Background: Indications for intra-osseous (IO) infusion are increasing in adults requiring administration of fluids and medications during initial resuscitation. However, this route is rarely used nowadays due to a lack of knowlegde and training. We reviewed the current evidence for its use in adults requiring resuscitative procedures, the contraindications of the technique, and modalities for catheter implementation and skill acquisition.Methods: A PubMed search for all articles published up to December 2015 was performed by using the terms "Intra-osseous" AND "Adult". Additional articles were included by using the "related citations" feature of PubMed or checking references of selected articles. Editorials, comments and case reports were excluded. Abstracts of all the articles that the search yielded were independently screened for eligibility by two authors and included in the analysis after mutual consensus. In total, 84 full-text articles were reviewed and 49 of these were useful for answering the following question "when, how, and for which population should an IO infusion be used in adults" were selected to prepare independent drafts. Once this step had been completed, all authors met, reviewed the drafts together, resolved disagreements by consensus with all the authors, and decided on the final version.Results: IO infusion should be implemented in all critical situations when peripheral venous access is not easily obtainable. Contraindications are few and complications are uncommon, most of the time bound to prolonged use. The IO infusion allows for blood sampling and administration of virtually all types of fluids and medications including vasopressors, with a bioavailability close to the intravenous route. Unfortunately, IO infusion remains underused in adults even though learning the technique is rapid and easy.Conclusions: Indications for IO infusion use in adults requiring urgent parenteral access and having difficult intravenous access are increasing. Physicians working in emergency departments or intensive care units should learn the procedures for catheter insertion and maintenance, the contraindications of the technique, and the possibilities this access offers.
Background: Lumbar spinal stenosis is a major public health issue. Interspinous devices implanted using minimally invasive techniques may constitute an alternative to the reference standard of bony decompression with or without intervertebral fusion. However, their indications remain unclear, due to a paucity of clinical and biomechanical data. Our objective was to evaluate the effects of four interspinous process devices implanted at L4–L5 on the intervertebral foramen surface areas at the treated and adjacent levels, in flexion and in extension. Materials and method: Six fresh frozen human cadaver lumbar spines (L2–sacrum) were tested on a dedicated spinal loading frame, in flexion and extension, from 0 to 10 N·m, after preparation and marking of the L3–L4, L4–L5, and L5–S1 foramina. Stereoscopic 3D images were acquired at baseline then after implantation at L4–L5 of each of the four devices (Inspace®, Synthes; X-Stop®, Medtronic; Wallis®, Zimmer; and Diam®, Medtronic). The surface areas of the three foramina of interest were computed. Results: All four devices significantly opened the L4–L5 foramen in extension. The effects in flexion separated the devices into two categories. With the two devices characterized by fixation in the spinous processes (Wallis® and Diam®), the L4–L5 foramen opened only in extension; whereas with the other two devices (X-Stop® and Inspace®), the L4–L5 foramen opened not only in extension, but also in flexion and in the neutral position. None of the devices implanted at L4–L5 modified the size of the L3–L4 foramen. X-Stop® and Diam® closed the L5–S1 foramen in extension, whereas the other two devices had no effect at this level. Conclusion: Our results demonstrate that interspinous process devices modify the surface area of the interspinous foramina in vitro. Clinical studies are needed to clarify patient selection criteria for interspinous process device implantation. Level of evidence: Level IV. Investigating an orthopaedic device.
Le traitement de la sténose lombaire représente aujourd’hui un enjeu de santé publique. Les dispositifs inter épineux mis en place par voie mini-invasive représenteraient une alternative thérapeutique à la technique classique de décompression ± arthrodèse intervertébrale. En raison d’un manque de données biomécaniques et cliniques, leurs indications de pose ne sont pas clairement établies. Le but de ce travail est de caractériser l’effet d’implants inter épineux disposés à l’étage L4-L5 sur la surface des foramens intervertébraux à l’étage implanté et aux étages adjacents en flexion et en extension. L’étude a nécessité la préparation de 6 colonnes lombaires humaines fraîches (L2-sacrum). Chaque colonne était testée en flexion et en extension de 0 à 10 N/m après préparation et marquage des trois foramens d’intérêts. Une acquisition stéréoscopique 3D était réalisée à vide puis après implantation de chaque dispositif en L4-L5. La surface foraminale était calculée en L4-L5 ainsi qu’aux étages adjacents. En L4-L5, deux catégories d’implants se distinguent : les implants possédant une fixation épineuse (Wallis® et Diam®) ouvrent le foramen en extension. Les implants qui ne possèdent pas de fixation épineuse (Xstop® et In-Space®) ouvrent le foramen au repos, en flexion et en extension. La mise en place des implants en L4-L5 n’avait aucune conséquence sur la taille du foramen à l’étage L3-L4. Xstop® et Diam® entraînaient une fermeture du foramen L5-S1 vers l’extension. Ce travail montre que les implants inter épineux ont bien un effet sur la surface des foramens inter épineux in vitro. Des études cliniques sont nécessaires pour préciser leurs indications. Le réseau microvasculaire est une cible majeure de l’ischémie reperfusion. Le but de cette étude était de caractériser le remodelage vasculaire du cortex rénal après ischémie reperfusion et de déterminer la partie la plus affectée. Nous avons utilisé un modèle préclinique porcin d’auto-transplantation rénale avec néphrectomie controlatérale dans lequel le greffon a été conservé 24 h à 4 °C dans la solution de conservation : University of Wisconsin. Trois mois après auto-transplantation, les reins porcins (n = 5) ont été étudiés ex vivo par micro-tomographie en les comparant à des reins natifs (n = 5 ; Fig. 1a). La morphologie du réseau vasculaire et notamment la densité et la tortuosité des segments vasculaires ont été analysés à partir d’un procédé d’analyse tridimensionnelle. Le débit sanguin cortical a également été évalué, ainsi que la fonction rénale et les lésions tissulaires. L’ischémie-reperfusion rénale a conduit à une diminution du volume des segments vasculaires associée à une raréfaction des petits vaisseaux de moins de 30 μm, en particulier dans le cortex profond (Fig. 1b). Cette raréfaction microvasculaire a été corrélée à une détérioration de la fonction rénale accompagnée de protéinurie et de dysfonction tubulaire (Fig. 1c). Le débit sanguin cortical des greffons était diminué à 1 h et à 3mois après la transplantation. Dans le cortex total des greffons, il a été mis en évidence une augmentation du nombre de bifurcations des segments vasculaires et le développement d’un tissu fibreux qui participe au remodelage vasculaire (Fig. 1d). Ce travail apporte des indications sur le spectre lésionnel de l’ischémie reperfusion rénale et permettra de développer des futures thérapies dans le but d’améliorer la préservation du réseau microvasculaire du greffon rénal.
L’ouvrage collectif, Traumatismes de la cheville, publie dans le cadre de la collection « References en medecine d’urgence » de la SFMU, est consacre a une pathologie tellement frequente qu’elle est souvent banalisee au point de rassembler sous le vocable « d’entorse » de nombreuses entites pathologiques differentes. La redaction collective de cet ouvrage permet de realiser une mise au point actualisee et tres pragmatique de la prise en charge aux urgences des traumatismes de cheville. Des auteurs de renom en matiere de traumatologie ont contribue a ce travail et ont permis d’envisager tous les aspects des traumatismes fermes de facon tres pratique en « collant au terrain ». Chaque chapitre presente les cinq points essentiels de la pathologie traitee, ce qui rend la lecture aisee et claire. Les aspects diagnostiques et leurs principaux pieges sont exposes ainsi que les indications therapeutiques et le suivi. L’urgentiste aura ainsi tous les elements du diagnostic du ou des traitement(s) ainsi que les informations necessaires au patient concernant le suicide ulterieur. Il faut egalement souligner la richesse et la qualite de l’iconographie qui illustre parfaitement ce propos. Le plan general de l’ouvrage suit un cheminement de raisonnement clinique, partant de l’examen clinique, en passant par les examens complementaires pour aboutir aux moyens therapeutiques. Au total, il s’agit d’un ouvrage de reference, actualise, dont le collectif des auteurs, tous experts dans leurs specialite et participant dans leur activite quotidienne a la prise en charge en urgence des traumatismes de la cheville, met en exergue la necessaire collaboration des urgentistes, des radiologues, des chirurgiens orthopedistes et des medecins de readaptation physique. La modernite de cet ouvrage tient a son aspect pratique qui demythifie le diagnostic banalise d’entorse et donne les elements pour deceler et traiter au mieux les nombreuses entites pathologiques qui peuvent s’associer autour du traumatisme de la cheville. En ce sens, cet ouvrage de reference devrait avoir un beau succes aupres des urgentistes, mais aussi des chirurgiens orthopedistes et des medecins de readaptation physique.
Protection of endothelial cell function may explain the benefits of nonoxygenated hypothermic machine perfusion (MP) for marginal kidney preservation. However, this hypothesis remains to be tested with a preclinical model. We postulated that MP protects the nitric oxide (NO) signaling pathway, altered by static cold storage (CS), and improves renal circulation recovery compared to CS. The endothelium releases the vasodilator NO in response to flow via either increased endothelial NO synthase (eNOS) expression (KLF2-dependent) or activation of eNOS by phosphorylation (via Akt, PKA or AMPK). Using a porcine model of kidney transplantation, including 1 h of warm ischemia and preserved 24 h by CS or MP (n=5), we reported that MP did not alter the cortical levels of KLF2 and eNOS at the end of preservation, but significantly increased eNOS activating phosphorylation compared to CS. eNOS phosphorylation appeared AMPK-dependent and was concomitant to an increased NO-dependent vasodilation of renal arteries measured, ex situ, at the end of preservation. In vivo, laser Doppler showed that cortical microcirculation was improved at reperfusion in MP kidneys. In conclusion, we demonstrate for the first time, in a large-animal model, that MP protects the NO signaling pathway, confirming the value of MP for marginal kidney preservation.
La connaissance de l’anatomie clinique de la cheville et du pied est indispensable pour l’urgentiste qui aborde les pathologies traumatiques et fonctionnelles de ces deux entités. Ce chapitre est différent des classiques descriptions dans le sens où il se veut volontairement pratique, « dynamique » (description des structures dans le mouvement) et en relation directe avec la clinique aux urgences. L’anatomie clinique de la cheville et du pied aux urgences se conçoit comme un ensemble fonctionnel comprenant les articulations de la cheville (talo-crurale, tibio-fibulaire distale et subtalaire) et celles du pied (articulation transverse du tarse ou tarso-métatarsienne en particulier). Nous envisagerons donc ces différentes entités anatomo-fonctionnelles et les points essentiels de l’anatomie afin de comprendre les lésions et d’interpréter au mieux les différentes imageries aux urgences.
Fractures of the tibial plateau are in constant progression. They affect an elderly population suffering from a number of comorbidities, but also a young population increasingly practicing high-risk sports and using two-wheeled vehicles. The objective of this study was therefore to propose a new technique for the treatment of this type of fracture. There are a variety of classical pitfalls of conservative treatment such as defective reduction resulting in early osteoarthritis and alignment defects. Conventional treatments lead to joint stiffness and amyotrophy of the quadriceps, caused by the open technique and late loading. We propose an osteosynthesis technique for tibial plateau fractures with minimally invasive surgery. A minimally invasive technique would be more appropriate to remedy all of the surgical drawbacks resulting from current practices. The surgical technique that we propose uses a balloon allowing progressive and total reduction, associated with percutaneous screw fixation and filling with polymethylmethacrylate (PMMA) cement. The advantages are optimal reduction, minimal devascularization, soft tissues kept intact, as well as early loading and mobilization. This simple technique seems to be a good alternative to conventional treatment. The most comminuted fractures as well as the most posterior compressions can be treated, while causing the least impairment possible. Arthroscopy can be used to verify fracture reduction and cement leakage. At the same time, it can be used to assess the associated meniscal lesions and to repair them if necessary.
Les fractures du plateau tibial sont en constante augmentation. Elles affectent une population âgée souffrant de nombreuses comorbidités, mais aussi une population jeune avec la pratique croissante des sports à risques et la recrudescence de l’usage des deux roues. L’objectif de ce travail est donc de proposer une nouvelle technique mini-invasive pour le traitement de ce type de fracture. Les écueils classiques du traitement conservateur sont multiples : défaut de réduction engendrant une arthrose précoce et un défaut d’axe. Les traitements conventionnels entraînent une raideur articulaire et une amyotrophie du quadriceps, dues à la technique à ciel ouvert et à une remise en charge tardive. Nous proposons une technique d’ostéosynthèse des fractures du plateau tibial par chirurgie mini-invasive. Une technique mini-invasive serait plus appropriée pour s’affranchir de tous ces écueils opératoires actuels. La technique opératoire que nous proposons utilise un ballonnet permettant une réduction souple progressive et totale, associé à une ostéosynthèse par vis percutanées et un comblement par ciment polymethyl methacrylate (PMMA). Les avantages sont : une réduction optimale, une dévascularisation minimale, un respect des parties molles, ainsi qu’une remise en charge et une mobilisation précoces. Cette technique simple semble une bonne alternative au traitement conventionnel. Elle permet de prendre en charge les fractures les plus comminutives ainsi que les tassements les plus postérieurs, tout en étant le moins délabrant possible. L’arthroscopie permet de vérifier la réduction de la fracture et d’éventuelles fuites de ciment. Elle permet, par la même occasion, de réaliser le bilan des lésions méniscales associées et de les prendre en charge si nécessaire.

The study aimed to assess the performance of a new ultrasound sign for correct intubation, the increase in the antero-posterior diameter of the trachea during cuff inflation (Fig. 1), by an experienced sonographer and its learnability among novices after a short training course. Freshly embalmed cadavers from people having donated their bodies to science were used in these studies, in accordance with French law (Collection number: DC-2008-137). Twenty freshly embalmed cadavers were randomly intubated correctly or not three times, leading to 60 assessments (30 tracheal intubations, 15 bronchial intubations, 15 oesophageal intubations). Correct tracheal intubation was defined as tube tip located into the trachea between the glottis and the carina. Incorrect intubation was defined as tube tip located either in the oesophagus or in a bronchus. All tube tip positions were checked with a laryngoscope and a disposable fibrescope (Ambu®, aScope™, Ballerup, Denmark). A piece of tissue was placed on the cadaver's head to hide the tracheal tube so that the sonographer was blinded to the tube position. A second operator, with good experience in ultrasound and blinded to the position of the tube, determined if the tracheal tube was correctly positioned or not. Ultrasounds were performed using a 10 MHz ultrasound probe flat (Vivid-e® General Electric, UK) according to the manufacturer's instructions. Then, the cuff of the tracheal tube was filled with an ultrasound contrast solution, a mixture of 1 ml of air and 15 ml of gelatin (Gelofusine 4%®, B/BRAUN, Melsungen, Germany). Then, easiness to learn this method was assessed after a 20 min training course using 32 novices performing 64 assessments in eight cadavers in the same manner as in the observations made by experimented operator. The trachea deformation during cuff inflation allowed identification of correct intubation by an experienced sonographer with a sensitivity of 90% [95% confidence interval (CI) 83–95], specificity of 97% (95% CI 92–99), positive predictive value of 97% (95% CI 91–99), and negative predictive value of 91% (95% CI 84–95). Using the ultrasound criteria, novices identified correct intubation with a sensitivity of 91% (95% CI 76–97), specificity of 94% (95% CI 80–98), positive predictive value of 94% (95% CI 79–98), and negative predictive value of 91% (95% CI 76–97). The performance of students was not significantly different from that of the experienced operator (P>0.20 for all criteria). The method proposed to diagnose correct intubation was safe. Only one cuff rupture among 124 assessments (0.8%) was noted and all cuffs could be completely emptied at the end of the experiment. Large studies are required to define the place of this new tool compared with actual techniques. However, in situations where conventional techniques like chest X-ray or bronchoscopy are not available quickly, ultrasound may be useful to diagnose proper placement of the tracheal tube in combination with capnography. Indeed, capnography makes it possible to rule out oesophageal intubation in a few seconds (time to observe three capnograms), but cannot differentiate between tracheal intubation and selective intubation. The proposed ultrasound criterion may help to differentiate these two situations in a short time, in <1 min. L'urgentiste, confronté au diagnostic lésionnel devant une cheville traumatique, se doit de connaître les éléments de base de la physiologie de la consolidation des lésions, qu'elles soient ligamentaires ou ostéo-chondrales. Cette connaissance est un élément fondamental pour guider les indications thérapeutiques et les informations à donner au patient et à sa famille. Nous envisagerons les éléments de consolidation ligamentaire et ostéo-chondrale de l'adulte, puis ceux de l'enfant.
The authors present the concepts of simulation and its utilization in pediatrics. Simulation in medicine is a teaching method that has not yet been developed in Europe and has not spread in pediatrics in France. Motivations for simulation are first and foremost ethical: "Never the first time on patients!" Simulation also provides benefits in teaching communication skills and theoretical concepts. It is an essential means to maintain patient safety by limiting the risk of errors. It covers teaching procedures requiring realistic models such as in teaching communication and crisis resource management. Simulation can also be used for teaching disclosure of bad news, using actors. Simulation skills are acquired during debriefing, when the supervisor acts as a facilitator. Evaluation is mandatory in simulation, dependent on the how realistic the models are and on the performance of a procedure or multidisciplinary team management. Performance can be objectively assessed only with validated tools. Simulation will become a mandatory teaching method in medicine. (c) 2013 Elsevier Masson SAS. All rights reserved.
I traumi dell’avambraccio e del polso sono frequenti e, in genere, di diagnosi lesionale facile. La loro evoluzione e la loro gestione devono avere per obiettivo il mantenimento della funzione di pronosupinazione. È per questo che devono essere rilevati alcuni elementi, come le specificità del bambino, alcune forme cliniche particolari delle fratture dell’avambraccio e, infine, gli aspetti clinici dei traumi aperti (ferite del polso in particolare), tutti elementi che possono condurre l’urgentista in trappole diagnostiche. I microtraumi (tendinopatie) e le ustioni non sono qui presi in considerazione. I principi terapeutici e le indicazioni sono precisati per definire la condotta da tenere nel servizio di accettazione del Pronto Soccorso. Sono anche prese in considerazione le principali complicanze acute nei casi in cui il paziente ferito resta a volte in osservazione nel servizio di osservazione breve del Pronto Soccorso in attesa della sua ammissione al blocco operatorio.
La pratique médicale inclut un savoir théorique, un savoir-faire et un savoir-être. La simulation a un bénéfice dans ces trois domaines et représente un outil pédagogique considérable pour la médecine d’urgence. Les médecins anesthésistes ont été les pionniers de la simulation médicale. La simulation est une technique pédagogique, pas une technologie. Elle permet l’acquisition d’une compétence individuelle par l’utilisation de task-trainers jusqu’à développer une performance d’équipe. Elle concerne aussi bien la formation initiale que continue et vise à comprendre et à éviter les erreurs humaines, source de manque de performance. Le domaine de l’urgence offre de nombreuses possibilités d’utilisation des task-trainers pour s’entraîner aux gestes pratiques, notamment quand ils sont rares, avec un bénéfice de la simulation par rapport à l’enseignement uniquement théorique. L’utilisation de scénarios complexes avec des mannequins haute-fidélité améliore les compétences, le comportement et les interactions interindividuelles. La communication et le travail d’équipe sont les éléments essentiels de la performance d’une gestion de situation de crise (Crisis Resource Management). L’apprentissage par simulation doit être évalué selon un processus codifié jusqu’à atteindre l’évaluation de l’impact clinique. L’extension de la simulation est liée au développement d’une recherche mesurant son impact clinique, c’està-dire le passage du modèle de simulation au patient. L’essor de la simulation passe par la création de centres de simulation ainsi que la valorisation de la formation en équipe pluridisciplinaire. Les services d’urgence devraient être dans les premiers à bénéficier de cette nouvelle pédagogie. L’éthique, la technique et l’économie y seront gagnants.
