目的 探讨基于根因分析法的针对性干预在复杂胸主动脉夹层腔内隔绝术围手术期患者护理中的应用效果.方法 选取2020年6月—2022年6月在郑州大学第一附属医院行腔内隔绝术治疗的84例复杂胸主动脉夹层患者,采用随机数字表法分为对照组与观察组,每组42例.对照组患者围手术期采用常规护理干预,观察组患者在对照组基础上采用基于根因分析法的针对性干预.比较2组患者的护理满意度,干预前后的心理状态及术后并发症发生情况.结果 观察组患者干预后健康教育、护理态度、护理环境及工作效率评分均高于对照组,差异均有统计学意义(P<0.05).2组患者干预前焦虑自评量表(SAS)及抑郁自评量表(SDS)评分比较,差异均无统计学意义(P>0.05);2组患者干预后SAS及SDS评分均低于干预前,且观察组低于对照组,差异均有统计学意义(P<0.05).观察组患者术后并发症发生率为7.14%,低于对照组的23.81%,差异有统计学意义(x2=4.459,P=0.035).结论 对复杂胸主动脉夹层腔内隔绝术围手术期患者进行基于根因分析法的针对性干预效果显著,能有效改善患者心理状态,降低术后并发症发生风险,且患者护理满意度高,值得推广.
基于多年教学经验,发现近年来药剂学和药动学的发展对生物药剂学与药物动力学的一些理论和应用产生了重要的影响,因此对《生物药剂学与药物动力学》教材部分内容提出编写建议.
目的 比较强脉冲光(intense pulsed light,IPL)技术联合A型肉毒毒素注射治疗与单纯IPL治疗面部轻中度痤疮的疗效.方法 回顾性分析118例面部轻中度痤疮患者的临床资料.根据治疗方法分为两组,每组59例.对照组单纯采用IPL治疗2次,每次治疗间隔4周;观察组给予A型肉毒毒素15~50 U多点微量皮内注射治疗,4、8周后各行1次IPL治疗.同时采用ELISA法检测治疗前、治疗结束后4周血清中炎症因子IL-6、TNF-α的含量,并对治疗结果进行分析比较.结果 118例患者获随访1~3个月.观察组有效率(86.44%)高于对照组(67.80%),其差异有统计学意义(P<0.05);治疗后4周两组血清中IL-6、TNF-α含量均低于治疗前(P<0.05);观察组低于对照组(P<0.05).结论 IPL联合A型肉毒毒素注射治疗面部轻中度痤疮的疗效优于单纯采用IPL治疗,是一种有效的治疗方法.
在面临环境保护,建筑质量效率提升以及农民工产业化转型等诸多问题下,我国开始大力推广装配式建筑的发展.在装配式住宅的应用中,轻质隔墙条板具有广阔的空间,但依然面临诸多问题,为了体现其标准化生产、性能、绿色的优势,与装配式建筑整体有机结合,需要从设计生产施工各个阶段进行规范生产、严格把控,从而能够在装配式住宅中广泛推广.
本文将"生物药剂学与药物动力学"这门课分为两部分,分别针对性提出教学改革建议.对"生物药剂学",主要实践"翻转教学"、"情景教学"、"双语教学"和最新文献教学等新的教学方法;而对"药物动力学",主要实践以实验教学带动理论教学,并在实验教学中增加开放型实验.结果表明,学生学习积极性和课堂参与感明显增强,学生的专业英语水平和科研思维能力明显增强,明显提高了教学质量和效果.
综述了近年来多孔泡沫金属强化池沸腾换热的实验研究,从多孔泡沫金属的材料、厚度、孔密度、孔隙率及泡沫金属复合槽道对强化传热性能的影响进行总结,并对多孔泡沫金属强化沸腾换热的研究方向进行展望.
基于WORKBENCH,研究了雨滴型凹坑流道换热板的流动换热性能,并对其进行多目标优化研究.以努塞尔数Nu,阻力系数f,综合传热性能η为目标函数,考察了凹坑的前缘半径R1,后缘半径R2,凹坑深度Hd,凹坑间距P等结构参数对雨滴型凹坑流道各优化目标的影响.结果表明:雨滴型凹坑流道内的流动分离和涡流碰撞增强了湍流强度,有效强化了传热,并且高传热区出现在凹坑后缘.雨滴型凹坑板的Nu是平板的1.9~4.5倍,阻力系数是平板的2.3~3.5倍,η可达1.42~3.04.凹坑深度Hd对Nu、f、η影响都比较显著,而凹坑间距P对这三者的影响都比较小,当3.5 mm<Hd<4.5 mm且25.0 mm<R1<30.0 mm时有较好的η.通过对雨滴型凹坑板在1358≤Re≤6926的数据结果进行关联,得到了凹坑板流动换热的准则关系式.
Objective To determine whether intrauterine human chorionic gonadotropin (hCG) infusion before embryo transfer impacts the in vitro fertilization-embryo transfer (IVF-ET) pregnancy outcomes.Methods Databases included PubMed,EMBase,ISI Web of Science Cochrane Library,CNKI,Wanfang and VIP were searched for the study of the effect of hCG on pregnancy outcome.The retrieval period was from the date of database construction to July 2017.The literature screening according to the inclusion and exclusion criteria,followed by quality evaluation and data extraction.The RevMan 5.3.5 software was used for data analysis.Results Twelve randomized controlled trials were analyzed.The results showed that there was a significant increase in clinical pregnancy rates (OR=1.24,95% CI=1.08-1.43) and implantation rate (OR=1.21,95% CI=1.00-1.46) in intrauterine hCG group compared with control group,but no significant difference in live birth rate (OR=1.05,95% CI=0.81-1.36) and abortion rate (OR=1.02,95% CI=0.78-1.34).Subgroup analysis showed that,for the patients who received the intrauterine hCG ≥ 500 IU before cleavage stage embryo transfer,the implantation rate (OR=1.61,95% CI=1.32-1.97),the clinical pregnancy rates (OR=1.46,95% CI=1.26-1.70) and the live birth rate (OR=1.52,95% CI=1.27-1.84) were greater than those in controls.Conclusion The intrauterine perfusion of hCG can improve pregnancy outcomes of patients with embryo transfer.
目的:建立HPLC法测定长春胺缓释胶囊的含量及有关物质.方法:采用Agela Durashell C18(L)色谱柱(4.6 mm× 250mm,5μm),流动相为乙腈-0.1 mol· L-1碳酸铵溶液(65∶35),流量为1.0 mL· min-1,柱温为30℃,检测波长为280 nm.结果:在选定的色谱条件下,主成分峰与各杂质峰之间的分离度良好,长春胺在9.66 ~72.48 μg· mL-1的浓度范围内与其峰面积呈良好的线性关系(r=0.999 9),平均回收率为100.27%,其RSD =0.81%(n=9).结论:所建方法快速简便,灵敏度高,专属性强,可用于长春胺缓释胶囊的质量控制.
Objective Discussion of diagnosing insulinoma.Methods Retrospective analysis based on 45 cases of insulinoma patients that are collected by General Surgery Department in the First Affiliated Hospital of Zhengzhou University from June 2008 to October 2014.Results Forty-five cases of patients with insulinoma including 41 cases (91.11%) of functional insulinoma and 4 cases (8.89%) of non-functional insulinoma.Apply selective arteriography,preoperative ultrasound,intraoperative ultrasound and CT to diagnosis.Forty-five cases of patients with insulinoma,in which 18 cases with laparoscopic simple tumor resection,6 cases with laparoscopic splenectomy and tail of the pancreas resection;45 cases of insulinoma with laprotomy,in which 8 cases with pancreaticoduodenectomy,12 cases which the tumors were located in the neck,neck-line of the pancreas with local excision of the pancreas and pancreatic tail-jejunal Roux-en-Y anastomosis,one cases of laparotomy found no placeholder.Conclusions Surgery removal is currently the most successful way to eliminate insulinoma.And localization diagnosis can determine the outcome of the surgery.Co-operation of surgery removal and localization diagnosis can be the optimal approach to diagnose certain insulinoma.
目的 制备Flavokawain A脂质体并考察其理化性质.方法 采用薄膜分散法制备Flavokawain A脂质体,在单因素考察的基础上,采用正交实验设计优化最佳处方和工艺;纳米粒度仪测定其粒径和电位分布,葡聚糖凝胶柱法分离脂质体与未包裹的药物,HPLC法测定包封率和载药量,透析法探讨其体外释放行为.结果 通过单因素和正交实验,筛选出制备FKA脂质体的最佳处方为磷脂180mg,胆固醇60mg,FKA3mg,37℃成膜1h,55℃水化1.5h,300W超声90秒,制备的FKA脂质体的平均粒径为153.28±11.36nm,平均电位为0.42±0.11mV,平均包封率为(87.57±5.49)%,FKA脂质体在体外约48h释放80%,为原料药的释放时间的4倍.结论 采用薄膜分散法制备的Flavokawain A脂质体,包封率高,稳定性好,且具有一定的缓释性.
目的 探讨腹腔镜下大子宫全切术的可行性及临床应用价值.方法 回顾性分析腹腔镜下大子宫全切术45例患者的临床资料.结果 45例均在腹腔镜下完成手术,无1例中转开腹,手术时间平均75 min(50~130 min),术中出血量平均50mL(30 ~ 170 mL),术后平均住院时间5d(3~7d).结论 腹腔镜下大子宫切除术安全可行,不会增加手术危险性和手术并发症.

患者女,19岁,因停经6个月,多尿、多饮40 d,间断发热15 d于2011年9月入院.6个月前无明显诱因停经,无泌乳.40d前出现烦渴、多尿、多饮,日饮水量增至约8~10 L,每l~2h排尿1次,每次尿量约400~500 ml,伴恶心、呕吐、间断头痛,无视力下降,无口干、眼干.既往无腮腺炎及龋齿病史.曾应用"黄体酮针"治疗,月经未来潮,当地医院行垂体磁共振成像(MRI)检查示:垂体柄略增粗.15d前受凉后出现发热,多出现在午后,最高体温达38℃,伴畏寒、咽痛、乏力,至当地诊所间断输液(具体药物不详),体温可降至正常,余症仍存.5d前于我院行头颅MRI平扫显示垂体柄增粗.发病以来体质量下降约4.5 kg.既往史:间断头痛2年,对症支持治疗效差.月经生育史:13岁月经初潮,月经规律.未婚未育.家族史无特殊记载.体格检查:体温37.2℃,脉搏76次/min,呼吸19次/min,血压118/93 mm Hg(1 mm Hg=0.133 kPa).
药学院开设《基因工程药物》公选课具有重要的意义,但在教学过程中也存在一些不容忽视的问题,改进对策一是端正认识,提高兴趣;二是重在普及,强调重点;三是加强交流,尝试讲座式教学;四是利用多媒体手段,加强启发式教学。
Objective To investigate the intravenous lipid emulsion of lomefloxacin aspartate and evaluate its stability. Methods After lecithin in glycerin and poloxamer were dissolved in purified water, heated by ultrasound machine then gently mixed with the oil phase at the same temperature, dispersed in tissue grinder before homogenized to the required standard. Its stability of the normal experiments was tested. Results Pharmacy properties of the lipid emulsion were studied, the average particle size was (162.50±9.32)nm; Zeta potential was (-30.67±1.93)mV and pH value was 7.32±0.02, all consistent with the intravenous standards of Pharmacopoeia 2010.Its stability was tested and verified by normal stability test and there was no significant change occurred. Conclusion The production process is simple and agent consumption is moderate and basically atoxic. The quality of the final product is stable. It could provide foundations for further animal experiments and clinic researches.
目的分析建立紫外分光光度法,测定动物组织中盐酸克伦特罗的含量。方法对盐酸克伦特罗标准溶液进行全波长扫描,以确定最佳吸收波长,在此波长处制作盐酸克伦特罗标准曲线,进行动物组织中盐酸克伦特罗的含量测定。结果在95%乙醇溶液中,吸收波长为247nm,盐酸克伦特罗在4.84~38.72mg/L范围内线性良好。结论该方法操作简便,耗材少,其准确性、线性关系良好。
Aim:To explore the effects of resveratrol (RES) on the learning memory performance and oxidative stress in lead exposure mice. Methods: Fifty Kunming mice were randomly divided into five groups, which were control group, lead exposure only group (Pb), low (L), middle (M), and high (H) dose RES interfering groups. The mice were allowed freely to drink water. After eight weeks feeding, Morris water maze (MWM) test was employed to evaluate the learning memory ability of the mice. The lead levels in blood and hippocampus were analyzed by graphite furnace atomic absorption spectrometry. Serum and hippocampal SOD activity and MDA level were measured. Results: MWM test showed that the escape latency among the 5 groups had significant differences (F=9.044,P0.001). The lead levels in blood and hippocampus had significant differences (F=24.142 and 17.511,P0.001).Compared with Pb group,the lead levels in hippocampus of RES interfering groups reduced (P0.05). The SOD activity in blood and hippocampus showed significant differences among groups (F=35.190 and 11.933,P0.001). The MDA levels also showed significant differences among groups (F=5.610 and 4.406,P0.05). Compared with control group,the serum and hippocampal SOD activity significantly decreased in Pb group and the three RES interfering groups (P0.05). Compared with Pb group,the SOD activity in M and H groups significantly increased (P0.05). Conclusion: RES could reduce the lead levels in blood and hippocampus, reverse the learning memory damage in lead exposure mice,as well as increase SOD activity and reduce the level of MDA.

Objective To investigate the protective effect of zinc in the lead exposed mice through observing the blood and hippocampal lead and zinc levels,hippocampal BDNF mRNA expression,and the spatial learning memory performance.Methods The 60 mice were equally divided into five groups,i.e.the control group,lead exposed group,low dose zinc intervention group,medium dose zinc intervention group,and high dose zinc intervention group.Under the same feeding condition,the mice were provided with distilled water,1.0 g/L lead acetate,0.2 g/L zinc acetate+1.0 g/L lead acetate,0.4 g/L zinc acetate+1.0 g/L lead acetate,and 0.6 g/L zinc acetate+1.0 g/L lead acetate.Morris water maze(MWM)test was employed for evaluating the spatial learning memory ability in the mice.After eight weeks,the levels of lead and zinc in blood and hippocampus,and the hippocampal BDNF mRNA expression were determined.Results The escape latency increased in lead and low dose groups(P0.01),while the moderate and high doses groups showed shorter escape latency than those of the lead group and low dose group(P0.01).Compared to the control,the difference was not statistically significant(P0.05).The expression level of hippocampal BDNF mRNA in lead exposed group was the lowest and in the control group was the highest,and also the expression was positively correlated with the doses of zinc intervention.The expression levels of hippocampal BDNF mRNA in various zinc doses intervention groups were higher than that of the lead exposed group(P0.01).Conclusions Zinc supplement in certain amount could partially antagonize the elevation of lead and lowering of zinc in blood and hippocampal induced by lead exposure.Elevation of the expression level in hippocampal BDNF mRNA may mitigate the learning memory injury induced by lead exposure.
