Several studies have suggested the potential value of Houttuynia cordata as a therapeutic agent in lung cancer, but direct evidence is still lacking. The study aimed to determine the regulatory impact of a major H. cordata constituent derivative (sodium new houttuyfonate [SNH]) on lncRNA networks in non‐small cell lung cancer (NSCLC) to identify new potential therapeutic targets. After exposing NSCLC cells to SNH, we analysed the following: cell death (via flow cytometry, TUNEL and ASC speck formation assays), immune factors (via ELISA), gene transcription (via RT‐qPCR), subcellular localisation (via FISH), gene–gene and gene‐protein interactions (via dual‐luciferase reporter and RNA immunoprecipitation assays, respectively) and protein expression and distribution (via western blotting and immunocytochemistry or immunohistochemistry). In addition, statistical analysis (via one‐way ANOVA or unpaired t ‐tests) was performed. Exposure to SNH promoted NSCLC cell pyroptosis, concomitant with significant up‐regulation of TCONS‐14036, a novel lncRNA. Mechanistic research demonstrated that TCONS‐14036 functions as a competing endogenous (ce)RNA by sequestering microRNA (miR)‐1228‐5p, thereby up‐regulating PRKCDBP‐encoding transcript levels. Indeed, PRKCDBP promoted pyroptosis by activating the NLRP3 inflammasome, resulting in CASP1, IL‐1β and GSDMD cleavage. Our findings elucidate the potential molecular mechanisms underlying the ability of SNH to suppress NSCLC growth through activation of pyroptosis via the TCONS‐14036/miR‐1228‐5p/PRKCDBP pathway. Thus, we identify a new potential therapeutic targets for NSCLC.
目的 探讨应用加速康复外科对腹腔镜腹股沟疝修补术后康复的影响.方法 对2016年1月至2020年6月该院经治的562例接受腹腔镜疝修补术的腹股沟疝患者的临床资料进行回顾性分析,其中2018年1月至2020年6月收治的351例患者采用围术期加速康复外科(ERAS)方案(ERAS组),主要措施包括术前宣教、饮食与液体管理、多模式镇痛及术后早期活动等;2016年1月至2017年12月收治的211例患者采用传统围术期处理方案(对照组);按腹腔镜经腹腹膜外修补术(TAPP)和腹腔镜完全腹膜外修补术(TEP)两种手术方式分层比较两组术后康复情况,术后急、慢性疼痛程度,并发症发生率及术后住院时间等.结果 ERAS组患者T A P P及T E P术后恢复正常进食时间[(5.65±2.80)、(5.89±3.82)h]、术后下床活动时间[(10.31±8.20)、(10.21±7.67)h]、术后住院时间[(2.58±1.73)、(2.12±0.80)d]均较对照组明显缩短(P<0.01).接受TAPP患者中ERAS组尿潴留发生率明显低于对照组(P<0.05).分别比较两种术式术后第1天疼痛数字评分法(NRS)评分、术后3个月腹股沟疼痛调查表(IPQ)评分、术后各种并发症发生率,两组间差异均无统计学意义(P>0.05).结论 在腹腔镜腹股沟疝修补术患者管理中应用ERAS理念,可促进术后康复,缩短住院时间,且安全有效.
BACKGROUND:This study aims to demonstrate the underlying correlation between the resolution of liver fibrosis induced by Gexia-Zhuyu decoction (GZD) treatment and myeloid cell-mediated angiogenesis. MATERIAL AND METHODS:A liver fibrosis mouse model induced by carbon tetrachloride (CCl4) intervention was employed in this study. Dynamics of blood liver function parameters were followed. The liver pathology was detected by Sirius Red and Masson staining. Matrix metalloproteinase (MMP) 2/9, tissue inhibitors of metalloproteinase (TIMP)-1/2, and vascular endothelial growth factor (VEGF)-A expression levels were measured. Bone marrow chimera mice were generated by transfer of bone morrow cells from green fluorescent protein (GFP)-knockin mice into irradiated wild-type mice, and were used it to visualize the role of myeloid cells on the fibrosis resolution induced by GZD treatment. RESULTS:The result of Sirius Red and Masson staining and the dynamics of blood liver function parameters showed that 5 weeks of GZD treatment attenuated the severity of liver fibrosis with continual CCl4 administration. GZD treatment promoted the expression of MMP2/9 and repressed the heightened level of TIMP-1/2 in the recovery phase. More notably, the increased VEGF-A and augmented endothelial progenitor cells were observed in the liver and blood in mice that received GZD, and contributed to the remodeling of hepatic vascular though the CXCL12/CXCR4 axis. Then, chimera mice with GFP-positive bone marrow cells were used to show angiogenesis driven by GZD-induced myeloid cell motivation. We found that GZD facilitated myeloid cells binding to the vascular CXCR4 and induced the resolution of fibrosis. CONCLUSIONS:This study shows that activation of myeloid cells induced by GZD administration accelerates the functional angiogenesis, which benefits the resolution of CCl4-induced liver fibrosis.
目的 探讨自拟健脾补肾方对结直肠癌肝转移患者血清MMP?9、TIMP?1的影响.方法 将82例结直肠癌肝转移患者分为观察组和对照组.对照组采用常规FOLFOX4方案化疗.观察组在对照组的基础上联合自拟健脾补肾方治疗.采用RECIST实体瘤标准评估两组的临床疗效;检测两组治疗前后血清MMP?9、TIMP?1水平;采用欧洲癌症治疗研究组织制定的生存质量测定量表(QLQ?C30)评估两组患者的生存质量.结果 观察组的控制率高于对照组,差异有统计学意义(P<0.05);两组的缓解率无明显差异(P>0.05);两组治疗后血清MMP?9、TIMP?1水平明显降低,QLQ?C30评分明显升高(P<0.05);观察组治疗后血清MMP?9水平低于对照组,血清TIMP?1水平和QLQ?C30评分高于对照组,差异均有统计学意义(P<0.05);观察组恶心呕吐、白细胞减少、肝功能损伤、肾功能损伤、血小板减少的发生率均低于对照组,差异有统计学意义(P<0.05).结论 自拟健脾补肾方可提高结直肠癌肝转移的疗效,改善血清MMP?9、TIMP?1水平,降低不良反应发生率.
Objective To summarize the ultrasonographic and pathological features of benign thyroid nodules in thyroid imaging reporting and data system (TI-RADS) 4 class. Methods The preoperative ultrasonographic features and pathological features of 46 patients (52 nodules) with TI-RADS 4 class benign thyroid nodules who got treatment in Affiliated Hospital of Nanjing University of Traditional Chinese Medicine between June 2016 and June 2017 were retrospectively analyzed. Results Among the 52 thyroid nodules, 32 nodules (61.54%) were categorized as TI-RADS 4A, 12 nodules (23.08%) were categorized as TI-RADS 4B, and 8 nodules (15.38%) were categorized as TI-RADS 4C. On ultrasonographic images, the 52 thyroid nodules showed hypoechogenicity (90.38%), solid or predominantly solid (98.08%), nodules>1 cm in diameter (61.54%), defined margin (63.46%), microcalcification (50.00%), abundant or pointy blood flow (65.39%), morphological rule (90.38%), aspect ratio≤1 (92.31%), and no suspicious lymph nodes (86.54%). On histology, these thyroid nodules revealed nodular goiter with fibrosis or calcification (38.46%), nodular goiter with adenoma (30.77%), nodular goiter (9.61%), Hashimoto's thyroiditis (17.31%), and thyroid follicular adenoma (3.85%). Conclusion Nodular goiter (thyroid nodules with fibrosis, calcification or adenomatoid hyperplasia), Hashimoto's thyroiditis, and thyroid follicular adenoma will lead to malignant signs on ultrasonographic images, we should refer to other examination methods to make combinated evaluation.
Objective To investigate the homing specificity of bone marrow (BM)-derived endothelial progenitor cells (EPCs) of transgenic mice carrying enhanced green fluorescent protein (EGFP) using a murine intraperitoneal hepatocellular carcinoma (HCC) model and its potential clinical value.Methods BM mononuclear cells were isolated by ficoll density-gradient centrifugation and serial differential adhesion method was used to purify the EPCs carrying EGFP which had been identified by morphology,uptake of DiI-Ac-LDL,binding of UEA-1 lection,flow cytometry and in vitro tube formation.Ex vivo expanded mouse BM-derived EGFP-EPCs were randomly injected into tumor-bearing nude mouse and normal nude mouse via the tail vein.Two weeks after injection of EPCs,the livers and other organs in each group were examined for distribution of transplanted EPCs.Results Fluorescence image analysis revealed that systemically transplanted EPCs'homed'to HCC with significantly higher specificity,respectively,in the number of cells in the experimental group were 169.40 ± 10.87 in the livers,15.40 ± 2.87 in the heart,14.70 ±2.02 in the lung,1.70 ±0.50 in the kidney,28.30 ±4.20 in the spleen,6.90 ±1.31 in the pancreas,and 4.60 ± 1.22 in the intestine,and those in the control group were respectively 76.50 ±4.20,16.70 ± 1.32,76.00 ± 3.31,24.70 ± 1.70,77.90 ± 4.50,18.60 ± 2.00 and 12.70 ±1.70 correspondingly.The number of cells in the liver more than lung (P =0.001),heart (P =0.032),kidney (P =0.002),spleen(P =0.036),pancreas (P =0.001) and bowel (P =0.001),the number of lung and spleen cells was higher in heart(P =0.036),pancreas (P =0.010),kidney (P =0.030) and bowel(P =0.008).as compared to other organs within the experimental group and to anatomically matched liver sections from the control groups (P =0.001).The quantify of EGFP-EPCs in lungs and spleen were higher specificity as compared to other organs (no liver).Immunohischemical showed the transplanted cells differentiate into vascular cells and recruitmented tumor angiogenesis.Conclusion The results suggested that the autologous EPCs in vivo could be recruited specifically into HCC,and take part in the angiogenesis of HCC in the nude mice.
A simple, sensitive and selective liquid chromatography coupled with electrospray ionization mass spectrometry (LC/ESI/MS) method for determination of skullcapflavone-II in dog plasma has been developed and validated. Analyses were conducted at a flow rate of 0.2 mL/min with the mobile phase consisted of acetonitrile and deionized water (v:v, 80: 20) containing 0.1 % formic acid. The detection utilized selected positive ion at m/z 375.1 and 323.3 for skullcapflavone-II and neobavaisoflavone, respectively. The linearity was excellent over the concentration range from 0.001-4 mu g/mL of skullcapflavone-II. The lower limit of quantification of skullcapflavone-II was 0.001 mu g/mL. The intra- and inter-day precision (relative standard deviation, R.S.D. %) was lower than 10% and accuracy ranged from 98.9 to 102.2 % showed a good reproducibility. This new method has been successfully applied to the pharmacokinetic evaluation of skullcapflavone-II in beagle dogs after intravenous administration.
AKT is often hyper-activated in human colorectal cancers (CRC). This current study evaluated the potential anti-CRC activity by AT7867, a novel AKT and p70S6K1 (S6K1) dual inhibitor. We showed that AT7867 inhibited survival and proliferation of established (HT-29, HCT116 and DLD-1 lines) and primary human CRC cells. Meanwhile, it provoked caspase-dependent apoptosis in the CRC cells. Molecularly, AT7867 blocked AKT-S6K1 activation in CRC cells. Restoring AKT-S6K1 activation, via expression of a constitutively-active AKT1 ("ca-AKT1"), only partially attenuated AT7867-induced HT-29 cell death. Further studies demonstrated that AT7867 inhibited sphingosine kinase 1 (SphK1) activity to promote pro-apoptotic ceramide production in HT-29 cells. Such effects by AT7867 were independent of AKT inhibition. AT7867-indued ceramide production and subsequent HT-29 cell apoptosis were attenuated by co-treatment of sphingosine-1-phosphate (S1P), but were potentiated with the glucosylceramide synthase (GCS) inhibitor PDMP. In vivo, intraperitoneal injection of AT7867 inhibited HT-29 xenograft tumor growth in nude mice. AKT activation was also inhibited in AT7867-treated HT-29 tumors. Together, the preclinical results suggest that AT7867 inhibits CRC cells via AKT-dependent and -independent mechanisms.
目的:探讨纳米碳混悬液在甲状腺癌手术中对颈部淋巴清扫的引导作用和甲状旁腺功能的保护价值.方法:对同时实施甲状腺全切除及患侧颈部淋巴清扫的120例分化型甲状腺癌病例随机分为2组进行前瞻性研究,其中60例在术中甲状腺注射纳米碳混悬液(纳米碳组),60例行常规手术(对照组).分别统计两组手术时间、术中出血量、术后引流量、术中甲状旁腺移植病例数、cN0和cN1的清扫淋巴结总数和直径<5 mm淋巴结数量及转移率、术后甲状旁腺激素水平、血清钙浓度及术后出现低钙血症病例数.结果:纳米碳混悬液组手术时间长(P=0.002),术中出血、术后引流量无明显差异(P>0.05);纳米碳混悬液组清扫淋巴结较对照组多(P=0.000 1).在cN0病例中纳米碳混悬液使得淋巴结清扫总数和直径<5 mm的淋巴结清扫数目明显多于对照组(P=0.000 1),但在cN1病例中两组淋巴结总数无明显差异(P=0.086),但有助提高直径<5 mm淋巴结的检出数(P=0.000 1).对照组术后低钙血症多于纳米碳组(P=0.001).结论:纳米碳混悬液有助于提高甲状腺癌cN0病例转移性淋巴结的检出数,尤其是直径<5mm淋巴结的检出数,同时对甲状旁腺功能具有保护作用.
目的:总结腹腔镜下完全腹膜外腹股沟疝修补术(TEP)的临床经验及手术体会。方法回顾性分析55例(63侧)行T EP治疗患者的临床资料,均为原发疝,单侧47例,双侧8例;斜疝49例,直疝6例。结果53例顺利完成T EP ,2例中转腹腔镜经腹腹膜前疝修补术。单侧手术时间30~210(67.6±35.7)min ,双侧手术时间40~225(100.6±55.6)min;腹膜破损8例,无腹壁下血管、精索血管、输精管损伤,无一例使用止痛剂及尿潴留发生。术后1~7d出院,患者术后第1、6个月电话随访。2例出现术后远端疝囊血肿,经穿刺抽吸治疗后好转;无一例复发及发生切口感染、慢性疼痛、内脏损伤、感觉异常等并发症。结论 T EP是目前成人腹股沟疝修补术中较理想的术式,手术操作的要点与难点主要为腹膜外前间隙的建立、镜下解剖标记的辨认与合理应用、疝囊的处理及补片的放置。
Objective To investigate the safety and effectiveness of laparoscopic cholecystectomy (LC) for calot triangle chronic adhesion.Methods The clinical data of 39 calot triangle chronic adhesion patients undergoing LC were retrospectively analyzed, and the operation effect was observed.Results There were 37 cases underwent successful LC, and the operation time was 30~175 min, with an average time of (65±8)min. There were 2 cases underwent laparotomy instead. The average hospital stays of the 39 cases was (3.0±0.5)d. Follow-up lasted for 9~25 months in 36 cases, and there was no short-term complications.Conclusion LC is a safe and feasible surgery method for treating calot triangle chronic adhesion.
结石性萎缩性胆囊炎由于炎症反复发作,胆囊壁不断纤维化、增厚、变硬,体积缩小,胆囊三角解剖不清,其经腹腔镜下切除难度大.为探讨腹腔镜下胆囊切除术(laparoscopic cholecystectomy,LC)治疗结石性萎缩性胆囊炎的方法及注意事项,现回顾性分析我院2006年1月~2008年12月行LC的结石性萎缩性胆囊炎37例,总结如下.
目的:观察益气建中,攻里通便类中药对高龄肠梗阻患者的疗效。方法:将69例高龄肠梗阻患者随机分为两组。治疗组:35例运用建中承气汤灌肠结合西医对本病进行治疗。对照组:34例单纯西医药方法治疗。结果:治疗组35例患者,总有效率86%。对照组34例,总有效率62%。两组相比治疗组有效率明显高于对照组,死亡率明显低于对照组(P<0.05)。提示:建中承气汤对高龄肠梗阻患者有较好的疗效,能降低高龄肠梗阻患者死亡率。
目的观测安乳霜对乳腺增生模型大鼠激素水平的调节作用。方法先以苯甲酸雌二醇和黄体酮给大鼠肌注造模,造模成功后以安乳霜外涂乳房,治疗2周后以放射免疫法测定大鼠血清中催乳素(PRL)、孕酮(P)和雌二醇(E2)的含量,并观察乳腺增生病理改变。结果安乳霜能明显调节乳腺增生模型大鼠PRL、P和E2水平,抑制乳腺增生。结论安乳霜对乳腺增生大鼠具有良好的治疗作用。

Objective To observe the preventive and therapeutic effect of Chinese herbal medicine film microwave irradiation on mammary hyperplasia in rats. Methods Rat models of experimental mammary hyperplasia were established by intramuscular injection of estrogen and a small quantity of progestin. Two weeks after modeling, the rats were treated with Chinese herbal medicine film microwave irradiation. Before and after the treatment, mammary diameter of the second nipples were measured, serum concentrations of estrogen (E2), progesterone (P) and prolactin (PRL) were determined by radioimmunoassay,pathological changes of mammary hyperplasia were examined under microscope,and the rate of mammary hyperplasia was measured. Results Chinese herbal medicine film markedly inhibited experimental mammary hyperplasia in rats,and the rate of mammary hyperplasia were 23.4 %in medicine film group and 20.9 %in medicine film microwave irradiation group respectively. Medicine film regulated the concentrations of estrogen and progestin,reduced the level of PRL and alleviated the pathological severity of mammary hyperplasia obviously. Conclusion Chinese herbal medicine film microwave irradiation have certain preventive and therapeutic effect on mammary hyperplasia in rats.
目的观察辛温解表通络法治疗急性乳腺炎疗效.方法以解表通络为主内服中药(细辛、麻黄、桂枝、路路通、龟版等)配合患乳局部按摩治疗本病83例.结果总有效率为98.8%.提示本方法对本病有疏通乳络,消肿止痛的功效.
目的寻求对原发性肝癌介入治疗后并发症的有效治疗方法.方法治疗组运用中西医结合治疗方法,对照组单用西药.结果治疗组总有效率为97.87%,与对照组相比治疗组对介入治疗后并发症的疗效有明显优势(P<0.01).结论原发性肝癌介入治疗后行中西医结合用药具有良好疗效,能较快恢复因介入治疗而引起的诸多不良反应.
老龄患者因胆石梗阻胆总管而出现的黄疸, 在临床上并不少见, 笔者总结了我科1998年12月-2001年12月采用中西医结合方法治疗的56例70岁以上患者的结果, 现报道如下.
