The objective of this study is to summarize the efficacy and safety of traditional Chinese medicine (TCM) enema in the treatment of ulcerative colitis (UC). The randomized controlled trials on TCM enema intervention in the treatment of UC were searched in seven databases: PubMed, Embase, Web of Science, CBM, CNKI, Wanfang Database, and VIP Database from January 1, 2013 to June 6, 2022, and the data were analyzed using RevMan 5.3 software. A total of 18 studies involving 1514 UC patients were included. Meta analysis results showed that compared with conventional Western medicine, Chinese medicine enema had a significant effect on UC, and the clinical effective rate of the experimental group using Chinese medicine enema was 4.45 times that of the control group using conventional Western medicine (odds ratio = 4.45, 95% confidence interval [3.27, 6.06]). Therefore, Chinese medicine enema is effective in the treatment of UC, and can significantly reduce related symptoms.
Acupuncture can reduce blood pressure, heart rate (HR), and ameliorate cardiac damage by modulating the excitability of the sympathetic nervous system, but the exact mechanism of this effect remains unclear. This study investigated the potential mechanisms of acupuncture in the treatment of cardiac damage in hypertension. Spontaneously hypertensive rats (SHR) were used as the hypertension model with Wistar–Kyoto rats as the control. Manual acupuncture, electroacupuncture, and metoprolol were used as interventions. Systolic and diastolic blood pressure (SBP, DBP) plus HR were monitored with cardiac structure determined using Masson staining. Angiotensin II (Ang II) and norepinephrine in myocardium were detected with ELISA as was Ang(1-7) and gamma aminobutyric acid (GABA) in the rostral ventrolateral medulla (RVLM). Expression of mRNA for collagen type I (Col-I), Col-III, actin α1 (ACTA1), and thrombospondin 4 (THBS4) in myocardium was detected using real-time PCR. Expression of angiotensin converting enzyme (ACE), Ang II, angiotensin II type 1 receptor (AT1R), ACE2, and Mas receptor (MasR) proteins in RVLM was monitored using western blot. After manual acupuncture and electroacupuncture treatment, SHRs showed decreased SBP, DBP and HR, reduced myocardial damage. There was decreased expression of the ACE/Ang II/AT1R axis, and increased expression of the ACE2/Ang(1-7)/MasR axis within the RVLM. GABA levels were increased within the RVLM and norepinephrine levels were decreased in myocardial tissue. Metoprolol was more effective than either manual acupuncture or electroacupuncture. Acupuncture directed against hypertensive cardiac damage may be associated with regulation of ACE/Ang II/AT1R and the ACE2/Ang(1-7)/MasR pathway within the RLVM to reduce cardiac sympathetic excitability.
Ulcerative colitis (UC) is a chronic nonspecific intestinal inflammatory disease of unclear etiology, characterized by continuous and diffuse inflammatory changes of colorectal mucosa. In recent years, the incidence rate of UC in China has increased year by year, but there is no effective cure method in Western medicine. Traditional Chinese medicine treatment based on syndrome differentiation has the unique advantages of simplicity, convenience, effectiveness, and low cost. Therefore, integration of Chinese and Western medicine has become an important strategy for UC diagnosis and treatment, and has made significant progress in UC. On the basis of learning from a number of consensuses at home and abroad, combined with Chinese research results and the actual clinical situation of traditional Chinese and Western medicine, the UC group of the Chinese Association of Integrative Medicine has formulated the expert consensus on the diagnosis and treatment of UC with integrated traditional Chinese and Western medicine. In this consensus, the pathogenesis, etiology and pathogenesis, diagnosis, integrated traditional Chinese and Western medicine treatment, and chronic disease management with integrated traditional Chinese and Western medicine for UC were recommended by experts. This article interprets the consensus from the perspective of nursing in order to provide reference basis for clinical nursing workers.
Objective To study the pathway and effect of homogeneity management of medical quality in urban compact medical consortiums. Methods From the perspective of comprehensive resource theory, this paper took Xining First Medical Group as an example and conducted in-depth study on its typical cases. It studied the implementation path of homogeneity management of medical quality in compact medical consortiums, and analyzed its effecting mechanism and significance in the improvement of capacity and efficiency of primary healthcare. Results Taking the urban compact medical consortium as a carrier, with the county medical quality homogeneity as the focus, Xining First Medical Group promoted the subsidence of quality healthcare and resources, and improved the capacity and efficiency of primary healthcare at the county level, realizing the hierarchical and orderly visits. Conclusion The innovation of organization mode and mechanism of urban compact medical association can help the homogeneity management of county medical quality. The homogenization of medical quality in the medical consortium plays an important role in improving the grassroots ability and overall service efficiency.
Traditional Chinese medicine breast massage, also known as breast Tuina or manual expression of breast milk, is a technique of using certain manipulations to act on specific positions or acupoints of the breast to promote blood and lymphatic circulation of breast and facilitate the discharge of stagnant milk. This technique is widely used in China to treat breastfeeding problems such as breast engorgement, mastitis, and postpartum hypogalactia. In this article, we introduce the definition and standardization process of breast massage, elaborate the operating procedures of breast massage in detail, and summarize and discuss the current situation of its clinical application, so as to provide a reference for its wide application and in-depth research.
Cerebral infarction (CI) is also known as ischemic stroke. It is one of the most common cerebrovascular diseases, with high mortality and disability rate, which has seriously threatened human health. To better apply the diagnosis and treatment plan of integrated traditional Chinese and Western medicine to the treatment of patients with CI, the Chinese Association of Integrative Medicine issued the Expert Consensus on the Diagnosis and Treatment of Cerebral Infarction with the Integrated Traditional Chinese and Western Medicine in 2021. It involves etiology and pathogenesis of CI, diagnosis and treatment standards of Traditional Chinese Medicine (TCM) and Western Medicine, syndrome elements of TCM, as well as rehabilitation nursing of CI with integrated traditional Chinese and Western medicine during acute period and recovery period. This study interprets the consensus from routine nursing, dietary nursing, emotional nursing, complication nursing, rehabilitation nursing, and continuous nursing, aiming to provide evidence-based support for clinical TCM nursing practice.
Huatuo Jiuxin Pills (HJP), a traditional Chinese medicine (TCM) preparation, has been widely used to treat Cardiovascular Diseases (CVDs) for more than 20 years. However, there were still gaps in the study of chemical components and potential pharmacological effects in the HJP. In this study, ultra-performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF/MSE) combined with network pharmacology was used to comprehensively explore the chemical components in HJP and explore its potential active compounds and the mechanism for the treatment of CVDs. A total of 117 compounds, mainly including saponins, cholic acids, and bufadienolides, were rapidly identified and characterized. Simultaneously, the fragmentation mode and characteristic ion analysis of different types of representative compounds were carried out. Network pharmacology results showed that the more important active ingredients mainly include 5β‐hydroxybufotalin, 19 oxo‐cinobufagin, bufarenogin, etc. While, the main targets were PIK3CA, MAPK1, VEGFA and so on. Importantly, HJP has therapeutic effects on CVDs by acting on endocrine resistance, PI3K-Akt signaling pathway, HIF-1 signaling pathway, etc. In addition, molecular docking results showed that the core active ingredients with higher degrees in HJP have a strong affinity with the core targets of CVDs. The current work fills the gap in the chemical substance basis of HJP, and also facilitates a better understanding of the effective components, therapeutic targets, and signaling pathways of HJP in the treatment of CVDs.
Identification of critical quality attribute(CQA) is crucial in quality control of Tongren Niuhuang Qingxin Pills(TRNHQXP). In this study, 661 active components in TRNHQXP were selected by liquid chromatography-mass spectrometry(LC-MS) and network pharmacology based on reported data and TCMSP, BATMAN-TCM, and TCMID databases, as well as mass spectrometry data, and 1 413 targets of the active components were obtained through SwissTargetPrediction. The 152 potential targets obtained from the intersection of predicted targets with 456 stroke targets underwent functional enrichment analysis by Metascape. The 27 Chinese medicinals in TRNHQXP were divided into four sets according to efficacies. Thirty-seven key targets in the blood-activating and stasis-resolving set and 41 in the tonifying set were screened out. On the basis of these potential key targets, 137 potential key CQA of TRNHQXP for stroke were reversely predicted. This study revealed the possible mechanism of TRNHQXP in treating stroke and established a modular identification method for the potential CQA of big brand traditional Chinese medicine(TCM) based on efficacies and chemical properties. Consequently, the CQA of TRNHQXP were identified by this method, which has provided a reference for the following experimental studies of CQA.
Objective: The objective of this study is to explore the clinical effects of structured skin care plan of integrated Chinese and Western medicine in intervening elderly patients with incontinence-associated dermatitis (IAD). Materials and Methods: Totally, 66 elderly patients with IAD were randomly divided into the experiment group (32 cases) and control group (34 cases). The control group was given routine nursing care, while the experiment group was given a structured skin care plan. The observational course was 2 weeks. The treatment efficiency and healing time were compared between the two groups. Results: After 2-week intervention, the total effective rate of the experiment group was higher than that of the control group (97.1% vs. 78.1%, X2 = 3.913, P = 0.048). The skin assessment tool score of the experiment group was lower than that of the control group (0.56 ± 1.58 vs. 1.75 ± 2.46, Z = −−2.401, P = 0.016). The healing time of the experiment group was shorter than that of the control group (7.29 ± 4.76 days vs. 10.69 ± 6.36 days, Z = −2.280, P = 0.026). Conclusion: The structured skin care plan of integrated Chinese and Western medicine showed a good effect in elderly IAD patients, and provided a reference for clinical treatment and care of elderly patients with IAD.
Assessment of the status property(boiling time) is a challenge for the quality control of extraction process in pharmaceutical enterprises. In this study, the pilot extraction process of Phellodendron chinense was used as the research carrier to develop an online near-infrared(NIR) quality control method based on the status property(boiling time). First, the NIR spectra of P. chinense were collected during the two pilot-scale extraction processes, and the status property(boiling time) was assessed by observing the state of bubbles in the extraction tank using a transparent window during the extraction process, which was then used as a reference standard. Based on the moving block standard deviation(MBSD) algorithm, the assessment model using online NIR spectra for boiling time during extraction process was established. In addition, the model was optimized as follows: standard normal variable(SNV) for spectral pretreatment, modeling band of 800-2 200 nm, and window size of 4. The results showed that, with 0.002 0 as the MBSD model threshold, the boiling time can be accurately assessed using online NIR spectra during extraction process. Furthermore, the principal component analysis-moving block standard deviation(PCA-MBSD) model was developed by our group to reduce the influence of online NIR spectral noise and background signal on the model, and the number of principal components was optimized into 2 in the PCA-MBSD model. The results showed that, with 0.000 075 as the PCA-MBSD model threshold, the boiling time can be accurately assessed using online NIR spectra during extraction process, with improved reliability. This study can provide a assessment method for boiling time during extraction process using online NIR spectra, which can replace the empirical judgment in manual observation, and realize the digitalization of the extraction process for big brand traditional Chinese medicine.
本文以医学本科生参与高压氧医学专业科研项目的具体实施过程为案例,学生通过实际参与科研项目的文献检索、项目选题、项目申请、项目实施、论文撰写等环节,理论和实践相结合,使其科研思维和科研技能,尤其是科研论文撰写能力得到了明显提高.通过本项目的实施,定期汇报文献、建立导师制、因材施教、撰写论文等方法是提升学生科研能力行之有效的方法,同时还能激发学生的科研兴趣,培养学生的科研思维,有助于增加学生的创新意识和核心竞争力,对高素质医学人才的培养具有重要的意义.
The physical properties of powder and granules are the critical quality attributes for the process control of Suhuang Zhike Capsules, a big brand traditional Chinese medicine. This paper took the production of 25 batches of real-world Suhuang Zhike Capsules dry extract powder and granules intermediates as the research object. Firstly, a method for testing the physical properties of Suhuang Zhike Capsules powder and granules with 19 physical indicators was established. The results showed that the granules of dry extract powder after granulation had a smaller particle size, wider particle size distribution range and poor fluidity, which easily caused the problem of over-limit capsule loading. Secondly, correlation analysis, principal component analysis and cluster analysis were used for mathematical statistics. The correlation analysis showed that the density of dry extract powder could affect the chroma and fluidity. At the same time, the particle size in the granules had a stronger effect on the chroma and fluidity than the density. The study also found that the particle size and hygroscopicity of dry extract powder were potentially key physical properties that affected the physical properties of granules. Furthermore, the results of principal component analysis and cluster analysis showed that the consistency of the physical properties between the dry extract powder and intermediate granules was relatively poor. To this end, similarity analysis was carried out, and the quality control method of powder and granules based on physical fingerprint was established. The results showed that the physical fingerprint similarity of 25 batches of dry extract powder was 0.639-0.976, and the physical fingerprint similarity of the gra-nules was 0.716-0.983. With the similarity of 0.85 as the threshold, the batches with abnormal physical properties could be identified. In this study, the process quality control method of Suhuang Zhike Capsules based on the physical properties of powder and granules was established finally, which realized the identification of abnormal batches, and provided a reference for the process quality control of Suhuang Zhike Capsules.
Objective: The objective of this study is to establish a nursing standard of integrated traditional Chinese and Western medicine for patients with COVID-19 (mild and common) in Beijing, to provide reference for clinical nursing of patients with COVID-19 (mild and common).Methods: Through online communication meeting with nurses who are in the frontline of anti-epidemic, clinical investigation, literature research, and expert demonstration meeting are carried out to prepare the draft of the standard, and the Delphi method is applied to determine the standard of integrated traditional Chinese and Western medicine care for patients with COVID-19 (mild and common) in Beijing.Results: The nursing standard of integrated traditional Chinese and Western medicine for patients with COVID-19 (mild and common) was established, which included 5 first-level indicators, 14 second-level indicators and 60 third-level indicators. After two rounds of Delphi method, the positive coefficients of experts were 96% and 83%, the authoritative coefficients of experts were 0.89 and 0.91, and the Kendall's coefficient of concordance (W) of experts were 0.12, 0.09, 0.10, 0.13 (P < 0.05) and 0.44, 0.43, 0.37, 0.39 (P < 0.05).Conclusion: The standard of integrated traditional Chinese and Western medicine nursing for patients with COVID-19 (mild and common) in Beijing constructed by the Delphi method is scientific and practical, which provides a reference for clinical application of integrated traditional Chinese and Western medicine nursing to fight against COVID-19 infection.
人体是以五脏为中心的有机整体,而五脏的合和则以脾胃为中心,脾胃共居中州,执中央以灌四傍,为后天气血之本.调脾胃为后天之本,五脏之源,化生气血,滋养百骸,脾主四时,为气机升降之枢的作用.重视脾胃调理,除了能治愈脾胃不适等相关病症外,对其他脏腑疾病兼具疗养作用.只有脾胃健运,气机升降有序,受纳适度,燥湿相宜,气血津液化源充足,脏腑才能得以安养.因此"调脾胃以安五脏"为治疗内伤杂病之大法.本文对王京奇主任医师"调脾胃以安五脏"的论治内伤杂病的经验作以论述,同时从脾胃为五脏之源,脾主四时,脾胃为气机之枢,脾胃化生营卫和滋养五官九窍等方面论述"调脾胃以安五脏"的学术思想内涵,并结合临床实际经典病例进行佐证,以期为内伤杂病的临床治疗提供指导意义.
目的 通过网络药理学方法研究新型冠状病毒肺炎(COVID-19)中期诊疗方(XGZQZLF)治疗COVID-19的作用机制.方法 采用网络药理学方法,以中药系统药理学数据库、GeneCards数据库等为支撑,通过Cytoscape构建“药物-活性成分-靶点-疾病”网络,进一步进行基因富集分析和通路注释,研究XGZQZLF治疗COVID-19的物质基础和作用机制.结果 筛选出XGZQZLF有效成分80个,通过调控蛋白激酶(AKT1)、白细胞介素6(IL-6)、丝裂原活化蛋白激酶3(MAPK3)、血管内皮生长因子A (VEGFA)、半胱氨酸蛋白酶3(CASP3)等208个肺炎相关靶点,以及主要通过调控TNF信号通路、IL-17信号通路、Th17细胞分化、缺氧诱导因子1(HIF-1)信号通路等炎症和免疫相关通路发挥治疗COVID-19的作用.结论 XGZQZLF通过多成分、多靶点、多通路协同作用发挥治疗COVID-19的作用,为XGZQZLF治疗COVID-19的生物机制研究提供数据支撑,同时为COVID-19药物研发提供参考.
OBJECTIVE: Clinical practice recommendations issued by the American Diabetes Association in 2019 include the health status and quality of life of people with diabetes as part of their daily care , and believe that it’s important to strengthen physical exercise for diabetic patients. Previous studies have shown that the Baduanjin, a traditional Chinese sport, can regulate blood glucose and blood lipids, weight loss and improve immunity.This study was aimed at investigating the effects and safety of Modified Baduanjin on patients with type 2 diabetes. METHODS: Forty patients were divided into the Modified Baduanjin group (Α group, n=22) and the control group (Β group, n=18) for 12 weeks according to their individual motivation. On the basis of conventional hypoglycemic treatment, Group Α practiced Baduanjin for 30 minutes per day, 4 times per week, while no exercise intervention was given in group Β. The main study outcomes included changes in fasting blood glucose, blood lipids, glycosylated hemoglobin,Quality of Life score( QoL score, 100 in total), muscle endurance, and flexibility after 12 weeks. RESULTS: There were no significant differences in patient characteristics between the two groups at baseline. Group Α in glycosylated hemoglobin (pre 6.56±0.70 vs post 6.28±0.70) , QoL score (pre 80.18±9.02 vs post 86.64±9.91) , muscle endurance (pre 3.76±4.38 vs post 6.91±5.73) and flexibility (pre 2.86±9.69 vs post 5.88±9.75) . All the above results were statistically significant (P<0.05).There were no statistical significance in group Β (P>0.05) . 2) Few changes were found in fasting blood glucose and lipid parameters both in two groups(P>0.05). 3) All patients completed the exercise programme with no adverse effects. CONCLUSION: Chinese traditional exercise is effective and safe in regulating and control the level of blood glucose, enhancing physical fitness and improving the quality of life.
OBJECTIVE: Insomnia is the most common diseases. Long-term insomnia can seriously affect normal life , even lead to a serious accident. Ba Duan Jin is a traditional Chinese exercise,which can regulate meridians qi -blood-body fluid,and improve the quality of sleep. This article comprehensively evaluates the effect of Ba Duan Jin on insomnia by meta-analysis. METHODS: Searching Wanfang platform, VIP consulting platform , CNKI, Pubmed and Springer on Ba Duanjin in the treatment of insomnia Randomized controlled trial (RCT) . Meta-analysis was performed using revman5.3 software. The main indicators were the number of recovered patients ,effective patients and invalid patients, the Pittsburgh sleep quality score (PSQI). RESULTS: A total of 98 articles were retrieved by searching in computer . Excluding the literature which is repeated or with incomplete date , the final including was 9 RCT.A total of 789 patients were included in the analysis. Meta-analysis showed that the number of patients with recovery was higher in the experimental group than in the control group. [OR=3.14, 95% CI (2.18, 4.52), p<0.00001], and the number of invalid patients was lower. [OR=0.19 , 95% CI (0, 12, 0.31), p < 0.00001], the difference was statistically significant. There was no statistical difference between the effective groups. PSQI were not statistically different due to excessive heterogeneity. CONCLUSION: This meta-analysis showed that in the treatment of insomnia, the clinical efficacy of the group with Ba Duan Jin intervented was better than the group with the conventional drug, and the difference was statistically significant.
推荐医师制度是《中医药法》规定的中医医术确有专长人员考核制下的全新制度设计,是考核报名要求中重要的一项.为贯彻落实推荐医师制度,国家卫健委、中医药管理局和各省级中医药主管部门相继发布了《暂行办法》和《实施细则》,使推荐医师在考核报名阶段发挥了很好的初步遴选作用.但由于是首次提出推荐医师的报名要求,在推荐医师的资质要求、推荐医师的权利义务、推荐程序、法律责任等方面仍存在一些问题.就这些问题进行分析论证,并提出相应的对策建议.
目的:探究血府逐瘀汤联合四联疗法治疗幽门螺旋杆菌(Hp)阳性慢性萎缩性胃炎的效果.方法:回顾性分析2014年5月-2017年12月在我院进行诊治的80例Hp阳性慢性萎缩性胃炎患者的临床资料,按照其入院顺序经随机数字表分为研究组和对照组,每组各40例患者.其中,对照组患者采用四联疗法,研究组患者在对照组基础上联合血府逐瘀汤进行治疗,对比两组患者的Hp转阴率、治疗前后胃黏膜病理积分的变化情况、临床症状积分的变化情况和不良反应发生率.结果:治疗后,研究组患者的Hp转阴率[85.0%(34/40)]显著高于对照组[62.5%(25/40)](P< 0.05).两组上腹痛、纳差、上腹胀、反酸、嗳气等临床症状积分以及胃黏膜萎缩、肠化、不典型增生等病理积分均显著低于治疗前(P<0.05),且研究组以上指标均明显低于对照组(P<0.05).研究组患者的不良反应发生率[5.0%(2/40)]与对照组[12.5%(5/40)]无显著性差异(P>0.05).结论:血府逐瘀汤联合四联疗法治疗Hp阳性慢性萎缩性胃炎的效果显著优于单用四联疗法,其可有效改善患者的病变程度和临床症状,且无明显不良反应产生.
