口腔住院医师规范化培训(简称住培)是口腔医学人才储备、建设的基础,口腔急诊教学是住培的重要组成部分,保证口腔急诊的住培效果和质量是医院人才培训工作的重要内容,而良好的师资队伍是住培基地顺利完成口腔急诊住培的关键因素.口腔急诊带教师资培训缺乏规范、指导老师带教缺少热情、教学认识存在偏差、奖惩措施尚不完善是当前口腔急诊住培过程中师资队伍建设存在的主要问题,通过严把带教师资准入条件、提高带教师资职业素养、促进教学团队整体提升、建立奖罚分明具体措施、调整医教工作合理比例、发挥住院医师反馈作用,对提升师资队伍水平、改善口腔急诊住培效果具有重要作用.只有不断提升口腔急诊带教师资素养,方能切实培训出有岗位胜任力的口腔住院医师.
目的 探究Er:YAG激光照射时间对牙本质小管封闭效果的影响.方法 将160个2mm厚牙本质片用0.5mmol/L EDTA去除玷污层以建立牙本质过敏症模型,然后将样本等分为16个组,应用聚焦式照射手具和散焦式照射手具按照2s×1次、2s×2次、2s×3次、2s×4次、4s×1次、6s×1次、8s×1次的照射时间照射牙本质片.照射后用扫描电镜(SEM)观察所有样本的牙本质小管形态,应用Image-Pro Plus 6.0图像分析软件计算牙本质小管的暴露面积和封闭率.结果 电镜扫描可见,对照组的牙本质小管形态完整、均匀开放,Er:YAG激光照射不同时间后牙本质小管口表层均有均匀致密的牙本质熔融层形成.图像分析结果 可见,Er:YAG激光照射后各组牙本质小管的暴露面积均明显小于对照组(P<0.05),牙本质小管封闭率均明显增加(P<0.05).但不同激光手具和照射时间实验组的牙本质小管暴露面积和牙本质小管封闭率比较,差异无统计学意义(P>0.05).结论 适宜的Er:YAG激光照射时间有效封闭牙本质小管后,增加照射时间不会进一步提高牙本质封闭效果,为确保操作的安全性及缩短临床就诊时间,建议结合临床实际减少激光脱敏照射时间.
During the period of orthodontic tooth movement, periodontal ligament stem cells (PDLSCs) play an important role in transducing mechanical stimulation and tissue remodeling. However, our previous studies verified that the periodontitis microenvironment causes damage to the biological functions of PDLSCs and abnormal mechanical sensitivity. Long noncoding RNAs (lncRNAs) participate in the inflammatory pathogenesis and development of many diseases. Whether lncRNAs are abnormally expressed in PDLSCs obtained from periodontal tissues of periodontitis patients (PPDLSCs) and whether putative lncRNAs participate in the mechanotransductive process in PDLSCs remain poorly understood. First, we subjected PDLSCs obtained from healthy periodontal tissues (HPDLSCs) and PPDLSCs to static mechanical strain (SMS) with 12% elongation at 0.1 Hz frequency using an FX-4000T system and screened overall lncRNA profiles in both cell types by microarray. Among lncRNAs with a fold change (FC) > 20.0, 27 lncRNAs were upregulated in strained HPDLSCs, and 16 lncRNAs (9 upregulated and 7 downregulated) were detected in strained PPDLSCs. For mRNAs with FC > 20.0, we detected 25 upregulated mRNAs and one downregulated mRNA in strained HPDLSCs and 7 upregulated and 5 downregulated mRNAs in strained PPDLSCs. Further enrichment analysis showed that, unlike HPDLSCs with annotations principally involving transduction-associated signaling pathways, dysregulated mRNAs in PPDLSCs are mainly responsible for pathological conditions. Moreover, coexpressed lncRNA-mRNA networks confirmed the pathological state and exacerbated inflammatory conditions in strained PPDLSCs. Taken together, when compared with strained HPDLSCs, various lncRNAs and mRNAs were dysregulated in PPDLSCs under mechanical forces, implicating the response of lncRNAs in PPDLSCs to mechanical stress. Moreover, we provide potential lncRNA targets, which may contribute to future intervention strategies for orthodontic treatment in periodontitis patients.
口腔专业卫生士官是近年来部队新生的一支卫勤保障力量.目前,我校是全军唯一一所培养口腔专业卫生士官专业人才的院校.依据口腔医学专业人才培养方案,学员在校前两年学习理论知识,第三年在医院临床实习.按照大纲要求,临床实习期间,学员掌握口腔常见疾病、急症的诊疗技术,口腔预防保健知识以及卫生装备的日常维护保养等.本文从实习医院科室的角度,针对口腔专业卫生士官培养要求及岗位能力需求,对学员在临床实习阶段的培养策略展开讨论,以期更好地完善口腔专业卫生士官的培养.
乳牙早失后,其天然间隙保持作用丧失,有可能影响继承恒牙的萌出,甚至导致错颌畸形的发生.所以,乳牙早失后的间隙管理至关重要. 本文对1例双侧上颌乳磨牙早失后间隙保持失败的病例进行分析,以期对间隙保持器的应用和监管给予更多的关注.
目的 对婴幼儿牙龈炎患病情况和用药效果分析,从而找到有效地治疗方法.方法 收集我院2010年1月至2013年1月就诊的0.5~3岁共690名婴幼儿就诊资料,牙龈炎病例数为120例,对这些病例的年龄、牙位和用药情况分别进行分析并做统计学处理.结果 婴幼儿牙龈炎在儿童口腔科就诊中的发病率为17.39%,患儿中男孩略多于女孩,但无统计学意义;以2~3岁年龄组患病人数为最多;患病牙位中以上前牙最为常见.治疗以局部结合全身用药效果理想.结论 婴幼儿牙龈炎发病率较高,治疗以局部和全身用药效果好.
目的:探讨儿童上颌恒前牙萌出异常的原因及早期干预效果。方法对150例上颌前牙萌出异常患儿作 X 线摄片检查及病因分类,其中上颌多生牙29例,牙龈增厚致恒牙迟萌48例,上唇系带过短致恒前牙间隙过大15例和乳牙疾病导致萌出异常58例。全部患儿均立即对症处置。随访1年,观察牙齿发育情况,并判定治疗有效率。结果上颌多生牙组处置有效率为96.55%;牙龈增厚组处置有效率为97.92%;上唇系带过短组处置有效率为86.67%;乳牙疾病组处置有效率为94.83%。儿童上颌恒前牙萌出异常与多种原因相关,150例患儿中,乳牙疾病组患者最多,占38.67%(58/150),其次是局部牙龈增厚组患儿占32.00%(48/150)。150例患儿处置总有效率为95.33%(143/150)。结论上前牙齿萌出异常一定是有原因的,早期发现和对症治疗对纠正和缓解错牙合畸形有重要意义。
Objective:To observe the clinical therapeutic effects of root-end filling material mineral trioxide aggregate(MTA) in the treatment of radicular cysts.Methods:30 patiants suffered with radicular cyst in the anterior upper jaw were enrolled in this retrospective study.The diseased teeth were treated with root canal therapy(RCT),and filled with gutta percha points.Later,3 mm of the diseased tooth root apex was cut.Enucleation of the cyst membrane.Finally,MTA as used to retrofill the diseased tooth root canal and cyst cavity.Follow-up period was 3 years.Results:Each of the cases had achieved great success.Conclusion:This study indicated that the usage of MTA in radicular cyst can obtain better effects which is a simple way without any side reactions.
Objective: To observe the clinical effect of which using high frequency electrotome for implanting rehabilitation in patients. Methods: 20 patients who imbed 22 implangts were treated with electrode, gingival shapes were reconstructed during operation. All the patients consistent with the indication of o-ral implantation. They were examined one day, one week, two weeks and four weeks after operation re-spectively. Results: All patients were successfull with satisfied gingival shapes exept 1 patient got pe-riodontitis, but it was controled after corrected treatment. Conclusion: High frequency electrotome can perform a high cutting together with coagulation and satisfied gingival configuration on control during the operation. It is a simple, practical, little hemorrhage, better effect and save time method.
<正>妊娠晚期羊水量少于300ml者称为羊水过少,发生率为0.4%~4%。羊水过少严重影响围生儿预后,应高度重视。近年来,随着围生医学的发展和超声诊断技术的不断提高,羊水过少的检出率已明显增高。及时发现和处理,对降低胎儿窒息率,降低剖宫产率,提高阴道分娩率有着重要意义。我科自2009年以来,针对300余例羊水过少孕妇进行产前及产时一系列治疗、监护及指导,保障了孕产妇及围生儿安全,现报道如下。

<正>替牙期年轻的恒前牙外伤后出现牙松动、移位、脱位,进行早期复位、固定是恢复正常咀嚼功能、保持牙列完整的有效方法。由于替牙期乳牙松动、脱落,恒牙萌出不全的特点,为牙齿的固定带来困难。近年来我科将2×4矫治技术应用在替牙期外伤的固定,取得良好疗效,现报道如下。

目的了解高中毕业生牙周健康状况和影响因素。方法参照第2次全国口腔健康流行病学调查标准,调查高中毕业生牙面软垢、牙结石及牙龈炎状况。结果 4434名高中毕业生中牙面软垢检出率86.02%;牙结石检出率84.35%;牙龈炎检出率81.28%。牙周病症检出率男生明显高于女生,乡村生明显高于城镇生,住校生明显高于走读生。结论高中毕业生口腔健康状况不佳,需要提高学校、家长及学生对口腔疾病的认识,加大预防和早治疗的力度。
目的利用CT图像重建秦岭野生大熊猫左上颌第一磨牙三维有限元模型,探讨大熊猫牙齿的三维重建方法,为大熊猫牙齿的研究探索新的途径,降低大熊猫牙齿在研究中的实体损耗与需求。方法以牙CT扫描图像结合Mimics图像生成和编辑处理软件,建立秦岭野生大熊猫左上颌第一磨牙三维有限元模型。结果在计算机上建立了秦岭野生大熊猫左上颌第一磨牙三维有限元模型。结论所建模型形态结构相对完整,具有较高的真实性与精确度,可满足大熊猫牙齿研究的需要。
口腔修复时,医生会遇到各种原因(如外伤、烫伤、肿瘤术后及肿瘤放疗)引起张口受限的患者。尤其是多个牙的缺失,需要取口内印模时,一般托盘难以取得精确的印模,给修复带来困难.我们采用组合托血法获得满意效果.
Objective To evaluate the effectiveness of urokinase and microwave therapy for secretory otitis media.Methods Sixty patients with secretory otitis media were treated with urokinase and microwave therapy.Results The most patients had a sitisfactary effectiveness.There was no complication in any patients.Conclusion The urokinase and microwave therapy was safe and effective for secretory otitis media.
大熊猫(Ailuropoda melanoleuca)是国家一级保护动物,被誉为国宝.牙齿作为大熊猫消化器官之一,相对于其他动物而言,大熊猫牙齿有其自身的构造特点.以往学者和专家对大熊猫牙齿形态的研究侧重于为大熊猫的种群分类寻找根据(Dvadi,1869; Milne-Edwards,1870;王将克,1974),更多关注牙齿在大熊猫进化、演变过程中的意义(王令红等,1982;黄万波,1993).研究的齿位大多局限于臼齿(张鹤宇和刘理,1959),所得结果或数据并不详细和全面.此外,还有一些学者对大熊猫牙齿釉质的超微组织结构进行研究(赵资奎等,1984 ).
