目的 探讨血清肿瘤标志物联合血常规检测在原发性肝癌诊断中的应用价值.方法 选取2019年1月至2020年12月间西安医学院第二附属医院收治的72例原发性肝癌患者为研究组,同期选取72例慢性乙肝患者为对照组,检测两组患者及研究组中不同肿瘤直径患者的血清肿瘤标志物与血常规指标,血清肿瘤标志物和血常规指标单独及联合检测的效果比较.结果 研究组患者中性粒细胞淋巴细胞比值(NLR)、白细胞、糖类抗原19-9(CA19-9)及甲胎蛋白(AFP)水平均较对照组高,差异均有统计学意义(均P<0.05).研究组中,肿瘤直径≥5cm患者的NLR、白细胞、CA19-9及AFP水平均高于<5cm者,差异均有统计学意义(均P<0.05).研究组患者血清肿瘤标志物+血常规指标检测阳性率97.2%(70例),血清肿瘤标志物检测阳性率73.6%(53例),血常规指标检测阳性率84.7%(61例).血清肿瘤标志物和血常规指标联合检测的敏感度、特异度、阳性预测值及阴性预测值均高于二者单独检测(均P<0.05).结论 原发性肝癌患者的血清肿瘤标志物与血常规指标均较高,血清肿瘤标志物与血常规联合检测的阳性率、敏感度、特异度、阳性预测值及阴性预测值均较高,值得临床研究与借鉴.
目的 分析白细胞介素(IL)-17、IL-35、补体C3、C4在慢性乙型肝炎(CHB)患者外周血中的表达及与肝纤维化程度的关系.方法 选取西安医学院第二附属医院2019年1月至2020年2月收治的110例CHB患者作为观察组,100例体检健康者作为对照组;收集血清检测血中IL-17、IL-35、补体C3和补体C4水平;采用Logistic回归模型分析IL-17、IL-35、补体C3、补体C4水平与患者肝纤维化程度的关系.结果 观察组血中IL-17、IL-35水平明显高于对照组(P<0.05),观察组血中补体C3及补体C4水平明显低于对照组(P<0.05);不同肝纤维化程度患者血中IL-17、IL-35、补体C3、补体C4水平比较,差异均有统计学意义(P<0.05),其中F0期患者血中IL-17、IL-35水平低于F1~F2期和F3~F4期患者(P<0.05),F1~F2期患者血中IL-17、IL-35水平低于F3~F4期患者(P<0.05),F0期患者血中补体C3、补体C4水平高于F1~F2期和F3~F4期患者(P<0.05),F1~F2期患者血中补体C3、补体C4水平高于F3~F4期患者(P<0.05);血中高IL-17及IL-35水平是CHB患者肝纤维化的独立性危险因素(P<0.05),血中高补体C3及补体C4水平是CHB患者纤维化的独立性保护因素(P<0.05).结论 CHB患者外周血中IL-17、IL-35水平明显升高,补体C3、补体C4水平明显降低,患者血中IL-17、IL-35、补体C3及C4补体是CHB患者肝纤维化的独立性影响因素.
目的 探讨血清肿瘤标志物联合粪便检验在结直肠癌中的诊断价值.方法 采用2019年1月至2020年12月间西安医学院第二附属医院收治的72例结直肠癌患者作为研究组,同期选取72例健康体检者作为对照组.分析两组研究对象的粪便检测结果、血清肿瘤标志物水平,研究组中Ⅰ期+Ⅱ期与Ⅲ期+Ⅳ期的血清肿瘤标志物水平,单项指标检测与联合检测的敏感度、特异度、阳性预测值及阴性预测值.结果 与对照组相比,研究组的粪便检测结果、血清肿瘤标志物水平均较高,差异均有统计学意义(均P<0.05).研究组血便检出率88.0%,粘液便检出率42.7%,痢疾样脓血便检出率20.0%;对照组血便检出率6.7%,粘液便检出率4.0%,痢疾样脓血便检出率0.0%.研究组中Ⅰ期+Ⅱ期与Ⅲ期+Ⅳ期相比,血清肿瘤标志物水平较低,差异均有统计学意义(均P<0.05).联合指标检测与单项指标检测相比,敏感度、特异度、阳性预测值及阴性预测值均较高,差异均有统计学意义(均P<0.05).结论 粪便联合血清肿瘤标志物检测结直肠癌患者诊断价值较高,阳性率、敏感度、特异度、阳性预测值及阴性预测值理想,值得临床研究与借鉴.
目的 探究血清炎症因子和凝血功能检测在下肢深静脉血栓形成患者中的应用价值.方法 选取2016年1月至2019年1月在本院接受治疗的72例下肢深静脉血栓患者作为观察组,另选取同期收治的72例健康体检人群作为对照组.对观察组患者进行治疗.比较治疗前两组受检者的血清炎症因子(CRP、IL-6、IL-8、IL-18)和凝血功能指标(FIB、TT、PT、APTT);比较治疗前、后观察组患者的血清炎症因子和凝血功能指标.结果 治疗前,观察组的血清炎症因子水平和凝血功能指标均明显高于对照组,差异具有统计学意义(P<0.05).治疗后,观察组患者的血清炎症因子水平和凝血功能指标均降低,差异具有统计学意义(P<0.05).结论 检测患者血清炎症因子和凝血功能能够为下肢深静脉血栓的预防、诊断、治疗提供全新的思路,可提高下肢深静脉血栓患者临床治疗效果,对患者诊断及治疗具有积极意义.
目的:了解未成年人体内微量元素(镉、铜、铁、铅、锌)和宏量元素(钙、镁)水平,探讨其在不同性别和年龄段人群中分布特点,对综合防治未成年人体内7种元素缺乏或过量提供科学依据.方法:回顾性分析2552例未成年人血液中镉、铜、铁、铅、锌、钙和镁的检测结果,按照未成年人生长发育特点分为八组:0~6个月组(350例),6~12个月组(930例),1~1.5岁组(345例),1.5~2岁组(277例),2~3岁组(227例),3~6岁组(226例),6~10岁组(128例),10~17岁组(69例).各元素检测采用原子吸收分光光度法.应用SPSS 19.0统计学软件对数据进行分析.结果:各年龄组人群铜和镁水平异常率很低;铅和镉的过量病例主要集中于6岁以上人群.7种元素中水平异常以钙、铁、锌为主,呈缺乏和过量并存现象,但以缺乏为主,且随年龄增大,缺乏率和过量率均呈下降态势,但在0~6个月组婴幼儿,三者缺乏较明显.各年龄组未成年人体内血镁含量比较无统计学差异(P>0.05),而铁、锌、钙、铅、镉、铜水平比较差异均有统计学意义(均P<0.05).其中,铅、镉、铁、锌随着年龄增大而升高,而钙、铜随着年龄增大而降低.不同性别未成年人群体内镉、镁、锌元素水平比较无统计学差异(P>0.05);在男性体内铜、铁、铅元素水平较女性高,而钙元素水平较女性低(均P<0.05).结论:镉、铜、铁、铅、锌、钙和镁元素失衡在未成年人群中普遍存在,应定期到医疗机构进行元素检测,必要时采取干预措施.
目的 分析肺癌患者D-二聚体和降钙素原(PCT)水平,探讨D-二聚体和PCT表达水平与肺癌的相关性.方法 选取2017年5月至2018年5月入住我院并确诊为肺癌的145例患者为试验组,并分为肺癌早期组(n=40)、肺癌中期组(n=50)、肺癌晚期组(n=55),另选择同期140位健康体检者为对照组.比较各组D-二聚体、PCT水平,并分析D-二聚体、PCT水平与肺癌的相关性.结果 试验组患者的D-二聚体、PCT水平均高于对照组(P<0.05).肺癌早期组与对照组的D-二聚体、PCT水平比较,差异无统计学意义(P>0.05);肺癌中、晚期组的D-二聚体、PCT水平均高于对照组和肺癌早期组,肺癌晚期组的D-二聚体、PCT水平均高于肺癌中期组,差异有统计学意义(P<0.05).肺癌早、中、晚期组D-二聚体、PCT水平均相关(P<0.05),且随着肺癌进展,D-二聚体和PCT水平的OR值明显上升,D-二聚体和PCT水平与肺癌的相关性明显增强.结论 肺癌患者D-二聚体和PCT水平检测可用于判断患者病情严重程度,可将其作为临床肺癌分期评估的参考依据.
目的 了解在慢性乙肝患者中HMGB1、IL-17表达水平与HBV-DNA载量相关性.方法 2016年6月~2017年1月期间随机从临床选取126例慢性乙型肝炎患者,分别通过实时荧光PCR仪和酶联免疫吸附法(ELISA)测定HBV-DNA、HMGB1、IL-17水平.结果 HBV-DNA检测,56例<1.0E+3IU/ml,53例≥1.0E+3IU/ml且<1.0E+7IU/ml,17例≥1.0E+7IU/ml;HMGB1水平(0.89±2.11)ng/ml;IL-17水平(2.44±7.75)ng/L;HMGB1与IL-17之间相关系数0.889,P<0.05;HMGB1与HBV-DNA之间相关系数0.093,P>0.05.结论 HMGB1与IL-17之间有相关性,HMGB1与HBV-DNA之间无相关性.
目的 探讨白细胞介素-6(IL-6)、降钙素原(PCT)和D-二聚体(D-dimer)联合检测在慢性阻塞性肺疾病急性加重期细菌感染患者中的表达及与预后的相关性.方法 选取慢性阻塞性肺疾病急性加重期细菌感染患者34例为A组,普通慢性肺阻塞性肺疾病患者34例为B组,并选取同期来我院接受健康体检的健康人34人作为对照组.结果 B组PCT、IL-6以及DD高于对照组和A组(P<0.05),A组PCT、IL-6以及DD高于对照组(P<0.05);治疗前PCT、IL-6以及DD高于治疗后(P<0.05);治疗后肺功能FVC、FEV1、MMEF以及PEF均高于治疗前(P<0.05);治疗后PaO2、SaO2明显高于治疗前(P<0.05),治疗后PaCO2指数明显低于治疗前(P<0.05).结论 在慢性阻塞性肺疾病急性加重期细菌感染患者中PCT、IL-6以及DD水平会明显升高,通过治疗可降低其水平表达,提升患者肺功能以及血气分析指数,改善预后.
在世界范围内都有乙型病毒性肝炎流行,且在各型肝炎中其危害最重,世界范围内约有3.5亿HBV感染者,其中15%~25%患者随病情发展最终将死于相关的肝损伤、肝硬化和肝细胞癌 [1],对人类健康构成极大威胁.医学检验已经开展了很多相关检查,理解不同方法学优缺点和相互扬长补短组合,对于慢性乙型肝炎诊治有重要意义.本研究对126例慢性乙型肝炎患者两对半模式、HBsAg定量与HBV-DNA进行回顾统计分析,以期为临床和检验工作者提供参考.
Pins are one of the most common parts of electronic equipment, which play a very important role in it. The reliability of pins is usually evaluated and improved by the high accelerated stress screening (HASS) test. A coupled thermal-structure model of a pin is proposed based on HASS test load in this paper. The pins are simulated commonly as the Euler-Bernoulli beam. At first, the finite-element model of a pin is established to analyze the temperature field and thermal stress stiffness. Then, the coupled thermal-structure equation of a pin is developed in which the additional stiffness matrix is superimposed on the dynamic equation. Last, the coupling equation is solved by the reciprocal iteration of the Galerkin and the Newmark methods. The results are verified by ANSYS simulation and HASS test. It showed that the simulation results are in good agreement with the experimental results, which reveal the correctness of the coupling model and provide the theoretical basis for failure analysis, simulation analysis, and forming reasonable HASS test profile of electronic equipment. It lays the foundation for the validity and safety analysis of HASS test.
目的 探讨急性脑卒中患者下呼吸道多重耐药菌感染的危险因素.方法 选择2016年1月至2018年4月在西安医学院第二附属医院就诊的602例急性脑卒中患者作为研究对象.翻阅患者病历资料,记录患者的一般情况、个人史、既往史、卒中性质、卒中部位、发病至住院时间、入院途径、格拉斯哥昏迷评分(GCS)、机械通气、低蛋白血症、抑酸药物应用、吞咽困难、手术治疗、鼻饲置管、应用抗生素、吸痰等资料,观察上述资料与多重耐药菌感染发生的相关性.结果 602例脑卒中患者,其中114例发生下呼吸道感染菌中有61例患者发生下呼吸道多重耐药菌感染;单因素分析显示,抑酸药物应用、吸烟、高血压、卒中位置、尿管、外科手术史、饮酒、过敏史不是影响急性脑卒中患者下呼吸道多重耐药菌感染的危险因素(P>0.05),鼻饲置管、高脂血症、吞咽困难、低蛋白血症、发病至入院时间、性别、慢性阻塞性肺疾病、体质量指数、糖尿病、年龄、卒中性质、GCS评分、前期碳青霉烯类药物、ICU住院时间、机械通气、手术治疗是影响急性脑卒中患者下呼吸道多重耐药菌感染的危险因素(P<0.05);多因素二分类Logistic回归分析结果显示,年龄>75岁、前期碳青霉烯类抗生素应用、吞咽障碍、院外转入、ICU住院时间≥72 h、机械通气>7 d是影响急性脑卒中下呼吸道多重耐药菌感染的独立危险因素(P<0.05),GCS≥9分是急性脑卒中下呼吸多重耐药菌感染的保护性因素(P<0.05).结论 急性脑卒中患者下呼吸道多重耐药菌感染是多因素综合作用的结果,临床应注意根据危险因素进行针对性防控以减少多重耐药菌的发生.
目的:探讨肺炎球菌DNA负荷(LytA基因)、降钙素原(PCT)和可溶性尿激酶型纤溶酶原激活物受体(suPAR)水平与肺炎链球菌感染患者病情严重程度的相关性.方法:选择184例肺炎链球菌感染患者作为研究对象,按照病情严重程度将患者分为轻度组(71例)、中度组(68例)和重度组(45例).采用定量荧光PCR反应测定血液LytA基因,采用酶联免疫吸附法(ELISA法)检测血浆suPAR水平,采用量免疫色谱法检测PCT水平,根据肺炎严重指数评分对患者病情进行分度.观察LytA、PCT及suPAR水平与疾病严重程度的相关性.结果:3组患者LytA阳性率相比较差异无统计学意义(F=1.900,P>0.05);LytA、PCT及suPAR水平随着病情分度增加逐渐升高,3组比较差异均有统计学意义(F=62.057,F=73.892,F=48.509;P<0.05).相关性分析显示,LytA、PCT及suPAR水平与病情严重程度均呈显著正相关(r=0.472,r=0.506,r=0.438;P<0.05);LytA与PCT、suPAR水平呈显著正相关(r=0.603,r=0.682;P<0.05).结论:肺炎链球菌肺炎患者血液中lytA、PCT及suPAR水平与病情严重程度均呈显著正相关;LytA基因量与PCT、suPAR水平呈显著正相关.
目的 探讨循环miR-146a水平对亚临床甲状腺功能减退症病人冠状动脉粥样硬化性心脏病(冠心病)的预测价值.方法 选择2015年1月-2018年1月在我院就诊的246例亚临床甲状腺功能减退症病人作为研究对象,以冠脉造影为金标准,观察病人一般情况、实验室检查指标与冠心病发病的相关性.结果 246例病人检出冠心痛102例,检出率为41.46%,冠心病组miR-146a表达量为2.209±0.636,非冠心病组miR-146a表达量为1.586±0.586,差异有统计学意义(f=7.925,P<0.001).单因素分析显示,饮酒不是亚临床甲状腺功能减退症病人冠心痛发病的危险因素(P>0.05),性别、年龄、体重指数、冠心病家族史、吸烟、高血压、糖尿病、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、总胆固醇(TC)、三酰甘油(TG)、血清同型半胱氨酸(Hcy)、血清促甲状腺激素(TSH)是影响亚临床甲状腺减退症病人冠心病发病的危险因素(P<0.05).多因素Logistic分析结果显示,年龄≥60岁、糖尿病、LDL-C≥3.12 mmol/L、血清促甲状腺激素(TSH)≥10 mlU/L、miR-146a是影响亚临床甲状腺功能减退症病人冠心病发病的独立危险因素(P<0.05).ROC显示,miR-146a预测冠心病的灵敏度、特异性、Cut-off、AUC和95% CI分别是62.75%、82.64%、2.06、0.767和0.709~0.818.结论 miRNA-146a是亚临床甲状腺功能减退症病人冠心病发病的独立危险因素,血浆miR-146a水平对亚临床甲状腺功能减退病人冠心病具有一定的预测价值.
目的:探讨冠心病(CHD)患者血清软骨寡聚基质蛋白(COMP)水平变化及其与冠状动脉钙化的关系.方法:选择在医院就诊的237例CHD患者,将其纳入观察组,同时抽取237名健康体检者纳入对照组.收集两组受试者一般资料、血清COMP水平.观察组患者同时检测血脂、同型半胱氨酸(Hcy)、白细胞(WBC)、C反应蛋白(CRP)水平和冠状动脉钙化积分(CACS),观察CHD患者一般资料、血清学指标与冠状动脉明显钙化的相关性.结果:对照组受试者COMP水平为(111.74±21.86)ng/ml,观察组患者COMP水平为(162.74±22.61)ng/ml,两组比较差异有统计学意义(t=19.082,P<0.001).观察组中明显钙化患者COMP水平显著高于无明显钙化患者,其差异有统计学意义(t=8.719,P<0.001).COMP预测明显冠状动脉钙化患者的最佳截断值为157.2 ng/ml,预测的灵敏度为73.5%,特异度为86.6%,明显冠状动脉钙化患者的受试者工作(ROC)曲线下面积(AUC)为0.85,95%置信区间(95%CI)为0.806~0.905.多因素Logistic分析,年龄、吸烟、糖尿病、CRP和COMP水平为冠状动脉钙化的独立影响因素(OR=5.857,OR=2.688,OR=3.684,OR=1.082,OR=4.639;P<0.05).结论:冠心病患者COMP水平高于正常人群,是CHD患者冠状动脉钙化的独立危险因素,预测冠状动脉明显钙化具有中等偏上价值.
This paper studies the mechanism of the corrosion of typical radar component working for the longterm in the environment of high temperature, high humidity and high salt fog. It puts forward the simulation calculation method of radar typical components in the environment, establishes the finite element model of radar typical components and conducts simulation analysis with computational fluid dynamics (CFD) software FLUENT for platform. The distributions of temperature, humidity and salinity of typical radar components are obtained. The simulation result shows the corrosion degree of parts surface is above 0.6mm and serious, which is in agreement with the actual radar equipment test result. In addition, from distributions of the simulation, the single temperature or salinity has little influence on the corrosion of the surface of the parts, while the humidity has a significant impact on the corrosion. It proves the rationality and correctness of the simulation calculation method, and provides a theoretical guidance for the further study and prevention of radar equipment corrosion of the typical parts of the radar.
Objective To establish an ultra high performance liquid chromatography coupled with electrospray ionization tandem mass spectrometry (UHPLC-ESI-MS/MS) method based on multiple-reactionmonitoring information-dependent-acquisition enhanced-product-ion (MRM-IDA-EPI) which was used to determinate simultaneously multiple mycotoxins residues in Chinese medicine health wine.Methods The samples were treated by QuEChERS-based extraction procedure,determined by UFLC-ESI-MS/MS,separated on Acquity UPLC BEH C18 column (150 mm × 2.1 mm,I.D,1.7 μtm) with water-methanol (both containing 0.1% formic acid) system at the flow rate of 0.3 ml/min and monitored in positive/negative ion monitoring mode.Matrix-matched calibration curves with external standard method were used for accurate quantitation of target mycotoxins in real samples.Results Linearity was excellent in the concentration ranges of 0.05 ~ 250 ng/ml and correlation coefficients were more than 0.999 2.The limit of quantification was 0.04 ~ 0.60 ng/ml,and the mean recoveries were 79.9% ~ 103.8% with relative standard deviation (RSD) value was less than 5.1%.Conclusion In this study,the pretreatment method was simple and the method has high sensitivity and fast detection speed,which has been successfully applied to determine rapidly and confirm multiple mycotoxins residues in 25 batches of Chinese medicine health wine samples purchased in various supermarkets.
Objective To analyze the distribution and drug resistance of carbapenem-resistant Klebsiella pneumoniae (CRKP), and to study the prognosis of patients infected with CRKP.Methods One hundred and fifteen patients with K.pneumoniae hospital infection were selected from January 2012 to December 2015 in the present study.Bacterial culture, identification and drug sensitivity test were carried out;then these 115 patients were divided into the CRKP group (53 cases) and non-CRKP group (62 cases) according to the results of drug sensitivity test, and prognosis of them were observed and analyzed.Results ①One hundred and sixty-four strains of K.pneumoniae were isolated from the 115 patients, and they were distributed in different specimens such as 48 strains (29.3%) in the sputum, 37 strains (22.6%) in the blood, 26 strains (15.9%) in the drainage liquid, 22 strains (13.4%) in the Pus, 15 strains (9.1%) in the bile, 9 strains (5.5%) in the urine and 7 strains (4.3%) in the wound secretion.②K.Pneumoniae isolated from 53 patients were identified as the CRKP (75 strains), and that isolated from the other 62 patients were identified as the non-CRKP (89 strains).The resistance rate of K.Pneumoniae: it was more than 95% to ampicillin, amoxicillin/clavulanate, cefalotin, cefotaxime, aztreonam, meropenem, gentamicin and ciprofloxacin.But that was less than 50% to tetracycline, tigecycline, minocycline, sulfamethoxazole and trimethoprim, fosfomycin.The resistance rate of K.Pneumoniae to piperacillin/tazobactam, imipenem and ertapenem were in 80%~90%.③Compared with the non-CRKP group, the hospitalization days after infection[(64±22) days], hospital mortality (23%) and infection related mortality (32%) of patients in the CRKP group were obviously higher (P<0.05), but the total effective rate (55%) were significantly lower (P<0.05).ConclusionK.pneumoniae are widely distributed in sputum of patients with infection, and CRKP has a high drug resistance to most antibiotics that used in clinical practice commonly.So patients infected with CRKP suffered a poor prognosis but a high fatality rate.

OBJECTIVE To explore the correlation between the serum IL-28B level,single nucleotide polymorphism (SNP) and viral genotypes and disease progression of patients with hepatitis C.METHODS Totally 258 patients with chronic hepatitis C (the chronic hepatitis C group),55 hepatitis C patients with liver cirrhosis (the hepatitis C with cirrhosis group),and 31 hepatitis C patients with liver cancer (liver cancer group),who were treated in hospitals from Jan 2009 to Dec 2015,were recruited as the study objects,meanwhile,223 health people who received physical examination were set as the control group.The IL-28B SNPs genotypes were detected by means of TaqMan@SNP genotyping assay and real-time quantitative polymerase-chain-reaction,the serum IL-28B concentration was determined by using ELISA,and the diagnostic values of serum IL-28B level and SNPs were analyzed via robust regression and ROC curve.RESULTS There was significant difference in 6 IL-28B SNP gene frequencies between the patients with hepatitis C and the healthy people(P<0.05).The serum IL-28B level of the patients with chronic hepatitis C was (112.8±40.9)pg/ml,significantly lower than (354.8±40.3)pg/ml of the hepatitis C patients with liver cirrhosis and (302.5-±-50.1) pg/ml of the hepatitis C patients with liver cirrhosis and liver cancer (P<0.01).The serum IL-28B level of the patients who carried with rs8099917 TT was (147.8±40.5)pg/ ml,significently lower than (295.8±50.2)pg/ml of the patients who carried with TG (P<0.05).The robust regression showed that the serum IL-28B level was closely associated with rs8099917 TG,chronic hepatitis C,and hepatitis C plus liver cirrhosis plus liver cancer (all P<0.05).The ROC analysis indicated that the 239 pg/ml of serum IL-28B was the cutoff value of the chronic hepatitis C and hepatitis C liver cirrhosis plus liver cancer.CONCLUSION The serum IL-28B can be used as an index for auxiliary diagnosis of progression of hepatitis C virus (HCV) disease.
Radar equipment are prone to surface abrasion or damage for a long-term working in the bad complex northwest region of in the large temperature difference and multi wind sand and strong ultraviolet. In this paper, a simulation analysis method of radar to the comprehensive environmental adaptability of the northwest area is established. Based on the computational fluid dynamics (CFD) software FLUENT, the simulation of the surface state of radar typical components in the large temperature difference, multi wind sand and strong ultraviolet environment is simulated. The simulation results are consistent with the actual situation of radar equipment, which provides a theoretical basis for further study of the physical mechanism and design of protective measures.
