Femoral shaft fractures mostly result from high-energy trauma and young adult males are typically affected. The standard treatment is antegrade reamed interlocking medullary nailing. Retrograde nailing and plate fixation are other treatment options. In polytraumatized patients, primary management according to the guidelines of damage control surgery includes initial fracture fixation with an external fixator before scheduled early secondary definitive fixation is performed. Particularly in complex fractures, restoration of the length and rotation can represent some challenges for the surgeon. This article provides tips and tricks for the effective management of femoral shaft fractures.
Femurschaftfrakturen entstehen meistens im Rahmen von Hochrasanztraumen und betreffen überwiegend jüngere Patienten. Standardtherapie ist die aufgebohrte antegrade Marknagelosteosynthese, wobei auch retrograde Nagelung oder Plattenosteosynthese möglich sind. Bei polytraumatisierten Patienten erfolgt zuerst im Rahmen von Damage Control Surgery die Stabilisierung mit Fixateur externe, bevor die endgültige Versorgung durchgeführt wird. Die operative Therapie einer Femurschaftfraktur stellt den Operateur vor einige Herausforderungen. Vor allem bei Trümmerfrakturen kann die Einstellung der richtigen Länge und Rotation starke Schwierigkeiten bereiten. Der Beitrag vermittelt Tipps und Tricks für die operative Behandlung der Femurschaftfraktur.
Proximal humerus fractures are among the most common fractures. Intramedullary nailing is a valuable treatment option for displaced fractures. The treatment aim is anatomic reduction and stable fixation allowing an early functional after treatment. In this study, we describe the technique of third-generation nailing, characterized by: straight nail, multiple angular stable proximal fixation options (aimed at the area of optimal bone quality), calcar screw, multiplanar (angular stable) distal fixation. Of the 35 fractures ( 7, two-fragment fractures; 16, three-fragment fractures; and 12, four-fragment fractures), we observed 2 losses of reduction, 3 screw penetrations, and 1 calcar screw back out. In 4 patients, this led to a second operation (1 reversed shoulder prosthesis, 3 removals of 1 screw). We conclude that third-generation is a valuable option in the treatment of 2-, 3-, and 4-part fractures of the proximal humerus.
BackgroundWhole-body computed tomography (WBCT) plays an important role in the management of severely injured patients. We evaluated the radiation exposure of WBCT scans using different positioning boards and arm positions.MethodsIn this retrospective study, the radiation exposure of WBCT using a 16-slice multislice computed tomography scanner was evaluated. Individual effective doses (E, mSV) was calculated. Patients were assigned to two groups according to placement on a plastic transfer mat (PTM, group 1) or on the Trauma Transfer™-Board (TTB, group 2). Data were collected for each group with arm placement on the abdomen (a) or in raising position (b), respectively. The maximum ventro-dorsal diameter [VDD] at the trunk was measured.Results100 patients with potentially life-threatening injuries were analysed. Patient demographics and VDD did not differ in the two groups. Radiation exposure in term of E did not reveal any significant differences between the two positioning boards using same arm position [group 1a (n=26) vs. 2a (n=24) (mSV): 16.7±4.7 vs. 17.1±4.4, group 1b (n=26) vs. 2b (n=24) (mSV): 13.1±3.9 vs. 14.3±1.5]. The arm raising positioning showed a significant reduction in E in comparison to the placement on abdomen position [group 1b vs. 1a (mSV): 13.1±3.9 vs. 16.7±4.7, p<0.05, group 2b vs. 2a (mSV): 14.3±1.5 vs. 17.1±4.4, p<0.05].ConclusionsPatient arm positioning for WBCT has an important influence on radiation exposure. Effective dose was 16–22% lower when arms were raised. An individual placement algorithm may lead to a relevant reduction of radiation exposure of severely injured patients.
Ziele: Die Multidetektor-Computertomographie (MDCT) hat einen zentralen Stellenwert in der Schockraumdiagnostik des Schwerverletzten, ist aber mit einer nicht unerheblichen Strahlenbelastung assoziiert. Ziel der vorliegenden Untersuchung war es, diese Strahlenbelastung durch den Einsatz verschiedener Lagerungshilfen und unterschiedlichen Lagerungstechniken zu reduzieren. Methode: In der retrospektiven Untersuchung wurde die mit einem 16-Zeilen-MDCT assoziierte Strahlendosis [DLP (mGy*cm)] des Thorax-Abdomenscans (geräteseitige Angaben im Dosisprotokoll) und der maximale ventro-dorsale Querdurchmesser [VDD (cm)] am Körperstamm erfasst. Es wurden zwei Patientengruppen unterschieden: (1) Lagerung auf flexibler Plastikumlagerungshilfe, (2) Lagerung auf Trauma TransferTM–Board (TTB, MedicalSCA, Wien, Österreich). Jeweils Gruppe 1 und 2 mit Lagerung der Arme auf dem Körperstamm (a) vs. Arme über Kopf (b). Ergebnis: Der VDD zeigte in allen Gruppen keinen signifikanten Unterschied [VDD (cm): 1a: 26,7±3,7 vs. 1b: 25,8±4,7 vs. 2a: 25,0±2,8 vs. 2b: 24,4±3,1, p=n.s.]. Die Verwendung des TTB führte nicht zu einer erhöhten Strahlenbelastung im Vergleich zu einer flexiblen Plastikumlagerungshilfe [DLP (mGy*cm): 1a: 1082,1±279,1 vs. 2a: 1087,5±258,3, p=n.s.; 1b: 846,5±250,8 vs. 2b: 891,5±171,6, p=n.s.]. Durch die Lagerung der Arme über Kopf des Patienten konnte im Vergleich zur Lagerung der Arme auf dem Körperstamm unter Berücksichtigung der gleichen Lagerungshilfe eine Dosisreduktion von 20% erreicht werden [DLP (mGy*cm): 1a: 1082±279,1 vs. 1b: 846,5±250,9, p<0,01; DLP (mGy*cm) 2a: 1087,5±258,3 vs. 2b: 891,5±171,6, p<0,01]. Schlussfolgerung: Als Lagerungsalgorithmus werden zur Reduktion der Strahlenbelastung spontanatmende, kreislaufstabile Patienten mit lagerungsfähiger oberer Extremität ("Arme hoch") von spontanatmenden, kreislaufstabilen Patienten mit Verletzung der oberen Extremität bzw. intubiert/beatmeten Patienten ("Arme auf Körperstamm") unterschieden.

Introduction: Radial Head Arthroplasty is considered the treatment of choice for unreconstructable radial head fractures. Short-term results in the current literatue are promising. Due to the lack of long-term results, radial head arthroplasty is looked at critically by many surgeons. In our the study we provide the 8.4 years results after treatment with the floating radial head prosthesis by Judet (Tornier, France). Methods: In our department 19 patients were treated with bipolar radial head arthroplasty between 1997 and 2001. 11 prostheses were implantated primary and 6 secondary. The other two were implanted because of a loosening of a prior implanted prosthesis and one after resection of a vast chondrosarcoma. 12 of these patients − 10 men and 2 women – were now examined retrospectively after 101 months (78–132). Results: 6 Patients were treated primary, 5 secondary and one was treated because of a vast chondrosarcoma. There were 5 proximal ulna fractures and 8 processus coronoideus fractures as concomitant injuries of the elbow. Following complications were seen: 2 dislocations, 2 capitellar erosions and 4 cases of heterotopic ossifications. According to the Mayo Elbow Performance Score 4 patients achieved an excellent result, 7 a good, and one a satisfactory result. The mean DASH was 13.7 (0–44). No differences were seen between primary and secondary implantation. The flexion arc was 123° (110–140°), the extension deficit was 20° (0–40°), pronation 61° (30–90°) and supination 62° (40–90°). Conclusion: Our 8.4 years results show that radial head arthroplasty with Judet’s bipolar prosthesis leads to mostly excellent and good – subjective as well as objective – results.

To determine whether there are differences in stability between double and triple interlocked intramedullary nails used for the fixation of extraarticular proximal tibial fractures.
Tecnica quirurgica Se talla un colgajo cutaneo triangular de avance palmar (Tranquilli-Leali, Atasoy) o un colgajo bilateral (Geissendorfer, Kutler), se desplaza en sentido distal cubriendo el defecto y se fija con una sutura en Y sin tension. Con la tecnica de Tranquilli-Leali se obtiene un colgajo neurovascular de avance, mientras que, con la tecnica de Atasoy, la intervencion microquirurgica adicional de los dos haces neurovasculares permite obtener un colgajo en isla pediculado con un avance maximo de unos 10 mm en sentido distal.
Die pathophysiologische Bedeutung starker Schmerzen mit der stressbedingten Folge kritischer Kreislaufbelastungen ist im Bereich der perioperativen Medizin gut dokumentiert. Aber auch unabhängig von chirurgischen Eingriffen im engeren Sinne können sowohl relativ einfache, kurz andauernde Maßnahmen (z. B. Entfernung einer Thoraxdrainage, Verbandswechsel, Reposition einer Fraktur/Luxation) als auch länger andauernde diagnostische Interventionen (z. B. MRT) belastend sein. Für die letztgenannten Prozeduren ist eine begleitende Medikation gelegentlich bereits etabliert, bei kurz andauernden Interventionen jedoch weniger verbreitet. Spezielle Herausforderungen ergeben sich durch die Tatsache, dass im Rahmen der prozeduralen Analgesie dynamische, d. h. sehr wechselhafte Schmerzintensitäten abgefangen werden müssen. Darüber hinaus ist die prozedurale Analgesie eine fachübergreifende Maßnahme und keinesfalls der Anästhesiologie vorbehalten, zumal o. g. Eingriffe oft ohne Beteiligung der Anästhesiologie durchgeführt werden. Gleichwohl müssen bei der Auswahl der Medikamente sowie hinsichtlich der Überwachung während des Eingriffs Mindeststandards erfüllt werden, um die Patienten nicht zu gefährden. Vor allem aber sind an die Qualifikation des Personals entsprechende Anforderungen zu stellen, wobei der Sicherung der Atemwege eine herausragende Stellung zukommt (Stichwort: Facharztstandard).
Introduction Segmental fractures of the tibial shaft (AO type 42-C2) often occur after a high energy direct trauma with consecutive severe soft tissue injury and a high rate of open fractures. The blood supply of the intermediate bone fragment can be severely disturbed and therefore operative treatment is demanding. In this retrospective study, we compared three different methods of stabilisation. Materials and methods We reviewed the clinical charts and postoperative problems of 26 patients in a ten year period. Sixteen (62%) fractures were stabilised with an intramedullary nail, five (19%) fractures by plate osteosynthesis and five (19%) fractures with an external fixator. Results Bony consolidation was obtained in all patients after an average time of 11.4 months. In 17 patients (65.4%) a second operative procedure had to be performed. Pseudarthrosis was seen in 11 fractures (40%), three times in the proximal and eight times in the distal fracture. In the intramedullary nailing group, pseudarthrosis occurred in nine patients. In the group treated with an external fixator, pseudarthrosis was seen three times. After changing to a different implant bony consolidation was achieved in every patient. Conclusion Conventional intramedullary nailing is not suitable for stabilisation of segmental fracture types with a short metaphyseal fracture fragment. New nails with proximal and distal interlocking in three different planes offer better stability. Bone vascularisation should not be endangered by the stabilisation procedure and optimal reduction of the fracture is an important prerequisite for uneventful fracture healing, especially in this difficult fracture configuration
