A post-acne hyperpigmentation index (PAHPI) has been developed in the United States to better compare therapeutic modalities. Our aim in this study was to validate the PAHPI score in patients with skin type VI from sub-Saharan Africa.The study was conducted in Dakar, Senegal. Twenty-one patients with Fitzpatrick skin type VI, aged 17 to 55 years, presenting hyperpigmentation secondary to acne were included. Ongoing use of skin bleaching products or acne treatments was allowed. Four trained dermatologists rated the patients using the PAHPI. A narrow-band reflectance spectrophotometer (Mexameter MX-18, Cologne, Germany) was used to measure the degree of pigmentation of involved and adjacent skin on 6 representative facial lesions.The average inter-rater reliability (weighted Kappa) showed substantial agreement for intensity (0.67), moderate agreement for number (0.53) and fair agreement for lesion size (0.28). Inter-rater reliability for the total PAHPI was excellent for both day 1 and day 2 (interclass correlation coefficient of 0.87 and 0.85, respectively; P < 0.0001). Intra-rater reliability for total PAHPI ranged from 0.83 to 0.93 (P < 0.0001). PAHPI scoring thus demonstrated excellent reliability both between and within raters. The association was moderate to substantial for all raters on both days (range for rho on day 1: 0.531 to 0.815; range for rho on day 2: 0.448 0.762). The correlations between the Mexameter (Courage and Khazaka) measurements and PAHPI scores showed moderate to substantial agreement.Although tested primarily in African American women to date, PAHPI is also valid for patients from sub-Saharan Africa.

Materials and methods. - Analysis of the initial and end of treatment CT scans of 56 patients with pulmonary tuberculosis according to a reading grid including parenchymatous and airways lesions. The CT data at the end of treatment were analysed in relation to the clinical and microbiological data, and the original CT scan. Results. - Active lesions (thick walled cavities and/or centrilobular micronodules) persisted in 24 patients (43%) after a mean treatment period of 7 months. The persistence of these signs of activity was correlated with the initial presence of a cavitary syndrome (p=0.027), with predominant sub-segmentary bronchial involvement, with extensive micronodular spread (p=0.024) and with bronchiectasis (p=0.04). These residual lesions were not associated with an increased risk of relapse. Conclusion. - The persistence of signs of activity on the CT scan at the end of treatment of tuberculosis do not necessarily correspond to an absence of cure but to a radiological delay. This imaging is nevertheless useful to make an assessment of any subsequent changes in the bronchial tree and to estimate the risk of later complications. (C) 2018 SPLF. Décrire les lésions broncho-pulmonaires résiduelles et évaluer l’intérêt de la tomodensitométrie (TDM) en fin de traitement d’une tuberculose pulmonaire. Analyse des TDM thoraciques initiales et de fin de traitement de 56 patients avec tuberculose pulmonaire selon une grille de lecture incluant les lésions parenchymateuses et des voies aériennes. Les données TDM de fin de traitement ont été analysées en fonction des données cliniques, mycobactériologiques et du TDM initial. Des lésions actives (cavité à paroi épaisse et/ou micronodules centrolobulaires) persistaient chez 24 patients (43 %) après un traitement moyen de 7 mois. La persistance de ces signes d’activité était corrélée initialement à la présence d’un syndrome cavitaire (p = 0,027), à une atteinte bronchique prédominant en sous-segmentaire, à une extension plus importante des micronodules (p = 0,024) et à des bronchectasies (p = 0,04). Ces lésions résiduelles n’étaient pas associées à un risque plus élevé de rechute. La persistance de signes d’activité sur la TDM de fin de traitement d’une tuberculose ne correspond pas obligatoirement à une absence de guérison mais à un retard radiologique. Cette imagerie est néanmoins utile pour faire le bilan des séquelles de l’arbre bronchique, voire estimer le risque de complications ultérieures. Les pemphigoïdes des muqueuses (PM) sont des maladies bulleuses auto-immunes sous-épithéliales, aux complications cicatricielles sténosantes graves, touchant les épithéliums malpighiens. L'atteinte bronchique est très peu connue avec seulement 6 cas dans la littérature, nous en rapportons 2 nouveaux. Cas 1 : femme de 28 ans atteinte d'épidermolyse bulleuse acquise (laryngée, nasale, buccale), prouvée en immunomicroscopie électronique (IME). Sous corticothérapie générale, apparaissent une toux et une dyspnée fébrile avec opacités radiologiques systématisées lobaires supérieures droites. La fibroscopie bronchique (FB) montrait des bulles et des érosions bronchiques avec histologiquement une métaplasie malpighienne (MM) de l'épithélium respiratoire, une bulle sous-épithéliale et des dépôts d'IgG anti-membrane basale (MB) en immunofluorescence directe (IFD). Le lavage broncho-alvéolaire était normal. L'évolution était favorable sous antibiothérapie. Des explorations fonctionnelles respiratoires (EFR) réalisées à distance pour explorer l'atteinte laryngée révélaient une hyper réactivité bronchique (HRB) témoignant d'une inflammation bronchique. Traitée successivement par salazopyrine, prednisone, ciclosporine, puis rituximab du fait de l'atteinte laryngée asphyxiante non contrôlée, la patiente est décédée d'une pneumocystose. Cas 2 : homme de 60 ans suivi pour une pemphigoïde cicatricielle (oculaire, nasale, buccale), prouvée en IME. Il présentait une toux sèche, une dyspnée d'effort avec un trouble ventilatoire obstructif (TVO) aux EFR. La FB révélait 2 sténoses bronchiques proximales focales, confirmées par le scanner, sans lésion bulleuse active. L'histologie bronchique montrait des décollements bulleux sous-épithéliaux témoignant d'une fragilité muqueuse en zone de MM et l'IFD des dépôts linéaires d'IgG, IgA anti-MB. Il était mis en rémission sous cyclophosphamide et dapsone. Les épithéliums respiratoires ciliés qui n'expriment ni la BP180 ni le collagène 7 sont épargnés au cours des PM. Pour ces 2 patients non fumeurs, non asthmatiques, le diagnostic d'atteinte spécifique bronchique de PM a été formellement porté en zone de MM. Dans nos 2 cas, la symptomatologie respiratoire était frustre malgré l'atteinte proximale. Le cas 1 illustre la possibilité de lésions bulleuses actives, le cas 2 leur évolution sténosante possible. Comme dans les 6 cas de la littérature, l'atteinte bronchique est survenue chez des patients avec une PM active et sévère. L'atteinte bronchique des PM est rare et mal connue. Son évolution sténosante peut engager le pronostic respiratoire. Une dyspnée ou une toux chez un patient atteint de PM doit faire rechercher une atteinte bronchique, même en absence de cause évidente de MM, par la réalisation d'EFR éventuellement complétées d'une FB (examen de référence) et d'un scanner haute résolution à la recherche de sténoses.
Évaluer l'apport d'un scanner thoracique en fin de traitement antituberculeux. Étudier les facteurs prédictifs de persistance de signes d'activité (cavités à parois épaisses +/- nodules centrolobulaires) et de lésions séquellaires bronchiques en fin de traitement. Étude comparative rétrospective au CHU Avicenne d'octobre 2006 à mars 2012 par trois radiologues des scanners thoraciques pour tuberculose pulmonaire réalisés au début et à la fin du traitement. L'étude a porté sur 56 patients dont 43 (77 %) hommes. Six patients étaient VIH+ (11 %). La persistance de signes d'activité radiologique résiduelle (ARR) en fin de traitement était retrouvée chez 24 patients asymptomatiques sur 56 (43 %) et était associée significativement à la présence initiale de cavités (p = 0,03), de micronodules (p = 0,022) et d'une atteinte bronchique sous segmentaire. Les symptômes cliniques et l'immunodépression n'étaient pas associés significativement à une ARR. Des séquelles bronchiques, retrouvées chez 36 patients sur 56 (64 %), avaient pour facteurs prédictifs initiaux les cavités (p = 0,003), les condensations (p = 0,004), les micronodules (p = 0,03) et une atteinte bronchique initiale à type de DDB (p < 0,001) mais aussi la toux (p = 0,01) et le tabagisme (p = 0,014). Un scanner thoracique à distance (18,5+/-9,1 mois post traitement) a été réalisé chez 4 patients avec ARR en fin de traitement. Les signes d'activité avaient disparu chez tous les patients. La présence isolée de signes d'ARR en fin de traitement témoigne probablement le plus souvent d'un retard radioclinique et ne doit pas faire prolonger le traitement par le clinicien. Le scanner thoracique peut néanmoins servir d'imagerie de référence en situation de rechute tuberculeuse, d'hémoptysie, de surinfection bactérienne ou de greffe aspergillaire.
Interstitial syndrome is a radiological syndrome grouping together all signs of pulmonary impairment affecting the interstitium, the supporting connective tissue of the lung. Interstitial syndrome disease and diffuse interstitial lung disease (PID) are terms with different meanings. The interstitial lung structure is mainly affected in the PID, but not exclusively. In addition to the pulmonary interstitium, PID can affect all the other structures of the deep lung, including the alveoli, bronchioles and small vessels and also in some cases the other thoracic structures: pleura, respiratory muscles, heart and chest, with a potential impact on the lung. Le syndrome interstitiel radiologique est l’ensemble des signes radiologiques indiquant une atteinte de l’interstitium pulmonaire, c’est-à-dire du tissu conjonctif de soutien du poumon. Syndrome interstitiel et pneumopathies interstitielles diffuses (PID) sont des termes dont la signification est différente. L’interstitium pulmonaire est la structure pulmonaire principalement atteinte dans les PID mais de façon non exclusive. Outre l’interstitium pulmonaire, les PID peuvent atteindre toutes les autres structures du poumon profond, notamment les alvéoles, les bronchioles et les petits vaisseaux et également dans certains cas les autres structures thoraciques : plèvre, muscles respiratoires, cœur et cage thoracique avec un retentissement éventuel sur le poumon. L’atteinte du secteur interstitiel peut être isolée ou associée, concomitante ou secondaire à l’atteinte des autres secteurs du poumon.
Granulomatose de cause inconnue, la sarcoïdose est une maladie protéiforme avec une atteinte pulmonaire quasi constante et souvent prédominante, et un profil évolutif difficile à prédire à la prise en charge initiale. L’approche diagnostique consiste devant un tableau clinico-radiologique évocateur, à mettre en évidence des lésions granulomateuses sans nécrose caséeuse par des biopsies de lésions superficielles, per-endoscopiques bronchiques ou encore ganglionnaires médiastinales sous contrôle écho-endoscopique et à éliminer les diagnostics différentiels à envisager selon la présentation. La prise en charge thérapeutique de l’atteinte respiratoire prend idéalement en compte l’évolution initiale spontanée. La corticothérapie constitue le traitement de première ligne en cas d’indication thérapeutique, situation rencontrée chez la moitié des patients. Les traitements de deuxième et de troisième ligne sont basés respectivement sur les immunosuppresseurs et les anti-TNFα. Les formes pulmonaires sévères – fibrose sarcoïdienne, hypertension pulmonaire, sténoses bronchiques et greffe aspergillaire – grèvent le pronostic vital et fonctionnel des patients. Les traitements « non anti-inflammatoires » ont également une place importante. La place du clinicien reste essentielle pour poser les indications thérapeutiques, proposer des cibles thérapeutiques, évaluer la réponse thérapeutique et trouver le meilleur rapport bénéfice–risque. Les progrès de la pharmacogénétique devraient permettre d’offrir un traitement plus personnalisé.Sarcoidosis is a granulomatous disease of unknown cause. This proteiform disease is characterized by an almost constant and often predominant lung involvement. The natural history of disease is difficult to predict at presentation. Diagnosis is based on a compatible clinical and radiological presentation and evidence of non-caseating granulomas. Exclusion of alternative diseases is also required according to clinical presentation. Biopsy samples of superficial lesions should be considered before other sites like per-endoscopic bronchial biopsies or endobronchial ultrasound-guided transbronchial needle aspiration. Therapeutic strategy for lung disease has to take into account the possible spontaneous resolution observed in newly diagnosed patients. Corticosteroids are the first choice when a treatment is decided, which concerns half of patients. Second and third line therapy are based respectively on immunosuppressive drugs and anti-TNFα drugs. Sarcoidosis mortality and morbidity are mainly linked to advanced pulmonary sarcoidosis – lung fibrosis, pulmonary hypertension, bronchial stenosis and chronic pulmonary aspergillosis. “Non anti-inflammatory” treatments have to be considered as well. Clinicians have an essential role in treatment indication, end-point targets and evaluation of response to treatment during follow-up and in finding the best benefice to risk balance. Progress made on pharmacogenetics may offer more personalized treatments for the patients.
BACKGROUND:Screening for latent tuberculosis infection (LTBI) using a protocol comprising chest X-ray and tuberculin skin test (TST) interpreted with medical history, Sc1, reduces LTBI reactivation on treatment with anti-tumour necrosis factor-alpha (anti-TNF-α). In the district of Seine-Saint-Denis, France, where tuberculosis (TB) incidence ranges from 30 to >100/100 000 person-years, however, Sc1 might be insensitive as a screening tool. We adopted another protocol, Sc2, comprising Sc1 plus two additional tests: the QuantiFERON(®)-TB Gold In-Tube (QFT-GIT) and chest computed tomography (CT).METHODS:We screened 123 consecutive patients with inflammatory rheumatic diseases (IRDs), candidates for anti-TNF-α treatment, and evaluated the impact of Sc2 vs. Sc1 on the prescription of prophylactic anti-tuberculosis treatment.RESULTS:Sc2 led to a diagnosis of LTBI in 69 patients vs. 59 when using Sc1: eight were QFT-GIT-positive. Diagnosis was based on CT findings in two patients. QFT-GIT had higher diagnostic accuracy than TST, but no single diagnostic test could detect all patients at high risk for LTBI reactivation (respectively 30.2% and 37.5% of patients positive with only TST or QFT-GIT). CT detected TB sequelae in 3/46 rheumatoid arthritis patients who were negative to all tests.CONCLUSIONS:Testing with both TST and QFT-GIT seems the safest strategy for detecting LTBI in patients with IRD from populations with high incidence of TB. Systematic screening with CT warrants further evaluation.
This paper compares the contact-repulsion movement of mutant and wild-type macrophages using a novel interaction detection mechanism. The migrating macrophages are observed in Drosophila embryos. The study is carried out by a framework called macrosight, which analyses the movement and interaction of migrating macrophages. The framework incorporates a segmentation and tracking algorithm into analysing motion characteristics of cells after contact. In this particular study, the interactions between cells is characterised in the case of control embryos and Shot3 mutants, where the cells have been altered to suppress a specific protein, looking to understand what drives the movement. Statistical significance between control and mutant cells was found when comparing the direction of motion after contact in specific conditions. Introduction: Various interstitial lung diseases (ILD) have been reported in inflammatory bowel diseases (IBD) but most publications are limited to small series or predate the 2002 ATS/ERS statement on idiopathic interstitial pneumonias. Aims: To describe the epidemiological, clinical, radiological and pathological characteristics of IBD-associated ILD. Methods: This is a monocentric retrospective study of 9 patients with IBD-associated ILD refered from 1990 to 2010. Patients were classified according to HRCT and pathological patterns of ILD as recommended by 2002 ATS/ERS statement. Results: There were 5 men and 4 women with a mean age of 49±7 years (Crohn's disease: n=4, ulcerative colitis: n=3, undetermined colitis: n=2). ILD developped in the course of previously known IBD in 7 cases, with a median delay of 10 years, while IBD was diagnosed after ILD in 2 cases. Surgical lung biopsy was available in 4 patients. ILD final diagnoses were: “hypersensitivity pneumonitis” (“HSP”): n=4, nonspecific interstitial pneumonia (NSIP): n=2, NSIP secondary to “HSP”: n=1, combined pulmonary fibrosis and emphysema: n=1, bronchiolitis: n=1. No patient had overt environnemental exposure but 4 patients received mesalazine at the onset of ILD, of which only one with “HSP”. One patient with NSIP also had dermatomyositis. Baseline FVC was 65±19% and DLCO 34±8%. ILD worsened despite corticosteroids in 7 patients who required other immunosuppressive drugs. At the end of follow-up (4.3±2.6 years), 2 patients died and 1 was transplanted. Conclusion: Our study outlines the severity of IBD-associated ILD and the high frequency of “HSP”, raising the possible role of a particular unrecognised antigen in such a context.
Introduction: Owing the absence of published prospective cohorts, the epidemiology and natural history of IPF remains unclear. Aims: To determine the factors associated with the occurrence of pre-defined evolutive events: slow progression of IPF, acute exacerbation (AE), subacute exacerbation (subAE), pulmonary hypertension (PH) and death. Methods: Prospective cohort involving all the 24 French University Hospitals. The diagnosis of IPF was based on 2000 ATS/ERS criteria and centrally reviewed. Results: From 12/2007 to 12/2010, 240 incident cases of IPF (diagnosis 25 in 68%. A familial form of IPF was noted in 8%. A combined pulmonary fibrosis and emphysema syndrome was seen in 18% and asymetrical IPF in 7%. Baseline FVC was 76±21% and DLCO was 47±17%. On BAL, lymphocytes were ≥30% in 4.1% and eosinophils ≥20% in 2.1%. Surgical lung biopsy was performed in 31%. After a follow-up of 14±9 months (range: 1-36 months) following events were observed: 34 slow progressions, 19 AE, 10 subAE, 9 PH and 46 deaths. Eight patients were transplanted. The incidence of AE was 12% at 1 year. Survival was 80% and 63% at 1 and 2 years. Mortality was related to respiratory causes in 78% (AE: 37% and end-stage respiratory insufficiency: 26%). Conclusion: Our results confirm the high frequency of overweight and severe co-morbidities in IPF. Despite a relatively low incidence of evolutive events, mortality is already high, with AE being the major cause of death.
The objectives of this study were to compare the survival of sarcoid patients with pulmonary fibrosis with that of the general population and to determine the causes of death and the incidence of evolutive complications. This retrospective cohort included 142 sarcoid patients in radiographic stage IV (74 males; mean ± SD age 48.1 ± 12 yrs). Their survival was compared with that of the general French population, matched for the year and age at diagnosis of stage IV disease, sex and length of follow-up. Expected survival probabilities were calculated year-by-year on the basis of probabilities provided by official demographic data for France. Survival curves were based on the Kaplan-Meier method and compared using the log-rank test. During the follow-up period (7.1 ± 4.8 yrs), pulmonary hypertension (PH) was observed in 29.7% of cases and aspergilloma in 11.3%. Long-term oxygen therapy was required in 12%. Survival was 84.1% at 10 yrs, which was worse than for the general population (p = 0.013). 16 (11.3%) patients died from the following causes: refractory PH (n = 5), chronic respiratory insufficiency (n = 4), acute respiratory insufficiency (n = 2), haemoptysis due to aspergilloma (n = 1), heart sarcoidosis (n = 1), nocardiosis (n = 1) and unknown causes (n = 2). Survival is significantly decreased in stage IV patients. 75% of fatalities are directly attributable to respiratory causes.
L'hyperexpression des récepteurs de la somatostatine, notamment le sous-type 2, est une caractéristique des tumeurs neuroendocrines gastro-entéropancréatiques (TNE-GEP) différenciées, mise à profit depuis de nombreuses années pour l'imagerie et plus récemment pour la radiothérapie interne vectorisée (RIV) dans une approche théranostique. L'accès à la RIV a longtemps été limité à quelques centres spécialisés européens malgré des résultats cliniques très prometteurs et un nombre important de patients porteurs de TNE traitées. Ces deux dernières années sont marquées par la validation, grâce à l'essai randomisé (NETTER-1), de l'intérêt de la RIV par 177Lu-DOTATATE pour le traitement des TNE de l'intestin moyen ayant abouti à l'autorisation de mise sur le marché du 177Lu-DOTATATE (LUTATHER®) par les autorités de santé américaine et européenne. L'accès à la RIV des TNE-GEP est devenu une réalité dans une vingtaine de centres français, avec une prise en charge financière de la RIV attendue dans les TNE intestinales en début 2019. La place de la RIV par rapport aux autres modalités thérapeutiques des TNE non intestinales reste à valider par les essais en cours, mais on anticipe déjà un développement important de cette nouvelle modalité thérapeutique. Les succès rapportés de la RIV et le développement de nouveaux radiopharmaceutiques devraient dépasser le champ des TNE et nécessitera une organisation des services de médecine nucléaire et une formation des équipes avec une orientation plus clinique du métier de médecin nucléaire. L'objectif de cet article est de faire le point sur la RIV par les analogues radiomarqués de la somatostatine en France en 2019, en précisant les indications et les modalités d'accès au traitement, et en abordant des aspects pratiques de sélection et de prise en charge des patients, en se basant sur les données de la littérature et des retours d'expériences des centres (incluant les résultats de l'enquête conduite par le groupe endocrinologie de la SFMN en 2017). Seront également abordés les aspects de radioprotection et les perspectives d'optimisation futures dans les TNE-GEP.Somatostatin receptors are overexpressed in differentiated gastroenteropancreatic neuroendocrine tumors (GEP-NET). Radiolabeled somatostatin analogs have been used for a few decades for imaging and more recently for peptide receptor radionuclide therapy (PRRT) in a theranostic approach. Medical access to PRRT has long been limited to a few European specialized medical centers despite promising results in large cohorts of patients. NETTER-1, a phase 3 randomized trial, has demonstrated a drastic improvement of midgut NET patients progression-free survival in PRRT arm as compared to somatostatin analogs, leading to marketing authorizations in USA and Europe. PRRT clinical availability is growing in France, with around 20 medical centers offering this innovative treatment for GEP-NET patients care in 2019. PPRT success-story should lead to improvements of radionuclide therapy developments, which will reshape our medical specialty to a more "clinically" practice. This review aims to detail PRRT in clinical practice in France in 2019, with emphasize on treatment indications, planning and practical aspects. Radioprotection aspects and future optimization perspectives will also be discussed.
