Deze oratietekst gaat over psychotherapie in relatie tot wetenschap, werkveld en onderwijs. Wetenschappelijk onderzoek was de afgelopen decennia sterk gericht op specifieke interventies en behandelprotocollen en de daarin werkzame factoren. Daarentegen werd relatief weinig aandacht besteed aan de zogenaamde therapeutfactor, dat wil zeggen: de invloed van therapeutkenmerken en -vaardigheden op de effectiviteit van psychotherapie. Er zijn echter aanwijzingen dat de therapeutfactor verantwoordelijk is voor 5–9 % van de behandeleffecten; dat is, in vergelijking met andere voorspellers, een flink effect. In de oratie worden kenmerken van effectieve therapeuten besproken. Daarna wordt ingegaan op de positie van de psychotherapeut in de beroepenstructuur in de ggz en volgt een pleidooi om de psychotherapeut op te nemen als specialist van de gz-psycholoog, naast de klinisch psycholoog en de klinisch neuropsycholoog. Tot slot komt aan de orde hoe de opleiding tot psychotherapeut kan worden verbeterd. Belangrijk is dat men in de opleiding tot psychotherapeut primair leert om effectief bevonden interventies goed toe te passen en tevens dat men leert om als therapeut maximaal effectief te zijn.
Substance use disorders and intimate partner violence (IPV) perpetration frequently co-occur, and it has been hypothesized that alcohol use and IPV perpetration are causally related. This led to the development of an Integrated treatment for Substance abuse and Partner violence (I-StoP). This case study describes the treatment of Henry, who repeatedly abused his partner and was diagnosed with alcohol and cannabis dependence. Treatment with I-StoP was highly successful with respect to IPV: At posttreatment and 6 months follow-up, no IPV had taken place. However, Henry had not changed substance use. This can be explained by the spurious model that states that a third variable, such as inadequate problem-solving skills (which was addressed in I-StoP), may be responsible for both IPV and substance abuse. In addition, Henry's partner was involved in treatment and became more assertive and, in contrast, he did change substance use. This case study illustrates that IPV is a very complex problem and that it is important to involve the partner.
BACKGROUND:Attention-Deficit/Hyperactivity Disorder (ADHD) in adults is one of the predictive and treatable risk factors for delinquency, including intimate partner violence (IPV). Effective treatment of IPV needs to address personal dynamic risk factors, offender typology, and dynamics of the domestic violence. It is unknown whether treatment of ADHD symptoms contributes to a decrease in IPV. The ITAP study aims to investigate the relationship between treatment of ADHD symptoms and IPV in patients in forensic mental health care. Moreover, this study examines the role of comorbid psychopathology, subtype of the offender, and dynamics of the domestic violence. METHODS/DESIGN:The ITAP study is a longitudinal observational study. Participants are followed one year through various assessments: one before starting treatment (t0), and four during treatment (8, 16, 24 and 52 weeks after start of the treatment). All participants receive treatment for IPV, ADHD, and comorbid psychopathology, if present. The primary outcome measure is the change in severity of IPV; the primary predictive variable is the change in severity of ADHD symptoms. The secondary outcome measure is the observation of the therapist about change in the offender's general violent behaviour, within and outside the partner relationship. Data are analysed in a multiple regression model with change in severity of IPV as the dependent variable and change in severity of ADHD symptoms as the primary predictor. Other predictive variables taken into account in the analyses are presence of comorbid psychopathology and personality disorder, subtype of the offender, and dynamics of the domestic violence. In addition, compliance with treatment and content of the treatment are documented. DISCUSSION:Research on the treatment process of IPV offenders and victims is complicated by many factors. This observational design will not allow inferences about causality but may reveal clinically important factors that contribute to more effective treatment of IPV. Background About 50% of patients in substance abuse treatment with a partner perpetrated and/or experienced intimate partner violence in the past year. To date, there are no screeners to identify both perpetrators and victims of partner intimate violence in a substance abusing population. We developed a 4 item screening instrument for this purpose, the Jellinek Inventory for assessing Partner Violence (J-IPV). Important strengths of the J-IPV are that it takes only 2 minutes to administer and is easy to use and to score. Methods To investigate the validity of the J-IPV, two independent studies were conducted including 98 and 99 participants, respectively. Aim of the second study was to cross-validate findings from the first study. Psychometric properties of the J-IPV were determined by calculating sensitivity, specificity, positive and negative predictive value, and positive and negative likelihood ratio’s by comparing J-IPV outcomes to outcomes on the Revised Conflict Tactics Scales (‘gold standard’). Also, receiver operator characteristics (ROC)-curves were determined to weight sensitivity and specificity as a result of different J-IPV cutoffs, and the area under the curve (AUC) was calculated. Results Results of the first study demonstrated that the J-IPV possesses good psychometric properties to detect perpetrators and victims of any as well as severe intimate partner violence. Results from the second study replicated findings from the first study. Conclusions We recommend administering the J-IPV to patients entering substance abuse treatment. If perpetrators and victims of partner violence are identified, action can be taken to stop IPV perpetration and arrange help for victims, for example by offering perpetrators treatment or by providing safety planning or advocacy interventions to victims.
The present study investigated whether (combinations of) specific substance use disorders predicted any and severe perpetration and victimization in males and females entering substance abuse treatment. All patients (N = 1799) were screened for IPV perpetration and victimization; almost one third of the sample committed or experienced any IPV in the past year. For males, an alcohol use disorder in combination with a cannabis and/or cocaine use disorder significantly predicted any IPV (perpetration and/or victimization) as well as severe IPV perpetration. For females, alcohol and cocaine abuse/dependence predicted both any IPV (perpetration and/or victimization) and severe IPV perpetration. Results from the present study emphasize the importance of routinely assessing IPV in patients in substance abuse treatment and demonstrate that clinicians should be particularly alert for IPV in patients with specific substance use disorder combinations.

Angststoornissen komen bij kinderen en jeugdigen vaak voor. Onderzoek heeft herhaaldelijk aangetoond dat cognitieve gedragstherapie bij deze stoornissen een effectieve interventie is, evenals bij andere stoornissen waarbij angst of spanning centraal staat, zoals traumagerelateerde en obsessief-compulsieve stoornissen. Interventies binnen de cognitieve gedragstherapie zijn te verdelen in twee groepen. De eerste is gebaseerd op het principe van klassieke conditionering (leren van nieuwe betekenissen), de tweede op het principe van operante conditionering (leren door de gevolgen van gedrag). In dit hoofdstuk worden eerst overwegingen voor de diagnostische fase besproken. Daarna volgt een beschrijving van beide groepen interventies. De nadruk bij de interventies ligt op exposure in vivo, als belangrijkste en meest effectieve interventie bij angst. Ook komen imaginaire exposure, cognitieve interventies, beloningsstrategieën en responspreventie aan de orde. Het hoofdstuk eindigt met een beschrijving van de behandeling van Anouk, een meisje met ernstige separatieangst en angst voor het donker, en een bespreking van recente ontwikkelingen in de behandeling van angstige kinderen.
This is the first study that compared different types of offenders in forensic outpatient treatment (i.e., offenders of general violence [GV], intimate partner violence [IPV], sex crimes, and “other offenses” such as drug smuggling and property crimes) regarding the prevalence of substance use disorders at the time of the offense. In total, 35.8% of participants ( n = 187) were diagnosed with any substance use disorder. Specifically, 61.5% of GV perpetrators, 30.9% of IPV perpetrators, 9.1% of sex offenders, and 26.7% of “other offenders” were diagnosed with substance abuse or dependence. More GV offenders and less sex offenders fulfilled diagnostic criteria for a substance use disorder. Furthermore, 29.9% of the offenders were intoxicated by substances at the moment they committed the offense (48.5% of GV perpetrators, 25.0% of IPV perpetrators, 17.4% of sex offenders, and 21.0% of other offenders). More GV perpetrators were intoxicated during the offense. As there is a clear association between substance abuse and criminal behavior, substance abuse in offenders should be assessed and, if present, be treated.
Op 8 december 2009 werden op het Richtlijnencongres in de Meervaart in Amsterdam nieuwe multidisciplinaire richtlijnen voor de GGZ gepresenteerd. Een daarvan was de Richtlijn Familiaal Huiselijk Geweld, de eerste richtlijn op dit gebied in Nederland. Hier geven we een korte bespreking van deze richtlijn.
This study investigates the point prevalence of substance use disorders in 150 perpetrators of intimate partner violence (IPV) in a forensic setting and compares participants with and without substance use disorders on demographic and offence-related variables. Furthermore, it investigates the frequency of IPV perpetrated under the influence of substances. Half the sample (50.0%) meets diagnostic criteria for at least one substance-related diagnosis. Significantly more IPV perpetrators without substance use disorders compared with IPV perpetrators with substance use disorders have children living at home and have abused their children. Relative to IPV perpetrators without substance use disorders, significantly more IPV perpetrators with substance-related disorders are found to be under the influence of substances at the time of the offence. Results highlight the importance of understanding the prevalence of substance use disorders in IPV perpetrators in forensic settings.
Deze bijdrage gaat over de antisociale persoonlijkheidsstoornis (APS), een psychische stoornis die zich kenmerkt door antisociaal, niet zelden agressief gedrag tegenover andere mensen en soms ook tegen dieren. Iemand met APS maakt misbruik van andere mensen, bijvoorbeeld door hen -al dan niet met geweld- te bestelen, te mishandelen of te verkrachten. Daar komt bij dat hij over het algemeen geen oog heeft voor de gevoelens van zijn slachtoffers en weinig of geen spijt heeft. Dit alles leidt tot gedrag waarvan vooral de mensen uit zijn omgeving last hebben. Dat kunnen bekenden en zelfs ouders, partners of kinderen zijn, maar vaak ook zijn het mensen die toevallig in de buurt waren: een medewerker van een pompstation of bank die wordt overvallen; een voorbijganger die zich in een vechtpartij mengt en vervolgens zelf de klappen moet opvangen. Samenvatting Het onderzoek betreft het eerste Nederlandse recidiveonderzoek waarin recidivepercentages werden vergeleken binnen subgroepen van volwassen mannelijke zedendelinquenten. Recidive werd gedefinieerd als een nieuwe veroordeling voor het plegen van een seksueel, gewelddadig en ander delict (d.i., vermogen, brandstichting of verkeer). De onderzoeksgroep bestond uit 149 zedendelinquenten die waren geindiceerd voor ambulante forensisch psychiatrische behandeling. De follow-up periode varieerde van 12 tot 88 maanden met een gemiddelde van 32 maanden. Survivalanalyses wezen uit dat van de totale groep 9.5% recidiveerde met een seksueel delict, 3.8% met een geweldsdelict en 4.2% met een vermogensof verkeersdelict. De hoogste recidivepercentages werden zoals verwacht gevonden bij exhibitionisten (waarvan 75% recidiveerde met een hands-on delict), drop-outs en zedendelinquenten bij wie het recidiverisico hoog werd ingeschat. Ondanks de beperkingen van het onderzoek dragen de resultaten bij aan een groter inzicht in de poliklinische behandeling van zedendelinquenten.
Hoewel de effectiviteit van cognitieve gedragstherapie (CGT) bij angstige kinderen reeds in een aantal studies is aangetoond, werden de resultaten ervan niet eerder in de reguliere praktijkinstellingen getoetst. Bovendien was het de vraag of het noodzakelijk is om naast een behandeling van het kind ook intensieve oudertraining te geven. In deze studie onderzochten wij de effectiviteit van 12 sessies CGT bij angstige kinderen + 2 sessies psychoeducatie voor ouders en de mogelijke meerwaarde van een cognitieve oudertraining (COT) van 7 sessies. Aan deze studie namen 79 kinderen en jongeren (7 tot 18 jaar) met angststoornissen deel. Het ging om kinderen die waren verwezen naar zowel reguliere praktijkinstellingen als een universitair centrum. Over het algemeen hadden de kinderen meer baat bij CGT dan bij een wachtlijst. Na CGT (met of zonder COT) was er een aanzienlijke verbetering op zelfgerapporteerde angst en depressie en op ouderrapportages van angst en internaliserend gedrag. Deze verbetering hield stand tot drie en twaalf maanden na behandeling. CGT bleek even effectief in de reguliere pratijkinstellingen als in het universitaire centrum. Tegen de verwachting in had de cognitieve oudertraining geen meerwaarde boven de individuele behandeling van het kind. Concluderend kunnen de meeste angstige kinderen in praktijkinstellingen effectief behandeld worden met 12 sessies individuele CGT en 2 sessies psycho-educatie voor ouders. Intensieve cognitieve oudertraining voegt hier niets aan toe.
Stalkers lijden aan preoccupaties: ze zijn de hele dag in gedachten bezig met het doen en laten van hun slachtoffer. Er is discussie over de vraag of hun preoccupaties moeten worden opgevat als (ego-dystone) dwanggedachten of als positief beleefde (ego-syntone) fantasieën. We bespreken het categoriseren van stalkers op grond van onderliggende psychodynamiek, gedachten en gedrag. Ook komt de psychologische behandeling van stalkers aan de orde. Stalkers kwamen tot voor kort nauwelijks in behandeling, omdat ze vinden dat ze in hun recht staan. Ze zijn moeilijk te motiveren om kritisch naar hun eigen gedrag te kijken, laat staan om het stalken te stoppen. De behandelaar moet veel investeren in een goede relatie en motivering van de cliënt. Aan het einde doen we suggesties voor behandelingsstrategieën die werden ontwikkeld voor vermindering van zorgelijke preoccupaties, maar die tot nu toe in de behandeling van stalkers niet zijn gebruikt, zoals imaginaire exposure, emdr, acceptance and commitment therapy en mindfulness-based cognitive therapy.
