规治疗护理模式的对照组基础上给予热敏灸治疗.结果 干预治疗后两组患者认知功能及日常生活能力均有改善,但观察组改善程度优于对照组,差异具有统计学意义(P<0.05).结论 热敏灸疗法能有效促进脑卒中后认知功能障碍患者的康复,提高其生活质量.
随着人口老龄化及慢性病的增加,居家护理的重要性逐渐凸显.智能健康管理APP具有护患互知、加强安全,双方互选、就近服务,满足城乡患者家庭护理需要的特点,不仅促进了护士的职业多样化,同时也满足了患者的居家护理需求,促进了智能健康管理的发展.为此,本次拟对一款交互式双向双端家庭护理智能健康管理手机APP的设计进行前期分析,从而初步设计出此款手机APP.文章首先通过需求分析,总结了护患双方在"互联网+"时代下对健康管理的需求特点;之后阐述了该APP护士端和患者端各自的具体功能,通过对同类产品特点的分析和总结,结合"约护"的设计背景,具体说明了本产品的优点;最后通过登录界面、项目栏界面、个人信息界面及护士搜索界面展示了初步设计效果.
目的:探讨《儿科护理学》岗位教学模式的应用效果.方法:采用量性与质性研究相结合的方法.选取2012级护理学专业本科学生100名作为对照组,采用传统教学模式;选取2013级护理学专业本科学生90名作为观察组,采用岗位教学模式.比较两组《儿科护理学》期末考试成绩.采用目的抽样招募2013级实习结束本科学生12名采用一对一访谈法进行质性研究.结果:观察组《儿科护理学》期末考试成绩优于对照组(P<0.01).质性研究组访谈结果形成三个主题,即岗位教学模式培养了学生的核心能力;岗位教学模式儿科护理专科实践不足;优化教学方法,强化专科实践.结论:《儿科护理学》岗位教学模式提高了学生的岗位核心能力,但需进一步优化教学方法,发挥学生自主学习能动性,提高学生的理论、实践能力,提升教学质量和职业素养.
[目的]调查厦门市脑卒中患者中医体质特点.[方法]采用便利抽样的方法对厦门市162名符合条件的脑卒中患者进行中医体质调查,描述不同体质类型分布情况,按不同的年龄、性别对体质类型进行比较.[结果]①厦门市脑卒中患者中医体质类型为平和质6人,偏颇体质156人;其中气虚质54人(33.3%),痰湿质48人(29.6%),湿热质18人(11.1%).②年龄<60岁患者以痰湿质最为常见,≥60岁则以气虚质最为多见;不同年龄的脑卒中患者对其气虚质的构成差异有统计学意义(P<0.05).③男性患者中常见体质类型(>20%)为气虚质,女性患者中常见体质类型(>20%)为痰湿质和气虚质.不同性别患者平和质、气虚质、痰湿质及血瘀质的构成差异有统计学意义(P<0.05).[结论]厦门市脑卒中患者中医体质类型以气虚质、痰湿质及湿热质为多见;不同性别、年龄的患者,其中医体质类型分布也有所不同.
目的:探讨脑卒中后尿失禁患者的膀胱管理现状及影响因素.方法:釆用便利抽样的方法,从福州、厦门某两所三甲医院神经内科抽取100例脑卒中急性期、恢复期、后遗症期住院患者为调查对象,采用郭丽的脑卒中尿失禁情况调查问卷进行调查.结果:脑卒中后尿失禁总发生率为48%,48名患者都有不同程度的漏尿,且都没有进行过尿动力学检查,膀胱管理中使用防护用品、记录排尿日记、执行饮水计划、进行膀胱训练、接收健康宣教、盆底肌锻炼总是或者经常的比例分别只有77%、10.4%、20.9%、22.9%、38.6%、0%,性别、照顾者、BMI指数、卒中类型、吸烟史、饮酒史等可能是影响脑卒中后尿失禁的相关因素.结论:脑卒中后尿失禁发生率高,尿失禁患者的膀胱管理仍需进一步加强与完善,脑卒中后尿失禁危险因素的确立有待进一步研究,医护人员应重视此类患者并寻找适当的措施使患者的身心健康得到最大程度的恢复.
目的:观察特制病员服对脑卒中后单侧空间忽略(USN)患者康复的影响.方法:选取脑卒中后USN患者30例,随机分为对照组(n=15例)和干预组(n=15例),对照组给予常规康复治疗和护理穿普通病员服,干预组在常规康复治疗和护理同时穿特制病员服,比较2组患者干预2个月后凯瑟琳-波哥量表(CBS)评分和Bartthel指数(BI)评分.结果:干预2个月后,2组患者的CBS和BI比较,干预组均优于对照组,差异具有统计学意义(P<0.05).结论:特制病员服可促进脑卒中后单侧空间忽略患者的康复,有效提高患者日常生活活动能力.
目的:对初产顺产的产妇产时接受的护理干预的评价,为优化产时护理服务提供依据。方法:运用质性研究的方法,采用深度访谈法收集符合纳入标准的10例资料,用现象学方法进行分析。结果:产妇对护理程序认同度较高;对护理干预舒适度还不太满意;产时助产士给予的情感支持不能满足产妇的需要,专业支持基本能满足;待产时护理人员应给陪伴的家属更多的健康教育。结论:对产时服务总的评价给予较多肯定,但在舒适度等方面与期望值存在差异。建议改善产科病房及产房环境,增加护理人员配置,尤其是夜班人数;加强护理人员的人文素质培训。
