
OBJECTIVE:To observe the effects of the "Zhibian" (BL54)-toward-"Shuidao" (ST28) acupuncture on key regulatory factors during mitochondrial apoptosis of testicular tissue in asthenozoospermia mice, and explore the potential mechanism of the protective effect of acupuncture on reproductive function. METHODS:Thirty C57BL/6 male mice were randomly divided into a blank group, a model group and an acupuncture group, 10 mice in each group. In the model and the acupuncture groups, the intraperitoneal injection of cyclophosphamide (30 mg•kg-1•d-1) was delivered for 7 days to prepare the asthenozoospermia model. After the success of modeling, the modeled mice in the acupuncture group were intervened with "Zhibian" (BL54)-toward-"Shuidao" (ST28) acupuncture, once daily and the needles were retained for 20 min. The duration of the intervention was 2 weeks. The general condition of each mouse was observed, and the body mass was recorded before modeling, after modeling and after intervention completion. After intervention, the testicular mass was recorded and the weight coefficient was calculated, and the mouse sperm quality was examined; the serum contents of testosterone (T), follicle stimulating hormone (FSH) and luteinizing hormone (LH) were detected using ELISA, the morphology of testicular tissue was observed using HE, the mitochondrial ultra-microstructure of testicular tissue was observed under transmission electrone microscopy, the mitochondrial membrane potential level of testicular tissue was detected using JC-1 staining, the positive rate of apoptosis cell of testicular tissue was observed using TUNEL; and the mRNA and protein expression of b-cell lymphocytoma-2 (Bcl-2), Bcl-2 associated X protein (Bax), cytochrome c (Cyt C), apoptotic protease-activating factor1 (Apaf-1), Caspase-9 and Caspase-3 of testicular tissue was detected using real-time quantitative fluorescence PCR and Western blot methods separately; and the positive expression of Cleaved Caspase-3 of the testicular tissue was detected using immunohistochemistry. RESULTS:Compared with the blank group, the mice were in listless spirits, had shaggy hairs, the reduced appetite and movement, and weight loss in the model group (P<0.01); the testicular mass and the weight coefficient decreased (P<0.01); the total number of sperms, sperm motility, and sperm viability were declined (P<0.01); while the levels of serum T, FSH, and LH were dropped (P<0.01). The morphology of seminiferous tubules in testicular tissue was abnormal, the number of spermatogenic cells and the number of mitochondria decreased, the inner mitochondrial crest was fractured and lost, and vacuoles appeared. The level of mitochondrial membrane potential was reduced (P<0.01); and the positive rate of apoptosis cell in testicular tissue increased (P<0.01). The mRNA and protein expression of Bax, Cyt C, Apaf-1, Caspase-9 and Caspase-3 was elevated (P<0.01, P<0.05), the mRNA and protein expression of Bcl-2 was dropped (P<0.01), and the average absorbance value of Cleaved Caspase-3 increased (P<0.01). When compared with the model group, in the acupuncture group, the general condition of mice was improved, the testicular mass and the weight coefficient elevated (P<0.01); the total number of sperms, sperm motility, and sperm viability increased (P<0.01); while the levels of serum T, FSH, and LH rose (P<0.01). The pathological morphology of testicular tissue and the inner mitochondrial ultra-microstructure were ameliorated, the level of mitochondrial membrane potential was elevated (P<0.01); the positive rate of apoptosis cell was reduced (P<0.01). The mRNA and protein expression of Bax, Cyt C, Apaf-1, Caspase-9 and Caspase-3 was dropped (P<0.01, P<0.05), the mRNA and protein expression of Bcl-2 elevated (P<0.05), and the average absorbance value of Cleaved Caspase-3 declined (P<0.01). CONCLUSION:"Zhibian" (BL54)-toward- "Shuidao" (ST28) acupuncture may ameliorate mouse reproductive function by inhibiting mitochondrial apoptosis pathway, alleviating testicular tissue damage in the asthenospermia mice induced by cyclophosphamide.
Diabetic peripheral neuropathy(DPN) is a chronic complication resulted from peripheral nerve injury in the late stage of diabetes. It involves a variety of pathological changes such as oxidative stress, endoplasmic reticulum stress, neuroinflammation, and apoptosis of Schwann cells(SCs). DPN is the main factor leading to lower limb disability or amputation in diabetic patients, with high incidence, long disease course, and poor prognosis. The modern medicine treatment of DPN mainly focuses on controlling blood glucose and improving microcirculation and nerve nutrition, which can only mitigate the clinical symptoms and not fundamentally reverse the pathological changes of peripheral nerves. Autophagy is a self-clearing mechanism that maintains cellular homeostasis by removing excess metabolites. Traditional Chinese medicine(TCM), featuring the holistic concept and syndrome differentiation, can treat chronic diseases in a multi-target, multi-pathway, and wide-range manner. Modern studies have shown that the occurrence and development of DPN are related to a variety of pathological changes, and autophagy is a key mechanism associated with DPN. The environment with persistent high glucose can lead to the inhibition or over-activation of peripheral nerve cells, which causes irreversible damage of nerve cells and the occurrence and development of DPN. Therefore, restoring autophagy balance and reducing nerve damage is one of the key ways to treat DPN. The recent studies have confirmed that some active ingredients in traditional Chinese medicines and TCM compound prescriptions can inhibit the oxidative stress, endoplasmic reticulum stress, mitochondrial damage, inflammation, and apoptosis of SCs in DPN by regulating the autophagy pathway, thus playing a role in the prevention and treatment of DPN. However, the systematic induction in this field remains to be carried out. This paper reviewed the relevant literature, explained the mechanism of TCM in the prevention and treatment of DPN by regulating autophagy, and summarized the potential targets of TCM in the treatment of DPN, with a view to providing new ideas for clinical research and drug development.
Piperis Longi Fructus, made from the mature and immature fruit spikes of Piper longum, is a commonly used Mongolian medicine. In recent years, researchers have gradually deepened the research on ethnic medicines and found that Piperis Longi Fructus has significant effects in adjusting blood lipids and anti-cancer. Its new chemical components and pharmacological activities are also constantly updated. Subsequently, the development and application of Piperis Longi Fructus have attracted extensive attention. Thus, it is quite urgent to establish and improve a quality evaluation system for the medicine. On the basis of summarizing the chemical components and pharmacological effects of Piperis Longi Fructus and understanding the new concept of quality marker(Q-marker), the components which can be used as its Q-markers were analyzed from the aspects of the genetic relationship, traditional medicinal effects and properties, rules of compounding and compatibility, absorbed components and testability. The research can provide reference for the establishment of a quality evaluation system for Piperis Longi Fructus.
Objective To explore the effects of ginsenoside RG1 through LncRNA MALAT1 pathway on spinal cord injury to the regeneration of rats nerve regeneration. Methods Thirty rats were randomly divided into RG1 treatment group, sham operation group and spinal cord injury control group. Adult female rat was randomly divided into sham operation group, spinal cord injury control group and RG1 treatment group, and a rat spinal cord injury model was established. The rats in Rg1 treatment group were intraperitoneally injected with Rg1 solution(100 mg/kg) every day, and the rats in spinal cord injury control group and sham operation group were intraperitoneally injected with the same amount of normal saline every day. Basso Beattie Bresnahan(BBB) score was used to evaluate the motor recovery after spinal cord injury in rats. The rats were killed after 28 days of continuous treatment. The spinal cord tissue was taken out and the expressions of laminin(LN), fibronectin(FN), glial cell derived neurotrophic factor(GDNF) and nerve growth factor(NGF) were detected by Western blot. Real time fluorescent quantitative PCR was used to measure LncRNA MALAT1, and immunohistochemistry was used to analyze the expression level of LAMC1. Results The BBB score of the RG1 treatment group was significantly better than that of the sham operation group and spinal cord injury control group(P<0.05). Immunization and immune fluorescent displayed RG1 the expressions of LN,FN,GDNF,NGF and LAMC1 in the treatment group were significantly increased compared with those of the control group. Conclusion RG1 can increase the expressions of LNCRNA MALAT1 and further regulate LAMC1 to promote the repair of spinal cord injury in rats.
Alzheimer’s disease(AD) is the most common neurodegenerative disease, which causes memory, daily function decline and behavior disorders due to the gradual degradation of the brain, commonly known as "senile dementia". There are many types of dementia patients, and AD accounts for the largest proportion. At present, there is no specific drug to reverse the course of the disease. In recent years, substantial progress has been made in the pathogenesis and drugs of this disease, especially in the treatment of traditional Chinese medicine and compound prescription. Using animal and cell models, many researchers have confirmed the effectiveness of Renshen(Ginseng Radix Et Rhizoma) extract from different angles. This paper combed and summarized the research literature on the new progress of Renshen(Ginseng Radix Et Rhizoma) in the treatment of AD at home and abroad in recent years, so as to provide reference and scientific basis for developing it into a healthy product for the treatment of senile dementia and various types of memory disorders.
目的 研究知母多糖的超声波辅助碱液最佳提取工艺及其抗氧化活性.方法 以知母多糖得率为指标,通过单因素实验,以超声功率、提取时间、料液比和碱液浓度为考察因素,结合Box-Behnken实验,利用响应面法优化超声波辅助碱液提取工艺.通过对1,1-二苯基-2-三硝基苯肼(1,1-diphenyl-2-picrylhydrazyl,DPPH)的清除率来评价知母多糖的抗氧化能力.结果 当超声功率为92 W,提取时间为20 min,料液比为1:40,碱液浓度为0.05 mol/L时,知母多糖的提取率可达34.059%.抗氧化活性实验结果显示,纯化的知母多糖具有较强的DPPH自由基清除能力,在质量浓度为1.772 mg/mL时,其对DPPH自由基的清除率高达88.11%.结论 通过响应面优化所得提取工艺具有提取率高,稳定的优点,可用于知母多糖的提取;知母多糖具有较强的体外抗氧化活性.
The increasingly serious aging of the population has brought about an increase in the incidence of cardiovascular and cerebrovascular events. The early intervention and treatment of cerebral ischemia reperfusion injury after cardiovascular and cerebrovascular events is still a difficult problem. Cerebral ischemia-reperfusion injury is a complex cascade reaction in modern medicine, and modern medical treatment usually requires the combined use of multiple drugs, which may affect the patients’ homeostasis. In traditional Chinese medicine, there is no clear classification, but it can be classified into the category of stroke. Bingpian(Borneol) has excellent clinical effect in the treatment of cerebral apoplexy. Cerebral ischemia and reperfusion also belong to cerebral apoplexy, and so Bingpian(Borneol) can also be used in the treatment of cerebral ischemia and reperfusion injury. In this paper, the pharmacological effects of Bingpian(Borneol) were discussed from the perspectives of traditional Chinese and modern medicine, and the effects of Bingpian(Borneol) on cerebral ischemia-reperfusion injury were reviewed in terms of inhibiting iron death and calcium overload to protect cerebral nerves and affecting the permeability of blood-brain barrier. The specific mechanism of Bingpian(Borneol) on cerebral ischemia-reperfusion injury was discussed.
Mingmen was put forward as the viscera in Nanjing,and from this, the theory of Mingmen was established. In the debate between the tangible and intangible of Mingmen,Nanjing concludes that Mingmen is “tangible”and clearly puts forward the view of“kidney on the left and Mingmen on the right”,the theory of“visceral Mingmen”and establishes its congenital root status. Primordial Qi is the root of Zang-Fu organs and meridians,the central axis of life and Qi,the door of breathing and the god of guarding evils. The axis of Mingmen and primordial Qi runs through the five Zang-organs and six Fu-organs and is the source of birth and the root of life. On the basis of Lingshu and Suwen,Nanjing has established a systematic life view of life,Qi and spirit from the material dimension,functional dimension and coordination dimension.
目的 观察加减五苓散治疗气虚血瘀型慢性心力衰竭患者的临床疗效.方法 选取2020年12月-2021年12月本院80例心力衰竭患者(气虚血瘀型),随机均分为对照组和五苓散组,对照组予以西医常规治疗,五苓散组在对照组基础上予以加减五苓散治疗.观察并比较两组患者的临床疗效以及治疗前后心功能、血流动力学和血清指标水平变化.结果 五苓散组的总有效率显著上调(P<0.05).治疗前两组患者的LVEDD、LVESD以及LVEF水平差异无统计学意义.治疗4周后,五苓散组患者的LVEDD和LVESD相较于对照组显著下调,而LVEF相较于对照组显著上调(P<0.01).治疗后五苓散组患者的外周血中NT-pro BNP和cTnI水平相较于对照组显著下调;而6MWT水平相较于对照组显著上调(P<0.01).治疗前两组患者的血流动力学指标均差异无统计学意义(P>0.05).治疗4周后,五苓散组患者的外周阻力水平相较于对照组显著下调;而心排出量和SV水平相较于对照组显著上调(P<0.01).治疗前两组患者的心衰积分和MLHFQ差异均无统计学意义(P>0.05).治疗4周后,五苓散组患者的心衰积分和MLHFQ相较于对照组显著下调(P<0.01).治疗后五苓散组患者的NYHA心功能分级相较于对照组显著改善(P<0.01).两组总不良反应发生率差异不具有统计学意义(P>0.05).结论 加减五苓散联合西医常规用药治疗气虚血瘀型心力衰竭能够显著提高临床疗效,改善患者心功能、血流动力学以及心衰血清学指标,且无不良反应增加,临床适宜推广.
目的 研究针刺结合盐酸阿芬太尼用于老年患者无痛肠镜检查中效果。方法选择于2019年3月到2020年6月在本院行无痛肠镜检查的老年患者78例作为研究对象,随机数字表法分为两组,各39例。对照组患者采用静脉输注丙泊酚麻醉,观察组患者联合盐酸阿芬太尼麻醉以及针刺治疗。数据观察:两组患者进镜成功率、麻醉优良率、麻醉效果指标(置镜检查时间及意识消失时间、恢复自主呼吸时间、睁眼时间、言语应答时间)、麻醉前及麻醉后两组患者血流动力学指标变化、麻醉时期炎症因子水平情况、麻醉疼痛分级情况、麻醉不良反应发生率、患者复查意愿率及肠镜检查满意率。结果 两组患者进镜成功率均为100%,P>0.05;观察组患者麻醉优良率高于对照组患者,P<0.05;观察组患者麻醉效果,置镜检查时间及意识消失时间、恢复自主呼吸时间、睁眼时间、言语应答时间等指标均优于对照组患者,P<0.05;两组麻醉前血流动力学无统计意义(P>0.05);两组麻醉后血流动力学水平均有所变化,观察组患者麻醉期间舒张压(Diastolicbloodpressure,DBP)、收缩压(Systolicbloodpressure,SBP)及血氧饱和度(pulseoxygensaturation,SPO2)水平高于对照组患者(P<0.05);两组T0时间点炎性因子无统计意义(P>0.05);麻醉诱导后(T0)两组hs-CRP和TNF-α水平无显著性差异(P>0.05);观察组麻醉后超敏C反应蛋白(HighsensitiveC-reactiveprotein,hs-CRP)、肿瘤坏死因子(TumorNecrosisFactor-α,TNF-α)水平在T1、T2、T3时间段均低于对照组(P<0.05);观察组患者麻醉疼痛分级情况优于对照组,P<0.05;观察组患者麻醉不良反应发生率低于对照组患者,P<0.05;观察组患者复查意愿率及肠镜检查满意率均高于对照组患者,P<0.05。结论 针刺结合盐酸阿芬太尼用于老年患者无痛肠镜检查中效果良好,患者麻醉效果佳,麻醉后恢复好,且炎症因子水平较低,对患者血流情况影响小,患者疼痛轻,麻醉不良反应少,患者查意愿率及肠镜检查满意率高。
南蛇藤(Celastrus orbiculatus Thunb.或C.articulatunm Thunb.)是卫矛科(Celastraceae)南蛇藤属植物,是常见的传统中药之一。现代研究表明其含有多种活性成分,除了在中医药理论指导下发挥祛风除湿通络作用之外,现代研究表明其还具有抗肿瘤、抗多重耐药、抗动脉硬化、抗炎、抗风湿、抗病毒、抗氧化活性等多种生物学活性。现代药理研究表明南蛇藤不仅有良好抗肿瘤活性,而且抗肿瘤的作用机制是多样性的。本文就近年来关于南蛇藤抗肿瘤作用的国内外文献,从南蛇藤抑制肿瘤细胞的增殖、诱导肿瘤细胞凋亡、抑制肿瘤血管形成、抑制肿瘤的侵袭转移以及联合化疗药物增效等5个方面进行综述,以期望为南蛇藤后续研发和临床应用提供理论参考依据。
目的:观察口服中药汤剂清金化痰饮联合中药外用治疗小儿支原体肺炎的临床疗效。方法:选取近5年内在附属医院儿科住院的70例支原体肺炎患儿,将其随机分为观察组和对照组,每组各35例。对照组采用静脉滴注红霉素和盐酸氨溴索,以及常规对症治疗。观察组在对照组治疗基础上,口服自拟中药汤剂清金化痰饮并配合中药敷胸散外敷,两组均治疗10天,疗程结束后观察临床疗效。结果:两组临床总有效率比较,观察组总有效率为97.1%,对照组总有效率为74.3%,两组相比,观察组总有效率显著高于对照组,有统计学差异(P<0.05)。两组治疗后发热、咳嗽、喘息、肺部啰音消失时间比较,观察组显著短于对照组(P<0.05)。两组治疗前后中医证候及体征积分变化比较,治疗前相比无统计学意义(P>0.05),治疗后观察组显著低于对照组(P<0.05)。两组治疗前后肺功能最大呼气流量(peak expiratory flow,PEF)、用力肺活量(forced vital capacity,FVC)、第1秒用力呼气容积(forced expiratory volume in one second,FEV 1 )比较,治疗前相比无统计学意义(P>0.05),治疗后观察组改善情况显著优于对照组(P<0.05)。两组治疗前后血清炎性因子白细胞介素-6(interleukin-6,IL-6)、反应蛋白(C-reactive protein,CRP)、血清淀粉样蛋白A(Serumamyloid A,SAA)水平比较,治疗前相比无统计学意义(P>0.05),治疗后观察组显著低于对照组(P<0.05)。两组均未发生严重不良反应。结论:口服中药清金化痰饮联合中药敷胸散外用治疗小儿支原体肺炎可以明显提高临床疗效,能有效改善临床症状和肺功能,降低血清炎性因子水平,并且安全性好,副作用小,能促进患儿较快康复,值得临床推广应用。
目的 探讨康妇炎胶囊对支原体属感染慢性盆腔炎患者血清炎症因子及盆腔血流动力学的影响。方法 100例支原体属感染慢性盆腔炎患者,随机分组,每组50例,对照组治疗药物是阿奇霉素片,观察组是康妇炎胶囊联合阿奇霉素片,4周后停药,观察中医症候积分和血清炎症因子、盆腔卵巢和子宫动脉血流动力学变化。结果 中医症候积分:治疗前无统计学意义(P>0.05),治疗后积分降低(P<0.05),且观察组更低(P<0.05);血清白细胞介素-4(IL-4)、肿瘤坏死因子-a(TNF-a)、白细胞介素-2(IL-2)、C反应蛋白(CRP):治疗前无统计学意义(P>0.05),IL-2和IL-4治疗后升高(P<0.05),且观察组更高(P<0.05),TNF-a和CRP治疗后降低(P<0.05),且观察组更低(P<0.05);卵巢动脉及子宫动脉搏动指数(PI)、阻力指数(RI)、收缩期峰值流速(PSV):治疗前无统计学意义(P>0.05),PI及PSV治疗后升高(P<0.05),且观察组更高(P<0.05),RI降低(P<0.05),且观察组更低(P<0.05);停药3个月、6个月时;复发率对照组20.00%、32.00%,观察组4.00%%、12.00%,有统计学意义(P<0.05);治疗总有效率观察组96.00%,对照组82.00%,有统计学意义(P<0.05)。结论 康妇炎胶囊治疗支原体属感染慢性盆腔炎对改善临床症状有促进作用,利于炎症的控制,有助于改善盆腔血流循环,降低复发率,提高治疗疗效。
目的 利用气相色谱-质谱联用(GasChromatography-massSpectrography,GC-MS)和高效液相(HighPerformanceLiquidChromatography,HPLC)技术,探讨香附不同部位(花、茎、根、须)间化学成分变化和含量差异,为非药用部位再利用提供参考。方法 采用GC-MS技术研究香附不同部位中的挥发性成分的分布情况,借用HPLC对4种化学成分含量进行含量测定,完成对香附不同部位的定性、定量分析。结果 不同部位共检测到49个化学成分,其中花部共指认20个成分,茎部共指认17个成分,根部共指认38个成分,须部共指认17个成分;不同部位共存在8个主要挥发性成分,其中花部与根部8种成分相对含量均接近;定量分析结果显示花部与根部的香附烯酮、α-香附酮和阿魏酸含量接近;聚类分析结果显示多个产地花部与根部可聚为一类。结论 花部与根部化学成分较为接近,有较高的开发价值,该结果为香附非药用部位的合理利用和后续研究提供有效的数据支撑。
Objective To investigate the effects of electroacupuncture on the expressions of vascular endothelial growth factor(VEGF),VEGF receptor-1/fms-like tyrosine kinase-1(Flt-1) and brain-derived neurotrophic factor(BDNF) in patients with cerebral infarction and their clinical effects. Methods A total of 160 patients with acute cerebral infarction diagnosed and treated in the hospital were collected. Random number table method was used to divide patients into control group(80 cases) and study group(80 cases). The qRT-PCR method was used to detect the expressions of VEGF mRNA and Flt-1 mRNA. The level of serum BDNF was detected by ELISA. The clinical treatment effect, neurological deficit score and activity of daily living scale(ADL) score were compared between the two groups of patients. The levels of serum interleukin-1β(IL-1β),interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP) and tumor necrosis factor-α(TNF-α) were detected by ELISA. Patients’ cerebral hemodynamic indexes[average blood flow(Qm),average blood flow velocity(Vm),peripheral resistance(Rv),dynamic resistance(DR)] of common carotid artery were detected. Results Compared with those of the control group, the improvement rate and total effective rate of patients in the study group were significantly increased(P<0.05),and the incidence of no change and the rate of deterioration were significantly reduced(P<0.05). After treatment, the neurological deficit score was significantly reduced in both groups(P<0.05),and the ADL score was significantly increased(P<0.05),and the difference between the control group and the study group was statistically significant(P<0.05). After treatment, compared with those of the control group, the expression levels of serum VEGF mRNA and BDNF in the study group were significantly increased(P<0.05),and the expression levels of Flt-1 mRNA were significantly reduced(P<0.05),the levels of hs-CRP and TNF-α were significantly reduced(P<0.05),the levels of Qm and Vm were significantly increased(P<0.05),and the levels of Rv and DR were significantly reduced(P<0.05). Conclusion Electroacupuncture can effectively increase the expression levels of VEGF and BDNF in serum and decrease the expression level of Flt-1 in patients with acute cerebral infarction, and can improve the neurological function and cerebral hemodynamics.
目的 探讨"培元通腑法"针药结合治疗痰热腑实型卒中的临床疗效及对脑肠肽、肠道菌群的影响.方法 研究纳入痰热腑实型中风病患者94例,采用随机数字表的分组方法分为观察组与对照组各47例.对照组给予卒中的常规支持治疗,观察组在对照组治疗的基础上,给予培元通腑汤口服联合培元通腑针法治疗,均连续治疗4周后判定疗效.比较两组患者治疗前后的"痰热腑实证"中医症状积分、NIHSS和SS-QOL量表评分.检测并比较两组患者治疗前后的血清脑肠肽水平和粪便中的肠道菌群计数.结果 观察组治疗后的燥屎不行、脘腹胀满、口气臭秽等中医症状积分及总积分较对照组均显著降低,观察组治疗后的有效率(93.62%,44/47)显著高于对照组(78.72%,31/47),组间比较差异有统计学意义(x2=4.374,P<0.05).NIHSS评分、血清血管活性肠肽(vasoactive intestinal peptide,VIP)水平和粪便中的大肠杆菌、肠球菌计数,观察组均显著低于对照组(P<0.05);SS-QOL评分、血清胃动素(motilin,MTL)、胃泌素(gas-trin,GAS)、P 物质(substance P,SP)水平和粪便中的双歧杆菌、乳酸杆菌计数,观察组均显著高于对照组(P<0.05).结论 "培元通腑法"针药结合治疗痰热腑实型卒中,能够明显改善卒中患者痰热腑实的临床表现,促进患者神经功能和生活质量的恢复,该治疗作用与机体脑肠肽分泌紊乱和肠道菌群失调相关.
目的研究揿针疗法治疗脾虚湿阻型单纯性肥胖(SO)的效果及对血清中脂肪因子瘦素(LEP)、脂联素(ADP)、胰岛素(INS)水平的影响。方法 选取2019年8月—2021年8月我院诊治的108例脾虚湿阻型SO患者,随机分为对照组与观察组,各54例。对照组给予饮食调整和有氧运动干预,观察组在以上基础上实施揿针疗法。两组持续治疗8周后,比较两组疗效、中医证候积分、腰臀围与体质量指数、脂肪因子指标、血脂水平。结果 两组治疗后具有一定疗效,且观察组有效率96.30%高于对照组85.19%(P<0.05)。两组治疗后较治疗前比较,中医证候积分显著下降,且观察组积分低于对照组(P<0.05)。两组治疗后较治疗前比较,腰围、臀围、体质量指数显著下降,且观察组水平低于对照组(P<0.05)。两组治疗后较治疗前比较,LEP、ADP、Ins水平下降,且观察组水平低于对照组(P<0.05)。两组治疗后较治疗前比较,总胆固醇、甘油三酯、低密度脂蛋白水平有所下降,且研究组水平低于对照组(P<0.05)。结论揿针疗法可有效改善脾虚湿阻型SO患者症状体征,提高疗效,降低血清脂肪因子与血脂水平,可推荐临床使用。
目的 观察应用益气升阳针法对慢性萎缩性胃炎大鼠胃黏膜GLi和COX-2蛋白表达。方法 将60只健康SPF级Wistar大鼠采用随机数字表法依次分为正常组、模型组、西药维酶素组及针灸组各15只,除正常组外,其余各组大鼠均采用2%水杨酸钠和30%乙醇混合液1mL/100g混合灌胃,每天将100μg/mLMNNG作为饮用水自由饮用,同时结合饥饱失常法,建立慢性萎缩性胃炎大鼠模型。造模成功后,针灸组给予益气升阳针法治疗,西药维酶素组给予0.3g/(kg·d)维酶素治疗,正常组及模型组每天给予生理盐水治疗,均连续治疗4周。治疗结束后,观察各组大鼠的一般情况、体重及食量,各组胃黏膜GLi1、GLi2、GLi3、COX-2蛋白表达采用免疫组化染色观察,采用酶联免疫吸附法测定各组血清TNF-α、G-17、IL-1β及IL-4表达。结果 造模前,各组体重、食量相比(P<0.05);造模后,针灸组、西药维酶素组、模型组体重、食量均低于正常组(P<0.05);干预后,正常组体重、食量高于针灸组、西药维酶素组、模型组,针灸组体重、食量高于西药维酶素组、模型组,西药维酶素组体重、食量高于模型组(P<0.05)。正常组GLi1、GLi2、GLi3、COX-2蛋白表达及TNF-α、G-17、IL-1β及IL-4表达低于针灸组、西药维酶素组、模型组,针灸组GLi1、GLi2、GLi3、COX-2蛋白表达及TNF-α、G-17、IL-1β及IL-4表达低于西药维酶素组、模型组,西药维酶素组GLi1、GLi2、GLi3、COX-2蛋白表达及TNF-α、G-17、IL-1β及IL-4表达低于模型组(P<0.05)。结论 针灸可增加慢性萎缩性胃炎大鼠体重及食量,可能与降低GLi1、GLi2、GLi3、COX-2蛋白表达及TNF-α、G-17、IL-1β及IL-4表达有关,也是针灸防治慢性萎缩性胃炎的作用机制之一。
苄基异喹啉类生物碱(Benzylisoquinoline alkaloids,BIAs)属于异喹啉类生物碱中的一类,根据其化学结构特点可分为小檗碱和原小檗碱类、阿朴菲类、苄基四氢异喹啉类、普罗托品类、双苄基异喹啉类等。此类生物碱来源中药及中药组方在治疗心悸、怔忡、厥证、胸痹等心血管相关疾病方面也有着丰富的临床用药经验,现代研究表明苄基异喹啉类生物碱具有较强的抗心律失常活性,从中药中开发抗心律失常单体活性成分,毒性、副作用更清楚,开发成本更低。基于心律失常发病机制复杂的特点,通过多途径,多靶点作用的抗心律失常药物具有更高的安全性和有效性,现在临床使用的抗心律失常药物大多属于单离子通道阻滞剂,具有致心律失常风险,而苄基异喹啉类生物碱可通过影响Na + 、K + 、Ca 2+ 多条离子通道、抗炎及氧化应激途径达到治疗多种心律失常的目的,极具开发前景。本文通过查阅、整理近40年的国内外文献,结合中医药理论对苄基异喹啉类生物碱的抗心律失常离子机制进行总结,可为开发安全高效的抗心律失常药物和治疗手段提供参考。
目的 观察参芪补胰方对2型糖尿病(type2diabetesmellitus,T2DM)大鼠TLR4/MyD88/NF-κB信号通路的影响,探讨参芪补胰方治疗T2DM的作用机制。方法 采用随机对照法将48只SD大鼠随机分为7组,正常组予以基础饲料喂养,其余各组动物采用高糖高脂饲养加腹腔注射STZ法造模,二甲双胍组给予盐酸二甲双胍片,中药组给予不同剂量参芪补胰方,给药6周后留取标本。ELISA法检测大鼠肿瘤坏死因子-α(TumorNecrosisFactor,TNF-α)、白细胞介素-1β(Interleukin1β,IL-1β)、白细胞介素-10(Interleukin10,IL-10)、白细胞介素-17(Interleukin17,IL-17)水平,HE染色法观察大鼠胰腺组织病理变化,TUNEL染色法观察大鼠胰岛细胞凋亡情况,RT-PCR法检测大鼠胰腺组织TLR4、MyD88、NF-κB的mRNA表达水平,Westernblot法检测大鼠胰腺组织TLR4、MyD88、NF-κB的蛋白表达水平。结果 参芪补胰方对DM大鼠FBG水平方面有明显下降作用,与模型组比较有统计学差异(P<0.05);参芪补胰方组大鼠血清TNF-α、IL-1β、IL-10、IL-17表达水平明显降低(P<0.05);可以通过改善大鼠胰腺组织损伤、促进细胞结构组织修复、减少细胞凋亡起到抗T2DM的作用;同时,与模型组比较,参芪补胰方组大鼠胰腺组织TLR4、MyD88、NF-κBmRNA及蛋白表达均明显降低(P<0.05)。结论 参芪补胰方可能通过调控T2DM大鼠炎症因子及TLR4、MyD88、NF-κB表达发挥其抗T2DM的作用。
