AbstractBackgroundHuman immunodeficiency virus (HIV) infection is an important risk factor for Coronavirus Disease 2019 (COVID‐19), but data on the prevalence of COVID‐19 among people living with HIV (PLWH) is limited in low‐income countries. Our aim was to assess the seroprevalence of the severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) specific antibodies and associated factors among PLWH in Sierra Leone.MethodsWe conducted a cross‐sectional survey of PLWH aged 18 years or older in Sierra Leone between August 2022 and January 2023. Participants were tested for SARS‐CoV‐2 antibodies using a rapid SARS‐CoV‐2 antibody (immunoglobulin M/immunoglobulin G [IgG]) kits. Stepwise logistic regression was used to explore factors associated with SARS‐CoV‐2 antibody seroprevalence with a significance level of p < .05.ResultsIn our study, 33.4% (1031/3085) participants had received a COVID‐19 vaccine, and 75.7% were SARS‐CoV‐2 IgG positive. Higher IgG seroprevalence was observed in females (77.2% vs. 71.4%, p = .001), adults over 60 years (88.2%), those with suppressed HIV RNA (80.7% vs. 51.7%, p < .001), antiretroviral therapy (ART)‐experienced individuals (77.9% vs. 44.6%, p < .001), and vaccinated participants (80.7% vs. 73.2%, p < .001). Patients 60 years or older had the highest odds of IgG seroprevalence (adjusted odds ratio [aOR] = 2.73, 95% CI = 1.68–4.65). Female sex (aOR = 1.28, 95%CI = 1.05–1.56), COVID‐19 vaccination (aOR = 1.54, 95% CI = 1.27–1.86), and ART (aOR = 2.20, 95% CI = 1.56–3.11) increased the odds, whereas HIV RNA ≥ 1000 copies/mL (aOR = 0.32, 95% CI = 0.26–0.40) reduced the odds of IgG seroprevalence.ConclusionsWe observed a high seroprevalence of SARS‐CoV‐2 antibody among PLWH in Sierra Leone. We recommend the introduction of targeted vaccination for PLWH with a high risk of severe COVID‐19, especially those with an unsuppressed HIV viral load.

学校是典型的人群密集的公共场所,其传染病突发疫情事件占传染病突发疫情事件总数半数以上,学样人群密集且流动性较大,很容易造成传染病的暴发流行.本综述全面梳理学校传染病防控的关键点和学校传染病防控的关键要素,为学校传染病防控措施制定提供参考,从而有效提升学校传染病的防控效率.
目的 分析北京市2012-2018年抗结核药物的使用情况与变化趋势,为北京市进一步规范及修改结核药物的使用策略提供数据参考.方法 对2012-2018年北京市46家医院J04A类抗结核药物的销售金额、用药频度(Defined daily doses,DDDs)、药品限定日费用(Defined daily cost,DDC)等指标进行统计分析与评价.采用SPSS 22.0线性回归分析,判断变化趋势.结果 2012-2018年抗结核药销售金额逐年上升,而销售量在2016年到达高峰后缓慢下降;2012年和2018年相比抗结核的药物种类构成变动不大,各类药物销售金额构成变化较大;抗结核药物以一线、口服为主,二线、注射为辅,且二线用药的销售金额、DDDs和注射用药的销售金额有上升的趋势(P均<0.05);抗生素销售单价以注射类经济负担最重,且2018年较2012年,大部分种类药物都有不同程度的价格增长.结论 抗结核药物的DDDs在2012-2018年间保持平稳,但大部分种类抗结核药物的DDC均出现上升趋势,患者经济负担增大,且抗结核药物使用存在用药集中的现象,带来的耐药风险不容忽视.
目的 了解医院住院患者医院感染现状,为加强医院感染防控提供参考.方法 采用横断面调查方法,对某医院住院患者医院感染现患率进行调查.结果 调查当日共有住院患者1960例,实查1960例,实查率100%.共计查出医院感染233例,其中社区获得感染139例,医院内获得感染94例,医院感染现患率为4.8%.医院感染主要分布在重症监护病房(ICU)和有创性手术科室.医院感染部位所占比重居前3位的是下呼吸道、器官腔隙和尿管相关感染.患者抗菌药物使用率为21.43%,一联用药占73.33%,治疗用药占89.29%.结论 该医院住院患者医院感染现患率在规范控制范围内,应加强目标监测和采取针对性防控措施.
目的 解决野战医疗所在应急救援过程中的医疗数据业务通信、现场应急指挥调度需求.方法 基于LTE 4G无线专网的技术,采用OFDM、MIMO等多种先进技术以及定制的专业宽带集群技术,建立了高速稳定的数据传输专网.结果 野战救援通信系统得到快速部署.结论 快速通信系统具备覆盖距离远、安装简易且不易受环境影响的特点,能够解决现有有线通信系统中难部署、易故障等问题,满足了在应急救援过程中的通信需求.
目的:实现对救援队的人员、伤员、药品、设备等全部元素的高效信息化管理.方法:依托信息化手段,构建省级应急医疗救援队信息平台,研究伤情分检、医疗救治、物资设备、指挥决策等资源最佳配置模型.结果:增强省级医疗救援队应急医疗救援的指挥控制能力.结论:提高了医疗救援的工作效率和救援质量.
Under the background of construction of research-oriented military hospitals, in order to meet the requirements of inner construction of research-oriented hospitals, reform and development of medical and healthcare system and management innovation of research-oriented hospitals, conforming with related laws and regulations, and based on the principles of control, manipulation, guidance and incentive, quality evaluation system for health service of research-oriented hospitals is established, so as to improve medical techniques and quality of service;three-level medical quality supervision and inspection system is established and key health indicators for assessment are set up, so as to achieve management according to law, standardize management, and precise management, to highlight the characteristics of research-oriented hospitals, to improve the quality evaluation system for health service, and to promote the construction and development of research-oriented hospitals.
Under the modern joint service system, military hospitals should, with the perspective, vision and method of joint service, analyze the current highlighted problem of disharmony between real combat maneuver of a military hospital and the standard of combat capability, take effective measures focusing on new orientation, new system and new mission, and accelerate the transition and reconstruction through joint operation, maneuver and support, so as to further improve the quality and benefits of actual combat maneuver of a military hospital.
Medical decision-making was the key of medical command, and also a precondition of effec-tive medical support mission.The medical decision-making service process was summed up in three steps:deter-mining the security target, security planning scheme,and formation of security determination.Combined with the characteristics and regulation of the medical service, the contents and methods of medical decision making were dis-cussed, and suggestions including overall planning in medical decision -making process, giving full play to the think tank, good parallel working and so on were proposed.
Objective To study the positioning of the medical equipment in the hospital by the leasing center.Methods The overall frame for the positioning scheme was designed based on the management by the leasing center,and then the scheme was accomplished accordingly.Results The positioning system for leased medical equipment was composed of RFID label system,network coordinator,fixed-point receiver,regional server and core management server,and the working flow and the differential application of RFID label and barcode were also fulfilled.Conclusion RFID technology is applied to the positioning scheme of the medical equipment in hospital leasing center,then the dynamic information on quality control and location can be tracked through the scheme,which is of great significance for overall supervision of hospital medical equipment.
Objective To explore the standardized management of emergency treatment equipment in lease center. Methods Self-evaluation of the existing emergency treatment equipment management was performed, and the items related to equipment safety and quality control in JCI standard were introduced into the standardized management. Results Risk evaluation, quality control and detection, digital management and etc of emergency treatment equipment were determined according to the self-evaluation result and JCI requirements. Conclusion The introduction of JCI standard into the management of emergency treatment equipment in lease center contributes to the standardized management of equipment.

Objective To investigate the current status of turnover and stability among contract system of medical staff in military hospitals.Methods Questionnaire was used to investigated the stabilization of human resources of three military third-tertiary hospitals in last three years,and the results was analyzed by statistic methods.Results ①According to the professional category,medical technology professional is the highest,the nursing profession is followed by with an increasing trend,and latest are the medical and pharmaceutical;②According to the structure of educational background,the proportions of resignation were relatively higher among graduate students than others.③According to the work experience,those who with work experience less than 3 years accounted for the highest percentage,showing an increasing tendency;④According to the reasons for departure,hired by the state,public institutions is the primary reason for leaving,occupational stress and family factors is followed by.Conclusion The instability of contract system of medical staff has become the key factor for the development of military hospital.Effective strategies and measures should be formulated to stabilize the hospital contract medical staff human resources in order to achieve the goal of 'health care and ensuring win'.
On the basis of good operation of equipment rental center in our hospital,this paper discusses the quality control viability of equipment in rental center.It can lay the foundation for quality control centralization management in the future,improve the quality level of the equipment in rental center,and prevent adverse events caused by medical equipment quality.
