BackgroundThe provision of high-quality healthcare services and patient satisfaction are fundamental objectives in modern healthcare. Humanistic nursing care, which emphasizes empathy, respect for individuality, and cultural sensitivity, aims to build trust and improve the overall experience for patients. This approach is especially relevant for rural patients in China, who often face additional challenges in accessing care in large tertiary hospitals.MethodsA multistage, stratified sampling method was employed to collect data from 8,263 patients aged 18 years or older in large public tertiary hospitals. Humanistic care satisfaction scores were measured using the Nurse Caring Instrument (NCI) questionnaire, a validated tool for assessing patient satisfaction with nursing care.ResultsSatisfaction with nursing humanistic care among rural Chinese patients attending large tertiary public hospitals was low with the overall mean satisfaction score 81.62 ± 16.85. Significant differences in satisfaction were found based on age, marital status, number of children, educational attainment, occupation, monthly household income, department visited, type of medical insurance, and first-time visitor. A multivariate analysis revealed positive correlations with satisfaction for factors such as having children, higher education, higher family monthly income, and first-time visitor, and negative correlations for factors such as older age, being widowed, department visited, and region.ConclusionOlder adults, widowed individuals, and first-time patients expressed lower levels of satisfaction, highlighting the need for tailored interventions. The findings provide insights into the impact of humanistic nursing care for rural patients and emphasize the importance of culturally sensitive approaches to improve patient satisfaction in rural China. This study has several limitations. The cross-sectional design restricts the ability to establish causal relationships, and there is a potential for selection bias, as participants who completed the survey may have higher educational and economic levels, possibly leading to an overestimation of satisfaction. Lastly, as this study focused on rural patients in large public tertiary hospitals in China, the findings may not be generalizable to other settings or patient groups. Future studies should address these limitations for broader applicability and insight.
Purpose: Nursing is a representative profession with a high prevalence of presenteeism, which is usually associated with negative outcomes. Therefore, it is important to explore the antecedent factors of nurse presenteeism behavior. This study aims to explore the impact and mechanism of head nurses' cognitive preference toward presenteeism on subordinate nurses' presenteeism (SNP), and the mediation effects of subordinate nurses' perception of head nurses' cognitive preference. Patient and Methods: A cross-sectional study was conducted from July to August 2022. A total of 256 head nurses and 1424 subordinate nurses were recruited from six hospitals located in Zhengzhou, Henan Province, China. The Nurse Presenteeism Questionnaire (NPQ) and Cognitive Preference Questionnaire were used to assess head nurses' cognitive preference toward presenteeism, SNP, and subordinate nurses' perception of head nurse's cognition of presenteeism. We conducted description, multilevel correlation, and multilevel structural equation models for the data analysis. Results: In the past 6 months, 93.4% of nurses experienced presenteeism. Within and between the team, head nurses' cognitive preference toward presenteeism is positively associated with SNP. The mediation effect of nurses' perception of head nurses' cognitive preference was also significant. Conclusion: Head nurses' cognitive preferences are essential predictors of subordinates' presenteeism. In this process, subordinates' subjective initiative plays a crucial role. Implications for Nursing Management: Hospital managers should focus on head nurses' values to formulate multiple interventions of presenteeism and strengthen communication between leaders and subordinates to promote transforming negative outcomes into positive outcomes.
This study aims to observe the current situation of nurses' presenteeism and the relationship between presenteeism among nurses and patient perceptions and examine its implications for nursing management. The study design was quantitative, correlational and cross-sectional. The researchers used convenience samples of nurses and patients from five hospitals who agreed to participate in an online survey distributed using Sojump Survey Software. A total of 500 in-service nurses from five tertiary hospitals in Henan Province in China were recruited as the nurse participants. Among them, 433 met the inclusion criteria and completed the general information questionnaire and the presenteeism scale. Patients who were hospitalised for three days or more and were cared for by one or more nurse participants were included in the study. In total, 435 patient participants answered the Inpatient Experience Questionnaire. The responses collected from both groups were analysed using descriptive and inferential statistics.Nurses' presenteeism was a key factor that affected patient experience. Presenteeism among nurses is a common phenomenon. Although patients' experience was overall positive, there is still room for improvement. Reducing presenteeism among nurses is crucial for improving patient experience, creating harmonious nurse-patient relationships and sharing a common mission.
Objective:To investigate the current situation and influencing factors of patients′ satisfaction with nursing humanistic care, and to provide reference for improving the quality of such care provided by hospitals.Methods:From July to August 2022, outpatients and inpatients in 30 provinces were selected by multi-stage stratified sampling as the survey objects. A cross-sectional survey was conducted on an online platform, using the general information questionnaire and Chinese version of methodist health care system nurse caring instrument revised by the research group. The latter instrument consists of 12 dimensions. namely care coordination, competence, teaching/learning, emotional support, respect for individuality, physical comfort, availability, helping/trusting relationship, patient/family engagement, physical environment, spiritual environment and outcomes. Descriptive analysis was performed on the data collected by the questionnaires, and independent sample t-test and one-way ANOVA were used to analyze the influencing factors of patient satisfaction. Results:A total of 107 hospitals were selected for questionnaire survey, including 86 tertiary hospitals and 21 secondary hospitals, and 29 108 valid questionnaires were recovered. The patient satisfaction with nursing humanistic care scored (5.40±0.86); the top three dimensions were competence (5.50±0.89), emotional support (5.47±0.88) and helping/trusting relationship (5.46±0.86); the lowest scoring dimensions were teaching/learning (5.38±1.01), spiritual environment (5.36±1.04) and patient/family engagement (5.11±1.28). Differences with gender, age, marital status, child status, educational level, occupation, place of residence, economic region, per capita monthly income of the family, type of medical insurance, medical department visited and surgery or not presented significant differences on the patient satisfaction with nursing humanistic care scores ( P<0.05). Conclusions:The satisfaction of patients with hospital′s nursing humanistic care in China was at the middle to upper level. In the future, health education for patients should be strengthened, and a mode of family-engaged nursing humanistic care should be constructed in line with the Chinese cultural background. In the process of nursing services, the particularity of patient groups should be considered to better meet their needs.
BackgroundHumanistic care pertains to the abilities, attitudes, and behaviors central to patient-centered care, contributing to patients' sense of safety and wellbeing. This study aimed to assess the satisfaction of patients with humanistic nursing care in Chinese secondary and tertiary public hospitals.MethodsA national cross-sectional survey was conducted across 30 provinces and 83 hospitals in China. Patient satisfaction with humanistic care was assessed using the Methodist Health Care System Nurse Caring Instrument (NCI), which encompasses 20 items across 12 dimensions. Each item was rated on a 7-point Likert scale, yielding a total score of 140. Multiple linear regression analysis was employed to identify factors associated with patients' satisfaction.ResultsModerate satisfaction (mean score 91.26 ± 13.14) with humanistic nursing care was observed among the 17,593 participants. Factors significantly associated with patient satisfaction included age, hospital type, presence of children, educational attainment, place of residence, family monthly income, and medical insurance type.ConclusionThe study findings highlight the importance of tailored interventions, evidence-based practice guidelines, and patient-centered care in improving patients' satisfaction with humanistic nursing care. Continuous emphasis on nursing education and professional development is crucial for enhancing humanistic care and patient satisfaction.

Objective To investigate patients’satisfaction rate towards referral in a class A tertiary hospital,so as to provide the evidence for promoting the highly efficient implementation of referral.MethodsThe gender,age,type of re-ferral,referral efficiency and referral cost of patients receiving referral services in 2021 were collected through the inter-connected intelligent hierarchical diagnosis and treatment collaborative platform of Henan Provincial People’s Hospital.Patients’awareness and recognition of the two-way referral system were investigated using questionnaire surveys,and fac-tors affecting patients’satisfaction with referral were identified using a multivariable logistic regression model.Results A total of 1 268 patients receiving referral services were surveyed,including 576 men (45.43%) and 692 women(54.57%),and the respondents were predominant at ages of 18 years and older (935 cases,73.74%).There were 535cases with overdue referral (42.19%) and 1 157 cases with free referrals (91.25%).There were 749 cases that were aware of the two-way referral system (59.07%),and 587 cases recognized the two-way referral system (46.29%).There were 975 patients with a satisfactory attitude towards referral,with a satisfaction rate of 76.89%.There were 1 030 cas-es (81.23%) that considered fluent communication of referral information,with a 78.54%satisfaction rate towards refer-ral;569 cases (44.87%) that considered simple referral procedures,with an 81.37%satisfaction rate towards referral,and 797 cases that were satisfied with the services provided by the referred medical staff (62.85%),with a 79.42%sat-isfaction rate towards referral.Multivariable logistic regression analysis showed that non-overdue referrals (OR=1.846,95%CI:1.411-2.414),free referrals (OR=1.815,95%CI:1.188-2.773),awareness of the two-way referral system (OR=0.624,95%CI:0.472-0.826),recognition of the two-way referral system (OR=1.621,95%CI:1.235-2.129) were factors affecting the satisfaction with referral.ConclusionsThe satisfaction rate towards referral was 76.89%among the study patients,and the awareness and recognition of the two-way referral system,referral efficiency and convenience are fac-tors affecting the satisfaction with referral.
Background The nurse–patient relationship and nursing care satisfaction are important factors that represent whether patients experience the care they expect from nurses. However, research is lacking on the relationship between nursing staff and patients, and the correlation between nursing care satisfaction and relationship care in China. Therefore, this study aimed to explore the correlation between the nurse–patient relationship and patients' satisfaction with nursing care, to form a basis for corresponding intervention measures. Methods A total of 29,108 patients from 107 hospitals in 30 provinces/municipalities in China completed a general information questionnaire, the Nursing Care Satisfaction Scale, and Relational Care Scale. Results The average nurse–patient relational care scale score was 4.38 ± 0.57, and the average patients' satisfaction with nursing care scale score was 5.40 ± 0.86. Nursing care satisfaction score was significantly related to differences among patients in different age, gender, marital status, education level, occupation, residence, family per capita monthly income, type of medical insurance, medical department, and regional patient characteristics. The correlation analysis showed that the total nurse–patient relational care score and its three dimensions of caring, trust, and professional ethics correlated positively with nursing care satisfaction scores. The multiple linear regression analysis showed that patients' age, marital status, region, department, income, type of medical insurance and the caring, trust, and professional ethics dimensions of relational care predicted nursing care satisfaction. Conclusion Enhancing nurse–patient relational care improves nursing care satisfaction, reduces nurse–patient disputes, promotes early rehabilitation of patients, and ensures patient safety.
Objective: To investigate the humanistic care consciousness and ability of outpatient and emergency nurses in tertiary Grade A hospitals in Zhengzhou City. Methods: In June 2021, a total of 345 outpatient and emergency nurses from 6 tertiary Grade A hospitals in Zhengzhou City were selected as the survey objects by random number table method. The humanistic care ability of outpatient and emergency nurses was investigated. Multiple linear regression analysis was used to analyze the related factors influencing the humanistic care ability of outpatient and emergency nurses. Results: The total score of humanistic care ability of outpatient and emergency nurses in Zhengzhou tertiary Grade A hospital was (194.18±30.53). The scores of humanistic care ability of outpatient and emergency nurses with different gender, age, educational background, professional title, length of service, night shift frequency, marital status, children's status, employment patterns and average monthly household income were significantly different (P<0.05). Regression analysis showed that education background, length of service, professional title and night shift frequency were independent influencing factors for outpatient and emergency nurses' humanistic care ability (β=0.243, 0.139, 0.163, -0.126, P<0.05) . Conclusion: At present, the humanistic care ability of outpatient and emergency nurses in tertiary Grade A hospitals in Zhengzhou City is still low. Education, length of service, professional title and night shift frequency are independent influencing factors affecting the humanistic care ability of nurses.

目的 了解医务人员对新冠肺炎相关防治知识认知现况,为制定对策提供依据.方法 使用自行设计的问卷,通过网络对河南省18个地市医护人员进行调查.结果 共调查42 490名医护人员,参与过其他类似重大紧急突发事件救治工作者占17.51%,接受过相关知识培训者占97.72%.>90%对该病流行现状有良好的认知,52.99%认为比较了解,92.66%认为非常严重或严重,49.52%认为风险可控.90.07%认为居家隔离非常有必要,>85%正确掌握隔离相关知识,67.12%认为科普方式需要进行补充.结论 河南省医务人员对新型冠状病毒肺炎流行病学相关知识、隔离知识知晓率较高,仍应持续进行动态监测,加强引导和健康教育.
Background: Novel coronavirus 2019 (COVID-19) infections are highly contagious and have spread worldwide. Healthcare workers must understand the laws and regulations related to major public health emergencies to work effectively within this environment. Through investigation and analysis, a review was conducted to help gain a better understanding of a Level-1 response to public health events and the relevant laws and regulations applicable to medical staff. Based on the results, this study formulated measures for working in the current COVID-19 healthcare context. Methods: A total of 42,490 medical personnel in 18 cities in Henan Province (China) were reviewed and analysed using the convenience sampling method. A questionnaire was employed to address two areas of cognitive status quo (25 items), ie, "general information" and "major public events and rules of the law". Results: More than 90% of medical staff had a good understanding related to knowledge about prevention and control in the pandemic context, as well as their due diligence and legal responsibility for controlling the pandemic and preventing others from being infected. However, 3.47-32.61% of medical staff still had a minimal understanding of a Level-1 response to public health events and its relevant laws and regulations. Conclusion: The response to public health events required strengthening at all levels through promotion and education, by implementing an optimised treatment system and establishing an improved legal mechanism for the treatment of major conditions, such as hierarchical, stratified and triaged infectious diseases.
Objective:To understand the effect of presenteeism behavior of nurses on patient satisfaction and to explore the correlation between them.Methods:This study was a cross-sectional study. In December 2019, the convenient sampling method was adopted to select 500 in-service responsible nurses and 500 inpatients under the charge of the responsible nurses in 5 Class Ⅲ Grade A hospitals in Henan Province as the research objects. The responsible nurses were surveyed by general information questionnaire and Presenteeism Behavior Scale and patients in charge were surveyed by Inpatient Satisfaction and Experience Monitoring Scale.Results:A total of 435 valid questionnaires were collected from responsible nurses and patients in charge. The score of Presenteeism Behavior Scale of 435 nurses was (2.62±0.93) and the score of Inpatient Satisfaction and Experience Monitoring Scale of 435 patients was (4.50±0.49) , showing negative correlation ( r=-0.19, P<0.01) . Conclusions:The level of nurses' presenteeism behavior is at a moderate level and patient satisfaction is at an upper-middle level. Nurses' presenteeism behavior affects patient satisfaction. The higher the incidence of nurses' presenteeism behavior, the lower the patient satisfaction. It is recommended to reduce nurses' presenteeism behavior and improve patient satisfaction.
目的:了解郑州市护理人员人文关怀能力现状及影响因素,为后期采取相应干预措施提供理论依据.方法:使用问卷星制成电子问卷(一般资料调查表、护士关怀能力问卷及工作—家庭支持量表),于2021年4月15日-4月25日应用便利抽样法选取郑州市10所医院的1234名护理人员进行问卷调查.结果:1234名护理人员人文关怀能力总分为(191.48±26.76)分,工作—家庭支持总分为(109.49±24.69)分;pearson相关分析显示护理人员人文关怀能力与工作—家庭支持得分及各维度得分均呈正相关(r=0.343~0.404,P<0.01);多元线性回归分析显示工作—家庭支持得分、子女状况、同事关心程度是护理人员人文关怀能力的主要影响因素(P<0.05).结论:郑州市护理人员人文关怀能力较低,工作—家庭支持得分高、有子女、同事关心程度高的护理人员人文关怀能力较高.
目的 了解护士人文关怀能力与工作—家庭支持的状况,并探讨其相关性.方法 采用工作—家庭支持量表和护士人文关怀能力量表,对1 476名护士进行问卷调查.结果 护士工作—家庭支持总分为(107.54±25.59)分,人文关怀能力评分为(192.26 ±27.76)分.护士工作—家庭支持情况与人文关怀能力呈正相关(P<0.05).回归分析结果显示,护士工作—家庭支持、工龄对护士的人文关怀能力的影响效应为20.50%.结论 护士人文关怀能力与工作—家庭支持密切相关.应给予护士更多的工作与家庭支持,以促进其人文关怀能力的提升.
BACKGROUND:Nurses' health in the workplace is crucial for ensuring the quality of healthcare. However, presenteeism, the behavior of working in a state of ill health, is widespread in the nursing industry. Considering that the origin of authoritarian leadership and the prevalence of presenteeism are inseparable from Chinese workplace culture, this study aimed to explore the impact and mechanism of authoritarian leadership on presenteeism.METHODS:A total of 528 nurses were recruited from four grade III level A hospitals in the present survey, which was distributed across 98 nursing teams. Participants were required to complete self-report measures on authoritarian leadership, presenteeism, workload, and leader identification. Description, correlation, and multilevel linear regressions were applied for data analysis.RESULTS:The present study found that presenteeism was significantly related to participants' demographic characteristics, such as marital status, educational level, technological title, and general health. There was a positive relationship between authoritarian leadership and presenteeism, and workload acted as a mediator in authoritarian leadership and presenteeism. Furthermore, leader identification moderated the relationship between authoritarian leadership and workload. When nurses were under high leader identification, the positive impact of authoritarian leadership on workload was reinforced.CONCLUSIONS:This study revealed the potential antecedents and mechanisms of nurse presenteeism from the perspective of workplace culture. Results indicated that the excessive authoritarianism of leaders and the heavy workload faced by nurses may be the significant triggers for nurses' presenteeism. The role of leader identification is not always protective, which may heighten the relationship between dark leadership and its outcomes. These observations contribute to enriching research on presenteeism and authoritarian leadership, and provide valuable insights for cultivating healthy working behaviors.
Presenteeism refers to the behavior of people who turn up for work despite complaints of ill health that should prompt rest and absence from work. The high incidence of presenteeism in the nurse population has been extensively investigated using self-reported methods to explore its effects on individual outcomes. However, few studies have examined nurse presenteeism using an “other's” perspective to verify self-reported information. Our aim in this study was to evaluate the prevalence, consequences, and causes of presenteeism in Chinese nurses from the perspectives of nurses and chief nurses. A sample of 481 nurses and 282 chief nurses from five hospitals in Henan Province, China, took part in this cross-sectional study. Participants completed the Sickness Presenteeism Questionnaire, Social Productivity Loss Questionnaire, and Causes of Nurse Presenteeism Questionnaire. The human capital method was used to estimate the monetary loss because of nurse presenteeism. We found that 94.25 and 82.08% of nurses experienced presenteeism in the past 6 months from the perspective of nurses and chief nurses, respectively. The annual monetary loss was estimated to be ¥4.38 billion and ¥2.88 billion based on the presenteeism reports from nurses and chief nurses, respectively. Workload, leave system, and conscientiousness are the main reasons for nurse presenteeism, and financial need is another important reason that is likely overlooked by chief nurses. This study provides a foundation for future research by presenting new knowledge about the prevalence, consequences, and causes of presenteeism in Chinese nurses. The findings emphasize the need for nursing managers and nursing departments to establish policy systems around paid sick leave, workload, and communication with managers to reduce nurse presenteeism and the subsequent socio-economic financial losses.
通过综述患者就医体验概念、测评工具、国内外研究现状和影响因素,探索如何改善患者就医体验实践,提升医院核心竞争力,为推进医疗卫生服务事业发展提供参考,为后续研究和实践提供理论依据.在患者体验研究方面,国内外研究多采用量表或者质性访谈对患者进行调查.中国患者就医体验研究呈逐年上升趋势,以改善患者体验具体措施为主.就医体验影响因素主要是就医环境和服务流程、医疗技术、人文关怀和有效沟通、尊重患者隐私及合法权益等.建议提高关怀患者理念,引导患者正确就医,调查研究应完善研究内容和调查方法,深层挖掘影响因素.
