Las comunicaciones buconasosinusales son comunicaciones patológicas entre la cavidad oral, el seno maxilar y/o las fosas nasales. La gran mayoría se producen después de la extracción de un diente maxilar. Las otras causas principales son la cirugía de resección de tumores maxilares, la enucleación de quistes maxilares y la cirugía implantológica y preimplantológica. Pueden provocar una sinusitis unilateral crónica que es discapacitante para el paciente. Esta entidad requiere un tratamiento medicoquirúrgico especializado. Las pruebas de imagen ocupan un lugar importante en el diagnóstico, la planificación de la cirugía y también en la prevención de la aparición de comunicaciones buconasosinusales. Existen diferentes técnicas quirúrgicas, que dependen de la localización y del tamaño del orificio.
Mandibular reconstruction remains a real challenge for the maxillofacial surgeon. Reconstruction techniques have evolved considerably and currently the gold standard is the fibula free flap (LLF).In sub-Saharan Africa, this intervention is rarely practiced due to lack of human resources (few specialist practitioners practicing this type of intervention) and technical resources (equipment, under-medicalization of the population, difficult and expensive access to care).Furthermore, in low- and middle-income countries, certain maxillofacial pathologies are specific, such as giant mandibular ameloblastomas, benign tumors characterized by an often impressive volume responsible for facial deformities and functional impotence, in patients with poor oral hygiene.Surgical management of such tumors requires mandibular reconstruction to meet functional and aesthetic requirements.It is in this context that collaborative missions between the maxillofacial surgery services of the Pitié-Salpêtrière University Hospital Center (CHU) (Sorbonne University, Paris, France) and the Aristide le Dantec University Hospital (Dakar, Senegal) have been initiated since 2016. The objective of these missions is to allow patients from Senegal and the West African sub-region to benefit from mandibular reconstructions by LLF in Dakar (Senegal).
Le comunicazioni oro-naso-sinusali sono delle comunicazioni patologiche tra il cavo orale, il seno mascellare e/o la cavità nasale. La stragrande maggioranza di esse si verifica in seguito all’avulsione di un dente mascellare. Le altre cause principali sono l’intervento chirurgico per asportare tumori mascellari, l’enucleazione di cisti mascellari e la chirurgia implantare e preimpianto. Possono causare una sinusite cronica unilaterale invalidante per il paziente. Questa entità richiede un trattamento medico e chirurgico specializzato. Gli esami di imaging svolgono un ruolo importante nella diagnosi, nella pianificazione dell’intervento chirurgico ma anche nel prevenire l’insorgenza di comunicazioni oro-naso-sinusali. Esistono diverse tecniche chirurgiche che dipendono dalla posizione e dalle dimensioni dell’orifizio.
L’ histoire de la chirurgie orthognathique commence dès le milieu du xix e siècle. Son évolution s’appuie sur les bases de la chirurgie traumatologique et le développement des connaissances anatomiques. H. Obwegeser et W. Bell, l’un pour la chirurgie mandibulaire, l’autre pour la chirurgie maxillaire, ont été les deux personnages majeurs de cette chirurgie. L’ école française de chirurgie maxillo-faciale s’est essentiellement distinguée dans le domaine de l’ostéosynthèse, de la distraction ostéogénique et de la planification numérique. Cette histoire raconte surtout la place essentielle de la collaboration entre chirurgien et orthodontiste mise en œuvre dès la fin du xix e siècle par V. Blair et E. Angle.
This study aimed to evaluate the long-term stability of surgical maxillary expansion using patient-specific fixation implants (PSFIs) without intraoral retention. Fifteen patients who had undergone segmented Le Fort I osteotomy and PSFIs with available preoperative (t0) early (t1) and 1-year follow-up computed tomography (CT) scans (t2) were evaluated. The early and 1-year 3D models were superimposed to transfer the bony landmarks; the distances between each pair of landmarks at the different time points were then measured. The distances between the canines and second molars were also measured directly on the CT scans. The achieved maxillary expansions ranged from a median of 4.39 (2.00–6.27) mm at the greater palatine foramina to a median of 2.14 (1.56–2 > 83) mm at the canine level of the palatal bone. One year postoperatively, the changes in skeletal diameters ranged from a median of − 0.53 (− 1.65 to 0.41) mm at the greater palatine foramina (p = 0.12) to 0.17 (− 0.09 to 0.32) mm at the canine level of the palatal bone (p = 0.56). Changes in dental arch diameters ranged from a median of − 0.6 (− 2 to 0.00) mm between the second molars to − 1.3 (− 1.8 to − 0.25) mm between the canines (P < 0.05). This study showed the stability of maxillary expansion osteotomy using PSFIs, even without postoperative intraoral retention. PSFIs are a reliable method for the surgical treatment of transverse maxillary discrepancy. PFSIs are easy-to-use and improve surgical accuracy.
PurposeTo noninvasively assess spectroscopic and metabolic profiles of healthy tongue tissue and in an exploratory objective in nontreated and treated patients with tongue squamous cell carcinoma (SCC).MethodsFourteen healthy subjects (HSs), one patient with nontreated tongue SCC (NT-SCC), and two patients with treated tongue SCC (T-SCC) underwent MRI and single-voxel proton magnetic resonance spectroscopy (1H-MRS) evaluations (3 and 1.5T). Multi-echo-times 1H-MRS was performed at the medial superior part (MSP) and the anterior inferior part (AIP) of the tongue in HS, while 1H-MRS voxel was placed at the most aggressive part of the tumor for patients with tongue SCC. 1H-MRS data analysis yielded spectroscopic metabolite ratios quantified to total creatine.ResultsIn HS, compared to MSP and AIP, 1H-MRS spectra revealed higher levels of creatine, a more prominent and well-identified trimethylamine-choline (TMA-Cho) peak. However, larger prominent lipid peaks were better differentiated in the tongue MSP. Compared to HS, patients with NT-SCC exhibited very high levels of lipids and relatively higher values of TMA-Cho peak. Interestingly, patients with T-SCC showed almost nonproliferation activity. However, high lipids levels were measured, although they were relatively lower than lipids levels measured in patients with NT-SCC.ConclusionThe present study demonstrated the potential use of in-vivo1H-MRS to noninvasively assess spectroscopic and metabolic profiles of the healthy tongue tissue in a spatial location-dependent manner. Preliminary results revealed differences between HS and patients with tongue NT-SCC as well as tongue T-SCC, which should be confirmed with more patients. 1H-MRS could be included, in the future, in the arsenal of tools for treatment response evaluation and noninvasive monitoring of patients with tongue SCC.
El dolor facial asociado a la masticación es un motivo frecuente de consulta en odontología, en medicina general y en cirugía oral y maxilofacial. El acto de comer es una función constituida por varias acciones, comenzando por la toma de los alimentos con los labios, para finalizar por la deglución tras la insalivación y la masticación. Los síntomas se manifiestan de múltiples formas y dependen de la localización y del origen del trastorno. Las etiologías son diversas y las principales corresponden a cánceres, causas dentales y disfunciones del aparato masticador. Este dolor agudo o crónico suele tener una repercusión importante sobre la calidad de vida del paciente. Como suele suceder, la exploración física es determinante en el estudio histológico. En este artículo se presentan las características esenciales de cada entidad clínica que puede causar dolor asociado a la masticación. El tratamiento de estos cuadros de dolor a menudo es complejo y suele requerir un tratamiento pluridisciplinario.
Introduction : Les kystes et tumeurs des maxillaires représentent une multitude d’entités pour lesquelles le diagnostic est parfois difficile. L’examen anatomopathologique permet dans la majorité des cas de donner un diagnostic de certitude mais il est parfois nécessaire de confronter ces résultats à l’imagerie voir de réaliser des marquages d’immunohistochimie pour mieux caractériser la lésion. Le diagnostic différentiel apparaît comme primordial devant certaines entités dont l’évolution guide une prise en charge très différente. Cas clinique : Il s’agissait d’une jeune patiente de 25 ans sans antécédent médical, adressée par son chirurgien-dentiste à la suite de la découverte fortuite d’une lésion osseuse au niveau de l’angle mandibulaire droit. Une exploration radiographique de type CBCT ainsi qu’une biopsie osseuse ont été réalisées. La confrontation de l’imagerie et du diagnostic histologique ne permettait pas de dissocier deux diagnostics possibles, la lésion pouvant correspondre à une tumeur à cellules géantes (TCG) ou à un kyste osseux anévrismal (KOA). Devant cette incertitude, un marquage par immunohistochimie de la protéine p63 ainsi qu’une imagerie par résonance magnétique ont été demandés. Un marquage négatif à la p63 ainsi que l’analyse de l’ensemble des images radiologiques nous a permis de poser le diagnostic de kyste anévrismal. Devant le caractère asymptomatique de la lésion et de l’absence d’évolution à 9 mois, une attitude de surveillance a été décidée. Discussion : Les deux entités que sont le KOA et la TCG sont des lésions dont le diagnostic différentiel est complexe mais impératif du fait de leurs évolutions très différentes. Le KOA correspond à une dystrophie osseuse bénigne qui forme une lésion cavitaire constituée d’un réseau riche en fibroblastes et cellules géantes plurinucléés parfois bordées par un endothélium. Cette lésion est bénigne d’évolution lente. Le traitement consiste le plus souvent en un curetage de la cavité kystique lorsque la lésion entraîne déformation et/ou symptomatologie. La TCG présente de nombreuses caractéristiques histologiques en commun avec le kyste osseux anévrismal, avec un nombre de cellules géantes plus important. Toutefois la TCG présente un risque de récidive de l’ordre de 50% ainsi qu’un risque de transformation maligne (sarcome) de 10 à 20 %. Son évolution peut également être rapide. Le traitement doit être radical et consiste en une exérèse chirurgicale avec marges. Il a été montré par plusieurs auteurs que le marquage de p63, une protéine nucléaire de la famille du gène suppresseur p53, retrouvée dans différents tissus permettait de distinguer ces deux entités dans plusieurs localisations. Conclusion : Le marquage de la protéine p63 peut apparaît donc très utile dans le diagnostic différentiel de ces lésions dont les pronostics sont très différents lorsque l’imagerie et le marquage en HES ne sont pas suffisants.
Introduction: Caring for acute odontogenic cellulitis involves drainage, treatment of the tooth and the administration of antibiotic therapy (ANSM 2011). The emergence of bacterial resistance mechanisms has led to formulating actions to promote better use of antibiotics, but France stay one of the largest consumers in Europe. Objectives: Evaluate the impact of ANSM's recommendations on dental surgeons in France for treatment of this affection. Methods: We performed a national survey with 12365 practitioners who received the questionnaire by email. Results: On 690 responder practitioners, 13% followed the recommendations to the letter and 70.5% performed a clinical intervention on the day of emergency. Only 1/3 of cases involved the prescription of amoxicillin alone for first line treatment. Conclusion: Few data exist on the evaluation of the impact of recommendations regarding this subject, but it is generally accepted it they are seldom followed. The reasons expressed are many: disagreement between practitioners on the efficiency of recommendations, lack of time and organizational constraints.
Aim: To present a technical note on intra-sinusal bone ring concomitant with Le Fort 1 osteotomy. Material and method: A 57-year-old man was referred to our Department for full-mouth rehabilitation. Oral examination identified: uncompensated multiple tooth loss and a class 3 skeletal malocclusion. The treatment plan consisted in a Le Fort 1 osteotomy and short-arch dental implant rehabilitation. Intra-sinusal bone ring technique associated with Le Fort 1 osteotomy were carried out under general anaesthesia. Result: High primary retention was clinically observed of both the implant and the bone graft. Radiographic follow-up demonstrated satisfactory healing of the graft and implant osseointegration. Conclusion: Bone ring technique concomitant with Le Fort 1 osteotomy seems to be appropriate to correct jaw discrepancy associated to a single tooth loss of the upper molar region with residual bone height of at least 3 mm to ensure implant primary stability. Introduction: La prise en charge des collections circonscrites aiguës suppurées d’origine dentaire (CCASD) implique un drainage, le traitement de la dent causale ainsi que la mise en place d’une antibiothérapie dont le choix repose actuellement sur les recommandations de l’ANSM (recommandations AFSSAPS, 2011). En première intention, elles préconisent la prescription d’amoxicilline (2g/jours) ou clindamycine (1200g/jour) en cas d’allergie; en seconde intention, une bithérapie associant classiquement l’amoxicilline à l’acide clavulanique ou au métronidazole. L’émergence de mécanismes de résistances bactériennes a conduit à la mise en place d’actions favorisant une meilleure utilisation des antibiotiques. La France s’inscrit parmi les plus gros consommateurs en Europe (ANSM 2017): une surconsommation globale, une utilisation trop fréquente de molécules à large spectre en pratique courante. Dans ce contexte, l’ANSM a émis des recommandations sur la prescription des antibiotiques pour la prise en charge des CCASD. Sachant que 90% des prescriptions sont faites en ville dont 8% par les chirurgiens-dentistes (CD), il est paru nécessaire de connaître leur attitude. Nous avons donc souhaité évaluer l’impact des recommandations de l’ANSM chez les CD en France, dans la prise en charge des CCASD. Matériel et méthode: Nous avons réalisé une enquête nationale, en collaboration avec l’UFSBD (Union Francaise pour la Santé Bucco-Dentaire), regroupant 15000 praticiens, ayant recu le questionnaire par mail. Il comprenait 28 questions sur quatre thèmes principaux: prise en charge des CCASD, connaissance des recommandations, leur application, formations professionnelles initiales et continues. Résultats: Sur l’ensemble des praticiens, 690 (4,6%) ont participé à l’étude. 13% suivent de manière stricte les recommandations et 70,5% réalisent un acte clinique le jour de l’urgence : il correspond à 98% à un drainage (par voie muqueuse ou ouverture de la dent). Dans seulement 1/3 des cas la prescription de première intention se portait sur l’amoxicilline seule. 44% y associent des anti-inflammatoires. 68% des praticiens reconnaissent connaître les recommandations de l’ANSM par leur formation initiale (36%), une revue professionnelle (27%) ou un congrès (19%). La plupart suit une formation continue (77% une revue professionnelle, 90% un congrès, 38% un cursus de formation continue théorique). 89% prescrivent des anti-inflammatoires lors de la consultation d’urgence. Conclusion: Il existe peu de données évaluant l’impact des recommandations concernant ce sujet, mais il est communément admis qu’elles sont peu suivies (Gaillat et al 2005), ce que révèle également notre étude. Les raisons invoquées sont multiples : désaccord des praticiens sur l’efficacité des recommandations, manque de temps, contraintes organisationnelles. Bien que la bithérapie soit considérée comme plus efficace par les CD interrogés pour traiter les CCASD, les experts s’accordent à dire qu’elle participe à la création de la résistance bactérienne, considérée comme ≪ l’une des plus graves menaces pesant sur la santé mondiale ≫, et faisant l’objet d’un plan d’action mondial mis en place par l’OMS et l’ONU en 2015 (OMS, 2017).
Head and neck squamous cell carcinomas (HNSCC) are the seventh most frequent cancers. Among HNSCCs, oral squamous cell carcinomas (OSCCs) include several anatomical locations of the oral cavity, but exclude the oropharynx. The known risk factors for OSCCs are mainly alcohol consumption and tobacco use for at least 75-80% of cases. In addition to these risk factors, Human papillomavirus (HPV) types 16 and 18, classified as high-risk (HR) HPV genotypes, are considered as risk factors for oropharyngeal cancers, but their role in the development of OSCC remains unclear. We tested the hypothesis of viral etiology in a series of 68 well-characterized OSCCs and 14 potentially malignant disorders (PMD) in non-smoking, non-drinking (NSND) patients using broad-range, sensitive molecular methodologies. Deep-sequencing of the transcriptome did not reveal any vertebrate virus sequences other than HPV transcripts, detected in only one case. In contrast, HPV DNA was detected in 41.2% (28/68) and 35.7% (5/14) of OSCC and PMD cases, respectively. Importantly, 90.9% (30/33) of these belonged to the Betapapillomavirus genus, but no viral transcripts were detected. Finally, high-throughput sequencing revealed reads corresponding to transcripts of the Trichomonas vaginalis virus (TVV), which were confirmed by RT-PCR in two OSCCs. Our results strongly suggest that Alphapapillomavirus genotypes classified as HR are not involved in the development of OSCCs in NSND patients and that known oncogenic infectious agents are absent in these specific OSCCs. Any possible direct or indirect role of Betapapillomavirus genus members and TVV in OSCCs remains speculative and requires further investigation.
Temporo-mandibular disorders -TMD- involve a whole set of anatomical, physiological and clinical disorders that exceed by far the sole temporo-mandibular joint. The main symptom of this condition, most frequent in female patients, is pain. Three syndromes are associated with TMD: a muscular syndrome, not only involving masticatory muscles but also muscles of the neck and pain in seemingly unrelated anatomical regions, which can be misleading; an inflammatory articular syndrome; a mechanical syndrome affecting mouth-opening path. The main treatment of this very common and benign disorder is based on physical therapy. However, physicians should be warned that some patients might exhibit similar symptoms in rare cases of malignant tumor.
INTRODUCTION:The purpose of this study was to compare two structural cephalometric analysis (Delaire and Sassouni) as surgical decision tools. These two analyses do not include dental compensation for skeletal discrepancies and have been proven reliable especially for the Sendai surgery first protocol developed by Sugawara. MATERIAL AND METHOD:The two analysis were performed on 20 surgical cases and the proposed skeletal modifications were compared in order to measure the differences and see if those differences varied according to the facial type. For 10 cases the Virtual Surgical Planning (VSP) images obtained from Dolphin 11.95 Premium software (based on the presurgical CBCT and facial photographs) allowed a comparison of the two analyses as surgical decision indicators and to the realised surgical procedure. The VSP photographs obtained were graded by two panels: one professional and the other one non professional. RESULTS:Sassouni's analysis favored more forward movements for the maxilla (ENA +5,45) as well as more vertical modifications (Me 1,595). The facial type did not appear to have a significant effect. The quality of Dolphin VSP prediction was judged rather reliable above average (3.7/6) by the panels. The VSP photographs obtained from the two analyses were presented to the panels as well as the VSP obtained from the actual surgery. The three facial outcomes: Sassouni, Delaire and "Surgery Realised" called Dolphin showed some potentially interesting differences. The small size of the sample did not show any significant findings (t test) but a special analysis revealed a valid difference between the three outcomes. The most aesthetic being Dolphin (surgery realised) 17.4 then Sassouni 16.6 (diff 0.8) and finally Delaire 14 (diff 2.6). CONCLUSIONS:The two structural analysis Delaire and Sassouni did not give the same surgical objectives. Sassouni favored more forward movements for the maxilla as well as more vertical modifications. The Dolphin software appeared to be the quite reliable software to predict the facial outcomes of the surgery. The VSP photographs obtained using Sassouni's analysis were more balanced than when the Delaire analysis was followed (16.4/14 = +2.6). The VSP based on the actual surgery was judged to be even more aesthetic. This shows that beyond a cephalometric analysis the facial aesthetic details and the functional analysis are very important during the phase of surgical planning.
Introduction: Neoadjuvant chemotherapy (neo-CT) for osteosarcomas is the standard of care. Management of maxillo-facial osteosarcomas (MFOS) is challenging. In this rare disease, we collected a large cohort of patients with the aim to report the histological and radiological local response rates to neo-CT. Patients and Methods: All consecutive adult patients treated between 2001 and 2016 in two French sarcoma referral centers (Pitie-Salpetriere Hospital, APHP, RESAP France and Gustave Roussy Institute France), for a histologically proved MFOS were included. Clinical, histological and radiological data were independently reviewed. Tumor response to neo-CT was assessed clinically, radiologically with independent review using RECIST v1.1 criterion and pathologically (percentage of necrosis). Multivariate analysis was done for outcomes, tumor response and disease-free survival (DFS). Results: A total of 35 high grade MFOS were collected. The clinical tumor response was 4% (1/24 receiving neo-CT), the radiological response was 0% (0/18 with available data) and the pathological response was 5% (1/20 with available data). Three patients (12.5%) initially resectable became unresectable due to clinical and radiological progression during neo-CT. Tumor size and R0 (clear margins) surgical resections were significantly associated with DFS. Conclusion: MFOS is a rare disease. This large retrospective cohort of MFOS indicates the lack of benefit and potentially deleterious effects of neo-CT. We suggest privileging primary surgery in initially localized resectable MFOS. The benefit of adjuvant chemotherapy should be prospectively studied.
Introduction : Cet article présente une étude des analyses squelettiques structurales de Delaire et de Sassouni, indépendantes des compensations dentaires, comme prescripteurs des déplacements squelettiques en chirurgie orthognathique particulièrement dans les chirurgies de première intention (Surgery First) développées par Sugawara. Méthode : Les analyses céphalométriques ont été réalisées sur vingt cas chirurgicaux : les déplacements squelettiques induits ont été comparés pour quantifier les différences d’indications en fonction du type facial. Les morpho-simulations faciales obtenues avec le logiciel Dolphin 11.95 Premium à partir des photographies et scans préchirurgicaux ont été présentées à deux jurys (professionnel et non professionnel) pour valider l’outil Dolphin, puis comparer les analyses entre elles et à la chirurgie réalisée. Cette étude sur dix cas opérés dans le même cabinet a fait l’objet d’une étude statistique (laboratoire de statistique-CNRS). Résultats : L’analyse de Sassouni suggère beaucoup plus de déplacement vers l’avant du maxillaire (ENA +5,45*) et plus de modifications verticales (Me 1,595*). Les différences entre les deux analyses sont peu influencées par le type facial. La prédiction Dolphin jugée assez fiable (3,7/6) a permis de comparer l’effet esthétique des prescriptions des analyses avec la chirurgie réalisée. Le test de Student apparié n’est pas significatif (taille de l’échantillon), mais l’analyse par cotation montre une différence plus significative (chirurgie DO (17,4), Sassouni (16,6) et Delaire (14)). Conclusions : Les analyses de Delaire et Sassouni ne donnent pas les mêmes indications chirurgicales : plus d’avancée maxillaire et de modifications verticales pour Sassouni. L’outil Dolphin permet une prédiction raisonnable des effets d’une chirurgie. L’analyse de Sassouni donne des objectifs céphalométriques jugés plus esthétiques que ceux de Delaire (16,6/14 soit +2,6), mais l’analyse esthétique et fonctionnelle reste essentielle.
Since November, 2018, France has been facing violent contestation with the national so-called yellow vests protests, resulting in about 4000 casualties.1 We managed 21 patients who presented with face and eye injuries caused by rubber bullets from non-lethal hand-held weapons (NLHHWs). Because of the steady increase in the overall number and violence of protests worldwide,2 one might expect a rise in injuries caused by NLHHWs.
