目的 探讨小儿活血止痛颗粒对急性软组织损伤的药效作用.方法 将SPF级昆明种小鼠随机分为空白对照组(生理盐水)、阳性对照组(0.25 g/kg云南白药),以及小儿活血止痛颗粒低、中、高剂量组(分别相当于原药剂量2.31,4.62,6.94 g/kg),各10只.各组小鼠每天灌胃1次相应药物,连续给药7 d,采用醋酸致小鼠扭体模型和热刺激法致小鼠疼痛模型,考察镇痛作用;采用二甲苯致小鼠耳廓肿胀模型,考察消肿作用.取SPF级健康雄性SD大鼠,采用机械冲击挫伤法制备急性软组织损伤模型.将100只建模成功的大鼠随机分为模型组(B组,生理盐水),阳性对照组(C组,0.2 g/kg云南白药),以及小儿活血止痛颗粒高、中、低剂量组(D1组、D2组、D3组,相当于原药剂量7.00,3.50,1.75 g/kg),各20只;同时选择正常大鼠20只作为正常组(A组,生理盐水).各组大鼠每天灌胃1次相应药物,连续给药7 d,观察其组织损伤程度,并测定血液流变学参数.结果 与空白对照组比较,小儿活血止痛颗粒中、高剂量组醋酸所致疼痛小鼠的扭体潜伏期均显著延长,扭体次数均显著减少(P<0.05);小儿活血止痛颗粒高剂量组热刺激所致疼痛小鼠末次给药1.5 h后痛阈时间显著延长(P<0.05);小儿活血止痛颗粒中、高剂量组二甲苯所致耳廓肿胀小鼠的耳廓肿胀度均显著降低(P<0.05).对于机械冲击挫伤致急性软组织损伤,与B组大鼠比较,D1组、D2组、D3组大鼠的损伤证候评分分别在给药第3,5,7天显著降低(P<0.05);D1组、D2组大鼠的全血黏度和血浆黏度均在给药第3天显著降低(P<0.05).结论 小儿活血止痛颗粒具有消肿、镇痛、活血、化瘀功效,对软组织损伤有显著改善作用.其消肿、镇痛作用可能与减轻炎性因子渗出和肿胀有关,活血化瘀作用可能与降低全血黏度和血浆黏度有关.
目的 研究小儿活血止痛颗粒治疗儿童创伤性骨折的临床效果.方法 以2016年7月—2018年7月河南省洛阳正骨医院收治的儿童创伤性骨折患者100例为观察对象.根据随机数字表法分组研究,即对照组与观察组,各50例.2组均接受常规治疗,在此基础上,对照组采取云南白药胶囊治疗,观察组采用小儿活血止痛颗粒.比较2组疼痛程度、肿胀度、整体疗效以及炎性反应改善程度.结果 经7 d治疗后,观察组Wong-Baker面部表情疼痛3级发生率、自我疼痛主诉3级疼痛发生率(56.00%、58.00%)低于对照组(76.00%、80.00%),差异均有统计学意义(P<0.05).观察组轻度肿胀发生率(64.00%)高于对照组(42.00%);观察组重度肿胀发生率(4.00%)低于对照组(20.00%),差异均有统计学意义(P<0.05).观察组总有效率(84.00%)高于对照组(66.00%),差异有统计学意义(P<0.05).结论 小儿活血止痛颗粒可有效缓解儿童创伤性骨折疼痛,提升临床疗效,缓解肢体肿胀,促使小儿患者早日康复.

目的 探讨高职院校职业安全教育现状,为护理职业安全教育教学的改革和完善提供参考.方法 采用质性研究方法 ,自2020年5月~2020年6月分别对某高职院校8名护理教师、实习医院5名临床带教老师及16名高职实习护生进行护理职业安全教育现状的半结构式访谈,Colaizzi七步分析法整理分析访谈资料.结果 经分析提炼为三个主题:(1)高职护生护理职业安全知识学习兴趣不高;(2)高职实习护生职业安全行为影响因素主要包括教师教学能力、护生层面(学习能力、社交能力、临床护理技能熟练程度、性别等)及实习工作环境;(3)高职护理职业安全教育存在不足.结论 高职护理职业安全教育尚需深入研究.应基于高职护生学习需求,科学设置课程,以提高高职护生职业防护能力.
Pingle osteopathic medicine (POM) is a internal treatment based on syndrome differentiations of eight principles, meridians, viscera, defense Qi, construction and blood, triple burner and so on. It takes syndrome differentiations of defense Qi, construction and blood as the main program. It combines disease differentiation and syndrome differentiation to choose different treatment depending on the severity and priorities of the fractures, strength of the body, time of the injury. Thus, the analysis of the academic origin of syndrome differentiation of Pingle osteopathic medicine from perspectives of "principle-method-recipe-medicines" would help in the construction of the academic system of Pingle osteopathic medicine.
Objective:To optimize extraction process of Jiawei Xuezhongjie.Methods:Small Intestine Propulsion Dynamics Test was used to compare decoction and Steam distillation.With composite score of contents of J Rhein and Chrysophanol,yield of extract as index,orthogonal test was designed to optiminze extraction technology by taking solvent consumption,extracting time and times as factors.Results:Decoction was adopted optimal extraction conditions were as follows:extracted twice with eight times the water for 2 h each time.Conclusion:The extraction process was reasonable,simple and affordable,which was suitable for industrial production of Jiawei Xuezhongjie.
Objective:By measuring of the possible maximum scale of the personnel budget in public hospitals,providing ideas and references for the implementation of public hospital reform objectives,including gradually increasing the proportion of staff spending in business expenses.Methods:Based on the data from annual report of Henan provincial health finance in 2010-2016,fuzzy linear programming method was applied to construct the model of maximization hospital personnel expenditure budget and measure the optimized decision for calculating the personnel expenditure budget.Results:It put forward reasonable feasible region for public hospital expenditure budget;constructed the model for the maximization of public hospital personnel budget.It measured the largest scale with feasible for personnel budget in public hospitals of Henan in 2016 was 33.376 billion yuan,increased 3.804 billion yuan compared to the actual scale,which promoted 3.79% of the proportion for business expenditure.Conclusion:It could obtain the most feasible maximum scale of the public expenditure budget of the public hospital,based on the practices,reflect the policy requirement and development goal,and take into account the reasonable expenditure structure.
运用3阶段DEA模型,对河南省79家综合、中医、专科三级公立医院的投入产出效率进行测度.第1阶段结果显示,综合、中医、专科三类医院的综合效率分别为0.750、0.712、0.769;剔除政策环境因素后,第3阶段三类医院的效率分别为0.787、0.686和0.448.可见,各类公立医院的效率仍有较大提升空间,财政、科教投入等政策环境因素对各类医院的效率存在影响.
Background: Ginsenosides have been shown to exert beneficial pharmacological effects on the central nervous, cardiovascular, and endocrine systems. We sought to determine whether total ginsenosides (TG) inhibit monocrotaline (MCT)-induced pulmonary hypertension and to elucidate the underlying mechanism. Methods: MCT-intoxicated rats were treated with gradient doses of TG, with or without NG-nitro-l-arginine methyl ester. The levels of molecules involving the regulation of nitric oxide and mitogen-activated protein kinase pathways were determined. Results: TG ameliorated MCT-induced pulmonary hypertension in a dose-dependent manner, as assessed by the right ventricular systolic pressure, the right ventricular hypertrophy index, and pulmonary arterial remodeling. Furthermore, TG increased the levels of pulmonary nitric oxide, endothelial nitric oxide synthase, and cyclic guanosine monophosphate. Lastly, TG increased mitogen-activated protein kinase phosphatase-1 expression and promoted the dephosphorylation of extracellular signal-regulated protein kinases 1/2, p38 mitogen-activated protein kinase, and c-Jun NH2-terminal kinase 1/2. Conclusion: TG attenuates MCT-induced pulmonary hypertension, which may involve in part the regulation of nitric oxide and mitogen-activated protein kinase pathways.
平乐正骨即平乐郭氏正骨,发源于河南省洛阳市孟津县平乐村郭氏家族,盛传8代,已有200余年历史。新中国成立后,以第5代传人高云峰先生为院长,创办了世界上第一所骨伤科大学———河南平乐正骨学院,学员遍布全国各地。平乐正骨是祖国医学的一朵奇葩,目前已成为我国中医骨伤学科中有较大影响的正骨流派。但由于年代久远、单传秘授、战乱及建国后破四旧、文化大革命等影响,洛阳平乐正骨历史文献记载有限。文献资料的缺失,已成为进一步研究平乐正骨的制约因素。近年来随着考证研究的深入和重要史料的发现,平乐正骨的渊源逐渐清晰。本文从《秘授正骨心法》的发现、文献中关于郭尧民和祝尧民的记载、平乐正骨传人著作中关于其起源的记载、郭树信创益元堂、平乐正骨承《正体类要》《跌损妙方》的学术思想、平乐正骨续《医宗金鉴·正骨心法要旨》之方术、平乐正骨历代传人及其著作等方面对平乐正骨的渊源进行了阐述。
目的 从服务医院领导层决策的角度出发,构建适用于城市公立中医院的综合性管理控制指标体系,监控医院发展运行状况,预警医疗质量和医院安全等问题,为强化科学决策与管理,促进公立医院协调发展提供工具和思路。对象与方法 以河南省洛阳正骨医院为研究现场,通过文献分析法和专家访谈法构建中医医院管控指标框架,应用Delphi法对指标进行筛选。结果 确立了包括医疗质量、医院安全、中医特色、经营效益、创新能力和人力资源6大维度、20个二级指标,72个三级指标的城市公立中医院管控指标体系。讨论 管控指标体系可操作性强,体现了中医医院运营、发展特色,符合深化公立医院改革方向,对于加强公立中医院科学管理与考评具有重要参考和借鉴价值。
目的:优选芪芍活血颗粒的醇提工艺.方法:采用正交试验法,以芍药苷和干浸膏收率为评价指标,考察加醇量、醇浓度、提取时间和提取次数等4个因素对提取工艺的影响.结果:最佳提取工艺为12倍量醇,提取3次,每次1.5h;乙醇浓度为70%.结论:优选的工艺合理可行,提取效率高,适合工业生产.
文章以国内一家传统中医骨科三甲医院为研究对象,面对医院发展规模的扩大,为提升医院管理层次和科学决策水平,欲借助信息化手段加强对内管理和对外宣传,力求搭建一个标准化、集成化的信息平台,充分挖掘、整理和利用临床和管理信息资源,在提高临床服务能力的同时,拓展传统中医的影响力.
Objective:To build the model of health risk assessment of osteoporosis combined with the present situation in health risk assessment processing. Method:To sum up the indexes of osteoporosis in the orthopaedic hospital and evaluate the health risk assessment of osteoporosis by questionnaire. Result:It can help forecasting the risk of osteoporosis and providing prevention and treatment via health risk assessment of osteoporosis.Conclusion:It is convenient and scientific to evaluate the symptoms in order to provide the reference of treatment.
The hospital chinese medicine preparations has a long history in our country, act as the cradle and source of the research to the chinese new medicine preparations. I takes an important role to the health service and human health. In this paper, through a large number of literatures, and according to the development of hospital chinese medicine preparations in resent years of Luo Yang Orthopedic Traumatological hospital, we have an in-depth analysis to the existing problems and make a thorough inquiry of the pharmaceutical research. Aimed at provide a theoretical basis and policy guidance to the development and application of hospital chinese medicine preparations, it may has important significance for the scientific and reasonable development of hospital medicine preparations.
Objective:To optimize the water extraction and purification processes of Taoren Xikang granules. Method:The paeoniflorin and dry extract rate are indexes,using orthogonal test,inspecting the purification processes affection by the additive quantity of water of extract time and number of times,purification of the extracting solution by water extract-alcohol precipitation,the paeoniflorin and dry extract rate are indexes,inspecting the ethanol concentration. Results:Optimum extraction process was as follows:extracted 2 times with 8 times the amount of water,1. 5 hours each time;the consistency of ethanol is 70%. Conclusion:The optimized process is reasonable and feasible,and it can provide reference for the industrialized production.
目的:评估3D骨科医患沟通系统在骨关节病科试运行的临床效果.方法:选取河南省洛阳正骨医院2014年6月~2014年11月骨关节病科收治的达到膝关节置换手术指征的膝关节骨关节病患者(仅单膝)共67例,用抛硬币的方法随机分为常规谈话组和3D软件谈话组,记录各个患者术前谈话用时、患者对于病情及手术的理解程度评分以及医患沟通满意程度评分.并用统计学方法分析,比较常规谈话组和3D软件谈话组的统计学差异.结果:3D软件谈话组和常规谈话组的谈话时间为3D软件谈话组(393.06±80.03)秒和常规谈话组(490.20±124.78)秒,两者相比具有高度统计学差异(P<0.01);两组患者对于膝关节骨性关节炎术前谈话的理解程度评分为3D软件谈话组(8.16±0.81)和常规谈话组(8.14±0.76),两者相比无统计学差异(P>0.05);两组患者对其术前谈话的满意度评分为3D软件谈话组(8.19±0.74)和常规谈话组(7.66±0.91),两者相比具有统计学差异(P<0.05).结论:在使患者对术前谈话内容达到相同理解程度的情况下,借助3D交互式医患沟通系统进行谈话比常规模式用时更少,且患者最终满意度更高.
Objective:To establish the quality rating of Gugutouhuaisiyu granules.Methods:To use thin layer chromatography to make qualitation identification of Duzhong,Jixueteng and Buguzhi in Gugutouhuaisiyu granules.Use high performance liquid chromatography to determine the ecdysterone content.Results:The spots were clear with good resolution and non-interference negative.The content determination of ecdysterone had a good linear relationship in 0.064 0-0.320 0 g·L-1 range,r=0.999 6.The average sample recovery was 97.71%(n=6).Conclusion:This method is simple,scientific and reasonable and can be used as quality control of the Gugutouhuaisiyu granules.
Objective:To study the possibility of innovation training mode to cultivate Pingle bonesetting inherited talents.Methods:Pingle bonesetting inheriting experimental class(or Pingle class for short) is one of the three experimental classes for cultivating talents of Chinese medicine inheritance type established by Henan University of Traditional Chinese Medicine based on local resource advantages.Pingle class relying on pingle bonesetting advantage resources,innovate education system and the talent training mode through combining universities and hospitals,and combining universities education and master-apprentice education;Construct characteristic curriculum system of Pingle bonesetting inheritance talents according to the principles of "strengthen the foundation,base on inheritance,complete system and practicality and effectiveness";Pay attention to practice,and improve the clinical ability of students.Results:The talent training of traditional Chinese medicine(Pingle class) is under the idea of "Chinese medicine+traumatology and orthopedics of TCM+highlighting the characteristic therapy technology of Pingle bonesetting",and the cultivated students also become the inheritor and disseminator of Pingle bonesetting characteristics technology on the basis of possessing the quality of practitioners of traditional Chinese medicine.Conclusion:Pingle bonesetting inheriting talents cultivated by innovative training mode are relying on advantageous resources,and the innovation of personnel training mode,so the talents have necessarily characteristics.
Objective:To optimize extraction technology for Gusong Jiangu granules.Method:With yield of icariin and dry extract as indexes,orthogonal test was adopted to optimize extraction technology of Gusong Jiangu granules by taking the amount of water,extraction time and extraction times as factors.Result:Optimum extraction process was as following:extracted 3 times with 10 times the amount of water,1 h each time.Conclusion:This optimized extraction technology was effective,reasonable,feasible and applicable for industrial production.
