Background: Gukang Capsule has been used as a complementary and alternative medicine (CAM) for the treatment of primary osteoporosis (POP) in China. The primary aim of this study was to assess the clinical effectiveness and safety of Gukang Capsule in POP patients.Methods: A systematic search was conducted across multiple academic databases including PubMed, Web of science, Cochrane Library, China National Knowledge Infrastructure, Chongqing VIP Information, and Wanfang database to identify randomized controlled trials investigating the Gukang Capsule in the treatment of POP. The screening process, data extraction, and assessment of methodological quality were conducted independently by two reviewers. Statistical analysis was performed using the Rev Man 5.3 software. Subgroup analysis was carried out through the combination of OPF. Subgroup analysis was performed according to whether OPF were combined. Stata 12.0 was used for sensitivity and bias analysis.Results: Nineteen studies were assessed that included 1804 participants. It was found that compared with the control group, the total effective rate (RR = 1.26, 95% CI, 1.20, 1.33), the Medical Outcomes Study Short-form 36 [RR = 1.26, 95% CI(1.20, 1.33)], the bone mineral density (BMD) of lumbar vertebra (SMD = 0.77, 95% CI, 0.48, 1.07), the BMD of femoral neck [SMD = 0.84, 95% CI(0.53, 1.14)], and the BMD of Ward’s triangle (SMD = 0.64, 95% CI, 0.44, 0.85) of the Gukang Capsule experimental group were higher. Compared with the control group, the fracture healing time (SMD = −2.14, 95% CI, −2.45, −1.84), the bone specific alkaline phosphatase (BALP) levels in serum (SMD = −2.00, 95% CI, −2.83, −1.17), the tartrate resistant acid phosphatase 5b (TRACP-5b) levels in serum (SMD = −2.58, 95% CI, −3.87, −1.29) of the Gukang Capsule experimental group were lower. The bone glaprotein (BGP) levels in serum (SMD = −0.22, 95% CI, −1.86, 1.43) and the adverse events (RR = 0.80, 95% CI, 0.40, 1.63) of the experimental group and the control group have no difference.Conclusion: Gukang Capsule, as a CAM for the management of POP, exhibits the potential to enhance BMD and quality of life, expedite the healing time of OPF, diminish levels of BALP and TRACP-5b, and improve the total effective rate without increasing the adverse events.Systematic Review Registration:https://www.crd.york.ac.uk/prospero/display_record.php?ID=CRD42023477774, PROSPERO CRD42023477774.
目的 观察推拿功法"少林内功"对阳虚体质者督脉红外热像的效应,从督脉代谢热值的角度探讨推拿功法"少林内功"的效应机制.方法 纳入阳虚质志愿者30名(男16,女14名),先进行推拿功法"少林内功"的规范化培训后开始试验.功法训练时间:每次训练60 min,每周5次,共持续12周.采用红外热像技术分别于干预前后检测红外辐射轨迹以及大椎穴、命门穴与督脉循行体表的温度.结果 干预后受试者的督脉循经红外辐射轨迹被诱发,与干预前相比,长度更长、范围更大、更加连续.干预后大椎穴、命门穴及督脉线上体表代谢温度较干预前明显升高(P<0.01).结论 推拿功法"少林内功"可以提高阳虚质者督脉的经穴皮肤温度,增强督脉经络能量代谢.
Senile osteoporosis (SOP) is a common disease that has decreased bone strength as its main symptom. There is currently no medication that can treat SOP, and traditional Chinese medicine has advantages in slowing down bone aging. The present study aimed to observe the effects of Qing'e decoction on leptin, leptin receptor, sex hormone, and biochemical markers of bone metabolism in naturally aging rats and to explore its mechanism in regulating bone metabolism. The results revealed that, with the increase in age, the bone mineral density (BMD), bone strength, bone trabecula sparse, serum levels of leptin receptor (LEP-R), estradiol (E2), testosterone (T), core binding-factor α-1 (Cbfα-1), collagen-I (COL-I) and osteocalcin (OC), and the mRNA levels of leptin (LEP) and LEP-R in bone tissue decreased, while serum LEP levels increased in the female and male NS groups. The serum levels of LEP, LEP-R, E2, T, osteoprotegerin, Cbfα-1, COL-I, OC and bone alkaline phosphatase, and the mRNA levels of LEP and LEP-R in bone tissue in the female and male QED groups were higher than those in the same age and sex NS group, while the BMD, bone trabecular area percentage, maximum load, and maximum stress in the female and male QED groups were significantly higher than those in the same age and sex NS group. In conclusion, with the increase in age, the bone quality of naturally aging rats decreased gradually. Qing'e decoction can regulate the bone metabolism and increase the bone quality and delay bone aging, which may be achieved by increasing sex hormone, LEP, and LEP-R levels.
目的 观察健骨颗粒含药血清对去卵巢模型小鼠BMSCs成骨分化的影响,并研究其调控miR-141影响BMSCs分化的可能机制.方法 流式细胞计数法鉴定BMSCs;含药血清和空白血清分别干预体外培养的BMSCs;显微镜下观察细胞形态结构,碱性磷酸酶染色、定量检测,茜素红染色;RT-PCR检测miR-141及DLx5、Msx2、Runx2的基因表达情况;Western blot检测DLx5、Msx2及Runx2的蛋白表达情况.结果 流式细胞计数法鉴定提取细胞符合实验要求;镜下观察发现两组细胞有差异,含药血清组细胞群落间结晶更为密集;ALP染色及定量、茜素红染色结果含药血清组均优于空白血清组;RT-PCR检测Dlx5、Msx2、Runx2及miR-141的基因表达结果:含药血清组miR-141和Msx2基因表达较空白血清组显著降低(P<0.01),而Dlx5、Runx2的基因表达显著升高(P<0.01);Western blot检测DLx5、Msx2、Runx2的蛋白表达结果:含药血清组较空白血清组相比,Msx2的蛋白表达略微下降(P<0.05),而Dlx5、Runx2的蛋白表达相对升高(P<0.01).结论 健骨颗粒可以调低miR-141的表达,进而调高Dlx5的表达,削弱Dlx5的同源异形基因Msx2对Runx2抑制作用,通过miR-141调控Dlx5/Msx2/Runx2信号通路促进BMSCs成骨分化达到预防和治疗绝经后骨质疏松的目的.
Objective:To observe the expression difference and change trend of miR-141 and osteogenic gene Dlx5, Msx2 and Runx2 in osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs) in the ovariectomy model group and the sham operation group, and investigate the mechanism of postmenopausal osteoporosis.Methods:Twenty 4-week-old clean C57 female mice were selected as the model group and the sham operation group according to the random number table method, and ten cases in each group. The mouse model of postmenopausal osteoporosis in the model group was established by ovariectomy, and the ovaries were preserved and only the fat around the ovaries were removed in the sham operation group. The mice in the model group were identified and successfully modeled two months after modeling, then BMSCs of mice in the two groups were isolated and cultured through whole marrow culture method. The morphology of the cells was observed and the cell phenotype was identified by flow cytometry. The osteogenic differentiation of BMSCs was observed by alkaline phosphatase staining, quantification and alizarin red staining on the 3rd, 8th and 13th day, respectively to observe the osteogenic ability of BMSCs. The expression of miR-141 and osteogenic genes Dlx5, Msx2 and Runx2 were detected by q-PCR on the 3 rd, 8th and 13th day after osteogenic induction of BMSCs. The protein expression of osteogenic gene was detected by Western blot.Results:Positive expression of CD29 and CD44 and negative expression of CD34 and CD45 in BMSCs were found by flow cytometry. After osteogenic induction of BMSCs in both groups, the expression of ALP gradually increased, reaching a peak on the 8th day, and then gradually decreased, showing a"∧"type change. The model group was lighter than the sham operation group, and the result of activity detection was consistent with the degree of staining. The expression of ALP in model group was lower than that of the sham operation group after osteogenic differentiation on the 3rd, 8th and 13th day (P< 0.05). When the two groups of cells were stained with alizarin red, calcified nodules appeared on the 8th day, and the number of calcified nodules increased on the 13th day. There was no significant difference in the number of calcified nodules between the model group and the sham operation group on the 3rd and 8th day of osteogenic induction. On the 13th day, compared with the sham operation group, the number of calcified nodules in the model group was less, and osteogenic differentiation slowed down significantly. On the 3rd, 8th and 13th day of osteogenic induction, the expression of Dlx5 and Runx2 were increased continuously, and miR-141 and Msx2 were decreased continuously (P< 0.05). Compared with the 3rd day, the indexes of the 8th day and the 13th day were significantly different (P< 0.05). Comparison of between groups showed that the expression of miR-141 and Msx2 in the model group was higher, and the expression of Dlx5 and Runx2 was lower than those in the sham operation group (P< 0.05).Conclusion:The ability of osteogenic differentiation in BMSCs is affected by ovariectomy. MiR-141 and Msx2 are negative regulators, and Dlx5 and Runx2 are positive regulators in osteogenic differentiation of BMSCs.
Objective:To observe the effect of Jiangu granule on the expression of miR-141, Dlx5, Msx2 and Runx2 in bone tissue of ovariectomized mice, and to explore the mechanism of Jiangu granule in preventing and treating postmenopausal osteoporosis.Methods:A total of 30 C57 mice were randomly divided into sham operation group, saline group and Jiangu granule group, with 10 cases in each group. Fat around ovary of the sham operation group was removed, but not ovary, bilateral ovaries of the saline group and Jiangu granule group were removed surgically, and administrate respectively by normal saline and Jiangu granule.After eight weeks of intervention, the tibial morphology was observed by micro-CT, the expression of microRNA-141, Dlx5, Msx2 and Runx2 in bone tissue were detected by real-time PCR, protein contents of Dlx5, Msx2 and Runx2 in bone tissue were detected by Western blot.Results:Compared with the saline group, the number, width, length, shape and distribution of trabeculae in the Jiangu granule group recovered partially, and the density and connectivity of trabeculae increased, which could not be restored to the level of the sham operation group, The area of cortical bone increased, the bone marrow cavity decreased, and basically restored to the level of the sham operation group; BMC, BMD, BV/TV, BS/TV, Tb.N increased significantly; Tb.Sp, SMI reduced significantly;the relative amount of microRNA-141 and Msx2 in bone tissue decreased significantly;the relative contents of Runx2 and Dlx5 in bone tissue increased significantly, with statistically significant differences (P<0.05).Conclusion:Jiangu granule can effectively reduce the expression of miR-141, enhance the expression of key osteogenic factors such as Dlx5 and Runx2 by influencing BMP signaling pathway Dlx5/Msx2/Runx2 in bone marrow mesenchymal stem cells (BMSCs), and promote osteogenic potential, so as to prevent and treat osteoporosis.
目的:研究健骨颗粒乙酸乙酯萃取部位对卵巢切除大鼠骨质量的影响.方法:采用切除雌性SD大鼠双侧卵巢建立绝经后骨质疏松症模型.运用双能X线4500W骨密度仪测定骨密度,万能材料试验机检测骨组织的最大载荷.骨组织石蜡包埋切片后行Masson染色,观察骨组织形态,并做骨小梁面积百分比计量.通过酶联免疫吸附法检测血清中的骨碱性磷酸酶(BALP)、雌二醇(E2)、抗酒石酸酸性磷酸酶(TRAP)的含量.结果:与空白组比较,乙酸乙酯组术后4、12周BMD显著升高(P<0.01),术后12周胫骨三点抗弯最大载荷显著升高(P<0.01),术后8、12周腰椎、胫骨近干骺端骨小梁面积百分比及血清E2水平显著升高(P<0.01),术后12周血清BALP水平显著升高(P<0.01),术后8周血清TRAP水平显著下降(P<0.05).结论:健骨颗粒乙酸乙酯萃取部位能有效的改善卵巢切除术后大鼠机体的骨代谢水平,改善骨质量.
目的:观察健骨颗粒对去卵巢小鼠骨组织中miR-141、Dlx5、Msx2、Runx2表达的影响,探讨健骨颗粒防治绝经后骨质疏松的作用机制.方法:30只C57小鼠按照随机数字表法分为假手术组、生理盐水组和健骨颗粒组,每组10只.假手术组只切除卵巢周围脂肪,不切除卵巢;生理盐水组和健骨颗粒组均手术切除双侧卵巢,并分别灌服生理盐水和健骨颗粒.干预8周后处死取材,采用micro CT观察胫骨形态并计量;realtime PCR检测骨组织miR-141、Dlx5、Msx2、Runx2等mRNA的表达;Western blot法检测骨组织Dlx5、Msx2、Runx2等蛋白含量.结果:与生理盐水组比较,健骨颗粒组骨小梁数量、宽度、长度、形态和分布部分恢复,密度和联结性有所增加,仍无法恢复到假手术组水平,但皮质骨面积增加,骨髓腔缩小,基本恢复到假手术组水平;骨矿物质含量(BMC)、骨密度(BMD)、骨体积分数(BV/TV)、骨表面积和骨组织体积比值(BS/TV)、骨小梁数量(Tb.N)均明显升高;骨小梁分离度(Tb.Sp)、结构模型指数(SMI)指标均明显降低;小鼠骨组织miR-141、Msx2相对含量明显降低;骨组织Runx2、Dlx5相对含量明显升高,差异具有统计学意义(P<0.05).结论:健骨颗粒可以有效降低miR-141的表达,并通过影响骨髓间充质干细胞(BMSCs)成骨分化BMP信号通路Dlx5/Msx2/Runx2信号轴来提高Dlx5、Runx2等成骨关键因子的表达,提高成骨能力,达到预防和治疗骨质疏松的目的.
Objective To explore the distribution of constitutional types in patients with metabolic syndrome under the guidance of the theory of constitution in Chinese medicine,so as to provide an objective basis for the prevention and treatment of metabolic syndrome.Methods Using the case-control research method,60 patients under the diagnostic standard of metabolic syndrome were studied with 60 healthy check-up people as normal controls.With the single-factor logistic regression analysis and multivariate logistic regression analysis,this study explores the correlation between independent variables (such as gender,age,family history,occupation,education level,smoking,drinking,greasy and thick diet,exercise,mental stress and physical type) and metabolic syndrome,and clarifies the influence of physical type in causal factors of metabolic syndrome.Results tingle factor logistic regression analysis shows that independent variables phlegm-wetness type (OR=23.727,95%CI=8.223-68.461),yin-deficiency type (OR=5.515,95%CI=2.185-13.925),qi-depression type (OR=3.574,95%CI =1.374-9.299) were selected as model.Multivariate logistic regression analysis showed that phlegm dampness,alcohol consumption and metabolic syndrome were highly correlated,and the corresponding odds ratios were 20.016 and 19.834,respectively.Positive exercise has a significant negative corretion with the incidence of metabolic syndrome,with the ratio of-38.115.Conclusion Phlegm-wetness type,yin-deficiency type and qi-depression type are the main constitution related factors of metabolic syndrome.Smoking,drinking,greasy and thick diet and mental stress are positively correlated to metabolic syndrome,and are risk factors in the pathogenesis of the disease,while exercise and metabolic syndrome had a negative correlation.

目的:探讨南少林手法在颈性冠心病的治疗作用、效果和治疗机理。方法:将60例患者随机分为南少林手法治疗组和普通推拿治疗对照组,观察二组患者治疗前后的临床疗效并分析其作用机理。结果:南少林治疗组治愈22例,有效6例,无效2例,总有效率93.3%,疗效优于对照组(p<0.05)。结论:南少林手法疗效显著优于普通推拿治疗。
颈椎病是一种常见病和多发病,近10年来发病率正在逐年提高,且呈现低龄化特点.我们统计2010年至今在福建中医药大学国医堂、福建中医药大学附属第三人民医院和福建省中医药研究院门诊部就诊的颈椎病患者中,青年期颈型颈椎病占了1/3以上.我们采用南少林推拿疗法治疗青年期颈型颈椎病45例,收到良好的效果,现报道如下.

With the further study about pathogenesis of steroid-induced necrosis of femoral head,it is possible to maintain the patients′own articulus by giving early intervention to this disease.This article reviews some methods used by domestic and abroad scholars,such as durgs,bone tissue engineering,and physical treatment and so on,in order to provide new idea for the therapeutic method preserving the own articulus of patients with this disease.
