Background: Childhood asthma (CA), a chronic inflammatory disorder of the airways, and childhood obesity (CO), characterized by low-grade systemic inflammation, frequently coexist. This study seeks to elucidate shared biological mechanisms underlying CA and CO and to identify potential biomarkers via comprehensive bioinformatics analyses of public datasets. Methods: CA and CO gene expression datasets were retrieved from the Gene Expression Omnibus (GEO). Differentially expressed genes (DEGs) common to both conditions were identified, with hub genes (HGs) screened via four machine learning (ML) algorithms. The diagnostic performance of candidate HGs was evaluated utilizing receiver operating characteristic (ROC) curve analysis. Functional characterization was conducted utilizing Gene Ontology (GO), Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis, and single-gene gene set enrichment analysis (GSEA). In addition, competitive endogenous RNA (ceRNA) networks were constructed to further explore regulatory relationships and shared pathogenic mechanisms. Results: There were 25 key genes closely linked to CA and CO identified. Enrichment analyses indicated the main involvement of these genes in immune and inflammatory responses, as well as extracellular matrix organization and tissue remodeling. ML analyses ultimately identified NTRK2 and RGS1 as HGs, which demonstrated favorable diagnostic performance in ROC analyses. Furthermore, a ceRNA regulatory network comprising 104 long noncoding RNAs (lncRNAs), 80 microRNAs (miRNAs), and the two HGs was constructed. Conclusions: This study identified NTRK2 and RGS1 as key HGs shared between CA and CO, presenting fresh insights into their common pathogenic mechanisms. These findings possibly contribute to developing improved diagnostic biomarkers and targeted therapeutic strategies for CA comorbid with obesity.
Slow transit constipation (STC) is one of the most common gastrointestinal disorders in children and adults worldwide. Paeoniflorin (PF), a monoterpene glycoside compound extracted from the dried root of Paeonia lactiflora, has been found to alleviate STC, but the mechanisms of its effect remain unclear. The present study aimed to investigate the effects and mechanisms of PF on intestinal fluid metabolism and visceral sensitization in rats with compound diphenoxylate-induced STC. Based on the evaluation of the laxative effect, the abdominal withdrawal reflex test, enzyme-linked immunosorbent assay, quantitative real-time polymerase chain reaction, western blot, and immunohistochemistry were used to detect the visceral sensitivity, fluid metabolism-related proteins, and acid-sensitive ion channel 3/extracellular signal-regulated kinase (ASIC3/ERK) pathway-related molecules. PF treatment not only attenuated compound diphenoxylate-induced constipation symptoms and colonic pathological damage in rats but also ameliorated colonic fluid metabolic disorders and visceral sensitization abnormalities, as manifested by increased colonic goblet cell counts and mucin2 protein expression, decreased aquaporin3 protein expression, improved abdominal withdrawal reflex scores, reduced visceral pain threshold, upregulated serum 5-hydroxytryptamine, and downregulated vasoactive intestinal peptide levels. Furthermore, PF activated the colonic ASIC3/ERK pathway in STC rats, and ASIC3 inhibition partially counteracted PF's modulatory effects on intestinal fluid and visceral sensation. In conclusion, PF alleviated impaired intestinal fluid metabolism and abnormal visceral sensitization in STC rats and thus relieved their symptoms through activation of the ASIC3/ERK pathway.
Panax ginseng and Fructus mume (Renshen Wumei in Chinese, RW) are natural medicines with high nutritional and pharmacological value. They have been widely used together in China to treat gastrointestinal diseases, especially persistent diarrhea, but the potential mechanisms remain elusive. In this study, a diarrhea model was established in rats using a 30% aqueous extract of senna. The therapeutic effects of RW were evaluated by recording the prevalence of loose stools, the diarrhea index, and histopathological changes in colon tissue. The levels of mucins, tight junction (TJ) proteins, inflammatory cytokines, and phosphoinositide 3-kinase/Akt/nuclear factor-κB (PI3K/Akt/NF-κB) signaling pathway proteins were measured. Metagenomic sequencing was used to analyze the gut microbiota. Treatment with RW alleviated injury to the intestinal barrier in rats with diarrhea and also upregulated levels of Muc2 and TJ proteins, such as occludin, zonula occludens-1, and claudin-1. Administration of RW regulated the structure of the gut microbiota in diarrheal rats. Furthermore, RW suppressed levels of interleukin (IL), tumor necrosis factor (TNF)-α, IL-1, PI3K, Akt, and p-NF-κB p65 and also increased IL-4 levels. Our study indicates that P. ginseng and Fructus mume help improve the symptoms of diarrhea, possibly by alleviating the intestinal barrier injury, regulating intestinal flora composition, and inhibiting the PI3K/Akt/NF-κB signaling pathway.

酸甘化阴法广泛运用于临床,其代表方为芍药甘草汤.该文基于"肝常有余""肝主筋膜"理论,阐述小儿抽动障碍的病机,芍药甘草汤具有养血滋阴、敛肝柔肝、缓急舒筋之功,符合小儿抽动障碍病机.该文结合现代药理学研究探讨酸甘化阴法在小儿抽动障碍治疗中的运用.
Background This study summarized the available randomized controlled trials (RCTs) to assess the efficacy and safety of macrolides on pathogens, lung function, laboratory parameters, and safety in children with bronchiectasis. Methods PubMed, EMBASE, and the Cochrane Library were searched for available papers published up to June 2021. The outcomes were the pathogens, adverse events (AEs), and the forced expiratory volume in one second (FEV1%) predicted. Results Seven RCTs (633 participants) were included. The long-term use of macrolides reduced the risk of the presence of Moraxella catarrhalis (RR = 0.67, 95% CI: 0.30–1.50, P = 0.001; I 2 = 0.0%, P heterogeneity = 0.433), but not Haemophilus influenza (RR = 0.19, 95% CI: 0.08–0.49, P = 0.333; I 2 = 57.0%, P heterogeneity = 0.040), Streptococcus pneumonia (RR = 0.91, 95% CI: 0.61–1.35, P = 0.635; I 2 = 0.0%, P heterogeneity = 0.515), Staphylococcus aureus (RR = 1.01, 95% CI: 0.36–2.84, P = 0.986; I 2 = 61.9%, P heterogeneity = 0.033), and any pathogens present (RR = 0.61, 95% CI: 0.29–1.29, P = 0.195; I 2 = 80.3%, P heterogeneity = 0.006). Long-term macrolides had no effect on FEV1% predicted (WMD = 2.61, 95% CI: –1.31, 6.53, P = 0.192; I 2 = 0.0%, P heterogeneity = 0.896). Long-term macrolides did not increase the risk of AEs or serious AEs. Conclusion Macrolides do not significantly reduce the risk of pathogens present (except for Moraxella catarrhalis ) or increase FEV1% predicted among children with bronchiectasis. Moreover, macrolides were not associated with AEs. Considering the limitations of the meta-analysis, further larger-scale RCTs are needed to confirm the findings. Impact Macrolides do not significantly reduce the risk of pathogens present (except for Moraxella catarrhalis ) among children with bronchiectasis. Macrolides do not significantly increase FEV1% predicted among children with bronchiectasis. This meta-analysis reports on the efficacy and safety of macrolides in the treatment of children with bronchiectasis, providing evidence for the management of children with bronchiectasis. This meta-analysis does not support the use of macrolides in the management of children with bronchiectasis unless the presence of Moraxella catarrhalis is provenor suspected.
赵坤依据带状疱疹病因、病机、发病部位及临床表现,应用三焦理论进行辨治.邪犯上焦,多见于疾病早期,为风热毒邪上犯上焦所致,治以疏风散邪解毒,方选普济消毒饮加减.邪犯中焦,多由肝经湿热火毒瘀滞所致,治以清肝利湿、凉血解毒,方用龙胆泻肝汤合五味消毒饮汤,此阶段,由于局部症状明显,疼痛剧烈,赵老师另给予雄黄、冰片、黄柏、利多卡因乳膏调成糊状,涂于患处.邪壅下焦,多见于疾病后期,由中焦湿热火毒证余毒未清,瘀血等邪气阻滞于经络而致,治以活血化瘀、行气止痛,方选桃红四物汤加减.急性期强调清热解毒、利湿止痛之法,后期注重益气散血、搜风通络之功,活血化瘀贯穿整个病程.
目的:探讨金匮肾气丸加减方对小儿闭塞性细支气管炎(BO)中医证素的影响.方法:采用随机对照试验的研究方法,将符合纳排标准的BO患儿60例平分为两组,每组各30例.其中治疗组给予金匮肾气丸加减方治疗,对照组给予西医常规治疗,疗程均为14d.根据证素辨证方法,分别提取病位证素及病性证素,观察两组证素分布情况及治疗前后病位及病性证素积分的变化.结果:最终治疗组完成28例、治疗组完成27例,两组病位证素出现频率>20%的分别为肺、肾、脾、表,病性证素分别为阳虚、气虚、阴虚、血虚、痰、饮.经治疗后,治疗组各病位证素积分均较治疗前下降(P<0.05),且与对照组相比,肾、脾、表病位证素积分均明显下降(P<0.05);对照组肺、表2个病位证素积分,在治疗后较治疗前均有下降(P<0.05).治疗组各病性证素积分均较治疗前下降(P<0.05),且与对照组相比,阳虚、气虚、阴虚、血虚4个病性证素积分均明显下降(P<0.05);对照组痰、饮2个病性证素积分,在治疗后较治疗前下降(P<0.05).结论:金匮肾气丸可有效改善脾肾阳虚型BO患儿病位证素肺、脾、肾、表及病性证素阳虚、气虚、阴虚、血虚、痰、饮的积分情况,研究为金匮肾气方治疗儿童BO的有效性提供了客观依据.
课程思政是当代高校教育的重要环节,是人才培养的关键部分,是做好其他学校工作的前提.高校需要把立德树人贯穿教育工作的全过程,以立德树人为核心的"三全育人"理念是对课程思政教学思想的完善,使得课程思政贯穿教学始终,实现全员、全程、全方位育人,这与中医整体观良好契合,符合中医天人合一、大医精诚、以人为本的思想道德传统.学生标准化病人(SSP)教学以真实模拟典型案例,使学生通过扮演患者、家属、医师等不同角色,切实体验临床患者接诊、病史采集、四诊合参、病历书写、医患沟通、人文关怀等各个环节,可有效提升学生积极性及主动性,符合现代高校中医儿科学临床学科的教学需要.基于"三全育人"理念指导下的SSP教学可在有效提升中医儿科学医学生专业技能的同时,潜移默化地将大医精诚、人文关怀等课程思政内容融入教学过程,以培养出德才兼备的全能型优秀中医儿科人才.
目的:基于中医传承辅助系统,对临床治疗儿童闭塞性细支气管炎的方药进行数据分析整理,总结其遣方法则,为本病临床辨证组方及新方开发应用提供客观理论根据.方法:收集中国知网、万方及维普数据库中关于中医中药治疗儿童闭塞性细支气管炎的处方,利用中医传承辅助系统进行数据挖掘,分析总结高频药物的属性、核心组合及新方方析.结果:筛选文献获取71个组方,涵盖157味中药,使用较多的药物有甘草、杏仁、芦根、葶苈子、五味子、鱼腥草、紫苑、红花、炙麻黄,药味前三位为苦、甘、辛,药性为寒、温、平,15个核心组合和4个新处方.结论:儿童闭塞性细支气管炎的中医治疗原则以宣肺利气,化痰平喘,活血化瘀,温补肺肾为主,用药不宜过于寒凉及温热.
酸甘化阴法是中医学传统特色治则治法,吴鞠通所著《温病条辨》中酸甘化阴法理论的临床运用颇具特色,具有独特的代表性.吴氏创立三焦辨证,根据温病上、中、下三焦病邪初、中、后期阴伤程度不同,将顾护津液视为温病治疗第一要义,其使用贯穿温病治疗始终.吴氏以性味配伍理论为指导,选取生脉散、麦冬麻仁汤、人参乌梅汤等酸甘化阴分治上、中、下三焦病证,同时以灵活多样的酸甘化阴辛甘化阳、酸甘化阴酸苦泄热、酸甘化阴辛淡为阳等酸甘化阴复法,适应温病三焦实际存在的寒热虚实夹杂的复杂病机,灵活展现了酸甘化阴法及其复法的临床运用,具有极大的临床指导意义,值得深入探讨.
目的:研究加味人参乌梅汤对腹泻模型大鼠结肠组织中γ-氨基丁酸(GABA)能信号通路相关调控因子的影响,进一步揭示加味人参乌梅汤益气生津止泻的作用机制.方法:从48只SD幼龄大鼠中随机抽取12只作为正常组,其余大鼠以番泻叶泻下、劳倦力竭及饮食失宜等复合因素造模14 d.造模成功后,随机分为模型组、西药组及中药组,每组12只.正常组与模型组给予生理盐水灌胃(20 mL·kg-1),西药组给予妈咪爱灌胃(0.7g·kg-1),中药组给予加味人参乌梅汤灌胃(35g·kg-1),均1次/d,连续干预7d.每日观测记录大鼠一般情况、稀便率及腹泻指数.获取大鼠结肠标本,采用免疫组化法(IHC)检测大鼠结肠GABA蛋白积分吸光度表达;酶联免疫吸附测定法(ELISA)检测磷脂酰肌醇3-激酶(PI3K)、蛋白激酶B2(Akt2)、磷酸化(p-)Akt及白细胞介素-1β(IL-1β)的含量,实时荧光定量聚合酶链式反应(Real-time PCR)及蛋白免疫印迹法(Western blot)检测大鼠结肠PI3K、Akt2及γ-氨基丁酸A型受体β2亚基(GABRB2)mRNA和蛋白的表达水平.结果:与正常组比较,模型组大鼠一般情况明显变差,稀便率及腹泻指数差异无统计学意义,GABA蛋白表达明显升高(P<0.05),PI3K、Akt2、p-Akt及IL-lβ表达含量明显升高(P<0.05,P<0.01),PI3K、Akt2、GABRB2 mRNA及蛋白表达均极显著升高(P<0.01).与模型组比较,中药组、西药组大鼠一般情况明显好转,稀便率及腹泻指数显著下调(P<0.01),中药组、西药组PI3K、Akt2、p-Akt及IL-1β含量均明显下调(P<0.05),中药组PI3K、Akt2、GABRB2 mRNA均明显下调(P<0.05,P<0.01),GABA、PI3K及GABRB2蛋白表达明显下调(P<0.05,P<0.01);西药组PI3K及Akt2mRNA和PI3K蛋白表达均明显下降(P<0.05).结论:加味人参乌梅汤能通过调节腹泻大鼠结肠GABA能信号通路,减少肠上皮细胞中Cl-向肠腔流动,改善腹泻模型大鼠结肠水液代谢失衡状态,进而发挥其益气生津止泻效应.
目的 借助数据挖掘技术初步探讨近年来小儿便秘的用药规律.方法 检索2009~2019年中国知网、万方、维普数据库中关于中药治疗小儿便秘的研究,筛选符合纳入标准的文献,建立Excel数据库,对药物使用、性味及功效进行频数统计及关联规则分析.结果 共计175首方剂,中药163味,共计频次1894次.火麻仁、枳实、甘草、地黄、白术等十味药物为最常用药物.药物类别以补虚药为主;最常见药对为乌药-木香,玄参-麦冬,乌药-槟榔,玄参-地黄,麦冬-玄参,麦冬-地黄.三项关联中,麦冬、郁李仁→玄参,玄参、枳壳→地黄,麦冬、杏仁→火麻仁置信度最高.无论是药对或3味药物组合,显示行气药与行气药、滋阴药与滋阴药之间存在较大的关联性.结论 中药治疗小儿便秘疗效肯定,以标本兼治为原则;注重健脾补虚、理气运脾、泻下清热、导滞消积;临床治疗需重视"气"与"津"的指导意义.
To investigate the effects of Xiaoning liquid on gut microbiota in mouse during asthma. A total of 60 mice were randomly and averagely assigned to healthy control group, control group, budesonide group, and Xiaoning liquid group. The later three groups were used to establish an Ovalbumin (OVA) asthma model. The intestinal bacterial communities were compared among groups using 16S rRNA gene amplification. Analyzing the structure of gut microbiota with OTU analysis, Shannon-Wiener, PCA, PCOA, etc. 16S rDNA high-throughput sequencing. The abundance and diversity of the gut microbiota in asthmatic mice increased, most obviously in the control group. The Bacteroidetes and Firmicutes levels increased in all asthmatic mice. The level of Bacteroides increased most obviously, making Bacteroides a useful marker of gut microbiota changes in asthmatic mice. The levels of Proteobacterium, Deferribacteraceae and Mucispirillum dropped significantly in the Xiaoning liquid group. Xiaoning liquid can reduce the species and numbers of pathogenic bacteria and restored the intestinal microecology of asthmatic mice. Xiaoning liquid has a positive effect on the function of gut microbiota.
目的:探讨原发性纤毛运动障碍(PCD)的临床特征、基因突变情况和诊治要点.方法:回顾分析1 例PCD患儿的临床表现,实验室检查、影像学检查、基因检测结果和家系相关情况等,总结PCD的临床诊断与治疗.结果:患儿,女,12岁6个月,2 a前出现反复咳嗽、咳痰,经抗感染及中药口服治疗效果不佳.近半个月来症状加重.肺部CT示:双肺多发支气管扩张合并感染;双侧胸膜增厚粘连.电子支气管镜示:支气管扩张;支气管内膜炎.全外显子测序基因检测发现患儿及其弟弟、妹妹 DNAH5 基因杂合突变:63 号外显子 c.10616G>A(p.R3539H)和61号外显子c.10363G>T(p.Q3455*),分别来源于母亲和父亲,c.10616G>A为已知致病突变,c.10363G>T为新发突变,未见文献报道.结论:PCD临床表现多样,治疗主要为抗感染和对症支持.本研究报道了一种新的致PCD的DNAH5基因杂合突变.
Angelman综合征(angelman syndrome,AS)是一种由15号染色体长臂在11~13区(15q11~13)从头微缺失或父源性的15号染色体相关单亲二倍体等引起的遗传性相关的神经源性疾病,临床表现多样,临床诊断有一定困难.本文现回顾性分析1例Angelman综合征患儿的临床资料.患儿,男,1岁3个月,发现全面发育迟缓9个月,入院见表观异常,语言、运动发育落后,基因检测提示父源单亲二倍体,并复习国内外相关文献以了解该病的报道情况,为AS的临床诊治提供参考,以提高临床工作者对该病的认识.
目的 观察哮喘补益膏加减联合三伏贴治疗儿童哮喘缓解期的临床疗效.方法 选择2017年8月至2018年6月四川省中医院就诊的哮喘缓解期患儿70例为研究对象,随机分为对照组34例和观察组36例.对照组采用吸入用布地奈德混悬液及口服孟鲁司特钠咀嚼片治疗,观察组采用口服哮喘补益膏加减及三伏贴治疗.疗程3个月.治疗结束后,从哮喘症状控制水平、肺功能、中医症状积分、中医证候疗效、安全性方面对比两种方案的临床疗效.结果 治疗结束后观察组脱落6例,对照组脱落4例.两组在哮喘症状控制水平和无喘息症状时间方面比较差异无统计学意义(P>0.05).在肺功能方面,两组治疗后最大呼气流速峰值均低于治疗前,差异有统计学意义(P<0.05),治疗后两组最大呼气流速峰值比较差异无统计学意义(P>0.05).观察组治疗后中医症状总积分低于对照组,差异有统计学意义(P<0.05).观察组治疗后反复感冒次数、自汗、纳差、大便异常中医症状积分低于对照组,差异有统计学意义(P<0.05).观察组总有效率为96.7%(29/30),显著高于对照组43.3%(13/30),差异有统计学意义(P<0.05).在哮喘补益膏联合三伏贴对儿童哮喘缓解期肺脾气虚证的治疗期间、治疗结束后患儿均无明显不良反应.结论 哮喘补益膏加减联合三伏贴治疗儿童哮喘缓解期的临床疗效效果好且用药安全.
目的 研究番泻叶致幼龄大鼠腹泻模型量-时-效关系.方法 将96只幼龄大鼠随机抽取24只作为空白组,余72只随机分为低、中、高浓度组,分别给予相应浓度番泻叶灌胃4周,建立大鼠腹泻模型.观察各组大鼠每周稀便率、腹泻指数、体重及电解质(Na+、K+、CL-)变化.结果 ①腹泻指标:中、高浓度组每周稀便率、腹泻指数均显著高于低浓度组(P<0.01),中、高浓度第2、3、4周稀便率、腹泻指数高于第1周(P<0.05),高浓度组第4周稀便率显著高于第1周(P<0.01).②体重:中、高浓度组每周体重增长较空白组、低浓度组缓慢(P<0.05),而高浓度组差异有显著意义(P<0.01).③电解质:Na+:第2周中、高浓度组与空白组及高浓度组与低浓度组比较,下降有统计学意义(P<0.05);第3、4周中、高浓度组较空白组、低浓度组下降显著(P<0.01);中、高浓度组2、3、4周较第1周显著下降(P<0.01),而高浓度组第3、4周较第2周显著(P<0.01);K+:第2、3、4周中、高浓度组较空白组、低浓度组下降均有差异(P<0.05);中、高浓度组第2、3、4周较第1周下降均有差异(P<0.05),而高浓度组下降显著(P<0.01);CL-:第2周中、高浓度组较空白组、低浓度组下降有统计学意义(P<0.05),而第3、4周下降显著(P<0.01).中、高浓度组第2周较第1周下降有统计学意义(P<0.05),而第3、4周下降显著(P<0.01);④安全性:高浓度组较中浓度组死亡率高,差异有统计学意义(P<0.05).⑤结肠病理:中、高浓度组第3、4周较第1、2周相对较重,但差异无统计学意义(P>0.05).结论 中浓度番泻叶致幼龄大鼠腹泻2周时模型效果理想,安全性好.
Modified Renshen Wumei decoction (MRWD), a famous traditional Chinese medicine, is widely used for treating persistent diarrhea. However, as the mechanism by which MRWD regulates diarrhea remains unknown, we examined the protective effects of MRWD on intestinal barrier integrity in a diarrhea model. In total, 48 male rats were randomly distributed to four treatment groups: the blank group (CK group), model group (MC group), Medilac-Vita group (MV group) and Chinese herb group (MRWD group). After a 21-day experiment, serum and colon samples were assessed. The diarrhea index, pathological examination findings and change in D-lactate and diamine oxidase (DAO) contents illustrated that the induction of diarrhea caused intestinal injury, which was ameliorated by MV and MRWD infusion. Metabolomics analysis identified several metabolites in the serum. Some critical metabolites, such as phosphoric acid, taurine, cortisone, leukotriene B4 and calcitriol, were found to be significantly elevated by MRWD infusion. Importantly, these differences correlated with mineral absorption and metabolism and peroxisome proliferator-activated receptor (PPAR) pathways. Moreover, it significantly increased the expression levels of TLR4, MyD88 and p-NF-κB p65 proteins and the contents of IL-1 and TNF-α, while the expression levels of occludin, claudin-1 and ZO-1 proteins decreased. These deleterious effects were significantly alleviated by MV and MRWD infusion. Our findings indicate that MRWD infusion helps alleviate diarrhea, possibly by maintaining electrolyte homeostasis, improving the intestinal barrier integrity, and inhibiting the TLR4/NF-κB axis.
目的:探讨加味人参乌梅汤及其拆方对腹泻模型大鼠差异基因表达的影响,从基因层面揭示加味人参乌梅汤酸甘化阴生津止泻作用的机制.方法:84只SD大鼠随机抽取12只为空白组,其余72只采用复合因素制作腹泻模型,造模成功后随机分为模型组、西药组、全方组、去甘味药组、去酸味药组及去辛味药组,每组12只.造模21d后开始干预,空白组及模型组给予0.9%氯化钠溶液,其余各组分别给予5%枯草杆菌二联活菌颗粒溶液,加味人参乌梅汤全方、去甘味药、去酸味药及去辛味药,1次/d,连续给药7d.取各组大鼠结肠标本,采用全基因芯片技术进行检测,比较各组与模型组受试大鼠的基因表达谱,筛选差异基因,采用生物信息学方法获得基因富集的GO功能及pathway,明确差异基因的相关功能,并对部分差异基因进行RT-PCR验证.结果:与模型组比较,空白组筛选差异基因34个,西药组16个,全方组51个,去甘味药组45个,去酸味药组145个,去辛味药组126个.挑选8条基因进行RT-PCR验证,各组表达趋势与芯片结果基本一致.结论:加味人参乌梅汤对腹泻模型大鼠结肠基因表达具有调控作用,其作用依赖于药物间酸甘化阴、性味合化的协同作用共同发挥生津止泻效应.
