We conducted a comprehensive analysis of spectrum of neuroimaging findings in Subacute Sclerosing Panencephalitis (SSPE) across case reports and cohort studies. Major depressive disorder (MDD) is a heterogeneous, debilitating disorder. A distinct subgroup within the MDD spectrum is identified to have treatment-resistant depression (TRD). There is an impetus to find alternative treatments for TRD to reduce disease burden and improve quality of life. Psychedelics have been used in the treatment of various psychiatric disorders since the 1950s, with potential benefits in TRD. Aiming to summarize the evidence of using psychedelics for TRD by exploring mechanisms and potential benefits, we conducted a literature search on different psychedelics with their mechanisms from clinical and preclinical evidence. Psychedelics have been examined in many preclinical studies for efficacy and mechanisms with fewer clinical studies on patients with TRD. Besides serotonergic agonism, glutamate surge and monoamine release, psychedelics promote neuroplasticity, corticolimbic function, and epigenetic changes. Psilocybin, ketamine, and esketamine are the most researched agents of psychedelics in the context of TRD. Psilocybin-assisted therapy has been shown to induce short-term improvement in symptoms that can persist for weeks and months, with some meta-analyses consolidating such findings. Ketamine, as an atypical psychedelic, in many clinical trials of intravenous, subcutaneous, and oral forms, had rapid and robust effects in reducing depressive symptoms and relapses without decreasing cognitive function. Similarly, esketamine induced early and clinically meaningful improvements in function and productivity. Ayahuasca also showed fast and sustained effects with higher remission rates and good safety. Psychedelics have significant potential in TRD with superior mechanisms over traditional antidepressants. Despite the encouraging findings of the existing studies, large, well-designed studies are needed to extend their use as part of standard recommendations.
Background: Hypertension, commonly known as high blood pressure, a major risk factor for cardiovascular diseases, stroke, and frequently coexists with ckd which is dangerous because it is both a consequence and a cause of ckd. The eighth joint national committee (jnc) guidelines mention lifestyle modifications as the first step in management of hypertension alongside pharmacotherapy. Introduction: Yoga is a combination of three main parts dhyan, asanas and pranayam which reduces blood pressure, heart rate, respiratory rate, oxidative stress, psychological stress. Global health authorities, including the world health organization (who) and the american heart association (aha), recommend nonpharmacological approaches as first-line therapy, especially for individuals with prehypertension or stage 1 hypertension. Aim: To reduce onset of hypertension in chronic kidney disease patients through non pharmacological approach (yogic intervention and lifestyle modification). Objective: To assess the changes in mabp in ckd patients after 3 months of yogic intervention. Methods: An interventional case control study, place - department of physiology in collaboration with medicine (nephrology) department, at era's lucknow medical college and hospital lucknow, u.p, india after taking permission from institutional ethical committee. Patients were divided into two group, case group (who were on medication and with yoga practice for 3 months) and control group (who were only on medication without yoga practice). Results: After completion of three months duration of structured yoga program, significant change in sbp, dbp and mabp has been observed. Conclusion: Yoga had a significant effect on bp control in hypertensive patients and reduced the overall need for antihypertensive medications
The proliferating trichilemmal cysts (PTCs) are rare, typically benign neoplasms arising from hair follicles. The clinical details, computed tomography findings, and histopathological evaluation were carried out to reveal the statistics of PTC patients in a tertiary healthcare center of North India.
Invasive fungal infection (IFI) simultaneously due to three fungi is rare. IFI caused by two fungal agents is relatively commoner in transplant patients. Most common IFI worldwide is candidiasis, whereas in India, aspergillosis and mucormycosis infections are commoner. Severe dengue viral infection has been linked with invasive fungal infections. Following case report exemplifies more than two fungi can together cause pneumonia as confirmed by similar findings in 3 respiratory samples from the dengue shock syndrome patient. High clinical suspicion and prompt initiation of aggressive, high-dose, guideline-recommended antifungal therapy is essential at the slightest doubt of mucormycotic pneumonia.
