Rationale: Novel coronavirus pneumonia is spreading in many countries and regions. Coronavirus disease (COVID-19) is characterized by rapid onset, high infectivity, rapid progression, and variably effective treatment. Patient concerns: One 63-year-old woman presented to the fever clinic of our hospital on February 10, 2020, due to a fever for 12 days. Diagnoses: COVID-19 complicated with liver abscess. Interventions: A case report of a patient with COVID-19 complicated with liver abscess admitted to our hospital was used to explore the treatment methods for COVID-19 complicated with bacterial liver abscess. The sepsis caused by pulmonary viral infection and liver bacterial infection were correctly distinguished, and the patient was cured and discharged after targeted treatment, abscess, and pleural puncture and drainage. Outcomes: The patient was cured with a normal temperature is >3 days and coronavirus 3 consecutive negative nucleic acid detection. Lessons: COVID-19, elderly critically ill patients may be prone to rapid onset, complex disease, multiple organ damage, prolonged hospital stay, and high mortality. In the treatment of such COVID-19 patients, holistic thinking is required, pathology may not be limited to the lung, but may affect other organs, and treatment guidelines should not be blindly followed. Medication may need to be individualized for patients with COVID-19 complicated with liver abscess.
目的 了解湖北十堰地区某公司7421例员工普通体检代谢相关脂肪性肝病(MAFLD)的患病情况,以及MAFLD患病的影响因素.方法 进行健康体检的东风公司员工7421例,依据MAFLD诊断标准分为MAFLD组2839例、非MAFLD组4582例.收集入选体检者的性别、年龄等资料,计算不同性别及年龄MAFLD患病率.收集两组体检者的一般资料和血液生化指标信息,包括年龄、BMI、丙氨酸转氨酶(ALT)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、血小板计数(PLT)、空腹血糖、尿酸等.以是否患有MAFLD为因变量,以单因素分析中差异有统计学意义的指标为自变量并赋值,多因素Logistic回归分析7421例体检者MAFLD患病的影响因素.结果 7421例体检者中MAFLD患病率为38.3%,其中男性患病率为43.8%、女性患病率为17.1%,男性患病率显著高于女性(P<0.05),男性MAFLD患者年龄<30岁、30~39岁、40~49岁、≥50岁者患病率均高于女性(P均<0.05).3309例非MAFLD组男性体检者的年龄、ALT、TG、TC、HDL-C、LDL-C、PLT、超重/肥胖检出率、空腹血糖异常检出率、高尿酸血症检出率等指标与2576例MAFLD组男性体检者相比,P均<0.05.1273例非MAFLD组女性体检者的年龄、ALT、TG、TC、HDL-C、LDL-C、PLT、超重/肥胖检出率、空腹血糖异常检出率、高尿酸血症检出率等指标与263例MAFLD组女性体检者相比,P均<0.05.Logistic回归分析结果显示,年龄、ALT、TG、HDL-C、PLT、超重/肥胖、空腹血糖异常、高尿酸血症是5885例男性体检者MAFLD患病的影响因素,ALT、TG、HDL-C、超重/肥胖、高尿酸血症是1536例女性体检者MAFLD患病的影响因素.结论 湖北十堰地区某公司7421例员工中MAFLD患病率偏高,男性患病率高于女性,年龄、ALT、TG、HDL-C、PLT、超重/肥胖、空腹血糖异常、高尿酸血症是男性体检者MAFLD患病的影响因素,ALT、TG、HDL-C、超重/肥胖、高尿酸血症是女性体检者MAFLD患病的影响因素.
Non-alcoholic fatty liver disease(NAFLD) is the most common chronic liver disease in China and the primary cause of abnormal liver function. There are many risk factors affecting NAFLD, including not only common risk factors such as sex, age, metabolic syndrome, but also rare factors such as sleep disorders, myopathia, etc. With the changes in diet and lifestyle, the prevalence and incidence of NAFLD in China are showing a continuous increase and a trend of younger age.Epidemiological investigation is conducive to the diagnosis, treatment and prevention of NAFLD. In recent years, the epidemiological data of NAFLD in China are constantly improved. This article reviews the prevalence of NAFLD and related risk factors in the past 10 years in China.
目的:评价血必净治疗新型冠状病毒肺炎(COVID-19)的临床疗效.方法:采用回顾性研究方法,将某院隔离病房2020年1月21日-2月24日收治的44例普通型COVID-19患者作为研究对象,按治疗方案分为观察组(n=22)和对照组(n=22),2组同时接受常规抗病毒等对症治疗,观察组在此基础上加用50 mL血必净注射液与生理盐水100mL静脉滴注,bid,治疗7d.治疗结束后比较2组炎症指标变化,核酸转阴及肺CT病灶吸收情况.结果:治疗后观察组肺CT病灶吸收有效率及治疗总体有效率均高于对照组(P<0.05),但2组治疗前后炎症指标(WBC、L、L%、CRP、铁蛋白)恢复水平及核酸转阴率均无明显差异(P>0.05);用药过程中无明显不良反应.结论:血必净治疗COVID-19普通型可以促进肺部感染病灶吸收并提高疗效,安全性良好,但对炎症指标及核酸转阴率改善不明显.
目的 对本院71例出院的新型冠状病毒肺炎(简称新冠肺炎)患者的临床特点以及治疗方案进行评价分析.方法 回顾性分析2020年2月1日-2020年2月24日出院的新冠肺炎患者的病例资料及药物治疗方案.结果 71例新冠肺炎患者中,57例(80.3%)伴有咳嗽,56例(78.9%)有发热的表现,30例(42.3%)伴有肌肉酸痛、乏力等症状,20例(28.2%)有消化系统症状,12例(16.9%)有头晕、头疼等症状,11例(15.5%)有胸闷、气喘等症状.CT影像学出现进展与好转中位数分别为4.1 d与6.1 d,24例(33.8%)白细胞减低,29例(40.8%)出现C反应蛋白升高.患者均给予了抗病毒、抗菌、免疫调节、中药(注射液或方剂)等治疗措施,15例(21.1%)患者出现了严重不良反应.结论 所有新冠肺炎患者快速入院,救治及时;提倡早诊断,早隔离,早治疗;α-干扰素、阿比多尔、大剂量维生素C及中药联合治疗方案效果理想,同时应加强不良反应监测.
目的:探讨我院治愈出院的新型冠状病毒肺炎患儿的临床特点及药物治疗方案.方法:收集我院2020年2月1-26日治愈出院的新型冠状病毒肺炎患儿的病例资料,回顾性分析其临床特点及治疗药物使用情况.结果:共有4例新型冠状病毒肺炎患儿治愈出院,其中男3例,女1例;普通型3例,轻型1例;3例为家庭聚集性发病;2例首发症状为发热;2例中性粒细胞减少及淋巴细胞升高,1例肝酶升高,2例心肌酶谱异常;2例合并有其他呼吸道病原体感染;3例肺部CT示异常病变.4例患儿均给予隔离,卧床休息,抗病毒、增强免疫、中药清热解毒、对症支持等综合治疗,均治愈出院.结论:儿童新型冠状病毒肺炎有其自身临床特点,经综合治疗可痊愈.
目的 分析探讨新型冠状病毒肺炎(Corona virus disease 2019,COVID-19)治愈后再入院患者核酸复检阳性可能的原因,为COVID-19患者的诊断、治疗和治愈出院患者的管理、防护提供依据.方法 收集湖北医药学院附属东风医院2020年2月18-20日收治的5例C O V ID-19复查核酸阳性再次入院的患者病例,回顾性分析其临床资料、影像学特征和核酸检测结果等.结果 5例COVID-19再入院患者中,年龄最大60岁,最小19岁;4例患者有基础疾病;5例均有流行病学史,首发症状主要为发热和腹泻;入院诊断中4例为C O V ID-19普通型,1例为重型;确诊依据中4例核酸检测阳性确诊,1例临床诊断确诊;胸部影像学检查均表现为磨玻璃影,首次入院给予抗病毒药物和对症治疗后均好转;首次住院时间为(13.60±3.93)d,从首次出院到再次入院时间为(7.40±1.36)d.结论 治愈后再入院患者核酸复检阳性的原因可能与多种因素有关,应警惕复阳患者成为慢性病毒携带者;应从标本的采集运送、检测试剂的比对、药物治疗疗程、病毒本身原因、机体免疫力等方面入手,尽量避免复阳情况的发生.
目的:探讨金银花多糖(FLP-3)对卡介苗加脂多糖(BCG+LPS)致小鼠免疫性肝损伤的影响.方法:将72只昆明种小鼠按随机数字表法分为正常组、BCG+LPS免疫性肝损伤小鼠组(模型组)、对照组(联苯双酯)及FLP-3低、中、高剂量组,每组12只;制备小鼠免疫性肝损伤模型;观察各组小鼠肝组织Bcl-2、Bax阳性表达率、血清白细胞介素10(IL-10)、白细胞介素12(IL-12)水平和小鼠肝组织的病理改变.结果:与正常组比较,模型组小鼠肝组织Bax表达上升(P<0.01),Bcl-2表达降低(P<0.01);与模型组比较,FLP-3各剂量组Bax表达下降(P<0.01),Bcl-2表达上升(P<0.01),而FLP-3高剂量组与对照组肝组织Bax及Bcl-2表达比较差异无统计学意义(P>0.05).与正常组比较,模型组小鼠血清IL-10、IL-12水平升高(P<0.01);与模型组比较,FLP-3各剂量组IL-10、IL-12水平下降(P<0.01);与对照组比较,除FLP-3高剂量组血清IL-12水平差异无统计学意义外,其余剂量组血清IL-10、IL-12水平均降低(P<0.05).结论:金银花多糖对LPS+BCG致小鼠免疫性肝损伤有明显的保护作用,而防止肝细胞过度凋亡可能是其防治免疫性肝损伤的作用机制.
目的 调查乙型肝炎病毒(HBV)基因型、核苷(酸)类药物(NAs)耐药基因突变对湖北十堰地区慢性乙型肝炎(CHB)患者肝硬化、肝癌(HCC)的影响.方法 将521例HBV-DNA阳性CHB住院患者分为CHB组和CHB合并肝部病变组(包括肝硬化代偿、失代偿和HCC);对HBV基因型、逆转录酶主要耐药基因突变位点,血清透明质酸(HA)、层黏蛋白(LN)、Ⅲ型前胶原N端肽(PⅢNP)、Ⅳ型胶原(Ⅳ-C)以及甲胎蛋白(AFP)、糖类抗原125(CA125)、糖类抗原199(CA199)、癌胚抗原(CEA)水平进行检测.结果 感染C基因型H B V的患者肝脏病变比例高于B型(46.0%v s.34.1%;χ2=7.128,P=0.008),其肝硬化、肝癌O R值分别为1.34(1.04~1.73)、1.84(1.04~3.26);且C基因型血清LN、PⅢNP、Ⅳ-C、AFP和CA125水平高于B型(t=2.713、4.226、4.381、3.661、2.349;P分别为0.007、<0.001、<0.001、<0.001、0.019).有N A s用药史患者肝部病变比例低于无N A s用药史(33.1%v s.44.2%;χ2=6.785,P=0.009),其O R为0.75(0.60~0.93);且272例有N A s用药史患者中,耐药突变型患者的肝部病变比例、血清学指标与野生型均无统计学差异(均P>0.05).结论 本地CHB住院患者感染C基因型HBV的较B型更易产生肝脏病变;NAs药物治疗与低的肝脏病变发生率有关,而RT区主要耐药基因突变对肝脏病变无明显影响.
To investigate the efficacy of Yigan Decoction combined with western medicine antiviral therapy in the treatment of chronic hepatitis B,103 cases of chronic hepatitis B patients were randomly divided into control and observation groups.the control group was treated with chemical medicines,the observation group was treated with chemical medicines and Yigan Decoction,results showed that after treatment there was no significant difference in blood routine,urine routine and renal function between 2 groups,but the index of liver function,the level of inflammatory cytokines and serum HBV-DNA negative rate of the observation group were more significant than that of the control group;the total effective rate of ob-servation group was significantly higher than the observation group,suggesting Yigan Decoction may improve the efficacy of chemical medicines on chronic hepatitis B,and its mechanism may be related to the regulation of immune function.

Background: Tao-Hong-Si-Wu decoction (THSWD) is a traditional Chinese herbal medicine that has been used for centuries in the treatment of Chinese patients with chronic liver disease. Recently, THSWD has been reported to alter vascular endothelial growth factor (VEGF) induced angiogenesis, raising the possibility that in addition to its anti-inflammatory properties; THSWD might also inhibit hepatic blood flow associated fibrosis.Aim: To document the effects of THSWD on hepatic necroinflammatory disease activity, fibrosis and VEGF signaling in a murine model of chronic liver disease.Methods: Sixty adult mice were equally divided into six study groups. Five groups were exposed to subcutaneous carbon tetrachloride (0.1 m1/10 g BW) for six weeks. Three of the five groups were treated with different concentrations of THSWD (4.25, 8.50, 17.00 g/kg), one with 0.1 mg/kg of Colchicine (positive control), and one with physiologic saline (negative control). Mice in the sixth group were not exposed to CCI4 and remained untreated (healthy controls). Liver enzymes/function tests, hyaluronic acid and laminin levels were measured in serum, and hepatic histology, VEGF, Flt-1 and kinase insert domain containing receptor (KDR), Akt and phosphorylated Akt (pAkt) expression were documented in liver tissue at the end of treatment.Results: Hepatic necroinflammatory disease activity and fibrosis were significantly attenuated in THSWD treated mice in a dose dependent manner. These beneficial results were similar and often exceeded those achieved with Colchicine. In addition, VEGF, Flt-1, KDR, Akt and pAkt mRNA and protein expression were reduced in TSHWD treated mice.Conclusions: In this animal model of chronic liver disease, THSWD decreased hepatic necroinflammatory disease and fibrosis. Inhibition of VEGF expression and downstream signaling were associated with these findings. Further studies with this and other TCMs as treatment for chronic liver disease are warranted. Although rare, acute liver failure (ALF) is associated with high levels of mortality, warranting the development of novel therapies. Nuclear factor-κB (NF-κB), tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6) play roles in ALF. Lipoxin A4 (LXA4) has been shown to alleviate inflammation in non-hepatic tissues. In the present study, we explored whether LXA4 exerted hepatoprotective effects in a rat model of ALF. A rat model of ALF was generated by intraperitoneal injections of D-galactosamine (300 mg/kg) and lipopolysaccharide (50 µg/kg). Animals were randomly assigned to: control group (no ALF); model group (ALF); and the groups treated with a low dose (0.5 µg/kg), medium dose (1 µg/kg), and high dose (2 µg/kg) of LXA4 (all with ALF); and pyrrolidine dithiocarbamate (PDTC)-treated group (ALF and 100 mg/kg PDTC, an inhibitor of NF-κB). Liver histology was measured using H&E staining, serum levels by ELISA, and liver mRNA expression was measured by RT-PCR for the detection of the pro‑inflammatory cytokines TNF-α and IL-6. Liver cell apoptosis (as measured using the TUNEL method and examining caspase-3 activity), and Kupffer cell NF-κB activity [using an electrophoretic mobility shift assay (EMSA)] were examined. Serum levels of transaminases, TNF-α and interleukin-6 (IL-6) were substantially higher in the model group compared to controls. In the model group, significant increases in TNF-α and IL-6 mRNA expression, TUNEL‑positive cells, and caspase-3 activity in the liver tissue were noted. LXA4 improved liver pathology and significantly decreased the indicators of inflammatory response and apoptosis in a dose-dependent manner. High-dose LXA4 provided better protection than PDTC. LXA4 administration significantly decreased NF-κB expression in hepatocytes and Kupffer cells. These results indicated that LXA4 inhibited NF-κB activation, reduced the secretion of pro-inflammatory cytokines, and inhibited apoptosis of liver cells, thereby exerting protective effects against ALF.

Objective To establish a HPLC method for the determination of hyperoside in Fu'an granule. Method The HPLC system consisted of the Fortis C18( 4. 6mm × 250 mm,5μm) column,the mobile phase consisted of acetonitrile- 0. 1% H3PO4( 14: 86),the flow rate was 1. 0m L / min,the column temperature was 30℃,the UV detector was set at 360 nm. Results The linear response range was 1. 8- 36. 0μg /m L( r = 0. 9999). The average recovery of hyperoside was 99. 2%,and RSD was 1. 18%. Conclusions The method is simple,rapid,accurate and repeatable. It can be applied in determination of hyperoside in Fu'an granule.
Based on the etiology and pathogenesis and treatment of immunological liver injury, the pharmacological action of Plant polysaccharides on immunological liver injury was expounded, including regulation of the level of ALT and AST, reducing TNF-αand IL-6, influence on NO and NOS(Nitric oxide synthase), and lymphocyte subsets in peripheral blood lymphocytes.Plant polysaccharide is an innovative drug for the treatment of immune liver injury, which has a good development value and clinical application prospect.
s:Objective To establish a method for the determination of notoginsenoside R1 in Jinfuzhitong Pills.Method The HPLC system consisted of the FORTIS-C18(4.6mm ×250mm, 5μm)column, the mobile phase consisted of acetonitrile and 0.01% HAc(19:81), gradient elution flow rate was 1.0mL/min, the column temperature was 30℃, The UV detector was set at 203nm.Results The linear response range were 7.50μg/mL-150.0μg/mL(r=0.9999).The average recovery of notoginsenoside R1 was 98.4086%, and RSD was 1.3664%.Conclusions The method high sensitivity, accurate, repeatable and high secificity.It can be an important method to control the standard of Jinfuzhitong Pills.
Plant polysaccharides have significant bioactivity of immunoregulation and liver protection. They can induce immunocyte to produce various cytokines, which has significant potential to be used for treatment of immunological liver injury disorders. This review describes advances of immunological liver injury pharmacological effects of plant polysaccharides.
