Als interventionelles Verfahren zur Behandlung der benignen Prostatahyperplasie (BPH) wird die Embolisation der Vv. spermaticae (Gat-Goren-Technik) diskutiert.
Die 18F-Fluordeoxyglukose-Positronenemissionstomographie (FDG-PET) gewinnt zunehmenden Stellenwert in der Diagnostik onkologischer Erkrankungen, was zum einen auf der hohen Sensitivität der Methode und zum anderen auf der Möglichkeit einer Ganzkörperuntersuchung beruht. Insbesondere bei der Diagnostik von Kopf-Hals-Karzinomen können sich hierdurch Vorteile ergeben, da ein präzises Primärstaging entscheidender Prognoseparameter ist, woraus letzten Endes das Therapieregime für den Patienten resultiert. Die vorliegende Arbeit stellt die verschiedenen Einsatzmöglichkeiten der Untersuchung mittels Kombination aus PET und Computertomographie (CT) im Rahmen der Diagnostik von Kopf-Hals-Karzinomen dar. Besonderes Augenmerk wird hierbei auf das Primärstaging und die Tumornachsorge gelegt sowie den Stellenwert bei der Diagnostik des CUP-Syndroms („cancer of unknown primary origin“); auch der Einsatz bei der Strahlentherapieplanung sowie neue Aspekte in der PET-Technologie werden beschrieben.
Purpose: To analyze the outcome of patients undergoing percutaneous CT-guided radiofrequency ablation (RFA) of small renal masses (SRM) at a single center during a ten-year time period.Materials and Methods: Patient records of renal RFAs (07/2003 - 11/2013) were reviewed. Indications were SRM suspicious of malignancy on imaging and one of the following: severe comorbidity; old age; solitary kidney; impaired renal function; patient wish. Biopsy was performed at the time of RFA. Patients were excluded if no follow-up was available. Patient and procedural characteristics were recorded. Survival rates were calculated using the Kaplan-Meier's method and compared with log-rank or cox tests.Results: 38 patients (16 females, mean age 70.0 years [range 52-87]) presenting with a solitary SRM were included in the study. Biopsy showed malignancy in 29 patients; 9 had benign tumors. 26 patients suffered from cardiovascular, respiratory or hepatic comorbidities. Technical success (complete ablation on first follow-up) was achieved in 95 % of cases. Two major complications (bowel perforation; hematothorax) occurred. The 3-and 7-year overall survival (OS) [any cause] rates were 73.4 +/- 0.8 % and 50.3 +/- 1.0 %, respectively (mean follow-up 54.6 months, range 1-127). 4 recurrences and 2 metastases were observed. The presence of comorbidities was the only independent predictor of OS. There was no difference in survival between patients with benign and malignant tumors.Conclusion: RFA of SRM is successful in a large percentage of cases with a low complication rate and durable local control. As RFA is typically performed in multimorbid patients, overall survival seems to depend primarily on comorbidities rather than cancer progression.
PURPOSE:To determine effectiveness of lymphatic interventional procedures for treatment of chylothorax. MATERIAL AND METHODS:Analysis of interventions performed from 2001 to 2014. RESULTS:In 21 patients with therapy resistant chylothorax a lymphatic radiological intervention was attempted, which could be performed in 19 cases: 17 thoracic duct embolizations (15 transabdominal, one transzervical and one retrograde transvenous procedure), 2 percutaneous destructions of lymphatic vessels, one CT-guided injection of ethanol next to a duplicated thoracic duct. Fourteen of seventeen (82.3 %) of the technically successful embolizations lead to clinical cure. This encluded three patients with prior unsuccessful surgical thoracic duct ligation. Also the injection of ethanol was clinically effective. Complications were a bile peritonitis requiring operation, and one clinical deterioration of unknown cause. CONCLUSION:Interventional lymphatic procedures allow for effective treatment in many cases of chylothorax, and should be considered early during treatment. (Andrologia 2008) vorgeschlagenen Prinzip soll eine Embolisation der insuffizienten Testikularvenen bei Patienten mit benigner Prostatahyperplasie (BPH) einen erhöhten hydrostatischen Druck im prostatischen Venenplexus senken und eine Über-flutung der Prostata mit androgenreichem Blut aus dem Hoden verhindern, so dass eine Re-gression der BPH resultiert. Dieses neue Therapieprinzip sollte im Rahmen der klinischen Routine eingesetzt und über erste Ergebnisse berichtet werden.
Purpose: To effect regression of benign prostatic hyperplasia (BPH), Gat et al. (Andrologia 2008) proposed to occlude incompetent spermatic veins to reduce increased hydrostatic pressure on the prostatic venous plexus and prevent reflux with androgen rich blood from the testicles. Our aim was to implement this treatment strategy in clinical practice and to report about first results.Methods: Embolization of the spermatic veins was performed in 30 patients with BPH. In 16 patients, we obtained follow-up data from at least 6 months. The sonographic transabdominal prostatic volume, prostate-specific antigen (PSA) and peripheral total testosterone levels were determined before and 6 months after the intervention. Subjective symptomatology was assessed using standardized questionnaires (International Prostate Symptom Score [IPSS] and Quality of Life score [QoL]) before and 6 months after the procedure.Results: The age of all treated patients was 46-77 years. The age of the 16 patients who received follow-up was 51-77 years. IPSS (median 18 [IQR 20.75-14.50] vs. 9 [IQR 11.00-7.25], p<0.0001) and QoL score (4 [IQR 5-3] vs. 2 [IQR 3-1], p<0.001) were significantly decreased 6 months after the intervention. The subjective improvement of symptoms did not correspond with prostatic volumes, which did not change significantly (54.3130.90 vs. 50.5029.26ml, p=n.s.). 4/16 patients had a measurable post-void urine volume, which decreased in two patients 6 months after the procedure, remained unchanged in one patient, and was no longer detectable in one patient. 4 of the 11 had a sonographically detectable varicocele before the intervention, and one patient had a trabeculated bladder. Both the peripheral total testosterone levels (4.551.27 vs. 3.931.00ng/ml; p=n.s.) and PSA levels (3.742.83 vs. 4.063.34ng/ml; p=n.s.) showed no significant differences.Conclusion: Interventional occlusion of the spermatic veins in patients with BPH is a feasible outpatient procedure with a low complication rate. Intermediate results are satisfactory. Bei 4 der 16 behandelten Patienten lie ss sich ein Restharnvolumen nachweisen, welches bei zwei Patienten 6 Monate nach dem Eingriff unverandert, bei einem Patient rucklaufig war, bei einem Patient war kein Restharn mehr nachweisbar. Bei 4/16 Patienten war vor dem Eingriff auch sonografisch eine Varikozele nachweisbar, bei einem Patienten eine Balkenblase. Bei den peripheren Gesamttestosteronwerten (4,55 +/- 1,27 vs. 3,93 +/- 1,00ng/ml; p=n.s.) und den PSA-Werten (3,74 +/- 2,83 vs. 4,06 +/- 3,34ng/ml; p=n.s.) konnten im Verlauf keine signifikanten Unterschiede festgestellt werden.Schlussfolgerung: Die venose Embolisationsbehandlung der BPH ist ein ambulant durchfuhrbarer Eingriff mit geringer Komplikationsrate. Es zeigt sich ein zufriedenstellendes Zwischenergebnis. Mittel- und langerfristige Ergebnisse sowie pathophysiologische Mechanismen des Eingriffs sind weiter zu evaluieren.
Wir berichten über das Outcome von Patienten mit solitären kleinen Nierentumoren (small renal mass, SRM) nach perkutaner CT-gesteuerter Radiofrequenzablation (RFA) an einem Zentrum über einen Zeitraum von 10 Jahren.
SummaryThis study investigated the efficacy of 131iod- ine-labeled lipiodol (1311-lipiodol) as a palliative therapy, evaluated overall survival (OS) across Barcelona Clinic Liver Cancer (BCLC) stages, and determined the main prognostic factors influencing OS in patients with hepatocellular carcinoma (HCC). Patients, methods: We retrospectively analyzed 57 (44 men; mean age, 65.7 years; mean activity per session, 1.6 GBq; mean cumulative activity in patients with >1 sessions, 3.9 GBq) HCC patients who underwent 1311-lipiodol therapy. A majority of patients exhibited Child-Pugh class B (53.6%) disease and a good Eastern Cooperative Oncology Group performance status (0-1; 72%). Multinodular disease was observed in 87.7% patients, bilobar disease in 73%, and portal vein occlusion (PVO) in 54%. Furthermore, 21.1% patients were staged as BCLC B and 59.6 % as BCLC C. All patients were followed until death. Results: The median OS was 6.4 months, which varied significantly with disease stage (median OS for BCLC A, B, C, and D was 29.4, 12.0, 4.6, and 2.7 months, respectively; p = 0.009); Child-Pugh score and class; presence of ascites, PVO, or extrahepatic disease; largest lesion size; favourable treatment response; international normalized ratio, baseline albumin and alpha-fetopro- tein levels. Patients with a Child-Pugh A liver disease had a longer OS. Conclusion: Currently, different treatment modalities for HCC include radioembolization, transarterial chemoemboliz- ation, and systemic therapy with sorafenib; however, 1311-lipiodol therapy remains a feasible alternative for patients without a favourable response to other therapies, particularly for patients with Child-Pugh A liver cirrhosis.
Zusammenfassung Hintergrund und Fragestellung: Evaluation der sonographisch gesteuerten transnodalen Lymphangiographie als modifizierte Punktionstechnik für die Darstellung des abdomino-thorakalen Lymphsystems zur Vorbereitung eines interventionellen Verschlusses des Ductus thoracicus. Patienten und Methodik: Bei 10 Patienten (6 Männer, 4 Frauen, Durchschnittsalter 53 Jahre [22–72 Jahre]) mit Chylothorax nach herz- und thoraxchirurgischen Eingriffen wurde eine sonographisch gesteuerte transnodale Lymphangiographie durchgeführt. Hierzu wurde sonographisch gesteuert ein inguinaler Lymphknoten mittels Feinnadel punktiert und Lipiodol injiziert. Die Kontrastierung des Lymphsystems wurde unter Durchleuchtung kontrolliert. Der D. thoracicus wurde perkutan punktiert und verschlossen. Erfasst wurden die Durchschnittszeiten für die Lymphknotenpunktion bis zum initialen Lymphangiogramm, die Anzahl der Lymphknotenpunktionen, die Menge des injizierten Kontrastmittels, der Zeitraum der Kontrastierung der Zielgefäße, die Anzahl und Dauer der perkutanen Punktionen bis zur Katheterisierung des D. thoracicus sowie die Dauer der Embolisation des D. throacicus und die Anzahl der Komplikationen. Die Prozedurenzeiten bis zum initialen Lymphangiogramm in der Gruppe mit transnodaler Lymphangiographie wurden mit denen einer Kontrollgruppe (n=10) mit pedaler Lymphangiographie verglichen. Ergebnisse: In allen Fällen gelang die sonographisch gesteuerte inguinale Lymphangiographie zur Vorbereitung des perkutanen Verschlusses des D. thoracicus. Für die inguinale Lymphknotenpunktion bis zum initialen Lymphangiogramm wurde eine Durchschnittszeit von 12,3 ± 2 min (pedale Lymphangiographie 30,4 ± 5 min) benötigt. Die durchschnittliche Anzahl der inguinalen Punktionsversuche betrug 1,3 (1–3). Leichte Paravasate traten in 4 von 10 Fällen auf. In 2 Fällen musste bei Auftreten lymphovenöser Shunts die kontralaterale Leistenregion punktiert werde. In allen Fällen konnte das Lymphsystem für die Okklusion des D. thoracicus über eine durchschnittliche Dauer von 24,5 ± 9 min ohne erforderliche Nachinjektion kontrastiert werden. Für die Katheterisierung des D. thoracicus wurden durchschnittlich 1,5 (1–3) Punktionsversuche benötigt. Die Prozedurenzeiten für die Katheterisierung der Zielgefäße und der Embolisation des D. thoracicus betrugen 19,3 ± 9 und 14,5 ± 2 min. In allen Fällen wurde der D. thoracicus erfolgreich verschlossen. Folgerung: Die sonographisch gesteuerte transnodale Lymphangiographie ist eine technisch einfache Methode zur Darstellung des abdomino-thorakalen Lymphsystems vor perkutanen Interventionen am D. thoracicus.
Ziele: Auswertung der Ergebnisse, des Langzeitverlaufs sowie von Einflussfaktoren auf Offenheit und Überleben nach transjugulärem intrahepatischen porto-systemischen Stentshunt (TIPS). Methode: Seit 1992 bis August 2011 wurde bei 532 Pat. ein TIPS durchgeführt. Erfasst wurden demographische Daten, Ätiologie der Zirrhose, Child-Pugh- Einteilung, Indikation, klinisches Ergebnis und technische Daten der TIPS-Anlage und eventueller Re-Eingriffe. Die Nachsorge bestand in klinischen und farbdopplersonographischen Kontrollen und – wenn pathologisch oder inkonklusiv – mittels Venographie und Druckmessung. Im Rahmen dieser Studie wurden die prospektiv erfassten Daten retrospektiv ausgewertet. Ergebnis: Der initiale technische Erfolg betrug 98%, der sekundäre 100%. Die mittlere Beobachtungsdauer dauerte 61±32 Monate. Die gemittelte primäre Offenheitsrate bezogen auf Wallstents lag bei bei 654±91,4 Tagen verglichen mit 1078±109,5 Tagen bei beschichteten Stents. Die sekundäre Durchgängigkeit nach 5 Jahren war für Wallstents und beschichtete Stents mit 95% fast identisch, wenngleich bei Wallstents wesentlich mehr Reinterventionen notwendig waren. Die Offenheitsrate war bei beiden Stentarten abhängig vom MELD-Score. Die mittlere Überlebenszeit betrug 1645±111 Tage (1478±118 Tage für Wallstents, 998±85 Tage für Viatorr-Stents); die Überlebensraten nach einem Jahr waren für Wallstents und beschichtete Stents gleich (61 vs. 60%). Das Überleben war abhängig vom Alter, CHILD- und MELD-Score, bei Wallstents zusätzlich vom Geschlecht; Frauen hatten eine 34% geringere Mortalität. Schlussfolgerung: Die primäre Offenheitsrate wird durch die Implantation von beschichteten Stents deutlich verbessert, die sekundäre Offenheit ist jedoch weitgehend identisch. Das Überleben ist nicht abhängig vom Stent-Typ; Alter, CHILD- und MELD-Score waren die wichtigsten Einflussfaktoren.
Ziele: Evaluation der Radioembolisation mit Yttrium-90 Mikrosphären bei hochpalliativen Patientinnen mit nicht resektablen Lebermetastasen eines Mammakarzinoms. Methode: Bei 18 Patientinnen (Altersmedian 56 Jahre) mit bilobären, nicht resektablen Lebermetastasen eines invasiv duktalen Mammakarzinoms und über 50%-igem Organbefall wurden im Zeitraum zwischen 2007–2011 insgesamt 24 SIRT-Behandlungen mit Y-90 Mikrosphären der Leber durchgeführt. Bei 12 Pat. lag eine extrahepatische Filialisierung vor. Alle Pat. hatten bereits eine Chemo-, Hormon- oder Immuntherapie erhalten. Das postinterventionelle Tumoransprechen wurde mittels MRT, PET-CT und CT bewertet. Für die Auswertung wurden die RECIST-Kriterien zugrunde gelegt. Laborkontrollen, akute/späte Nebenwirkungen und die Überlebensraten wurden erfasst. Ergebnis: Eine Behandlung der Gesamtleber wurde bei 5 Pat. durchgeführt, sequentiell des rechten und nachfolgend des linken Leberlappens bei 6, unilobär des linken Leberlappens bei 4 und unilobär des rechten Leberlappens bei 3 Pat.. Die im Mittel applizierte Dosis für die unilobäre Behandlung betrug 0,7 GBq und 1,2 GBq für die Gesamtleber. Ab dem Zeitpunkt der ersten Behandlung erfolgten 3 Nachkontrollen im Median von 1, 3 und 6 Monaten mit folgenden Ergebnissen: partial response bei 11,1%, 27,8%, 0%, stable disease bei 72,3%, 33,4%, 33,4%, progressive disease bei 16,7%, 0%, 5,6%, complete response bei 0%, 5,6%, 5,6% und Tod bei 0%, 33,4% und 55,6% der Pat.. Bei einer Nachuntersuchungszeit mit einem Median von 22 Monaten waren 14 Pat. verstorben, 4 Pat. befanden sich in einem stable disease. Der Median der Überlebenszeit betrug 5 Monate. Frühe toxische Nebenwirkungen traten in 4, therapieassoziierte Todesfolgen in keinem Fall auf. Schlussfolgerung: Auch bei ausgedehnter, über 50%-iger hepatischer Metastasierung eines Mammakarzinoms stellt die Radioembolisation mit Yttrium-90 Mikrosphären eine effektive Therapieform mit geringem Nebenwirkungsprofil dar.
A man (age: 70 years) with long-lasting thoracic pain underwent contrast enhanced F-FDG PET/CT to rule out spondylitis and infection of the aortic prosthesis after unsuccessful antibiotic therapy for two weeks. He had undergone stent implantation in the thoracic aorta six months earlier because of a dissecting aneurysm. The fused PET/CT images ( Fig. 1b) demonstrated an intense tracer accumulation in the lateral margins of the 6 to 8 thoracic vertebral bodies adjacent to the treated aneurysm without any tracer accumulation in the stent region or the vertebral bodies, ruling out any infection in these regions. CT images ( Fig. 1c) showed corresponding bone erosion at the site of tracer uptake. These findings were interpreted as a chronic granulomatous process due to the long standing pressure of aortic prosthetic accompanied by an ongoing bone remodelling process. This was proven by histopathology. The combined anatomic and metabolic information of F-FDG PET/CT allowed an exact localization of enhanced F-FDG uptake in this patient and changed the therapeutic management. Discussion
Einleitung: Die Anlage eines transjugulären intrahepatischen portosystemischen Stents (TIPS) stellt eine effektive Therapie bei Komplikationen einer portalen Hypertension dar. Es kann allerdings nach TIPS-Anlage zu Stenosen oder Verschlüssen des TIPS kommen, die zu einer Revision führen.
PURPOSE:The aim of this prospective multicenter study was to assess the diagnostic role of CEUS in the diagnosis of newly discovered focal liver lesions in clinical practice. One important aspect is the comparison of CEUS with magnetic resonance imaging (MRI).MATERIALS AND METHODS:From 05 / 2004 to 12 / 2006, standardized CEUS was performed prospectively on 1349 patients with focal liver lesions that had been newly detected by fundamental ultrasound in order to determine tumor differentiation and tumor entity. 269 patients had a standardized MRI after CEUS. In typical liver hemangioma and focal nodular hyperplasia (FNH), the definitive diagnosis was based on the MRI as the "diagnostic gold standard" and on clinical evidence and additional follow-up (subgroup A) or on histology (subgroup B). 262 patients met the diagnostic standard that had been set.RESULTS:In the subcollective (n = 262), the tumor differentiation (malignant or benign) of CEUS and MRI was concordant in 225 cases (85.9%), and the assessment of tumor entity in 204 cases (77.9%). In subgroup A (n = 180), concordant results for tumor differentiation were obtained in 169 (93.2%) and for tumor entity in 160 (88.9%) cases. Liver hemangiomas (n = 122) and FNH (n = 43) were most frequent. Subgroup B (n = 82) comprised mainly malignant liver lesions (n = 55), with only a few of hemangiomas (n = 8) or FNH (n = 5). Tumor differentiation was concordant in 56 (68.3%) and tumor entity in 44 cases (53.7%). There were no statistically proven differences between CEUS and MRI.CONCLUSION:CEUS and MRI are of equal value for the differentiation and specification of newly discovered liver tumors in clinical practice. CEUS and MRI are extremely reliable for the differentiation of benign and malignant lesions, the diagnosis of liver hemangiomas and FNH. The characterization of metastases and HCC is also very reliable.
AIM:The aim of our study was to evaluate the diagnostic value of Contrast-Enhanced Ultrasound (CEUS) for the characterization of focal liver lesions in a prospective multi-center study in clinical practice. For this purpose CEUS was compared with the spiral-CT (SCT), the standard radiological method. MATERIAL AND METHODS:1349 patients with unclear liver lesions after fundamental ultrasound diagnostics including color doppler analysis were examined with standardized CEUS (pulse inversion method, mechanical index < 0.4) from May 2004 to December 2006 in 14 hospitals in a prospective study. The enhancement of contrast medium in the liver tumors was analyzed according to known tumor-specific vascular patterns, using standardized documentation and analysis methods for the differentiation of tumor differentiation (malign or benign) and tumor specification (entity). A subcollective of 267 patients was additionally examined by standardized SCT method. Final diagnosis was based on histology, SCT or MRI in typical findings of liver hemangioma and FNH and on proved clinical data and additional follow up. RESULTS:The subcollective of 267 patients was divided in two subgroups. In 109 of these patients (subgroup A) there was no histological verification, diagnoses based on clear SCT-findings in 79 cases of hemangioma or FNH, as well as in 20 cases with a clear clinical diagnosis. 6 cases (5.5 %) remained unclear. In this subgroup the assessment of tumor differentiation was concordant with CEUS in 90 cases, discordant in 19 cases and the assessment of tumor specification was concordant in 82 and and discordant in 27 cases. In 158 patients (subgroup B) a histological finding was also present, only in 4 cases no definitive tumor diagnosis was achieved. In this subgroup assessment of tumor differentiation with CEUS and SCT was concordant in 124 cases and discordant in 30 cases (CEUS/SCT: sensitivity 94.0 / 90.7 %, specificity 83.0 / 81.5 %, PPV 91.6 / 91.5 %, NPV 87.5 / 80.0 %, accuracy 90.3 / 87.8 %). Tumor specification matched in 103cases and were different in 51 cases (CEUS/SCT: sensitivity 95.3 / 90.6 %, specificity 83.7 / 81.6 %, PPV 92.7 / 91.4 %, NPV 89.1 / 80.0 %, accuracy 91.6 / 87.7 %). A statistically significant difference could not be established. The analysis of particular tumor specification showed a statistically non significant slight advantage in tumor differentiation for CEUS in the case of hemangioma, FNH, HCC and metastases. CONCLUSION:In a multi-center approach under routine clinical conditions, this prospective study demonstrates CEUS to be of equal rank to the CT-scan in regard to the assessment of tumor differentiation and specification. No statistically significant differences could be established. CEUS should be employed before computed tomography is performed for the differentiation of liver tumors, because radiation exposure and invasive biopsies can be avoided in veritable numbers of cases, when precise clinical evaluation of the findings is implemented.
Ziel: Tumorspezifische Vaskularisationsmuster fokaler Leberläsionen in der Kontrastmittelsonografie sollten in ihrer Häufigkeit und diagnostischen Treffsicherheit bei B-Bild-morphologisch unklaren Leberraumforderungen überprüft werden.
Purpose: To evaluate the diagnostic benefit of contrast-enhanced Ultrasound for the differential diagnosis of liver tumors in clinical practice.Materials and Methods: From May 2004 to December 2006 1349 patients (male 677, female 672) with a hepatic tumor lacking a definite diagnosis based on B-mode Ultrasound and power Doppler Ultrasound were examined at 14 hospitals by contrast-enhanced Ultrasound using a standardized protocol (pulse/phase inversion imaging, mechanical index 0.4). The Tumor status was assessed based on the vascularity pattern and contrast enhancement seen in focal lesions during the arterial, portal, and late phase. The diagnosis established after contrast-enhanced ultrasound was compared to histology (>75% cases) or in some cases to CT or MRI.Results: The final diagnosis of hepatic tumors included 573 benign hepatic tumors (hemangiomas n=242, focal nodular hyperplasia n=170, hepatocellular adenoma n=19, other benign lesions n=142) and 755 malignant hepatic tumors (metastases n=383, hepatocellular carcinoma n=279, other malignant lesions n=93). The overall diagnostic accuracy of contrast-enhanced ultrasound in comparison to the correct final diagnosis based on the combined gold standard was 90.3%. Contrast-enhanced ultrasound was able to correctly assess 723/755 malignant lesions (sensitivity 95.8%) and 476/573 benign lesions (specificity 83.1%). The positive predictive value of contrast-enhanced ultrasound for the diagnosis of a malignant tumor was 95.4% and the negative predictive value of contrast-enhanced ultrasound was 95.7%.Conclusion: Contrast-enhanced ultrasound clearly improves the differential diagnosis of hepatic tumors and is very helpful in clinical practice when B-scan or power Doppler morphological criteria are missing (page 16).
